The effect of ıncentıve spırometry and oscıllatory posıtıve expıratory pressure therapıes on functıonal capacıty, dyspnea, and saturatıon ın burn patıents wıth ınhalatıon ınjury: A randomızed controlled study
Objective: The aim of this study is to investigate the effects of incentive spirometry (Triflo) and oscillatory positive expiratory pressure therapy (use of Acapella) in addition to standard pulmonary physiotherapy exercises on functional capacity, dyspnea, and saturation levels in burn patients with inhalation injury. Method: A total of 24 patients hospitalized in the intensive care and service units of the Gaziantep City Hospital Burn Center were included in the study. Patients were divided into three groups using stratified randomization method. The first group received standard physiotherapy exercises in addition to medical and surgical treatment, the second group received incentive spirometry (Triflo) in addition to this standard treatment, and the third group received standard treatment+oscillatory positive expiratory pressure therapy (Acapella). All groups were followed for 4 weeks. Six-minute walk test (6MWT), dyspnea modified medical research council (MMRC) scale, and saturation values were compared for all groups before and after treatment. Results: A total of 24 patients, 22 males and 2 females, aged 19-63 (38.66±16.77), were included in the study. When the initial and final week measurements of the six-minute walk test, dyspnea scale (MRC), and saturation values of all groups were compared, no difference was found (p>0.05). Conclusion: Oscillatory positive expiratory pressure therapy (Acapella Use) and incentive spirometry (Triflo) can be given in addition to standard physiotherapy exercises in burn patients with inhalation injury. These exercises are equally effective in the functional capacity, dyspnea, and saturation levels of burn patients with inhalation injury
Developing a novel hybrid model based on deep neural networks and discrete wavelet transform algorithm for prediction of daily air temperature
The precise predicting of air temperature has a significant influence in many sectors such as agriculture, industry, modeling environmental processes. In this work, to predict the mean daily time series air temperature in Mu & gbreve;la city (AT(m)), Turkey, initially, five different layer structures of Long Short-Term Memory (LSTM) and Gated Recurrent Unit (GRU) deep learning-based neural network models through the seq2seq regression forecast module are developed. Then, based on performance evaluation metrics, an optimal DL-based layer network structure designed is chosen to hybridize with the wavelet transform (WT) algorithm (i.e., WT-DNN model) to enhance the estimation capability. In this direction, among potential meteorological variables considered, the average daily sunshine duration (SSD) (hours), total global solar radiation (TGSR) (kw. hour/m(2)), and total global insolation intensity (TGSI) (watt/m(2)) from Jan 2014 to Dec 2019 are picked as the most effective input variables through correlation analysis to predict AT(m). To thwart overfitting and underfitting problems, different algorithm tuning along with trial-and-error procedures through diverse types of hyper-parameters are performed. Consistent with the performance evaluation standards, comparison plots, and Total Learnable Parameters (TLP) value, the state-of-the-art and unique proposed hybrid WT-(LSTM x GRU) model (i.e., hybrid WT with the coupled version of LSTM and GRU models via Multiplication layer (x)) is confirmed as the best model developed. This hybrid model under the ideal hyper-parameters resulted in an R-2 = 0.94, an RMSE = 0.56 (degrees C), an MBE = -0.5 (degrees C), AICc = -382.01, and a running time of 376 (s) in 2000 iterations. Nonetheless, the standard single LSTM layer network model as benchmark model resulted in an R-2 = 0.63, an RMSE = 4.69 (degrees C), an MBE = -0.89 (degrees C), AICc = 1021.8, and a running time of 186 (s) in 2000 iterations
Attitude towards service robots in tourism and hospitality services settings — The effect of multidimensional anthropomorphism and technology readiness
