    Making Foreign Aid Work: A Case Study of China-UK-Tanzania Pilot Project on Malaria Control

    Foreign aid is a topic with a long history. However, after so many years of practice, the effect of foreign aid is still controversial. This paper takes the China UK Tanzania malaria control pilot project as an example to understand the conditions under which foreign aid is effective by analyzing the reasons for the success of the project. This paper finds that there are several reasons that make foreign aid effective, including adjusting measures to local conditions, obtaining local people's support and participation, and having multilateral partners participate in projects

    Surgical Treatment of Hypertensive Cerebral Hemorrhage

    Intracerbral hemorrhage (ICH) refers to primary non-traumatic hemorrhage in the parenchyma, also known as spontaneous cerebral hemorrhage, accounting for 20%-30% of acute cerebrovascular diseases. Every year 2 million to 3 million people in the world suffer from intracerebral hemorrhage, accounting for 10%-15% of all new strokes, and the total incidence of intracerebral hemorrhage in the world is 24.6/ (100,000 · year). In China, the incidence of cerebral hemorrhage is 60-80/ (100,000 · year), which is much higher than that of other Asian countries. Primary intracerebral hemorrhage accounts for 80%-85%, and primary hypertensive intracerebral hemorrhage combined with hypertension accounts for 50%-70% [1]. Hypertensive intracerebral hemorrhage accounts for about 50% of the causes of non-traumatic intracerebral hemorrhage, and its mortality rate takes the first place in non-traumatic intracerebral hemorrhage [2]. Hypertensive intracerebral hemorrhage has rapid onset, high mortality rate and slow recovery, and often leaves various degrees of neurological dysfunction. This paper analyzes the pathological features of hypertensive intracerebral hemorrhage, the clinical features of the bleeding site, the indications, timing, surgical methods and the advantages and disadvantages of surgical treatment, hoping to provide reference for the selection of surgery for clinicians

    The Relationships of the Type of Needle and the Flushing of the Needle with the Satisfactory Rate of Thyroid Nodule Fine Needle Biopsy

    Objective: To explore the factors that affect the satisfactory rate of fine needle aspiration biospy(FBAB)sampling of thyroid nodules and the effects of different needle types and needle flushing methods on the satisfactory rate of FNAB sampling. Methods: The clinical data of 190 patients with thyroid nodules (200 nodules in total) who underwent FNAB from June 2020 to December 2020 were analyzed. The relationship of the satisfaction rate of sampling with the type of puncture needle and the flushing method of the puncture needle was analyzed. Results: In 200 thyroid nodules, we found that the nondiagnostic rate (37%) with a larger needle (22G) was lower than that with a smaller needle (25G) (55%), and the difference was statistically significant (P < 0.05). In the analysis of the comparison of the satisfaction rate of the flushing of the puncture needle, it was found that the nondiagnostic rate of the flushing method was lower (24%) than that of the nonflushing method (68%), with a statistically significant difference (< 0.05). In the randomized test according to the size of the puncture needle and the flushing method, the nondiagnostic rates of the four groups (25G needle with flushing group, 25G needle without flushing group, 22G needle with flushing group and 22G needle without flushing group) were 34%, 76%, 14% and 60%, respectively. Moreover, there was no significant change in the diagnosis rate of benign and malignant lesions in the obtained samples (p=0.912). Conclusion: In fine-needle biopsy of thyroid nodules, a larger needle should be used in combination with the flushing method to reduce the nondiagnostic rate and improve the pathological diagnosis rate

    Effectiveness of Ultrasound-Guided Minimally Invasive Spinotomy in the Treatment of Multiple Small Benign Breast Nodules

    Objective: To observe the efficacy of different surgical protocols in the management of multiple benign breast nodules. Methods: Patients diagnosed with multiple benign breast nodules from 2021.4 to 2023.2 were divided into two groups, blank and study groups, each with 51 cases, and treated with conventional open and ultrasound-guided minimally invasive rotational surgery respectively. Results: The study group had better incision diameter and VAS score at 24h postoperatively than the blank group, with significant differences (P < 0.05). Regarding the postoperative complication rate index, the study group vs. the blank group was 3.92% vs. 21.57% (P < 0.05). Conclusion: Minimally invasive spinotomy under ultrasound guidance for the treatment of patients with multiple breast nodules is safe and effective and has the potential to be used universally

