    Advanced Nasopharyngeal Carcinoma with Intracranial Extension and Bilateral Cervical Lymphadenopathy: A Case Report

    Background: Nasopharyngeal carcinoma (NPC) is an epithelial malignancy that is often found in Southeast Asia and is ranked sixth as the most common cancer in Indonesia. This disease is often diagnosed at an advanced stage due to nonspecific initial symptoms. Case Description: A 72-year-old man presented with chronic epistaxis, bilateral nasal obstruction, tinnitus, and bilateral neck node enlargement. Nasoendoscopy revealed a mass obscuring the fossa of Rosenmüller, while a contrast-enhanced CT scan revealed an infiltrative lesion in the nasopharynx extending to the skull base and intracranially with multiple cervical lymphadenopathy. Histopathology revealed nasopharyngeal nonkeratinizing squamous cell carcinoma, stage T4N3Mx. Management and Outcome: The patient was referred for concomitant chemoradiotherapy as standard therapy for advanced stage, with planned follow-up at an oncology center. Discussion: This case highlights the association between smoking and salted fish consumption with the occurrence of NPC and the challenges of early diagnosis due to symptoms resembling chronic rhinitis. Conclusion: Recognizing recurrent epistaxis and painless neck node enlargement is crucial for early detection of NPC. A multidisciplinary approach and integrated chemoradiotherapy play a significant role in improving patient prognosis

    Synthesizing The Evidence: TP53 Mutations as a Prognostic and Predictive Biomarker in Breast Cancer

    The journey to personalize breast cancer therapy increasingly navigates the complex landscape of somatic TP53 mutations, the most frequent genetic drivers of this disease. Moving beyond a simple prognostic badge, we now understand these mutations create a spectrum of clinical behaviors, deeply influenced by their specific functional impact and, critically, by their genetic companions. A systematic literature review was conducted across three major electronic databases: ScienceDirect, ProQuest, and Scopus. The search was restricted to peer-reviewed journal articles published in English between 2021 and 2024 to ensure the inclusion of recent evidence. The confluence of TP53 and PIK3CA mutations stands out as a devastating synergy that heralds a tumor phenotype with profound resistance to common chemotherapy regimens, especially taxanes, and the direst survival outcomes. This genetic context also subtly reshapes responses to HER2-targeted drugs, revealing that TP53's influence is both powerful and context-dependent. A more nuanced profiling, one that accounts for these critical genetic interactions, is no longer a future aspiration but a present necessity. These findings collectively advocate for the integration of comprehensive molecular profiling, including nuanced TP53 and PIK3CA assessment, into clinical decision-making

    The Relationship Between Stress Levels and Disease Perception in DM-TB Comorbid Patients in Gorontalo City

    Diabetes mellitus (DM) and tuberculosis (TB) are chronic diseases that remain major public health problems and frequently occur concurrently as DM–TB comorbidity. This condition not only worsens patients’ physical status but also affects psychological aspects, particularly stress levels and illness perception. Patients living with two chronic diseases simultaneously tend to experience stress due to long-term treatment burdens, complications, and demands for therapeutic adherence. High stress levels may influence how patients perceive their illness, reduce motivation, and shape negative illness perceptions, which can ultimately hinder optimal disease management. This study aims to analyze the relationship between stress level and illness perception among patients with DM–TB comorbidity in Gorontalo City. This study employed a quantitative, cross-sectional design. The sample consisted of patients with DM–TB comorbidity undergoing treatment at several community health centers in Gorontalo City. Stress level was measured using the Perceived Stress Scale (PSS), while illness perception was assessed using the Brief Illness Perception Questionnaire (BIPQ). Data were analyzed using statistical tests to determine the relationship between stress level and illness perception. The results showed that most patients experienced moderate to high stress levels and tended to have negative illness perceptions. Statistical analysis indicated a significant relationship between stress level and illness perception among patients with DM–TB comorbidity. The higher the patients’ stress levels, the more negative their perception of the illness. It can be concluded that psychological aspects play an essential role in the management of patients with DM–TB comorbidity; therefore, health interventions should integrate clinical and psychosocial approaches to improve treatment adherence and patients' quality of life

    Expectative Management of a Late-Diagnosis Twin Reversed Arterial Perfusion Sequences (TRAPS) - A Rare Case Report

    Twin Reversed Arterial Perfusion Sequence (TRAPS) is a rare and severe complication unique to monochorionic twin pregnancies, characterized by retrograde perfusion of a nonviable acardiac twin by a structurally normal pump twin. Management of TRAPS diagnosed in late gestation remains controversial due to limited interventional options and an unpredictable clinical course. While early fetal intervention is generally recommended, evidence supporting expectant management in late-diagnosed cases, particularly in resource-limited settings, remains scarce. We report a 31-year-old gravida 2, para 1 woman with a monochorionic diamniotic twin pregnancy complicated by TRAPS, who was referred to a maternal–fetal medicine clinic at 29–30 weeks of gestation. Given stable pump twin hemodynamics, late gestational age, and unavailability of fetal therapy, expectant management with intensive ultrasound surveillance was pursued. At 32 weeks, signs of evolving cardiac compromise prompted delivery by cesarean section. The pump twin survived with prematurity-related complications and was discharged in stable condition. This case highlights that expectant management may be a reasonable option in carefully selected late-diagnosed TRAPS cases when fetal therapy is unavailable, provided that close surveillance and timely delivery are ensured

