Comparison of Transforming Growth Factor-Beta 1 Concentration in Preeclampsia and Normal Pregnancy Women
BACKGROUND: According to the theory of endothelial dysfunction, the pathogenesis of preeclampsia is associated with the imbalance of angiogenic and anti-angiogenic factors. Transforming growth factor-beta 1 (TGF-β1) has also proposed as a proangiogenic factor that influences preeclampsia. This study was conducted to compare a mean difference of TGF-β1 between preeclampsia and normal pregnancy.METHODS: This study was an observational crosssectional study with 25 subjects of pregnant women with preeclampsia and 25 subjects of normotensive pregnant women. The study was conducted in Dr. Reksodiwiryo Hospital, Bhayangkara Hospital, and Dr. Rasidin Hospital in Padang, Indonesia from October 2015 to January 2016. For the determination of TGF-β1 concentration, peripheral Abstract venous blood samples were taken. The blood samples were analyzed by enzyme-linked immunosorbent assay (ELISA) in Biomedical Laboratory, Faculty of Medicine, Andalas University. The mean difference was statically analyzed by independent samples T-test.RESULTS: The mean difference of TGF-β1 was lower in preeclampsia group than normal pregnancy group (2.02±0.99 ng/mL vs. 3.24±2.67 ng/mL; p<0.05).CONCLUSION: The TGF-β1 concentration was lower in pregnant women with preeclampsia. The Association between Cardiovascular Risk and Elevated Triglycerides
BACKGROUND: The association between elevated triglycerides and cardiovascular risk has been extensively studied. The elevated level of triglycerides occurs through abnormalities in hepatic very low-density lipoprotein (VLDL) production and intestinal chylomicron synthesis, dysfunctional lipoprotein lipase (LPL)-mediated lipolysis or impaired remnant clearance.CONTENT: Hypertriglyceridemia (HTG) commonly leads to a reduction in high-density lipoprotein (HDL) and increase in atherogenic small dense low-density lipoprotein (LDL) cholesterol, called the atherogenic dyslipidemia (AD). Triglycerides may also stimulate atherogenesis by mechanisms, such excessive release of free fatty acids, and production of pro-inflammatory cytokines, fibrinogen, coagulation factors and impairment of fibrinolysis. Genetic studies strongly support hypertriglyceridemia (HTG) and high concentration of triglyceride-rich lipoprotein (TRL) as causal risk factors for cardiovascular disease. Therefore, lipid management is crucial in reducing cardiovascular risk. Combination of lipid lowering drug therapy may be needed to achieve both LDL and non-HDL cholesterols treatment goal for cardiovascular disease prevention in patients with elevated triglyceride levels, particularly those with triglyceride ≥500 mg/dL.SUMMARY: LDL and non-HDL cholesterol can be a promising target therapy in HTG. The Correlation Between TP53 Expression and Ki-67 Proliferation with Bartl Malignancy Degree of Plasma Cell Neoplasm
BACKGROUND: Plasma cell neoplasm (PCN) is a neoplastic plasma cell proliferation which includes solitary bone plasmacytoma (SBP), extramedullary plasmacytoma (EMP) and multiple myeloma (MM). Bartl classifies the degrees of PCN as low, intermediate and high. The aim of this study is to find the correlation between tumor suppressor gene p53 (TP53) expression and Ki-67 proliferation with Bartl malignancy degree of PCN. Therefore earlier PCN diagnostic method to prevent the development of PCN into MM can be found.METHODS: Thirty-two PCN cases were classified into three groups based on Bartl’s degrees of malignancy. TP53 and Ki-67 immunohistochemical staining were performed on samples and the percentage of positivity was evaluated.RESULTS: The Bartl’s low degree of malignancy was found in 10 MM cases (31.2%), intermediate degree in 5 SBP cases (15.6%) and high in 2 SBP and EMP cases (6.2%). TP53 expression was obtainable at 4% of low, 16% of intermediate and 10% of high degree. There was a significant difference between TP53 expression in low and intermediate degree (p=0.004). Mean proliferation index of Ki-67 was 57% in low, 44.6% in intermediate, and 32.6% in high degree. There was no significant difference of Ki-67 proliferation indexes among the group (p=0.339).CONCLUSION: Increasing expression TP53 was in accord with Bartl’s degrees of malignancy, especially in low and intermediate degree, but there was no significant difference between Ki-67 proliferation index and Bartl’s degrees of malignancy.KEYWORDS: plasmacytoma, myeloma, TP53, Ki-67, Bartl classificatio
Correlation Abnormal Ankle-Brachial Index and Multivessel Coronary Artery Disease in Acute Coronary Syndrome Patients
