    Microbiological and Biochemical Contamination Analysis of Refilled Drinking-water in Abeli, Kendari, Southeast Sulawesi

    BACKGROUND: The access to safe drinkingwater is increasingly difficult, especially in water catchment or coastal areas. Due to the difficulty, people in coastal areas tend to consume ready-to-use refilled drinking-water. However, the sanitation facilities on most drinking-water refill depots do not meet the requirements, hence really prone to microbial contamination. We conducted this study to determine the types of bacteria causing the contamination of refilled drinking-water from refill depots in Abeli, a coastal area in Kendari.METHODS: Samples were collected from all 6 drinkingwater refill depots in Abeli. Three-series fermentation tubes were used for the microbiological test, including the presumptive, confirmative and completed tests. The biochemical test was performed with indole test, methyl red test, Voges-Proskauer test, and citrate utilization test (IMViC) methods and the triple sugar iron agar (TSIA) test.RESULTS: Among 6 refilled drinking-water samples, we found the growth of coliform bacterial colonies in 3 samples, exhibiting various characteristics. Based on the characteristics, both microbiological and biochemical, the identified bacteria were Proteus sp., Escherichia coli and Klebsiella oxytoca.CONCLUSION: Bacteria that contaminated refilled drinking-water in Abeli were Proteus sp., E. coli and K. oxytoca.     Iron Administration Affects Cardiac Calcium Channel Expression in Mice: The Role of Cardiac Calcium Channel Expression in The Heart of Iron Overload Mice Model

    BACKGROUND: Iron-overload cardiomyopathy (IOC) is a major comorbidity in patients with chronic repetitive blood transfusion due to myocardial iron uptake that facilitated by calcium channels. As cardiac compensatory mechanism to IOC, we hypothesized the cardiac calcium channels expression would be increased and involved in cardiomyopathy progressivity. This study was aimed to investigate the gene expression of calcium channels in the heart of the iron overload mice model.METHODS: Mice were divided into three groups according to iron administration doses 0, 0.1, and 0.3 mg/day. Systolic blood pressure (SBP), diastolic blood pressure (DBP), and mean arterial pressure (MAP) were measured for the representation of cardiovascular outcomes. The heart tissues were harvested. Further mRNA levels of L-type calcium channels (LTCCs) and T-type calcium channels (TTCCs) were examined using semi-quantitative PCR. The expressions of cardiac calcium channels and blood pressure among the three groups were compared.RESULTS: The expressions of TTCCs in the two iron-injected groups were higher than the control group (p=0.018). The expressions of LTCCs were not different (p=0.413) among groups. SBP, DBP, and MAP of the iron-injected group were lower than the control group (p=0.025, p=0.011, and p=0.008, respectively).CONCLUSION: Iron administration affects the expression of TTCCs but not the LTCCs, accompanied by decreasing of systolic and diastolic blood pressure.KEYWORDS: cardiomyopathy, iron overload, L-type calcium channel, T-type calcium channel

    Secretomes of Primary Cancer-associated Fibroblasts Upregulate the Expression of Stemness Markers in HT-29 Human Colorectal Carcinoma Cells

    BACKGROUND: Cancer-associated fibroblast (CAF) is the most abundant tumor stroma. Our previous study has demonstrated that the secretomes of CAF isolated from colorectal carcinoma (CRC) patients could induce epithelial-mesenchymal transition in the HT-29 CRC cell line. However, the role of CAF secretomes in CRC stemness is needed to be further investigated. Therefore, the present study aimed to investigate the effect of CAF secretomes from CRC patients on the expression of stemness markers in HT-29 CRC cells in comparison with the secretomes from normal fibroblasts.METHODS: Fibroblasts were isolated from tumor (CAF) and their counterpart non-tumor (NF) areas of three CRC patients undergone surgical resection. Normal preputium fibroblasts (PF) were isolated during circumcision of three healthy boys aged 8 years. All fibroblasts were grown in free-serum culture medium for 24 hours to collect 50% (v/v) conditioned medium (CM). Then, CM was supplemented to HT-29 CRC cells for 72 hours. The effects of CAF- and NF-CM on the mRNA expression of CD44, CD133, OCT4, and ALDH1A1 were analysed using qRT-PCR. Cells proliferation was measured using the trypan blue exclusion assay.RESULTS: Supplementation of CAF-CM (50% v/v) significantly increased CD44, CD133, OCT4, and ALDH1A1 mRNA expressions compared to that of NF-CM and control without supplementation but had no effect on the proliferation of HT-29 cells.CONCLUSION: CAF secretomes from CRC patients upregulate the expression of CRC stemness.KEYWORDS: cancer-associated fibroblasts, ALDH1A1, OCT4, CD44, CD133, colorectal carcinom

    Correlation between Suppression of Tumorigenicity-2 with Left Ventricular Geometry, Left Ventricular Ejection Fraction and Quality of Life in Systolic Heart Failure Patients

