    Strengthening Pharmacy, Globally. Looking Outside the Box

    To strengthen pharmacy practice globally, new and more efficient strategies are needed. Pharmacy has traditionally viewed medicine as its reference group. This commentary questions the appropriateness of medicine as a benchmark for pharmacy and suggests the need for new thinking on the professionalization of pharmacy. Useful lessons from outside the medical field, in matters of professional training, practice and autonomy, are highlighted. The commentary concludes that, strategies that focus on the consumer are more effective in transforming practice globally.   Type: Commentar

    Novel Considerations about Diabetes Management Strategies in Chinese Immigrants in America: Possible Corollaries of the Use of Traditional Chinese Medicines

    Worldwide, the prevalence of type 2 diabetes mellitus (T2DM) and its related management costs have increased over time. The latest predictions anticipate this trend to continue during the next few decades. As individuals with T2DM in the United States (US) are burdened with escalating prescription drug expenses, the appeal and use of cost effective alternatives may also increase. Herbal supplements are an example of such an alternative, but are unregulated and mostly lacking scientific evidence of advertised claims. In general, plant based herbal preparations are often advertised as ‘natural’, ‘less-toxic’, ‘medicines’ and are widely available from public retailers throughout the country. Over half of the US adult population report the use of supplements. Eastern countries, and especially China, have a long history of the use of herbal preparations as traditional medicine to prevent and treat diabetes. Recently the US has experienced a growth in the number imports of herbal supplements and other Traditional Chinese Medicines (TCM) to the country as well as the most rapid population growth in individuals who identify themselves as Asian. To date, little is known about the prevalence TCM use in the management of T2DM either alone or in combination with prescription medicines in the US. Knowledge of possible implications to patient safety (e.g., interactions, adverse effects) with concurrent use of both TCM and prescription medicines is also lacking. This review begins with summaries of the T2DM management strategies employed in traditional modern medicine (TMM) and Chinese medicine (TCM). A review of literature follows where the most commonly used TCM herbal preparations and their ingredients are identified and found along with any published reports of their proposed mechanisms of action, efficacy, adverse effects, and contraindications.     Beyond the Pharmacists’ Patient Care Process: Cultivating Patient Care Practitioners by Utilizing the Pharmaceutical Care Framework

    The adoption of a standard pharmacists’ patient care process (PPCP) for the profession, and inclusion of the PPCP in the ACPE Standards 2016, are positive steps for pharmacy education and creates consistency among pharmacy practitioners, regardless of practice setting. The PPCP, and its implications for practice, needs to continue to be embraced by educators and emphasized with students. The PPCP should be the patient care process taught to students and integrated throughout didactic courses and experiential experiences. However, teaching the PPCP or a particular service, such as Medication Therapy Management (MTM) or Comprehensive Medication Management (CMM), is not enough. The patient care process must be taught as one component of pharmaceutical care. Without also learning the philosophy of practice and practice management systems, student pharmacists will not be prepared for the realities of practice. Pharmacists are taking on new roles, getting paid in new ways, and in positions to take responsibility for a patient’s medication-related needs. Student pharmacists need to be in a position to take advantage of these opportunities as they progress throughout their careers.     Use of Class Facebook Groups to Disseminate Evidence-Based Study Tips

    Objective: The purpose of this preliminary project was to determine the effectiveness of college administrators using Facebook® (FB) to disseminate information on study methods. Innovation: Eleven study tips in the format of riddles were posted in class FB groups as memes with links that lead to the riddle answers. Between 3.2-39.7% of students clicked on the links that accessed riddle answers. In a survey, 53.8% of respondents found the memes at least somewhat useful and 57.6% reported that they somewhat liked, liked, or liked them a lot. The average score on a study method knowledge assessment increased from 50% to 64%. Critical Analysis: The ratings of usefulness and likeability varied. However, students’ knowledge about the topic increased. Administrators considering using FB to share academic advice should post sparingly, begin posting when groups are initially formed and post early during the academic term.     An Approach for the Development and Implementation of an Assessment Tool for Interprofessional Education Learning Activities

    The Accreditation Council for Pharmacy Education Standards 2016 state that colleges of pharmacy must assess student achievement and readiness to contribute as a member of an interprofessional collaborative patient care team. There are a limited number of assessment tools available to achieve this part of the Standards. The purpose of this Case Study Report is to describe the process that one college of pharmacy took to develop an interprofessional education (IPE) assessment tool to be used for their longitudinal assessment approach for IPE in the didactic portion of the curriculum. Strategies for the development of an assessment tool are provided through three themes: continuous refinement, collaboration and streamlining. Next steps for the implementation of the assessment tool, as well as evaluating its validity and reliability, are discussed.   Type: Case Stud

    Impact of a Modified Jigsaw Method for Learning an Unfamiliar, Complex Topic

    Objective: The aim of this study was to use the jigsaw method with an unfamiliar, complex topic and to evaluate the effectiveness of the jigsaw teaching method on student learning of assigned material (“jigsaw expert”) versus non-assigned material (“jigsaw learner”). Innovation: The innovation was implemented in an advanced cardiology elective. Forty students were assigned a pre-reading and one of four valvular heart disorders, a topic not previously taught in the curriculum. A pre-test and post-test evaluated overall student learning. Student performance on pre/post tests as the “jigsaw expert” and “jigsaw learner” was also compared. Critical Analysis: Overall, the post-test mean score of 85.75% was significantly higher than that of the pre-test score of 56.75% (p<0.05). There was significant improvement in scores regardless of whether the material was assigned (“jigsaw experts” pre=58.8% and post=82.5%; p<0.05) or not assigned (“jigsaw learners” pre= 56.25% and post= 86.56%, p<0.05) for pre-study. Next Steps: The use of the jigsaw method to teach unfamiliar, complex content helps students to become both teachers and active listeners, which are essential to the skills and professionalism of a health care provider. Further studies are needed to evaluate use of the jigsaw method to teach unfamiliar, complex content on long-term retention and to further examine the effects of expert vs. non-expert roles.     The Effects of Personal Pharmacogenetic Testing on the Effects of Pharmacy Student Perceptions of Knowledge and Attitudes Towards Precision Medicine

