Proportion of Traumatic Avascular Necrosis on Hip Joint at Dr. Hasan Sadikin General Hospital Bandung in 2016–2020
Background: Traumatic factors affecting the femur such as fractures and dislocations can cause complications in the form of avascular necrosis (AVN), if not treated immediately, AVN is one of the contributors to total hip arthroplasty (THA) usage and disability which will have an impact on productivity and quality of life. Hence, it is necessary to understand the epidemiology of AVN caused by traumatic factors. This study aimed to determine the proportion of traumatic AVN on the hip joint at Dr. Hasan Sadikin General Hospital. Methods: A Descriptive-observational study with a cross-sectional study design was conducted on all patients registered in the medical record database, with a diagnosis of AVN on the hip joint at Dr. Hasan Sadikin General Hospital Bandung in 2016–2020. Traumatic AVN was included and patients with incomplete medical record data were excluded.Results: In total, 210 patients were registered in the medical records and only 56 had complete medical records, and 10.5% (22 of 210) were diagnosed with traumatic AVN of the hip joint, consisting of 14 male with most of the trauma (n=11) caused by AVN were femoral neck fractures and 20 patients were of productive age (18–64 years old). Most of the patients underwent surgical therapy with a THA (n=20).Conclusion: The proportion of AVN on the hip joint caused by trauma is 10.5%. However, this disease should not be underestimated because AVN has the potential to cause disability and impact the quality of life
Radiological Findings of Pulmonary Tuberculosis in Children at Dr. Hasan Sadikin General Hospital Bandung 2018–2019
Background: Tuberculosis (TB) remains a major cause of death worldwide, especially in children. Indonesia is ranked second as the largest contributor to TB. Chest X-rays have been used to diagnose pulmonary TB in children, however, the results are varied. Therefore, this study was performed to explore the radiological findings among children with pulmonary TB. Methods: Retrospective descriptive study was conducted by analyzing the chest X-ray findings retrieved from the medical records of all pulmonary TB children treated at Dr. Hasan Sadikin General Hospital during 2018 to 2019. Inclusion criteria were medical records of patients aged 1 month to 18 years diagnosed with pulmonary TB who underwent chest X-ray (n=223).Results: The highest incidence of pulmonary TB was found in the group of 10–18 years (43.9%) and female (50.2%). The results of chest X-ray were infiltrates (65.0%), lymphadenopathy (11.7%), cavity (7.6%), calcifications (7.6%), miliary (4.9%), pleural effusion (3.1%) and atelectasis (1.8%). Conclusion: Infiltrates are the major finding on Chest X-ray in children with TB.
Relationship between Narcissism, Self-Esteem, and Social Media Addiction in Preclinical Medical Students
Background: Narcissism is a condition in which people have a constant demand to be complimented and prioritized. Self-esteem is a subjective evaluation of one's worth. Several studies have linked narcissism and low self-esteem as a risk factor of social media addiction, a disorder where individuals displayed an unhealthy compulsion to access social media until it disrupts other activities. This study aimed to discover the relationship between narcissism, self-esteem, and social media addiction in preclinical medical students. Methods: This study was a cross-sectional observational analytical study on 211 preclinical medical students at the Atma Jaya Catholic University of Indonesia. Instruments used in this study were Narcissistic Personality Inventory-16 (NPI-16), Rosenberg Self-Esteem Scale (RSES), and Bergen Social Media Addiction Scale (BSMAS). Data were analyzed statistically using Chi-square in STATA analytic program.Results: Of all respondents, 6.6% were narcissistic, 19.9% had low self-esteem, and 34.6% had social media addiction. There was a significant relationship between narcissism and social media addiction (p=0.021) and between self-esteem and social media addiction (p=0.002).Conclusion: There is a relationship between narcissism, self-esteem, and social media addiction in university students. This indicates the importance of preliminary psychological screening in students, especially regarding self-esteem, narcissism, and social media addiction, to develop approaches that will ensure good academic achievements and physical health among students
Effect of Health Education Video on Knowledge about Stunting among Women in Childbearing Age
Background: Toddlers aged 24-59 months are experiencing a very rapid growth process; however, they are also prone to have nutritional problems, including stunting. Stunting occurs mainly within the first 2 to 5 years of life. The main cause is a lack of knowledge on toddler’s nutritional intake among women of childbearing age. This study aimed to explore the effect of health education using video media on the knowledge about stunting among women of childbearing age.Methods: This was a quantitative, experimental study with one group pretest-posttest design conducted in July-November 2019 on women of childbearing age in the working area of Public Health Center (Pusat Kesehatan Masyarakat, Puskesmas) Jatinangor, West Java, Indonesia. Respondents were recruited using the Multistage Random Sampling according to the inclusion criteria. A pre-test was administered before the health education session on stunting and a post-test was administered afterwards. Data collected were then analyzed using the Wilcoxon Test.Results: In total, 211 women were included. Education video on stunting was proved to significantly increase the respondent's knowledge on stunting (p=0.000, r value=0.690).Conclusion: Health education video clearly increases knowledge on stunting among women of childbearing age. Thus, video medium can be used as a preferred method for disseminating knowledge on various health topics