The use of technology in tourism is increasing day by day. Service robots are now on the agenda. The success of developments that envisage a new experience for consumer tourists depends on the accurate prediction of tourists' possible attitudes and behaviors and the influencing antecedents. This study examined consumer tourists' attitudes and usage intentions towards service robots on the multidimensional basis of anthropomorphism and technological readiness (TR). A survey was conducted with 1018 tourists in Türkiye face-to-face and using QR codes. Findings showed that attitude towards service robots (AR) mediated the relationship between anthropomorphism, technological readiness and intention to use (IS). Participating tourists anthropomorphically focused on the physical appearance of the robots rather than their functions and internal structures. Service robot experience is still limited in the tourism industry. Therefore, the empirical results obtained are guiding theory and practice in terms of academic studies and tourism service providers. © 2024 The Author(s). Assessing the impact of the COVID-19 pandemic on latex glove usage and latex allergy complaints among nurses: A descriptive study
BACKGROUND: The coronavirus 2019 (COVID-19) pandemic has led to a significant increase in the use of latex gloves among nurses. However, concerns about the rise in latex allergies and related complaints due to this increase remain uncertain. OBJECTİVE: This study aims to assess the rates of latex glove usage and allergy-related complaints among nurses working in hospitals during the COVID-19 pandemic. METHODS: Between May 15 and June 15, 2021, ethical approvals were obtained for a cross-sectional study involving 448 volunteer nurses. Descriptive statistics were used to represent categorical values as counts (n) and percentages (%), while continuous values were represented as mean±standard deviation. The normal distribution of the data was assessed using the Kolmogorov-Smirnov and Shapiro-Wilk tests. Comparative analyses were conducted using paired sample t-test, Pearson's chi-squared (x2) test, McNemar's chi-squared (x2) test, and Spearman correlation analysis. RESULTS: Before the pandemic, the average number of invasive procedures was 45.13±26.48, whereas during the pandemic, this rate increased to 50.23±29.14. The average glove usage duration went from 7.69±3.13 hours to 14.73±3.68 hours during the pandemic. Among nurses, the rate of allergic symptoms, which was previously at 31.5%, rose to 33.3% during the pandemic. CONCLUSİON: This study revealed a significant increase in daily invasive procedures and the use of latex gloves among nurses during the pandemic period. Simultaneously, the frequency of allergic symptoms also rose. These findings underscore the importance of awareness and preventive measures, particularly regarding latex allergies, in the healthcare field. © 2024 - IOS Press. All rights reserved
Pyruvate kinase deficiency in 29 Turkish patients with two novel intronic variants
Pyruvate kinase (PK) is a key enzyme of anaerobic glycolysis. The genetic heterogeneity of PK deficiency (PKD) is high, and over 400 unique variants have been identified. Twenty-nine patients who had been diagnosed as PKD genetically in seven distinct paediatric haematology departments were evaluated. Fifteen of 23 patients (65.2%) had low PK levels. The PK:hexokinase ratio had 100% sensitivity for PKD diagnosis, superior to PK enzyme assay. Two novel intronic variants (c.695-1G>A and c.694+43C>T) have been described. PKD should be suspected in patients with chronic non-spherocytic haemolytic anaemia, even if enzyme levels are falsely normal. Total PKLR gene sequencing is necessary for the characterization of patients with PKD and for genetic counselling
Morbid obez bireylerde uygulanan zayıflama diyeti ile farmakolojik tedavinin ağırlık kaybı ve bazı kan parametreleri üzerine etkisi
