Comparison of Fentanyl and Ketamine’s Effects on Post-General Anesthesia Recovery Scores Measured by QoR-40 and Changes in Blood Pressure and Pulse in Odontectomy SurgeryThe use of fentanyl in general anesthesia has side effects that may prolong postoperative recovery and hemodynamic decline when induction. Methods of general anesthesia without fentanyl may improve the quality of postoperative recovery. Ketamine in subanesthetic doses has analgesic effects with minimal side effects and more stable blood pressure and pulse changes. This study aimed to compare fentanyl and ketamine’s effects on quality of recovery and changes in blood pressure and pulse. This was a double-blinded clinical study in 30 patients with odontectomy under general anesthesia in Dr. Hasan Sadikin General Hospital from January–March 2020. Patients were divided into two groups, a ketamine group, induced using 0.5 mg/kgBW ketamine, and a fentanyl group, induced using 1,5 mcg/kgBW fentanyl. Data were analyzed using the unpaired t-test, Mann Whitney, and Kolmogorov-Smirnov test, a p-value of <0.05 was considered significant. Values of QoR-40 in the ketamine group had a significantly (p<0.05) higher value (181.07±5.32) compared to the fentanyl group (176,60±2,59). In conclusion, the quality of post-general anesthesia recovery using ketamine is higher than fentanyl in odontectomy evaluated using QoR-40. Induction using subanesthetic  doses of ketamine shows more stable hemodynamic than fentanyl.ABSTRAK Penggunaan fentanyl pada anestesi umum memiliki efek samping yang dapat memperpanjang pemulihan pascaoperasi. Background: In Indonesia about 18% of children, mostly in the first two years of life, are malnourished, causing a serious impact. Many factors are thought to affect nutritional status among young children. This study was conducted to determine factors affecting the nutritional status of children aged 12–23 months in West Java, Indonesia. Methods: This cross-sectional study used secondary data obtained from a Basic Health Research (Riset Kesehatan Dasar, RISKESDAS) 2010, conducted by Badan Penelitian dan Pengembangan Kesehatan (BALITBANGKES) Ministry of Health Republic of Indonesia. All of the data among children aged 12–23 months in West Java province was obtained. Statistical relationships between predisposing factors and nutritional status i.e. weight for height were analyzed using the chi-square test. Results: In total, 730 data were obtained, of which 567 data met the inclusion criteria. Malnutrition was detected in 101 (17.8%) children. There was a significant relationship (p=0.017, OR=2.6, 95% CI 1.1–5.8) between low birth weight and nutritional status. Nevertheless, no significant relationship was found between nutritional status and history of exclusive breastfeeding (p=0.629), complimentary feeding (p=0.949), vitamin A (p=0.209), infectious diseases (p=0.266), complete immunization status (p=0.420), and mother education level (p=0.251).Conclusions: The low birth weight is the only significant factor associated with the nutritional status among children less than 2 years old; resulting in that low birth weight had a 2.6 higher chance of malnutrition in the early years. Other factors are unexpectedly not significant in this study. There is thus a need to improve the quality of programs, focusing on childbearing mothers, during antenatal control

rimary and secondary osteoarthritis relates to inflammatory processes and inflammatory mediators and is destructive to the articular cartilage. Omega-3 is known to be an alternative treatment for rheumatoid arthritis due to the anti-inflammatory effect. This study is an experimental study with simple random sampling using 36 Wistar mice, which were divided into an intervention group and a control group, to understand the effect of omega-3 in slowing progress cartilage destruction in knee joint with osteoarthritis. This study is performed at the Clinical Pharmacology Laboratory, Faculty of Medicine, Universitas Padjadjaran, and the Anatomical Pathology Laboratory, Faculty of Medicine, Universitas Padjadjaran/Dr. Hasan Sadikin General Hospital, in November 2018. Osteoarthritis was induced using intraarticular 1 mg monosodium iodoacetate injection to the mouse knee joint. The intervention group received oral omega-3 every day while the control group did not. Samples from the knee joint were extracted to evaluate the cartilage destruction histopathologically. Results were then analyzed using the Mann-Whitney test and a significant difference of the osteoarthritis grades was identified between the intervention group and the control group on day 7 (p=0.003), day 14 (p=0.003), and day 21 (p=0.003). In addition, a significant difference in the osteoarthritis grading changes was also found between the study group and the control group on day 7 and day 21 (p=0.004). Hence, omega-3 has the ability to slow down the histopathological cartilage destruction progress in mice with knee joint osteoarthritis.

