    The Geriatric Patient: Ethical Issues in Care and Treatment

    In any discussion about medical ethics, it is always fair to ask whether the ethical issues that arise in a particular setting, or regarding a particular patient population, are unique to that setting or population or whether the same ethical concerns mark the care of patients in other facilities or from other special groups. It would be an easy, if not rather boring exercise if we could simply transfer the moral problems and any proposed solutions to them from one setting, or one patient population, to another. Yet it would be surprising if no common ethical problemsexisted from one area of medical practice to another. After all, medical treatment and research, nursing services, administration of health care facilities. and other activities in the sphere of medical and health care all focus on those occupying the role of patient: ill, ailing, or injured people.With regard to geriatric patients, these questions need reply: Are there special ethical problems that arise in caring for elderly patients, problems that never or rarely occur in general medicine or with other special populations? Are there special ethical problems that arise in caring for such patients in an extended care facility, problems that neveror rarely exist in the context of ambulatory care or shorttermmedical facilities

    Ultrasound contrast agents are widely used in clinical practice for left ventricle opacification in sub-optimal echocardiograms. Recently, significant research has focused on the use of contrast echocardiography as a non invasive means to evaluate myocardial perfusion. Advances in contrast agents as well as ultrasound technology have enabled investigations into myocardial contrast echocardiography as a possible alternative to nuclear imaging studies. This review will focus on the development and current uses of contrast echocardiography, as well as future indications, including myocardial perfusion and risk stratification following myocardial infarction.

    Intersex Management in the United States and Non-Western Cultures

    In Western nations, there is growing agreement about ethical approaches to clinical intersex management. At the same time, as Western-trained physicians increasingly encounter intersex patients in other parts of the world, new ethical tensions arise. Which cultural values are fair parameters for gender-assignment decision-making, particularly in cultural milieus where there is social and economic inequality between the sexes? How can physicians uphold universal bioethical principles while remaining culturally sensitive? Physicians have a primary commit- ment to patient beneficence and universal human rights, requiring physicians to promote concordance between the child’s assigned gender and his or her likely future gender identity. Ultimately, the potential patient distress posed by gender dysphoria fundamentally outweighs the influence of local cultural factors such as economics, gender politics, and homophobia.

In this article, we will explore the answers to these questions, review the historical bases for the pedagogical and andragogical paradigms, and discuss learning theories and their relevancy to teaching and learning in medicine.

    Feline Models of the Gangliosidoses: Research Applications for Investigating the Pathogenesis of Lysosomal Storage Disease

    Studies in recent years have demonstrated that ganglioside storage disease is characterized not only by lysosomal hydrolase deficiencies and concomitant storage of gangliosides, but also by specific changes in the structure of neurons such as meganeurite formation and aberrant dendritogenesis and synaptogenesis. The discovery of these unusual morphological alterations, unprecedented for mature neurons, has suggested new hypotheses relating to the pathogenesis of storage disorders, as well as to the functions of gangliosides in the normal nervous system. The availability of colony-reared feline models of GM,and GM2 gangliosidosis, which are near perfect analogues of these disorders in children, offers a unique opportunity to test these and other hypotheses related to neuronal dysfunction in these diseases. This paperreviews some of the characteristics of these models and their contributions made to date and, additionally, surveys some of the significant unanswered questions for future studies which are expected to contribute to a more complete understanding of the pathobiologyof neuronal storage disorders