Bu çalışma morbid obez bireylere uygulanan yağ oranı farklı ( enerjinin % 25-30' u ile enerjinin %20-24 'ü yağdan gelen düşük yağlı diyet) 2 zayıflama diyeti ile farmakolojik tedavinin ve diyetle kombinasyonunun ağırlık kaybı başta olmak üzere bazı önemli kan parametreleri üzerine etkilerini ortaya koymak amacıyla planlanmış ve yürütülmüştür. Çalışma, Gaziantep Üniversitesi Şahinbey Araştırma ve Uygulama Hastanesi Endokrinoloji Polikliniği'nde gerçekleşmiştir. Çalışmaya, bu çalışmanın süresi olan 3 ay içinde, 18-65 yaş arasında, 63 morbid obez (Beden Kütle İndeksi> 40 kg/m2) birey katılmıştır. Araştırmaya katılan tüm bireylerin demografik özelliklerini ve beslenme alışkanlıklarını saptamak amacıyla anket formu uygulanmıştır. Beslenme durumunun değerlendirmesi amacıyla besin tüketim kaydı, besin tüketim sıklığı formu, temel beslenme ve besin-sağlık bilgi düzeylerini ölçmek için yetişkinlere yönelik hazırlanan beslenme bilgi düzeyi ölçeği (YETBİD), fiziksel aktivite düzeyini belirlemek için fiziksel aktivite formu kullanılmıştır. Morbid obez bireyler randomize olarak üç gruba ayrılmıştır. 1.grup (n=21); standart diyet tedavisi, 2.grup (n=21); standart diyet tedavisi + farmakolojik tedavi, 3. grup (n=21); düşük yağlı (%20-24 yağ) diyet tedavisi+ farmakolojik tedavi almıştır. Üç aylık çalışma sonucunda gruplar sırasıyla (1., 2., ve 3. Grup) %8.7 (104,1 ± 7,15 kg'den 94,97 ± 6,81), %11.3 (105,3 ± 8,59 kg'den 93,47 ± 7,67 ), %12.9 (106,81 ± 10,26 kg'den 93,04 ± 8,84) vücut ağırlık kaybı yaşamıştır (p<0.05). Gruplar arası bel ve kalça çevresindeki azalma istatistiksel olarak anlamlı bulunmuştur (p<0.05) Üçüncü grupta ağırlık kaybı diğerlerine göre daha fazla olmuştur. Açlık kan glukozu (mg/dL), total kolesterol (mg/dL), LDL – kolesterol (mg/dL) gibi bazı kan parametrelerindeki azalma, tüm gruplarda istatistiksel olarak anlamlı düzeyde azalmış olup, bu açıdan gruplar arasında bir fark bulunmamıştır (p <0.05). Obezite tedavisinde yaşam tarzı değişikliği ve bireye özgü planlanmış diyet programı kabul edilebilir bir sonuç vermezse farmakolojik tedavi düşünülmelidir.This study examined the effects of two slimming diets with different fat content (25-30% of the energy and a low-fat diet with 20-24% of the energy coming from fat) applied to morbidly obese individuals, and the combination of pharmacological treatment and diet on some important blood parameters, especially weight loss. It was planned and carried out to reveal its effects. The study was carried out at Gaziantep University Şahinbey Research and Application Hospital Endocrinology Polyclinic. 63 morbidly obese (Body Mass Index > 40 kg/m2) individuals between the ages of 18-65 participated in the study within 3 months, which is the duration of this study. A survey form was applied to determine the demographic characteristics and nutritional habits of all individuals participating in the research. To evaluate the nutritional status, a food consumption record, a food consumption frequency form, a nutrition knowledge level scale (YETBİD) prepared for adults was used to measure basic nutrition and food-health knowledge levels, and a physical activity form was used to determine the physical activity level. Morbidly obese individuals were randomly divided into three groups. 1st group (n=21); standard diet treatment, group 2 (n=21); standard dietary treatment + pharmacological treatment, group 3 (n=21); received low-fat (20-24% fat) diet treatment + pharmacological treatment. At the end of the three-month study, the groups (1st, 2nd, and 3rd Group) respectively (1st, 2nd, and 3rd Group) lost 8.7% (94.97 ± 6.81 from 104.1 ± 7.15 kg), 11.3% (105.3 ± 8.59 kg). (93.47 ± 7.67 kg from 93.47 ± 7.67 kg), 12.9% (93.04 ± 8.84 from 106.81 ± 10.26 kg) body weight loss (p<0.05). The decrease in waist and hip circumference between groups was found to be statistically significant (p<0.05). The weight loss in the third group was higher than the others. The decrease in some blood parameters such as fasting blood glucose (mg/dL), total cholesterol (mg/dL), LDL - cholesterol (mg/dL) decreased at a statistically significant level in all groups, and there was no difference between the groups in this respect (p < 0.05). If lifestyle changes and an individually planned diet program do not yield acceptable results in the treatment of obesity, pharmacological treatment may be considered
Ecological security and agricultural production in the Arab League: Is financial development moderating the interaction?