    The laying han farming program is one of the Village Fund–supported initiatives aimed at strengthening the food security and economic development sector in Srowot Village, Banyumas Regency. This program is managed by the Village-Owned Enterprise (BUMDes) as a strategic effort to increase village income and create sustainable local businesses. However, the implementation of the program faces challenges related to inadequate cost calculation and the absence of systematic cost accounting practices, which affect the accuracy of production cost determination and financial decision-making. This community service activity aims to provide mentoring assistance to BUMDes managers in calculating production costs for the laying hen program through the application of cost accounting principles. The implementation method includes problem identification, training on basic cost accounting concepts, hands-on assistance in classifying and calculating production costs, and evaluation of the implementation results. Previous studies indicate that the application of cost accounting and financial mentoring in small-scale enterprises and village-owned enterprises can improve cost efficiency, financial transparency, and business sustainability (Hansen et al., 2018; Mulyadi, 2016; Putra et al., 2021). The results of this activity are expected to enhance the managerial capacity of BUMDes in determining accurate production costs, supporting better financial management and strengthening the sustainability of village-based food sector programs. &nbsp

    The rapid development of digital technology has significantly transformed economic activities, particularly payment systems used by Micro, Small, and Medium Enterprises (MSMEs). The implementation of financial technology (fintech), especially the Quick Response Code Indonesian Standard (QRIS), offers advantages including transaction efficiency, improved financial recording, and increased financial inclusion. However, digital payment adoption among MSMEs in traditional markets remains relatively low due to limited digital literacy, strong dependence on cash transactions, and concerns regarding transaction security. These conditions are evident among MSMEs in Pasar Kanoman, Cirebon City, where most traders still rely on conventional payment methods despite access to basic digital devices. This community service program aims to enhance understanding, awareness, and readiness of MSMEs to adopt fintech through QRIS utilization as a digital payment system. The program was implemented through structured educational sessions, socialization activities, interviews, and direct mentoring tailored to the needs and characteristics of traditional market traders. A participatory approach encouraged active involvement and ensured knowledge could be practically applied in daily business activities. The results show improved understanding of QRIS functions, benefits, and operational mechanisms, along with more positive attitudes and increased interest in digital payment adoption. Overall, the program strengthens financial literacy and supports sustainable inclusion

    In the current digital era, the utilization of information technology has become an essential requirement for small and medium enterprises (SMEs) to maintain business sustainability and competitiveness. SMEs in Sumbang District, Banyumas Regency, still face financial management problems, particularly related to manual bookkeeping, limited accounting knowledge, and low adoption of digital accounting systems. This community service activity aims to develop SME businesses through digital accounting training to improve financial management quality and support better decision making. The methods used include needs analysis, socialization, training on simple digital accounting applications, mentoring, and evaluation. The results show an improvement in SMEs’ understanding and skills in recording transactions, preparing simple financial statements, and utilizing financial information for business development. This activity contributes positively to strengthening financial governance and digital readiness of SMEs

    Antibiotic and Symptomatic Drug Use Patterns in URI Patients at Pratama Soedirman Clinic

    Upper Respiratory Tract Infections (URTIs) are among the most common diseases encountered in primary healthcare facilities and are a major cause of patient visits. The high incidence of URTIs may increase the use of medications, particularly antibiotics and symptomatic drugs, which can lead to irrational use and antibiotic resistance if not properly managed. This study aimed to analyze the pattern of antibiotic and symptomatic drug use among URTI patients at Soedirman Primary Clinic from July 2024 to June 2025. This study was a descriptive observational study with a retrospective design. Data were collected from the medical records of URTI patients at Soedirman Primary Clinic. Total sampling was applied based on inclusion and exclusion criteria, yielding 2.863 cases. Data were analyzed descriptively to describe patient characteristics, URTI diagnosis distribution, and patterns of antibiotic and symptomatic drug use. The results show that most URTI patients are female (65.4%) and aged 12–25 years (85%). The most common diagnosis is unspecified URTI (J06), accounting for 51,8% of cases. Antibiotics are prescribed in 57,5% of cases, with cephalosporins (60%) being the most frequently used antibiotic class. Corticosteroids are the most commonly prescribed symptomatic drugs. However, when active ingredients in combination products are considered, antihistamines are the most frequently used symptomatic drug class. This study indicates that antibiotic use among URTI patients at Soedirman Primary Clinic remains relatively high, highlighting the need to strengthen the implementation of rational drug use in accordance with URTI treatment guidelines in primary healthcare settings
