    Impacts of spilled debris from the X-Press Pearl disaster in Sri Lanka on marine plankton

    Accidents of ships carrying diverse hazardous chemicals and plastics can lead to complex spills of pollutants in marine ecosystems. One such incident was the catastrophic fire on the container ship X-Press Pearl which sank off the Sri Lankan coast in 2021. Explosions and fire resulted in plastic pellets and burnt clumps of melted plastic and combustion residues washing ashore. In this study, we analyzed the acute toxicity of the leached chemicals from this debris on various planktonic organisms: phytoplankton (Rhodomonas salina), meroplankton (Paracentrotus lividus larvae) and holoplankton (Acartia tonsa nauplii and adults). Acute exposures were conducted with a range of leachate dilutions for 72 h. The growth of R. salina was slightly affected by the leachates. Larvae of P. lividus showed a concentration-dependent reduction in growth and normal development (EC50 0.56 g L−1), with 94% of larvae showing malformations in the highest concentration. The hatching of A. tonsa decreased from 89% in the control to 29% at 0.75 g L−1. Nauplii mortality reached 46% and there was a trend of decreased growth. Mortality of the adults increased with concentration, reaching 51% in the highest concentration. Our results show that the complex mixture of spilled chemicals and debris from the X-Press Pearl accident can potentially harm the planktonic food web, particularly zooplankton. These findings highlight the urgent need for effective mitigation strategies and response measures to reduce impacts of accidental spills in sensitive and ecologically relevant areas, especially those located in major shipping lanes, such as the Sri Lankan coastal waters.121,6797,7Q1Q1SCI

    Comparing JAK inhibitors in ulcerative colitis: which one truly leads in the real world? Results from the ENEIDA registry of GETECCU

    Background: The addition of selective Janus kinase inhibitors (JAKi) –tofacitinib (TOFA), upadacitinib (UPA), and filgotinib (FILGO)– to the therapeutic arsenal for ulcerative colitis (UC) has provoked the need for comparative studies to establish their positioning in clinical practice. Aim: To compare the durability and the short- and long-term effectiveness of TOFA, UPA, and FILGO in UC, and to evaluate their real-world safety. Methods: Adult patients who started JAKi treatment for UC (from April 2021) at least 8 weeks prior to data collection and were part of the prospectively maintained ENEIDA registry of GETECCU were included. Patients receiving concomitant biologic agents or with a history of colectomy were excluded. Patients were followed from the first JAKi dose until treatment discontinuation or their last visit. Effectiveness analysis was restricted to patients with active disease [Partial Mayo Score (PMS)>2] at JAKi initiation. Clinical effectiveness was measured using PMS, and concomitant steroid use was assessed at each visit. Patients who discontinued JAKi before their last visit were considered not in remission at subsequent time points (negative imputation). The propensity score was calculated using the inverse probability weighting (IPTW) method to balance confounding factors. Results: A total of 611 patients were included (74 on FILGO, 369 on TOFA, and 168 on UPA). Baseline characteristics are summarised in Table 1. Discontinuation rates per patient-year were 57% for FILGO, 40% for TOFA, and 27% for UPA, with UPA showing significantly lower cumulative discontinuation rates compared to FILGO and TOFA (Fig. 1a). Reasons for discontinuation are detailed in Fig. 1b. Adjusted analysis indicated that baseline disease activity and prior JAKi exposure were associated with a higher discontinuation risk. Both TOFA and FILGO had significantly higher discontinuation risks than UPA. Steroid-free clinical remission (SFCR) rates during follow-up are shown in Fig. 2, with UPA demonstrating higher SFCR rates at most time points. At week 8, disease activity, prior JAKi exposure, and JAKi type were significantly associated with SFCR, with UPA having a higher probability of SFCR than both TOFA and FILGO (Table 2). The IPTW method confirmed all these results. Adverse events occurred in 12 (16%) patients with FILGO, 85 (23%) with TOFA, and 45 (27%) with UPA (p=0.2), with a minority leading to discontinuation (Fig. 1b). Conclusion: In real-world practice, UPA shows superior effectiveness and drug survival compared to TOFA and FILGO in UC patients, with a comparable safety profile. These findings support the positioning of JAKi treatments in clinical practice and may guide treatment decisions in UC management.13511349

    Actualización y adecuación de la batería estándar española de dermatitis alérgica de contacto