The jeopardizing of ecological security due to the detrimental effects of human activities necessitates the adoption of various actions to reduce ecological intensity. Though some studies have explored the moderating impact of financial development (FND) towards achieving ecological security, in Arab World it has not been thoroughly investigated. Against this backdrop, we investigate combined role of agricultural production (AFP), gross domestic product (GDP), energy consumption, population, direct and moderating impacts of FND on ecological intensity for a panel of 12 Arab League member states from 1995 to 2021. The empirical outcomes unveiled that AFP and GDP have U-shaped nexus with ecological intensity. It posits that at early stages of AFP, ecological intensity is reduced to a certain level, beyond which higher AFPhinders ecological security supporting the evidence against the Borlaug hypothesis. Our findings further unfolded that environmental Kuznets curve (EKC) hypothesis does not hold for the selected Arab League member states, denoting that real GDP has a U-shaped relationship with ecological intensity. Further findings confirm that energy consumption induces ecological deterioration in the absence of its interaction with FND, along with the interaction term. The causality results largely support these outcomes. Based on these findings, Arab League's climate-related policies should further explore FND to drive energy transition and environmentally friendly measures. © 2024 The Author(s
Suriye'de siberaktivizm ortaya çıkış, dönüşüm, potansiyeller ve sınırlamalar
Siber alanın ve sosyal medya platformlarının ortaya çıkışı, özellikle Orta Doğu ve Arap ülkelerinde sosyo-politik ortamda devrim yarattı. Otoriter rejimlerle uzun süren çatışmalar ve insanların susturulan sesleri arasında muhalefet, savunuculuk ve iletişim için bir platform sağladı. Siber aktivizm, taban hareketlerinin daha geniş bir kitleye ulaşması, desteği harekete geçirmesi ve değişimi etkilemesi için güçlü bir araç haline geldi. Ancak yanlış bilgilendirme, sansür ve karşı aktivizm gibi zorluklarla karşı karşıyadır. Bu araştırma, siber aktivizmin Suriye bağlamındaki rolünü inceliyor; evrimine, etkisine, zorluklarına ve gelecekteki toplumsal hareketlere yönelik potansiyel sonuçlarına odaklanıyor. Aynı zamanda Suriye'deki siber aktivizmin gidişatını aydınlatıyor ve siber aktivistlerin sonuçlarını çevreleyen koşulların anlaşılmasına yardımcı oluyor. Çalışma, profesyonel raporlardan, Suriyeli siber aktivistler ve siber uzmanlarla yapılan görüşmelerden ve bilimsel literatür taramasından toplanan verileri birleştiren nitel bir yaklaşım kullanıyor. Araştırma, çevrimiçi platformların Suriyeli aktivistler arasında bilginin seferber edilmesini, koordinasyonunu ve yayılmasını nasıl kolaylaştırdığını araştırıyor. Ek olarak siber aktivistlerin karşılaştığı gözetim, sansür, güvenlik kaygıları ve siber saldırılar da dahil olmak üzere zorlukları ve riskleri araştırıyor. Ayrıca bu çalışma, yoğun şekilde izlenen ve devlet tarafından yönetilen bir alanda siber aktivizmin dinamiklerini açıklamaktadır. Bulgular, Suriye'deki siber aktivizmin çok yönlü doğasını ve bunun anlatıları şekillendirme, dayanışmayı teşvik etme, otoriter rejimi etkileme ve uluslararası algıları etkileme üzerindeki etkisini vurguluyor. Sonuçta bu çalışma, Suriye bağlamında siber aktivizmin karmaşık dinamiklerinin daha derinlemesine anlaşılmasına katkıda bulunuyor. Dijital platformların sesleri yükseltme, anlatıları şekillendirme, dayanışmayı teşvik etme ve baskıcı ortamlarda değişimi savunma konusundaki dönüştürücü potansiyeline ışık tutuyor.The emergence of the cyber domain and social media platforms has revolutionized the socio-political landscape, especially in the Middle East and the Arab countries. It provided a platform for dissent, advocacy, and communication amidst protracted conflicts with authoritarian regimes and silencing voices of people. Cyber-activism has become a powerful tool for grassroots movements to reach a broader audience, mobilize support, and effect change. However, it faces challenges such as misinformation, censorship, and counter-activism. This research examines the role of cyber-activism in the Syrian context, focusing on its evolution, impact, challenges, and potential implications for future social movements. It also demystifies the trajectory of cyber-activism in Syria and helps to understand the circumstances surrounding the outcomes of cyber activists. The study