BACKGROUND: Many studies showed the association between peripheral artery disease (PAD) and coronary artery disease (CAD). The anklebrachial index (ABI) was a simple, noninvasive, and not expensive test that showed high sensitivity and specificity in the diagnosis of PAD. Previous studies showed PAD correlation with the number of coronary artery lesions and higher complexity of the lesions. These correlations might contribute to worse cardiovascular outcomes, especially acute coronary syndrome (ACS) cases. Aim of this study is to evaluate the correlation abnormal ABI with the risk of ACS patients to have multivessel coronary artery lesions.METHODS: This was a retrospective analytical casec ontrol study. The data were taken from Dr. Wahidin Sudirohusodo Hospital medical records from November 2015 to February 2016. The inclusion criteria were (>18 years old) patients with ACS who had underwent ABI examination and coronary angiography.RESULTS: The prevalence of patients with ST-elevation myocardial infarction (STEMI) was lower than non-ST elevation ACS (NSTE-ACS). The prevalence of patients with abnormal ABI was 43.3% and patients with multivessel CAD was 65%. Male patients dominated the ACS population with 73.3%, about half of patients were smoking, and 86.7% had dyslipidemia. Patients with abnormal ABI and multivessel disease had greater number than patients with normal ABI (p=0.025). Analysis with binary logistic regression model showed abnormal ABI (odd ratio [OR] 4.83; p=0.021) and male sex (OR 19.35; p=0.010) were associated with greater risk of multivessel CAD.CONCLUSION: An abnormal ABI is associated with greater risk of multivessel CAD in ACS patients.KEYWORDS: ankle-brachial index, multivessel, coronary, acute coronary syndrom
Association of Fat Mass and Obesity-associated Gene (FTO) rs9939609 Variant with Early Onset Obesity among Bataknese and Chinese Children in Indonesia: A Case-control Study
BACKGROUND: Childhood obesity is associated with the risk of adult obesity and obesity-related chronic disease. The fat mass and obesity-associated gene (FTO) rs9939609 variant are of particular interest because of its association with body mass index (BMI) and obesity-related phenotypes. This study was conducted to investigate the association between FTO gene rs9939609 variant with early onset obesity among Bataknese and Chinese children in Medan, Sumatera Utara, Indonesia.METHODS: The case-control study was carried out at 10 elementary schools in Medan. There were 212 children recruited; 56 early onset obesity and 61 control Bataknese and 49 early onset obesity and 46 control Chinese children. This study included a questionnaire, anthropometric measurements, and blood test analysis. rs9939609 polymorphism genotyping was performed using RT-PCR. Logistic regression, odds ratio (OR) and 95% confidence interval (CI) were calculated to determine the risk of obesity associated with the risk alleles, p<0.05 was considered as statistically significant.RESULTS: This study found a significant association between rs9939609 and early onset obesity in Chinese children (p=0.01), but not in Bataknese. The frequency of AA genotype was lower in the early onset obesity than in the normal weight children, while an OR of 0.69 showed that this genotype may protect against weight gaining and that the TT genotype may predispose to obesity.CONCLUSION: We concluded that the FTO gene rs9939609 is associated with early onset obesity in Chinese ethnicity but not in Bataknese.KEYWORDS: pediatric obesity, FTO gene, rs9939609, polymorphism, Indonesi
Cytotoxicity of Alpinia galanga Rhizome Crude Extract on NIH-3T3 Cells
BACKGROUND: Alpinia galanga (A. galanga) was reported as a potential medicinal source due to its wide effect. A. galanga rhizome crude extract (ARCE) was reported to have high cytotoxic effect in cancer cells, but low in normal cells. However half maximal inhibitory concentration (IC50) of ARCE is not clearly known yet. Hence, current study was conducted to investigate the IC50 of ARCE in normal standard fibroblast cell line, NIH-3T3 cells.METHODS: Rhizomes of A. galanga were collected, peeled, dried, milled and weighed. Extraction was performed using maceration method, then filtered and evaporated. ARCE with various concentrations were applied in NIH-3T3 cells for 24 or 48 hours. Cells were documented and counted with 3-(4,5-dimethylthiazol-2-yl)-2,5-Diphenyltetrazolium bromide (MTT) assay.RESULTS: Five hundreds grams of simplicia were macerated with ethanol and evaporated, 1 mg/mL crude extract with total volume of 114 mL was obtained. By addition of ARCE in NIH-3T3 cell culture, number of NIH-3T3 cells were shown less when treated with higher concentration of ARCE. Cell numbers of 0, 3.125, 6.25, 12.5, 25 and 50% ARCE treatment for 24 hours are 11,531, 11,352, 10,920, 10,365, 9,471, 8,360, respectively, meanwhile for 48 hours are 13,219, 12,686, 12,278, 11,390, 10,279, 8,390, respectively.CONCLUSION: IC50 of ARCE in 24 hours treatment was 620.5 mg/mL, while in 48 hours treatment was 666.6 mg/mL. Mesenchymal Stem Cells Manage Endogenous Tissue Regeneration
BACKGROUND: Current findings set a new understanding that every adult tissue has its own intrinsic progenitor or stem cell, give a potency for their innate turnover dynamics. This broke the old assumption that adult tissues cannot regenerate themselves. Localized tissue regeneration was regulatory oversight by a separate class of local cells originating as perivascular cells, suggested a profound influence on using specific cells for cell therapies as a health care delivery tool set.CONTENT: The mesenchymal stem cell (MSC) could be mobilized from the marrow or other depots or can be culture-expanded MSCs which are delivered to the damage site either by direct or systemic injection. MSCs act paracrine and autocrine by inducing a variety of cytokines and growth factors which suppress local immune system, inhibit fibrosis (scar formation) and apoptosis, enhanceangiogenesis, and stimulate mitosis and differentiation of tissue, intrinsic reparative or stem cells. These referred a trophic effects, different from the direct differentiation of MSCs into repair tissue. Thus, MSC suggested as a multidrug delivery vehicles in response of injury. In this regard, the trophic effects of MSCs may have profound clinical use.SUMMARY: Managing the body’s natural repair and regeneration capacities is the new frontier for modern medicine and the basis for the science of cell therapies. Measurement of Motor Evoked Potential in Acute Ischemic Stroke: Based on Latency, Amplitude, Central Motoric Conduction Time and Resting Motor Threshold