In patients with type 2 diabetes mellitus (Type 2 DM), glycemic control plays an essential role in reducing macro- and microvascular complications. The earliest marker for diabetic nephropathy is the presence of albuminuria. Meanwhile, HbA1c  has been recognized as a marker of glycemic control in the blood. This study aimed to identify the correlation between HbA1c and albuminuria in type 2 DM. This was a cross-sectional study involving 100 patients with type 2 DM aged 40–70 years visiting Dr. Soetomo Hospital, Surabaya, Indonesia. Secondary data from medical records of type 2 DM patients undergoing HbA1c and albuminuria examinations from January to December 2015 were used. The mean age of subjects was 55.4 ±8.7 years, with the majority (55%) being of female gender. The mean HbA1c concentration was 9.8±2.4%. Most subjects (83%) had an HbA1c level of ≥7 %. Albuminuria was found in the majority of the subjects (78%), where 33% of them had positive  1 albuminuria. The Spearman correlation test showed a positive strong significant correlation between HbA1c and albuminuria, with r=0.865 and p<0.001. Thus, it can be concluded that there is a significant correlation between HbA1c and albuminuria in type 2 DM.Korelasi HbA1c dan Albuminuria pada Diabetes Melitus Tipe 2 Pada penderita diabetes melitus tipe 2 (DM tipe 2), kontrol glikemik memegang peranan penting untuk menurunkan komplikasi makrovaskular dan mikrovaskular.

Relationship of Lipid Profile (Levels of Triglyceride, HDL, LDL, Total Cholesterol) with Types of Bacterial Infection in Sepsis PatientsImpaired lipid profiles are associated with an increased incidence of organ failure and mortality in septic patients. Disorders of the lipid profile may differ between bacterial infection types (gram-positive vs gram-negative). Differences could be due to dissimilarities in different endotoxin clearance and host immunological mechanisms related to lipid metabolism. The study analyzed differences in lipid profiles (HDL, LDL, triglycerides, total cholesterol) with the bacterial infection type in septic patients in the intensive care room of Dr. Soetomo Hospital in April–July 2020. This study was an observational analytic study of 38 adult patients who met the criteria for sepsis-3. Bacterial infection types were classified into gram-negative bacterial infections, gram-positive bacterial infections, mixed infections and culture negative. This study analyzed differences in lipid profiles between bacterial infection types, and used data normality analysis and data variant analysis. Results showed that 13 patients had gram-negative bacterial infection, 13 patients were with gram-positive infection, five patients were with mixed gram-positive gram-negative infections, and seven patients had no germ growth. Plasma HDL levels were lower in gram-negative bacterial infections and mixed gram-positive gram-negative bacterial infections (p<0.05). Plasma triglyceride levels were higher in gram-negative bacterial infections and mixed gram-positive gram-negative bacterial infections (p<0.05). There was no significant difference in LDL levels and total plasma cholesterol between bacterial infections types. In conclusion, plasma HDL levels are significantly lower, and triglyceride levels were significantly higher in septic patients with gram-negative and mixed gram-positive bacterial infections than in sepsis patients with gram-positive bacterial infections and culture negative

Background: Juvenile nasopharyngeal angiofibroma (JNA) is a fibrovascular tumor originating from the supero-posterior area of the sphenopalatine foramen. This tumor is histologically benign, but is clinically malignant because JNA has potentially destructive spread extending to the skull base and the cranium. The aim of the study was to evaluate the management of JNA in Dr. Hasan Sadikin General Hospital Bandung, a referral hospital in West Java. Methods: This was a descriptive retrospective study using medical record data of JNA patients during the period of 2011-2018. The stage of JNA was designated, based on the FISCH classification. Results: There were112 JNA cases, consisting of 99% male and 1% female. Most of them (97%) were aged 10-20 years old (median16 years old). The chief complaints were nasal obstruction (46%) and recurrent epistaxis (36%). Most of the JNA stage was stage II (43%). Major feeding vessel of JNA was from an internal maxillary artery (IMAX) and ascending pharyngeal artery (28%). Preoperative management was equal for ligation and embolization. The complication of embolization was local pain (75%). The most surgical technique performed was the transpalatal approach (41.36%). Blood loss in a different stage in JNA surgery with embolization was less than surgery with ligation. There were patients (10%)with residual disease after 6 months follow-up.Conclusions: JNA is essentially a disease of the adolescent males. The management of JNA patients is surgery, mostly using transpalatal technique. Preoperative embolization of JNA is a safe procedure, minimizes intraoperative blood loss, decrease the incidence of residual or recurrence, and shortening the duration of the surgery.
