Environmental life cycle assessment of surgical versus conservative care pathways for an anterior cruciate ligament injury
Environmental degradation is a major global health threat, and the healthcare sector functions as both its victim and perpetrator. To effectively inform mitigation strategies it is essential to quantify the environmental burden originating from healthcare activities and then favour environmentally preferable treatment options. This Switzerland-based study evaluated and compared the environmental impacts from surgical and conservative care pathways after an anterior cruciate ligament injury using Life Cycle Assessment; it also identified the environmental hotspots stemming from the surgery itself, as well as from physiotherapy. Environmental impacts were quantified across 18 ReCiPe midpoint impact categories. Results indicated that, on average, after an anterior cruciate ligament injury, the surgical pathway imposes at least an 8.5-fold greater relative environmental burden than the conservative pathway. Regarding the anterior cruciate ligament surgery alone, the hotspots of environmental impacts were identified as the manufacturing of single-use products, transportation of patient and staff, and the use of heating-ventilation-air-conditioning and electricity. Regarding physiotherapy, results indicated that a large share of environmental impacts can be attributed to transportation of patient and staff, laundry, and space heating. This difference in environmental impacts emphasises the importance of a careful patient allocation between these two care pathways. Recommendations to reduce the environmental impacts from surgery should prioritise investing in reusable equipment, streamlining surgical instrument trays, and opting for occupancy-based heating-ventilation-air-conditioning. To reduce the environmental impacts originating from physiotherapy, recommendations should encourage soft mobility, patient's self-management, and the physiotherapist's following of evidence-based guidelines. Übersetzung/Validierung/Implementierung AM-PAC "6-Clicks" Basic Mobility ::Mobilisation im Akutspital
Hintergrund: Die funktionelle Mobilität von Patient*innen im Akutspital ist ein entscheidender Faktor für Behandlungsergebnisse und Rehabilitation. Immobilität während Spitalaufenthalten kann zur Verschlechterung der Funktionsfähigkeit führen. Der Activity Measure for Post-Acute Care (AM-PAC) "6-Clicks" Basic Mobility Short Form stellt ein standardisiertes Messinstrument dar, das die Mobilitätsbeurteilung im klinischen Alltag erleichtert. Ziele: Das Forschungsprojekt zielt auf (1) die psychometrische Validierung der deutschen Version des AM-PAC "6-Clicks" Basic Mobility und (2) die Untersuchung der Interrater-Reliabilität zwischen Physiotherapeut:innen ab. Zudem präsentieren wir Erfahrungen aus der klinischen Implementierung im Rahmen der interprofessionellen Kommunikation und Mobilitätsförderung im Bürgerspital Solothurn. Methode: In einer Cross-sectional-Studie werden 50 Patientet*innen aus internistischen und chirurgischen Abteilungen durch jeweils zwei Physiotherapeut*innen unabhängig voneinander bewertet. Die Konstruktvalidität wird bei insgesamt 64 Patient*innen überprüft, indem Zusammenhänge mit Alter, Aufenthaltsdauer und Entlassungsort analysiert werden. Die Datenerhebung erfolgt während des regulären klinischen Betriebs. Klinische Anwendung: Das Instrument unterstützt bereits jetzt die tägliche Praxis in unserem Spital durch: (1) Standardisierte Erfassung der Mobilität bei physiotherapeutischer Erstkonsultation, (2) Unterstützung bei der Planung gezielter Therapiemaßnahmen, (3) Verbesserung der interprofessionellen Kommunikation durch gemeinsame Mobilitätssprache, (4) Ermöglichung individueller Mobilitätsziele und (5) Optimierung der Ressourcenallokation in der Physiotherapie. Bedeutung: Die Validierung des AM-PAC "6-Clicks" schaff t die wissenschaftliche Grundlage für dessen breite Anwendung im deutschsprachigen Raum. Als Teil eines systematischen Mobilitätsförderungsprogramms trägt das Instrument dazu bei, Immobilität im Spital entgegenzuwirken, funktionelle Unabhängigkeit zu fördern und patientenzentrierte Mobilisationsplanung zu unterstütze
Reliability and validity of smartphone measurement of spinal angular amplitude and velocity
Adapting nursing education to improve care for persons with disabilities ::a systematic review of curriculum changes and training effectiveness
Background : To ensure that nurses are equipped to provide effective care, it is essential to examine available evidence on how nursing curricula have adapted to meet the unique health needs of persons with disabilities. This study assesses how pedagogical strategies and interventions have changed in nursing education to develop disability competencies. It also examines whether such curriculum changes have been evaluated to assess their effectiveness in developing competencies. Methods : A systematic review based on a literature search in Web of Science, PubMed, Medline, PsycInfo, ERIC, CINAHL and Google Scholar. The study includes peer-reviewed papers, either qualitative or quantitative, that describe the development of disability-related education in undergraduate or postgraduate nursing training. With no time frame or publication restriction. The quality of the paper was assessed using the Mixed Methods Appraisal Tool (MMAT). 