Characteristics of Neonates with Hyperbilirubinemia in Cileunyi, West Java
Background: Neonates are prone to hyperbilirubinemia or jaundice due to the immaturity of various organ systems and complications at birth. Immature organ systems occur primarily in preterm babies and babies with low birth weight (LBW). The incidence of hyperbilirubinemia is about 60% among term neonates and is higher in preterm neonates. This study aimed to determine the characteristics of neonates with hyperbilirubinemia.Methods: This study used a cross sectional research design. Samples were obtained from medical records of neonates with hyperbilirubinemia throughout 2019 at Annisa Medical Center Hospital, Bandung Regency. Data on neonates that met the inclusion criteria were selected, including gender, gestational age, birth weight, mode of delivery, bilirubin levels (before and after phototherapy), and duration of treatment. Data analysis was performed using univariate statistical calculations.Results: From 786 data obtained, the prevalence rate of hyperbilirubinemia was 21.2%. Male neonates with hyperbilirubinemia were predominantly prevalent (n=103; 61.7%); whereas preterm neonates accounted for 51.5% (n=86), normal weight (85.6%), and born through vaginal delivery (n=116; 69.5%). After treatment, total bilirubin levels decreased, with the percentage of the >10 mg/dL group decreasing from 97.0% to 10.2%. The average duration of treatment for neonates with hyperbilirubinemia was 4.4 days, however, the neonates who were treated less than the average were 71.3%.Conclusion: Preterm male is predominantly prevalent among neonates with hyperbilirubinemia, although they have conformed birth weight and vaginal delivery mode. The duration of treatment for neonates with hyperbilirubinemia mostly does not exceed 4 days, as long as the treatment reduces the total bilirubin level. Good therapeutic management in the first week of hyperbilirubinemia neonates is required
IP-10 as a Non Sputum Biomarker in TB Treatment Monitoring
There are currently still limitations in the diagnosis of tuberculosis (TB). Sputum collection as specimen for diagnosis is not only difficult but also has low sensitivity.In blood, IP-10/CXCL-10 chemokine plays a role in inducing the movements of chemotactic inflammatory cells towards the sites of inflammation. A high level of IP-10 is found in active pulmonary TB patients and significantly decline after the patients have completed the TB treatments. The aim of this study was to analyze the decline of the IP-10 level before and after 2 months of TB treatment. This study was conducted from March toJuni 2020. This was a comparative observational cohort study on active pulmonary TB patients who were >18 years old at the DOTS Clinic of Dr. Hasan Sadikin General Hospital Bandung. Thirty patients who met the inclusion criteria were followed up until 2 months of TB treatment. Serum of these patients were collected and examined for the IP-10 level before and after 2 months of TB treatment. It was demonstrated that the median IP-10 level in new active pulmonary TB patients was 384.1 pg/mL (136.70–779.80) and dropped to 251.85 pg/mL (91.10–698.30) (p<0l001) two months of TB treatment. Thus, the IP-10 level in the active pulmonary TB patients is significantly declined (p<0.001) after 2 months of TB treatment and that serum IP-10 level could be considered as a non-sputum-based marker to monitor TB treatment
Correlation between Serum Uric Acid Level and Severity of Coronary Artery Stenosis in Patients with Acute Coronary Syndrome
Acute coronary syndrome (ACS) is a life-threatening disease which remains one of the causes of high morbidity and mortality despite the current advances in treatment. The relationship between the serum uric acid (SUA) level and ischemic heart disease continues to be controversial and still has not been established as a cardiovascular risk factor. The cooperative interaction between the two factors has not yet fully understood. Prior epidemiological evidence of the causal relationship between the too is still argumentative. Various studies have been done using the same methods; yet, the outcomes were different. This study aimeds to conduct a meta-analysis to synthesize the results of recent studies in order to obtain accurate quantitative data. This systematic study followed the preferred reporting items for systematic reviews and meta-analysis (PRISMA) guideline and studies published in the period of January 2010 to May 2020 were screened using the Cochrane Library, Ebsco, Medline/PubMed, ProQuest and Science Direct as the sources. Meta-analysis was conducted to synthesize the association between the SUA level and severity of coronary artery stenosis using random effect model to account for possible study heterogeneity. Heterogeneity was assessed using I2 and the meta-analysis was performed using Comprehensive Meta Analysis Version 3 (CMA3) software. Five studies (n = 601 subjects) identified a correlation between serum uric acid level and Gensini score (r = 0.548; p <0.001) in ACS patients. Heterogeneity bias was found in the analysis, whereas publication bias was not found. Thus, the severity of coronary artery stenosis in patients with ACS is positively correlated with serum uric acid level