BACKGROUND: After stroke, there are dynamic changes of motor evoked potential (MEP), including latency, amplitude, central motoric conduction time (CMCT) and resting motor threshold (RMT) in cerebral. However, latency, CMCT, amplitude and RMT have not been clearly shown in acute ischemic stroke patients with motoric function impairment based on Modified Motoric Research Council Scale (MRCs).METHODS: Patients with motoric function impairment after acute ischemic stroke were recruited, scored based on MRCs and grouped. Latency, amplitude, CMCT and RMT (% intensity) was measured using transcranial magnetic stimulation (TMS). Latency, amplitude, CMCT and RMT of subjects based on affected hemisphere (AH) and unaffected hemisphere (UH); stroke onset; and motoric severity; were analyzed and compared statistically.RESULTS: Thirty-seven subjects with complete assessments were selected. Results of MEP size measurement between AH and UH showed that latency, amplitude, CMCT and RMT of AH and UH were significantly different (p<0.05). In accordance to AH and UH results, latency, amplitude, CMCT and RMT of mild, moderate and severe groups based on motoric severity, showed that latency and CMCT were prolonged, RMT was increased, while amplitude was decreased along with severity increment. The amplitude and RMT among the groups were significantly different with p=0.034 and p=0.029, respectively.CONCLUSION: MEP size measurement including latency, amplitude, CMCT and RMT have significant different in AH and UH. In addition, amplitude and RMT were significantly different in MRCs groups, therefore the MEP size measurement could be suggested as prognostic tool.KEYWORDS: MEP, latency, amplitude, CMCT, RM
Lung Function Status of Workers Exposed to Welding Fume: A Preliminary Study
BACKGROUND: Exposure to welding fume in the workplace was associated with lung function disorders and occupational asthma. In this study, we determined lung function parameters in men workers exposed to welding fumes from heavy equipment manufacturer. This study is a preliminary study of biomonitoring program in worker exposed to welding fume as our main study. METHODS: A study with case-control design, random study, was conducted among welder (59 subjects) and non-welder (34 subjects) with more than one year experience in the same job task in a heavy equipment manufacturer. All subjects completed physical examination, informed consent, questionnaire and lung function status. Lung function status was measured by spirometer with vital capacity (VC), forced vital capacity (FCV), forced expiratory volume in one second (FEV1) and ratio of FEV1/FVC as test parameters. Linear regression model was developed to identify the risk factor of lung function parameter status using age, working period and smoking status as variables. RESULTS: This study showed that there were significant lower VC, FVC and FEV1 in welder than non-welder, but not difference in ratio of FEV1/FVC. However, there was no significant difference among welder from foundry and fabrication plan. By multivariate analysis, working period was found as a risk factor for lower parameters in lung function among welder. CONCLUSION: Lung function parameters status were significantly lower in welder than non-welder, and working period was the most important indicator for lung function status evaluation among welder. The Release of Total Metal Ion and Genotoxicity of Stainless Steel Brackets: Experimental Study Using Micronucleus Assay
BACKGROUND: Stainless steel brackets are composed of various metal that may corrode in oral cavity. Corrosion is caused by the release of metal ions such as chromium, nickel, and iron. The release of metal ions can cause adverse effects such as toxicity, allergic, and mutagenicity. To evaluate the biocompatibility of stainless steel brackets, micronucleus assay as one of genotoxicity assay is used in this study. To determine the differences and the correlation of metal ions release and genotoxic activity among three brand stainless steel brackets.METHODS: Three brands of brackets were immersed in artificial saliva for 672 hours and the release of ion chromium, nikel and iron were examined. The cytokynesis block micronucleus assay (CBMN) using lymphocytes was performed as well.RESULTS: The highest metal releasing were nickel, cromium, iron, respectively (30.5, 27.2, 23.4 ppb). There was a significant differences between total nickel and iron ion release among three brand brackets (p=0.04, p=0.02). Genotoxicity of metal ion released was correlated with durration of immersion brackets (p=0.01). Genotoxicity was significant correlated with the release of chromium (p=0.03) and nickel (p=0.01).CONCLUSION: Genotoxicity of stainless steel brackets was influenced by duration of immersion but not influenced by brand brackets.