46 studies have met al.l inclusion criteria and were included in the final analysis. Results : In total, were 2803 articles found in the initial search across seven online electronic databases. Finally, a thorough review was conducted, incorporating 46 studies. Most of the studies come from high income countries. Among the 46 studies reviewed, 16 employed qualitative methodologies, 22 studies adopted quantitative descriptive designs, relying primarily on cross-sectional surveys and pre-post evaluations. Seven studies used mixed methods.Three major themes emerged from this analysis a) enhancing nurses’ understanding of disability, b) pedagogical strategies to work with persons with disabilities and their families in a clinical setting, and c) diverse approaches to clinical simulation in nursing education. The results show that most educational interventions are not assessing their impact in developing disability competencies. Discussion : Many interventions aim to raise disability awareness, challenge prejudice, and promote more inclusive attitudes toward individuals with disabilities. While these goals are important, there is limited evidence demonstrating their effectiveness in enhancing nursing practice competencies. Research on strategies that equip nursing students with the core skills needed to provide quality care to people with disabilities remains scarce
Prevalence and determinants of constipation in community-dwelling adult women in the Democratic Republic of Congo ::a cross-sectional study
Introduction and hypothesis: Constipation is one of the most frequent functional gastrointestinal dysfunctions, with a negative impact on women’s quality of life. There are currently no epidemiological data on the magnitude of constipation among adult women in the Democratic Republic of Congo (DRC). To determine the prevalence of constipation among adult women in the DRC and to identify the risk factors associated with it. Methods: This was a community-based cross-sectional study conducted from 2021–2023 among 516 adult women (≥18 years) in six provinces of the DRC. A multistage, geographically and ethnolinguistically stratified sampling approach was used. Pregnant or postpartum women ≤ 6 months, survivors of sexual violence and those with anorectal and rectovaginal fistulas were excluded. The diagnosis of constipation was made for a score of ≥ 11 on the KESS constipation questionnaire. Pelvic floor muscles were assessed according to the PERFECT scheme. Binary logistic regression was performed to identify risk factors. Results: The prevalence of constipation was 16.1% (95% CI 13.1–19.6%), with a higher prevalence observed among women over the age of 50. Hard stool consistency and difficult, painful evacuating were the most common suggestive symptoms encountered in constipated women (p < 0.001). Constipation negatively affected sexual quality of life (p < 0.001). In the multivariable analysis, lower mental health scores of life quality (adjusted odds ratio [aOR] 0.98; 95% CI 0.97–0.99) and belonging to the Baluba ethnolinguistic group (aOR 0.31; 95% CI 0.13–0.68) were protective factors against. Conclusion: Constipation is a non-negligible health concern among adult women in the DRC
Where coal is produced really matters the environmental impacts
China is the largest coal producer. The environmental impacts caused by its vast production deserve attention. Previous site-generic life cycle assessment (LCA) studies focused on single or several coal mines, ignoring spatial heterogeneity of environmental impacts and hardly providing industry-level suggestions. Here, we propose a technical framework for regionalized LCA and evaluate provincial-level environmental impacts per tonne of coal in China. It reveals significant variations in climate change and particulate matter formation potential (PMFP), with water scarcity potential (WSP) showing the highest spatial disparities, and also demonstrates the significance of this work. Key processes dominating environmental impacts and underlying factors driving these variations are identified. Furthermore, the total PMFP and WSP of provincial-level coal production since 2002 are estimated, showing a growing trend in spatial variation. This study provides valuable insights to enhance the environmental performance of China's coal industry and serves as a methodological reference for regionalized LCA research
NEGOTIATOR ::a comprehensive framework for human-agent negotiation integrating preferences, interaction, and emotion
The paper introduces a comprehensive human-agent negotiation framework designed to facilitate the development and evaluation of research studies on human-agent negotiation without building each component from scratch. Leveraging the interoperability and reusability of its components, this framework offers various functionalities, including speech-to-text conversion, emotion recognition, a repository of negotiation strategies, and an interaction manager capable of managing gestures designed for Nao, Pepper, and QT, and coordinating message exchanges in a turn-taking fashion. This framework aims to lower the entry barrier for researchers in human-agent negotiation by providing a versatile platform that supports a wide range of research directions, including affective computing, natural language processing, decision-making, and non-verbal communication
