Changes of Serum TNF-α in Acute Pancreatitis Patients and Its Clinical Significance
Objective: The aimed of this study is to investigate the changes in serum levels of tumor necrosis factor (TNF-α) in the patients with acute pancreatitis treated with octreotide and its clinical significance. Method: Total of 65 patients of acute pancreatitis were selected as a case study, in which 30 patients with mild acute pancreatitis (MAP) and 35 severe acute pancreatitis (SAP) patients were treated with octreotide. 60 healthy subjects as control group and 65 case group was subjected to performed double antibody sandwich enzyme-linked immunosorbent assay (ELISA) to detect the serum levels of TNF-α. Results: The serum TNF-α level in the case group was (12.67 ± 3.45) pg/mL and the control group was (1.56 ± 0.57) pg/mL. Case group was significantly higher than control group (p < 0.05). Serum level of acute pancreatitis (AP) before treatment was (8.96 ± 2.12) pg/mL. After treatment, SAP group was (17.34 + 4.56) pg/mL, MAP group was significantly lower than SAP group, and the difference was statistically significant (p < 0.05). Conclusion: The serum levels of TNF-α in patient with acute pancreatitis were significantly higher than those of normal healthy people, and their serum level was closely related to the severity of illness
Clinical Observation of Coronary Artery Bypass Grafting with Heart Valve Replacement for Coronary Heart Disease with Heart Valvular Disease
Objective: To analyze and explore the clinical observation of coronary artery disease with coronary heart disease and coronary heart disease (CHD) undergoing coronary artery bypass grafting. Methods: From December 2015 to December 2016, 80 patients with coronary heart disease and heart valve were randomly divided into two groups: observation group and control group (40 cases). The observation group was treated with coronary artery bypass grafting combined with simultaneous heart valve replacement. The control group was treated with coronary stent implantation and non-synchronous valve replacement. The improvement of postoperative cardiac function, postoperative hospitalization, adverse events and bridge patency were compared between the two groups. Results: The improvement of cardiac function and the incidence of adverse events were significantly better in the observation group than in the control group (P <0.05). The time of hospitalization and the time of ventilator were significantly shorter than those of the control group (P <0.05). The patency rate of the bridge was significantly higher than that of the control group (P <0.05). Conclusion: Coronary artery bypass grafting combined with heart valve replacement in patients with coronary heart disease complicated with valvular heart disease can achieve significant curative effect, which is more ideal and safer than coronary artery bypass surgery combined with non-synchronous valve replacement
The Clinical Characteristics and Emergent Treatments of Patients with Anaphylactic Shock
Objective: To analyze the clinical characteristics of patients with anaphylactic shock, and discuss the appropriate emergent treatments. Methods: 18 cases of anaphylactic shock patients received emergent treatments in our hospital from October 2009 to October 2011 were randomly selected. Retrospective analysis was made on the basic information such as clinical characteristics, emergency treatments, etc. Results: Through retrospective analysis, it was found out that the drug-induced anaphylactic shock accounted for 88.89%, biotoxins-induced anaphylactic shock accounted for 5.55%, and blood transfusion-induced anaphylactic shock accounted for 5.55%. The main clinical characteristics of anaphylactic shock are: the illness occurred suddenly, developed fast, hard to predict, without indication, and can cause death, etc. The recovery rate reached 94.44% after emergent treatments, but there was only 1 case of anaphylactic shock patients who died because of not getting timely treatment. Conclusion: Understanding the clinical characteristics of anaphylactic shock, timely and effective treatment measures are the keys of successful treatment
Clinical Study on the Aerosol Inhalation of Budesonide in Treatment of Acute Exacerbation of COPD
