A Replication Study-Postoperative Complications in Bariatric Surgery using Age and BMI Stratification: A Stusy using ACS-NSQIP data (2014)
This study aims to replicate the findings of Sanni et al.\u27s (2014) research on postoperative complications in bariatric surgery patients. The original research utilized age and BMI stratification and data from the American College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) to identify the relationship between these variables and postoperative complications. The findings of the original study revealed significant associations between age, BMI, and postoperative complications. The replication plan aims to confirm these associations and identify the factors that lead to postoperative complications in patients undergoing bariatric surgery. The replication study proposes a comprehensive plan consisting of three types of analyses: pure replication, measurement and estimation analysis (MEA), and theory of change analysis (TCA). These analyses are designed to validate and expand on the discoveries of the original study.
The pure replication phase is a critical step in the replication process, aiming to reproduce the original analyses accurately. This phase involves performing the statistical tests used in the original study to confirm the significant risk factors identified in the original study. Overall, the pure replication phase ensures the accuracy of the research findings.
Conducting MEA (Measurement and Estimation Analysis) is crucial for ensuring the accuracy and reliability of the findings. Employing alternative methods such as nonparametric tests will provide a more comprehensive analysis and help validate the results obtained in the pure replication phase. It is important to ensure that the conclusions drawn from the original study are robust and can be replicated using different statistical techniques. The concluding step, TCA (Theory of Change Analysis), will broaden the scope of the original study by examining the correlation between wound complications and diabetes status (YES/NO) among patients undergoing bariatric surgery. This analysis will employ Chi-Square tests and logistic regression 3 models to assess the association between diabetes and wound complications among patients undergoing bariatric surgery while accounting for variables including age and BMI.
Through replicating the original study\u27s findings and employing alternative analytical approaches, this research aims to enhance understanding of the determinants influencing postoperative complications among individuals undergoing bariatric surgery. Furthermore, by exploring novel inquiries concerning wound complications in patients with diabetes, this study aspires to inform the establishment of evidence driven protocols for bariatric surgery selection criteria, thereby fostering advancements in patient care and outcomes
Exploring the Dynamics of Attracting and Retaining Acute Care Psychiatric Registered Nurses: An In-Depth Analysis Using Focus Groups
The recruitment and retention of in-patient psychiatric mental health registered nurses (PMH-RNs) remains a challenge. This qualitative study sought to identify factors impacting the recruitment and retention of PMH-RNs in acute-care settings. Participants (N = 15) were recruited for focus groups including one with in-patient unit administrators (n = 4), two with current PMH-RNs (n = 7), and two with nursing students (n = 4). Data were analyzed using a directed content analysis approach. Participants were informed about the study’s purpose, procedures, potential risks, and benefits, and they provided verbal consent before participating. Administrators emphasized a focus on retention and described a variety of supports they provided PMH-RNs, including formal and informal support and education. PMH-RNs’ most prevalent concerns were their safety, co-worker and/or management challenges, and emotional and/or physical exhaustion. Students also expressed concerns about safety in psychiatric settings and desired more interaction with PMH-RNs. They were also interested in learning more about the specialty as they valued the opportunity to see change in patients. All three groups mentioned a need for more interaction between students and PMH-RNs, while safety concerns were expressed by both PMH-RNs and students. Because PMH-RNs play a critical role for in-patient psychiatric care, recruiting and retaining specialist nurses can focus on increasing student engagement with the PMH-RNs, attending to PMH-RNs’ management and staffing concerns, and providing additional resources for responding to events that threaten safety in the workplace to prevent burnout
A Building Block of Empathy: Establishing a Graphic Medicine Collection in an Academic Health Sciences Library.
To help address the well-being of the campus and contribute to empathy building amongst students pursuing careers as healthcare providers, an academic health sciences library built a graphic novel collection focused on comics that discuss medical conditions and health-related topics. The collection contains the experiences of patients, providers, and caregivers. The reader-friendly format of graphic novels provides an easy entry point for discussing empathy with health professions faculty and students. The collection has been used in the classroom during library instruction sessions, with the idea of integrating it within the curriculum
Targeted Insertion of Conditional Expression Cassettes into the Mouse Genome Using the Modified i-PITT
BACKGROUND: Transgenic (Tg) mice are widely used in biomedical research, and they are typically generated by injecting transgenic DNA cassettes into pronuclei of one-cell stage zygotes. Such animals often show unreliable expression of the transgenic DNA, one of the major reasons for which is random insertion of the transgenes. We previously developed a method called pronuclear injection-based targeted transgenesis (PITT), in which DNA constructs are directed to insert at pre-designated genomic loci. PITT was achieved by pre-installing so called landing pad sequences (such as heterotypic LoxP sites or attP sites) to create seed mice and then injecting Cre recombinase or PhiC31 integrase mRNAs along with a compatible donor plasmid into zygotes derived from the seed mice. PITT and its subsequent version, improved PITT (i-PITT), overcome disadvantages of conventional Tg mice such as lack of consistent and reliable expression of the cassettes among different Tg mouse lines, and the PITT approach is superior in terms of cost and labor. One of the limitations of PITT, particularly using Cre-mRNA, is that the approach cannot be used for insertion of conditional expression cassettes using Cre-LoxP site-specific recombination. This is because the LoxP sites in the donor plasmids intended for achieving conditional expression of the transgene will interfere with the PITT recombination reaction with LoxP sites in the landing pad.
RESULTS: To enable the i-PITT method to insert a conditional expression cassette, we modified the approach by simultaneously using PhiC31o and FLPo mRNAs. We demonstrate the strategy by creating a model containing a conditional expression cassette at the Rosa26 locus with an efficiency of 13.7%. We also demonstrate that inclusion of FLPo mRNA excludes the insertion of vector backbones in the founder mice.
CONCLUSIONS: Simultaneous use of PhiC31 and FLP in i-PITT approach allows insertion of donor plasmids containing Cre-loxP-based conditional expression cassettes