    BACKGROUND: Heart failure (HF) is a clinical syndrome caused by structural or functional cardiac disorders and is the final stage of every heart disease, marked by decreased functional capacity and patients’ quality of life (QoL). Suppression of tumorigenicity-2 (ST2) is a biomarker depicting heart fibrosis and remodeling that altered left ventricular geometry, which in turn decreases left ventricular contractility, decreases functional capacity, and ultimately affects the QoL of the HF patient.METHODS: An observational study was conducted with a cross-sectional approach involving 60 patients with systolic heart failure. Left ventricular geometry, left ventricular ejection fraction (LVEF), ST2 level, and other biomarkers were examined, continued by QoL assessment.RESULTS: The ST2 level (33.25±23.55 ng/mL) was negatively correlated with LVEF (r=-0.257; p=0.024) and was positively correlated with QoL (r=0.255; p=0.05). The LVEF was negatively correlated with QoL (r=-0.224; p=0.031). However, no significant correlation was found between left ventricular geometry with ST2 level or patients’ QoL.CONCLUSION: Elevated ST2 levels are correlated with decreased LVEF and worse QoL in systolic heart failure subjects.     Correlations between Total Antioxidant Capacity and 8-Hydroxydeoxyguanosine with Carotid-Femoral Pulse Wave Velocity in Chronic Kidney Disease

    BACKGROUND: Chronic kidney disease (CKD) is often associated with an increased risk of cardiovascular disease. Cardiovascular disease in CKD can be affected by oxidative stress and inflammation. The oxidative stress can affect arterial stiffness, especially in patients with CKD, which will increase the risk of cardiovascular disease. This study aimed to explain the correlation between total antioxidant capacity (TAC) and 8-hydroxydeoxyguanosine (8-OHdG) with carotid-femoral pulse wave velocity (cf-PWV) in patients with CKD.METHODS: Forty-three CKD patients was included in this cross-sectional study. TAC was measured from serum samples by colorimetric method and 8-OHDG was measured from urine by enzyme-linked immunoassay method. cf-PWV was measured by Doppler ultrasound as a diagnostic tool for arterial stiffness.RESULTS: Significant correlations were found between age, estimated glomerular filtration rate (e-GFR), Hemoglobin A1C (HbA1C), serum creatinine, cystatin-C, and severity of CKD with urinary 8-OHdG levels. Serum creatinine, cystatin-C, severity of CKD, age, high-density lipoprotein (HDL), e-GFR, and HbA1C also had significant correlations with serum TAC levels. Other significant correlations between e-GFR, HbA1C, serum creatinine, cystatin-C, and severity of CKD with cf-PWV were also obtained. A significant negative correlation was obtained between serum TAC and cf-PWV with moderate levels of correlation coefficient (r=-0.504).CONCLUSION: There was a negative correlation between TAC and cf-PWV, but there was no significant correlation between 8-OHdG and cf-PWV in patients with CKD.KEYWORDS: chronic kidney disease, total antioxidant capacity, 8-hydroxydeoxyguanosine, carotid-femoral pulse wave velocit

    Hormesis in Health and Disease: Molecular Mechanisms

    BACKGROUND: Hormesis was initially defined as a phenomenon where a small dose of harmful agent exposure to living organisms gives beneficial effects. The dose and time of this ‘tress’ exposure has become the object of investigation across the broad range of biomedical studies.CONTENT: Hormesis characterized by the biphasic dose-effect or time-effect relationship for any substance. Some hormetic mechanisms performed biological plasticity, involve oxidative damage which instead induce antioxidant enzyme production in various cells. Early-life stress can increase resilience in later life and lack of stress can lead to vulnerability. Many stressors like dietary factors and natural environmental toxins can be occupied for healthy growth or homeostasis, which exemplifies how illness is the doorway to health.SUMMARY: Hormesis reconcile many paradoxical phenomena exert opposite effects of the same substance, either a xenobiotic or an endogenous substance, a hormone or a metabolite, a genetic manipulation or an epigenetic alteration, an experimental intervention or a natural event. Human bodies are highly adaptive. A resilient body would be resulted after the ‘training’.     Glutamine Supplementation Effects on Reducing Inflammation in The Ileum of Acute and Chronic Diarrhea Rats Induced by Enteropathogenic Escherichia coli

    BACKGROUND: Glutamine, a non-essential amino acid, is the main fuel in the gastrointestinal mucosa. It is thought to protect the intestinal mucosa against local or systemic injury from diarrhea. This study aimed to determine the relationship between glutamine supplementation and ileum histopathology in acute and chronic diarrhea rats induced by enteropathogenic Escherichia coli (EPEC).METHODS: A randomized post-test only control group design was conducted. Thirty Rattus norvegicus Wistar strain were divided into 5 groups: one negative control group, two acute, and two chronic diarrhea groups. All four diarrhea groups were induced by EPEC at a dose of 108 CFU/mL. One acute and one chronic groups were supplemented with glutamine at a dose of 810 mg/200 g body weight for 14 days. While the other two diarrhea groups were not treated. The intestinal histopathology of each group was assessed and the level of inflammation was classified.RESULTS: Significant differences in inflammation levels were found among the groups (p<0.05). The highest inflammation level was observed in the acute diarrhea group without glutamine supplementation. Inflammation levels of both acute and chronic diarrhea with glutamine supplementation groups were significantly lower than the inflammation levels of acute and chronic diarrhea without glutamine supplementation groups.CONCLUSION: Supplementation of glutamine reduces the level of inflammation and leads to the histopathological improvement of the rat’s ileum.KEYWORDS: enteropathogenic Escherichia coli, glutamine, gastrointestinal tract, histopathology, ileu