Objective: To observe the clinical effect and adverse reaction of aerosol inhalation of budesonide in treatment of acute exacerbation of COPD. 110 cases of patients with acute exacerbation of chronic obstructive pulmonary disease, who were treated in our department from January 2009 to January 2011, were selected as research objects (69 males, 41 females). All the patients were divided into control group and observation group. The two groups were treated routinely, in addition, the control group was treated with vein administration of methylprednisolone, the observation group received aerosol inhalation of budesonide, observed the clinical effect. Result: The clinical effect of observation group is better and the adverse reaction rate is clearly lower. Conclusion: Comparing with the control group which is treated with vein administration of methylprednisolone on the basis of routine treatment, the effect of observation group which is treated with aerosol inhalation of budesonide is extremely precise and its adverse reaction is also relatively less. Meanwhile, it has a higher security and its clinical effect is worth applying

Treatment of Toe-to-toe Defect with Arterial Skin Flap
Objective: To study the repairing effect of arterial skin flap on the toe defect. Methods: Six patients with hallux defect were treated in our hospital from February 2014 to February 2016. The skin flap of the thumb was used to repair the defect. Results: After the operation, the skin flap had symptom of venous crisis. One patient survived after the dressing and partial removal of the pedicle of the flap. The other 5 cases survived and the wounds were healed. The skin grafts in the donor site survived and the incision reached grade I healing. The average follow-up period was 10 months. The flap elasticity and texture were good. Patients’ foot felt with burden, with normal walking, without obvious discomfort and without ulceration. Conclusion: The toe-to-toe defect is repaired with the arterial skin flap. The skin flap does not cause damage to the blood vessels, and the clinical result is ideal
Long-term efficacy of concurrent chemoradiation therapy on patients with nasopharyngeal carcinoma
The objective of the study is to analyze and evaluate the long-term efficacy of concurrent chemoradiation therapy and the impact of other related factors to the prognosis of patients with nasopharyngeal carcinoma. A total of 897 cases of patients with nasopharyngeal carcinoma who were treated in our hospital in the period from December 2007 to February 2010 were divided into two groups according to the different treatment methods. The first group consists of 769 cases of patients treated with radical radiation therapy alone (radiotherapy group), and the second group consists of 128 patients treated with a combination of radical radiotherapy and chemotherapy (chemoradiotherapy group). While both groups of patients were subjected to the same modes of radiotherapy, an additional 2-3 cycles of concurrent chemotherapy were conducted for the second group of patients (using DDP + 5-Fu + BLM or DDP + 5-Fu). The overall 5-year survival rate of all patients was 51.23%, the 5-year survival rate among patients in the radiotherapy group was 48.46% and the 5-year survival rate of the chemoradiotherapy group was 56.80%. These results were found to be statistically significant (P <0.05). A multivariate stepwise regression analysis indicated that different N staging of nasopharyngeal carcinoma had a very significant effect (P <0.0001) for the patients’ survival. In addition, the different T staging also significantly affected (P <0.05) the patient's survival. The type of treatment (radiotherapy or chemoradiotherapy) administered can also have a significant impact on the prognosis, whereby the chemoradiotherapy group patients’ survival rate was significantly higher than the radiotherapy group (P <0.001). The implementation of concurrent chemoradiotherapy show long-term beneficial effects for naso-pharyngeal carcinoma patients. Diagnosing ovarian masses by using nuclear magnetic resonance
In the research, nuclear magnetic resonance (NMR) was used in order to study and diagnose ovarian tumour patients. A total of 80 cases of ovarian tumour patients who admitted to hospital in February 2014 to May 2016 were selected and randomly divided into 2 groups, with 40 cases in each group, namely; the experimental- and contrast group. The experimental group used NMR to diagnose whereas the contrast group utilized B-mode ultrasound in order to compare the character of masses and ascertain accuracy, sensitivity, and specificity of the clinical diagnostic afterwards. The accuracy, sensitivity, specificity, and characteristic of tumour masses in the experimental group was 95.84%, 94.75%, 90.92%, and 100%, respectively, which is apparently higher than the contrast group with 64.28%, 77.78%, 75.08%, and 70.83%, respectively. Both groups were found to be statistically significant different (p < 0.05). In comparison to NMR and ultrasound, former diagnostic method was better in terms of accuracy, sensitivity, and specificity which lead to significant detection and better diagnostic