    Programa de Doctorado en Investigación en Biomedicina por la Universidad de Las Palmas de Gran CanariaLa Dermatitis Alérgica de Contacto (DAC) es una enfermedad cutánea frecuente con una importante repercusión poblacional en los ámbitos de la salud, laboral, social y económico. Para su diagnóstico es imprescindible la realización de pruebas epicutáneas con las sustancias sospechosas, o alérgenos. La importancia de determinados alérgenos hace conveniente la existencia de una batería estándar, de carácter nacional, la cual debe aplicarse a todos los pacientes estudiados, además de otras baterías específicas y los productos propios del paciente, según el contexto epidemiológico y la sospecha clínica. Actualmente, en España se parchea la batería estándar española (BEEs) de 2022. El grupo encargado de la actualización periódica de la BEEs es el Grupo Español de Investigación en Dermatitis de contacto y Alergia Cutánea (GEIDAC). Para ello, existe un registro prospectivo online que permite llevar a cabo la vigilancia epidemiológica de las sustancias involucradas, el Registro Español de Investigación en Dermatitis de Contacto y Alergia Cutánea (REIDAC). El objetivo principal de esta tesis doctoral es establecer recomendaciones sobre las que llevar a cabo la actualización de la BEEs, para lo cual se han desarrollado tres trabajos que estudian varios alérgenos de interés durante diversos tramos del periodo 2018-2023. El primer trabajo analizó la sensibilización a las isotiazolinonas, detectando una frecuencia alta a la metilisotiazolinona (MI) (6,8%), metilcloroisotiazolinona/metilisotiazolinona (MCI/MI) (4,8%) y benzisotiazolinona (BIT) (3,5%), y baja a la octilisotiazolinona (OIT) (0,5%). La relevancia presente para las dos primeras fue alta (MI: 77%; MCI/MI: 68%), y moderada para las dos últimas (BIT: 39%; OIT: 39%). Ello indica que la MI y MCI/MI deben continuar en la BEEs, mientras que la BIT, por su menor relevancia, y la OIT, por su baja frecuencia de sensibilización, menor relevancia y que aún es objeto de estudio en el ámbito europeo, deben formar parte de la batería española ampliada. El segundo trabajo investigó la positividad a la mezcla de colorantes textiles (MCT) y, de forma selectiva, a los colorantes de la batería textil. La frecuencia de positividad a la MCT fue alta (3,3%), con una moderada relevancia presente (34%), y el colorante individual más frecuente fue el Disperse Orange 3 (16%). La TDM fue capaz de detectar al 83% de los positivos a cualquier alérgeno de la batería, lo cual refleja que es un buen marcador de la alergia textil en España. El tercer trabajo estudió la sensibilización a la fragancia hidroxiisohexil 3-ciclohexeno carboxaldehído (HICC), o Lyral®, y a la mezcla de fragancias II (MF II). La frecuencia de sensibilización al HICC fue baja (0,8%), mientras que a la MF II fue alta (3,3%). La relevancia presente fue alta para ambos (HICC: 53%; MF II: 64%). La MF II fue capaz de detectar a tres de cada cuatro pacientes sensibilizados al HICC. Estos hallazgos indican que el HICC debe permanecer fuera de la BEEs. Los trabajos de esta tesis doctoral han sustentado la actualización de 2022 de la BEEs y aportado información relevante que apoyará futuras actualizaciones de la misma. Han demostrado la factibilidad de organizar la información de la DAC en un registro online, el REIDAC, y su importancia como herramienta de vigilancia epidemiológica de la DAC. Ratifican la elevada frecuencia de sensibilización a las isotiazolinonas que persiste en España pese a las medidas regulatorias europeas, indican que la MCT es un buen marcador de la alergia textil en nuestro país y corroboran la disminución de la sensibilización al HICC, señalándolo actualmente como marcador de fragancias poco rentable que no debe formar parte de la BEEs.Allergic Contact Dermatitis (ACD) is a common skin disease which significantly impacts the health, work, social and economic population spheres. Diagnosis of ACD requires epicutaneous patch testing with the suspicious substances, called allergens. The importance of certain allergens makes it convenient to organise them in national baseline series, which must be applied to all patients studied, in addition to other specific series and the patient's own products, according to the epidemiological context and clinical suspicion. Currently, the 2022 version of the Spanish baseline series (SBS) is being patched in Spain. The group in charge of periodically updating the SBS is the Spanish Contact Dermatitis and Skin Allergy Research Group (GEIDAC). The update is based on a prospective online registry that allows carrying out the epidemiological surveillance of the substances involved in ACD, the so-called Spanish Registry of Research in Contact Dermatitis and Cutaneous Allergy (REIDAC). The main objective of this doctoral thesis is to establish recommendations on the update of the SBS. To this end, three works have been developed that study various interesting allergens during different sections of the period 2018-2023. The first work analysed the sensitization to isothiazolinones, detecting a high frequency to methylisothiazolinone (MI) (6.8%), methylchloroisothiazolinone/methylisothiazolinone (MCI/MI) (4.8%) and benzisothiazolinone (BIT) (3.5 %), and low to octylisothiazolinone (OIT) (0.5%). The current relevance for the first two was high (MI: 77%; MCI/MI: 68%), whereas moderate for the last two (BIT: 39%; ILO: 39%). This suggests that the MI and MCI/MI should continue in the SBS, while the BIT, due to its lower relevance, and the OIT, due to its low sensitization frequency, lower relevance and which is yet being studied in Europe, should be part of the extended Spanish series. The second work investigated the positivity to the textile dye mix (TDM) and, selectively, to the textile dye series. The frequency of positivity to the MCT was high (3.3%), while current relevance was moderate (34%). The most common individual dye was the Disperse Orange 3 (16%). The TDM was able to detect 83% of all the positives to any allergen of the textile dye series, highlighting it is a good marker of textile allergy in Spain. The third work studied the sensitization to the fragrance hydroxyisohexyl 3-cyclohexene carboxaldehyde (HICC), or Lyral®, and to the fragrance mix II (FM II). The frequency of sensitization to HICC was low (0.8%), while high to FM II (3.3%). Current relevance was high for both (HICC: 53%; FM II: 64%). FM II was able to detect three out of four patients sensitized to HICC. These findings support the exclusion of the HICC from the SBS. This doctoral thesis has supported the 2022 update of the SBS and provided relevant information that will assist future updates thereof. It has demonstrated the feasibility of organizing ACD data in an online registry, the REIDAC, and its importance as a tool for epidemiological surveillance. It confirms the high frequency of sensitization to isothiazolinones that persists in Spain despite the European regulatory measures, indicates that the TDM is a good marker of textile allergy in our country and corroborates the decrease in sensitization to HICC, showing it is not worth keeping it within the SBS