    Objective: To evaluate if pharmacy students’ participation in personal pharmacogenetic (Pgx) testing enhances their knowledge and attitude towards precision medicine (PM). Methods: First-year pharmacy students were offered personalized pharmacogenetic testing as a supplement to a required curricular pharmacogenomics course. Ninety-eight of 122 (80%) students completed pre- and post-course surveys assessing knowledge and attitudes regarding PM; 73 students also volunteered for personal pharmacogenetic testing of the following drug metabolizing enzymes (CYP2C19, CYP2D6, UGT1A1) and pharmacodynamics-relevant proteins (interleukin (IL)-28B & human lymphocyte antigen HLAB*5701). Results: Among the 122 students, we found that incorporating pharmacogenetic testing improved mean knowledge and attitude by 1.0 and 0.3 Likert points, respectively. We observed statistically significant improvements in 100% of knowledge and 70% of attitude-related statements for students who decided to undergo personal pharmacogenetic testing. Students who were enrolled in the course but did not partake in personalized pharmacogenetic testing had similar gains in knowledge and attitude. Conclusion: This study demonstrates the feasibility and importance of educating future pharmacists by incorporating pharmacogenetic testing into professional school curricula. Students who opt not to participate in genotyping may still benefit by learning vicariously through the shared learning environment created by genotyped students.     Imaginary Worlds and Efficiency Frontiers: Should We Abandon the IQWiG Health Technology Assessment Model?

    The contribution of cost-effectiveness analysis to the pricing of pharmaceuticals in Germany is at best marginal and in many, if not most cases, absent. While this may reflect a reasonable belief that cost-effectiveness analysis adds little if anything to pricing and formulary placement decisions, its marginalization reflects considerable dissatisfaction, if not frustration, with modeling efforts by the Institut für Qualität und Wirtschaftlichkeit im Gesundheitseesen (IQWiG). In part, this reflects the rejection of quality adjusted life years (QALYs) as the common outcome standard, together with the adoption of the efficiency frontier as the default framework for modeled claims. The purpose of this commentary is to consider the merits, in the German context, of an efficiency frontier framework for cost-effectiveness and pricing decisions. The commentary concludes that the efficiency frontier framework for health technology assessment, in supporting the creation of non-evaluable claims from models or simulations, fails of to meet the standards of normal science: it fails to support claims that are credible, evaluable and replicable. It should be abandoned. If cost-effectiveness modeling is to play a constructive role in pricing negotiations in Germany then manufacturers should be required to submit evaluable claims. The most effective way of ensuring this is to require manufacturers to accompany any submission for a new product with a protocol detailing how their claims, to include those for clinical outcomes, cost-effectiveness and budget impact, are to be evaluated and reported to decision makers in a meaningful time frame.   Type: Commentar

    Utilizing an Innovative Preceptor Video Mini-Series to Prepare Students for Experiential Rotations: Does it Work?

    Objectives: To determine whether an innovative Mini-Series training model originally developed for preceptors could be beneficial to pharmacy students prior to and/or after beginning their introductory or advanced pharmacy practice experiences. Methods: This program consists of twelve incremental video episodes, each ranging from five to eight minutes in length. It tells the story of a young pharmacy preceptor as she guides a third and fourth year student through a challenging six-week clinical hospital rotation. Two to three reflection questions were written for each individual episode, focusing on student issues portrayed in the videos. Two-hour viewing sessions, consisting of all (12) video episodes and facilitated student reflection were held for 2nd – 4th year professional students on two campuses. At conclusion of each session, students completed a short evaluation to gauge the effectiveness and potential application of the Mini-Series program. Results: Fifty-six (56) students (22 fourth-year, 6 third-ear, and 28 second-year) participated in the voluntary viewing sessions. All students either agreed or strongly agreed that the Mini-Series program was entertaining and educational. In addition, 82% of students strongly agreed this program would be beneficial for students prior to taking their first experiential rotation, while only 47% strongly agreed it would be beneficial after they had started rotations. On a 5-point Likert scale (1=strongly disagree, 5=strongly agree), participants reported a mean of 4.6 that this medium is more effective than traditional lecture orientations held by the Office of Experiential Programs. On three open-ended questions, students provided a diversity of suggestions for enhancing the Mini-Series to make it more effective for students. Implications: The “Mini-Series” model was well received by students as a training medium to deliver educational content. As a result, more programs are being developed utilizing this innovative teaching method to help prepare students for future experiential rotations.     Development of an Integrated Immunology and Vaccines Pharmacy Elective

    Objective: To describe an elective course on immunology and vaccines for pharmacy students that extends beyond basic immunization training. Design: A three credit-hour Immunology and Vaccines elective was developed and taught by an immunologist, policy research expert, and pharmacist. The learning objectives of the course included: understanding how the immune system works with vaccines to provide protection against infectious diseases, the history and policies involved in immunization practice, and how to counsel the vaccine hesitant individual. Classes were conducted using a variety of formats; group projects, lectures, films, literature reviews and guest speakers. An end-of-course evaluation was used to gauge student opinion on course value. Students were evaluated by four exams and a final group presentation. Conclusion: Students indicated that this course was valuable to their future pharmacy careers and provided insight into why people choose not to vaccinate and how they could use the course insight to properly educate such individuals.
