〔翻訳論文〕骨盤臓器脱に対する子宮温存腹腔鏡下仙骨腟固定術の１例
Get PDF骨盤臓器脱（以下POP：Pelvic organ prolapse）は主に中高齢女性に好発し，世界的にも増加している．POPに対する低侵襲の手術として，腹腔鏡下仙骨腟固定術（以下LSC：Laparoscopic sacrocolpopexy）が保険収載となり，徐々に件数が増加している．その際の子宮の扱いには複数のアプローチがあり，特に子宮温存を希望する患者に対しては，Sacrohysteropexy（子宮温存LSC）を行う．子宮を温存する場合は，メッシュの通し方など，子宮周囲の解剖学的特徴を理解する必要がある．しかし，子宮温存のための具体的な手技は，まだ十分に研究や報告がなされていない．今回，当院で施行した子宮温存LSCにおける手術手技の工夫について，症例を提示し考察する．34歳の女性がPOPに対して子宮温存LSCを希望され当院を受診した．子宮温存を希望した本症例では，前腟壁を剥離しメッシュを固定した後に，子宮頸部横の左右の子宮広間膜を展開・開放した．開放した広間膜に前腟壁メッシュの二脚を貫通させ，後腟壁のメッシュと一体化し腹膜下トンネルを利用して岬角前面へ固定した．術後経過は良好で，現在まで再発なく経過観察中である．温存する子宮と干渉するため鉗子ポートの挿入部位の工夫，子宮への愛護的なメッシュの固定，および牽引方法について考察した．departmental bulletin pape