Retrospective Analysis on Intraoperative Neuromonitoring (IOM) of Potential Nerve Injury in Scoliosis Correction Surgery
Iatrogenic spinal injury resulting in paraplegia or paraparesis after surgical correction of scoliosis deformity is a rare complication but is very detrimental to the patient. Intraoperative Neuromonitoring (IOM) has become the gold standard to monitor surgical procedures which has potential risks to damage the spinal cord. This study aimed to retrospectively analyze the role of IOM in predicting the severity and extent of neurological injury during and after spinal correction surgery in adult idiopathic scoliosis cases related to surgical variables. This was a retrospective cohort study conducted at Dr. Cipto Mangunkusumo National Central Hospital, Fatmawati Central Hospital, and dr. Drajat Prawiranegara General Hospital during the period of 20 March 2018 to 20 August 2019. The primary outcomes were intraoperative monitoring status and post-operative neurological deficits status. Confounder data on scoliosis correction degree, intraoperative hemorrhage, and type of anesthesia used during surgery were retrieved. Chi-Square statistic was used in the analysis. Out of the ninety three patients eligible for this study, twenty two patients was detected as positive in IOM assessment. Four of the patients were found to be positive for post-operative neuromuscular defect. Thereby it can be concluded that IOM procedure can effectively prevent neurological deficits post-surgery with 81.8% specificity and 95.7% sensitivity among thosepositively detected by IOM. Some of the factors that could potentially influence false positive IOM results such as anesthetic used; dosage and administration procedures; magnitude of the scoliosis correction angle; and amount of bleeding during surgery have to be carefully analyzed
Association between Malnutrition Inflammation Score and Latent Tuberculosis among Chronic Hemodialysis Patients
Objective: To investigate the association of malnutrition with latent tuberculosis (TB) among chronic kidney disease (CKD) patients on hemodialysis (HD).Methods: This was a cross-sectional study conducted at the Hemodiaylisis Unit of Dr. Hasan Sadikin General Hospital, Bandung, Indonesia. Subjects were patients aged >18 years who had undergone HD twice a week for at least three months. Patients suspected of active tuberculosis (TB), malignancy, or immunocompromised were excluded. Latent TB was diagnosed using the interferon-gamma release assays (IGRA). Malnutrition was defined by a malnutrition inflammation score (MIS) of less than 5. All data including age, sex, CKD etiologies, and laboratory findings were obtained and recorded in a case report form.Results: A total of 120 subjects were involved in this study. Subjects with positive, negative, and indeterminate IGRA results were 39.2%, 56.7%, and 4.2%, respectively. There was no significant differences in subjects characteristics between positive and negative IGRA subjects. The MIS>5 was shown to have no statistically significant association with positive IGRA subjects (OR=3.47, 95%CI 0.93–12.93).Conclusion: Malnutrition based on an MIS score of less than 5 is not statically associated, but clinically associated, with latent TB. Further causal inference study to investigate these associations is needed
White Turmeric Rhizome Protection Effect of against Lung Tissue Damage in Copper Sulfate-Induced Wistar Rats
One of common heavy metals that pollute rivers in Indonesia is copper, which can damage various organs, including the lungs. As a potential natural herbal medicine, white turmeric rhizome has antioxidant properties that potentially protect the body from copper pollution. This study aimed to explore the potential of the white turmeric rhizome (Curcuma zedoaria) to provide a protective effect on the lungs against copper contamination. This study was an experimental study performed in June 2020 on 25 copper-induced Wistar rats which were divided into five groups: negative group that received 1 ml of copper sulfate suspension at the 12th to 14th day; positive group that received 10 mg/200 g BW of ethanol extract from white turmeric without copper sulfate suspension; and three experimental groups that received 10 mg/200 gBW, 20 mg/200 gBW, and 40 mg/200 gBW of ethanol extract from white turmeric every day, respectively, followed by copper sulfate suspension at the 12th–14th day. After 14 days, the rats were sacrificed by chloroform inhalation and the lung was excised and processed for histopathology preparation. The edema, hemorrhage, leukocyte infiltration, and alveolar septal thickness were evaluated from the lung tissue perparation. The score of tissue damage was express as median (Range) and analyzed using the Kruskal-Wallis test. The result of this study shows that there was a significant difference in lung tissue score among all groups of treatment (p-Value=0.001). The experimental group with highest dosage extract presented a good protection effect as well as the positive group. Hence, white turmeric has a good protective property for the lung against damages caused by copper contamination