    Malignant hyperthermia (MH) is generally observed in people susceptible to volatile anesthetics, usually due to abnormalities in ryanodine receptor 1 (RYR1) leading to Ca2+ dysregulation in the sarcoplasmic reticulum (SR). CASQ1, a Ca2+ binding protein believed to be in the SR, also plays a regulatory role in RYR1. It has been hypothesized that mutations in CASQ1 protein may increase susceptibility to MH cases. We used the CASQ1 p.F186Y and p.I138T variants to compare Ca2+ binding ability with CASQ1 WT. It showed that CASQ1 F and I variants have reduced Ca2+ binding ability compared to CASQ1 WT

    Pituitary tumor is a common intracranial neuroendocrine tumor, generally belonging to benign tumors. It mainly originates from pituitary cells in the sella region. The tumor grows to the lower part of the parasellar thalamus, and may even reach the third ventricle to involve the cavernous sinus, extend into the middle cranial fossa, grow into the interpeduncular cistern, and enter the nasopharynx in the sphenoid sinus. A few tumors are rich in blood supply and easy to bleed, leading to pituitary tumor stroke. The incidence of pituitary tumor ranks the third among intracranial tumors.     Effect of Mechanical Ventilation Combined with Budesonide Suspension on Pulmonary Function Indices in the Treatment of Severe Asthma

    Objective: To observe the effectiveness of different methods in the treatment of severe asthma. Methods: Ninety patients admitted from 2020.2 to 2022.8 were divided into groups A and B, 45 patients each, and all were given basic symptomatic treatment, group A was given nebulized inhalation of budesonide suspension, group B was mechanically ventilated on the basis of group A's protocol, and the treatment was compared between groups. Results: After 3 weeks of continuous treatment, the lung function (FEV1, PEF, FEV1/FVC) levels of patients in group B were higher than those in group A, and the difference reached a significant level (P < 0.05). The incidence of adverse reactions in groups A and B was 8.89% and 6.67% respectively, which was not statistically significant (P > 0.05). Conclusion: In patients with severe asthma, early mechanical ventilation with inhalation of budesonide suspension can improve lung function more rapidly and with higher safety, and is worth promoting

    Analysis of the use of minimally invasive rotary mastectomy in the treatment of multiple small nodules in the breast

    Objective: To observe the effectiveness of different surgical excisional set-ups for the treatment of multiple small breast nodules disease. Methods: 60 patients who came to our department from 2020.10 to 2023.1 were divided into two groups A and B, 30 patients each, and were treated with conventional and minimally invasive mastectomy respectively, and the treatment status was compared between the groups. Results: Compared to group A, group B had better surgical indicators and 93.3% higher post-operative satisfaction than group A with 73.3%, forming a significant difference (p < 0.05). Conclusion: Minimally invasive rotary mastectomy is a minimally invasive technique for the treatment of breast nodule disease ear, which has the characteristics of short incision, less intraoperative bleeding and short operation time, which can increase the subjective satisfaction of patients and is worth promoting

    Clinical Effect of Yunkang Granule Combined with Low Dose Aspirin in the Treatment of Unexplained Recurrent Abortion and Its Influence on Pregnancy Outcome

    Purpose: To study the clinical effect of Yunkang Granule combined with low dose aspirin in the treatment of unexplained recurrent abortion (URSA) and its impact on pregnancy outcome. Methods: Seventy-two patients with URSA were randomly divided into a control group and an experimental group. The control group was given conventional treatment, and the experimental group was given pregnancy-kang granules combined with low-dose aspirin. The clinical efficacy of the two treatments and their effects on hormone levels, coagulation function indexes and pregnancy outcome were compared. Results: After medication, the levels of β-HCG, P and E2 in 2 groups were higher than before medication, and the levels of β-HCG, P and E2 in observation group were higher than control group, the difference was statistically significant (P < 0.05). Compared with before medication, the levels of coagulation function indexes PT, TT and APTT were increased in both groups after medication, while the levels of FIB were decreased. The levels of coagulation function indexes PT, TT, APTT and FIB in observation group were better than those in control group after medication, and the difference was statistically significant (P < 0.05). After treatment, the total effective rate of the experimental group was significantly increased, and the pregnancy outcome was significantly improved. Conclusion: The combination of Yunkang granule and low-dose aspirin in the treatment of URSA has obvious curative effect, can significantly improve the pregnancy outcome, and is worthy of clinical promotion