employs a qualitative approach, integrating data gathered from professional reports, interviews with Syrian cyber-activists and cyber experts, and a scholarly literature review. The research delves into how online platforms have facilitated the mobilization, coordination, and dissemination of information among Syrian activists. Additionally, it investigates the challenges and risks cyber-activists face, including surveillance, censorship, security concerns, and cyberattacks. Also, this study explains the dynamics of cyber-activism within a heavily monitored and state-governed domain. The findings highlight the multifaceted nature of cyber-activism in Syria and its impact on shaping narratives, fostering solidarity, impacting the authoritarian regime, and influencing international perceptions. Ultimately, this study contributes to a more profound understanding of the complex dynamics of cyber-activism within the Syrian context. It sheds light on the transformative potential of digital platforms in amplifying voices, shaping narratives, fostering solidarity, and advocating for change in repressive environments
DIAPH1-deficiency is associated with major t, nk and ılc defects in humans
Loss of function mutations in Diaphanous related formin 1 (DIAPH1) are associated with seizures, cortical blindness, and microcephaly syndrome (SCBMS) and are recently linked to combined immunodeficiency. However, the extent of defects in T and innate lymphoid cells (ILCs) remain unexplored. Herein, we characterized the primary T, natural killer (NK) and helper ILCs of six patients carrying two novel loss of function mutation in DIAPH1 and Jurkat cells after DIAPH1 knockdown. Mutations were identified by whole exome sequencing. T-cell immunophenotyping, proliferation, migration, cytokine signaling, survival, and NK cell cytotoxicity were studied via flow cytometry-based assays, confocal microscopy, and real-time qPCR. CD4+ T cell proteome was analyzed by mass spectrometry. p.R351* and p.R322*variants led to a significant reduction in the DIAPH1 mRNA and protein levels. DIAPH1-deficient T cells showed proliferation, activation, as well as TCR-mediated signaling defects. DIAPH1-deficient PBMCs also displayed impaired transwell migration, defective STAT5 phosphorylation in response to IL-2, IL-7 and IL-15. In vitro generation/expansion of Treg cells from naïve T cells was significantly reduced. shRNA-mediated silencing of DIAPH1 in Jurkat cells reduced DIAPH1 protein level and inhibited T cell proliferation and IL-2/STAT5 axis. Additionally, NK cells from patients had diminished cytotoxic activity, function and IL-2/STAT5 axis. Lastly, DIAPH1-deficient patients’ peripheral blood contained dramatically reduced numbers of all helper ILC subsets. DIAPH1 deficiency results in major functional defects in T, NK cells and helper ILCs underlining the critical role of formin DIAPH1 in the biology of those cell subsets. Yaygın anksiyete bozukluğunda çevrim içi yürütülen bilişsel ve davranışçı terapi: Bir olgu sunumu
Bu çalışma yaygın anksiyete bozukluğunda bilişsel ve davranışçı terapi müdahalelerinin çevrim içi ortamda uygulandığı ve bu müdahalelerin etkinliğinin bir vaka özelinde değerlendirildiği bir olgu sunumu çalışmasıdır. Bu çalışmada yaygın anksiyete bozukluğunda çevrim içi yürütülen bilişsel ve davranışçı terapi sürecinin aktarılarak bu terapinin etkinliğinin araştırılması ve literatüre katkı sağlanması amaçlanmaktadır. Uygulanan bilişsel ve davranışçı terapinin görüntülü görüşme uygulaması üzerinden çevrim içi yürütülmesi planlanmıştır. Uygulanan psikoterapi süreci; değerlendirme, psikoeğitim, bilişsel ve davranışçı müdahaleler ve yinelemeyi önleme seansları olmak üzere dört aşamadan oluşmaktadır. Danışanla haftada bir, 45-50 dakika süren, değerlendirme, psikoeğitim, bilişsel ve davranışsal müdahalelerden oluşan on iki seans gerçekleştirilmiştir. Ayrıca birer hafta arayla iki yinelemeyi önleme seansı ve birer aylık arayla iki izleme seansı gerçekleştirilmiştir. Danışanın belirtilerinin şiddeti yaygın anksiyete bozukluğu-7 ölçeği ile değerlendirilmiştir. Danışandan değerlendirme görüşmesinden ve ilk yinelemeyi önleme seansından sonra olmak üzere toplamda iki kere yaygın anksiyete bozukluğu-7 ölçeğini doldurması istenmiştir. Danışanın ilk ölçek puanı 17 iken son ölçek puanı 7 çıkmıştır. Süreç sonunda danışanın çevrim içi yürütülen bilişsel ve davranışçı terapiden fayda gördüğü sonucuna ulaşılmış ve sonuç literatür dikkate alınarak tartışılmıştır