    Momordica charantia L. Fruit Fractions inhibit Malondialdehyde Level and Regenerate Hepatic Damage of Hyperglycemic Rats

    BACKGROUND: Chronic hyperglycemia causes an increase of free radical production and in longterm, the hyperglycemia increases oxidative stress. Among medicinal plants, Momordica charantia L. fruit has been known to overcome hyperglycemia. However, role of M. charantia L. fruit on oxidative stress is not well understood. Therefore, current study was conducted to investigate the effect of M. charantia L. fruit extract on malondialdehyde (MDA) level and hepatic damage in hyperglicemic rat model.METHODS: Twenty five white rats (Rattus novergicus) were induced with Streptozotocin (STZ) and treated with/without glibenclamide, sodium carboxymethyl cellulose (Na-CMC), or M. charantia L. fruit ethanol/ethyl acetate/ n-hexane fraction. After the treatment, rat’s livers were collected and separated for histopathological examination and MDA analysis.RESULTS: The MDA level average of rats before the STZ induction was 1.37 μg/mL. MDA level average was markedly increased (23.85 μg/mL) in rats induced with STZ and treated with Na-CMC merely. The MDA level average of STZ-induced glibenclamide-treated rats was 3.12 μg/mL. Meanwhile, the MDA level averages of STZ-induced M. charantia L. fruit ethanol, ethyl acetate and n-hexane fractions-treated rats were 14.95, 8.98 and 5.37 μg/mL, respectively. The histopathology results of this study showed that adipocytes, dilated sinusoids and central vein thickening were mostly observed in STZ-induced Na-CMC-treated rats. Meanwhile, the STZ-induced ethanol/ethyl acetate/n-hexane fraction-treated rats did not exhibitthose expressions.CONCLUSION: M. charantia L. fruit fractions inhibit the MDA level average in liver tissue and regenerate hepatic damage of STZ-induced rats, especially the n-hexane fraction which could be a potential hepatic antioxidant and regenerative agent.KEYWORDS: Momordica charantia L., malondialdehyde, oxidative stress, hyperglycemia, diabetes mellitu

    Association of CCL7 Promoter Polymorphism with Responsiveness to Allergen in Cynomolgus Macaque Model of Asthma

    BACKGROUND: C-C motif Ligand 7 (CCL7) has been reported to be associated with asthma severity in humans. Cynomolgus macaques (Macaca fascicularis; Mf) are often used as animal model of asthma but little is known about Mf genetic profile such as polymorphism. Our aim was to identify CCL7 polymorphism in Mf as a potential surrogate marker for identification of allergen responsiveness in the Mf model of asthma.METHODS: Real-time PCR was performed on archive of bronchoalveolar fluid samples previously collected from Mf that were exposed to allergen. Expression of CCL7 mRNA was evaluated, and sequencing technique was used to identify polymorphism in this gene.RESULTS: The results showed that CCL7 expression did not differ between Mf, despite a trend of lower expression in Mf that exhibited high response to allergen. By direct DNA sequencing of CCL7, 10 sequence variants were identified; three in promoter region (-460 G/A, -459 A/G, -456 -/A ), two in exon 1 (9 A/G, 65 G/C), four in intron 1 (135 T/C, 254 T/C, 420 T/C, 453 A/G), and one in intron 2 (1205 T/A).CONCLUSION: There was an association between Mf sensitivity to allergen with CCL7 promoter polymorphism at (-456 -/A). These results suggest that CCL7 may be a potential genetic marker to identify Mf sensitivity to allergen, which could be a useful tool to efficiently select for Mf model of asthma.KEYWORDS: asthma, CCL7, allergy, Ascaris suum, nonhuman primat

    Low Level of Vitamin D is Correlated with High C-Reactive Protein (CRP) and Disease Activity in Indonesian Juvenile Idiopathic Arthritis (JIA) Patients

    BACKGROUND: Vitamin D plays essential role in the regulation of inflammation, such as in pathogenesis of Juvenile Idiopathic Arthritis (JIA). Vitamin D deficiency has been reported among JIA patients, but there were conflicting results regarding the correlation with disease activity. This study aimed to assess vitamin D serum level and its correlation with C-Reactive Protein (CRP) and disease activity in JIA patients.METHODS: Children who were diagnosed with JIA according to International League of Associations for Rheumatology (ILAR) criterias were enrolled as JIA group subjects, while age and sex-matched healthy children were enrolled as the control group subjects. Vitamin D and CRP serum level were measured. Disease activity of JIA patients was calculated by Juvenile Arthritis Disease ActivityScore-27 (JADAS-27).RESULTS: Vitamin D serum level was lower in the JIA group compared to the healthy control group (p=0.000). Among 26 JIA patients, 61.5% were deficient, 30.8% were insufficient, and 7.7% had normal vitamin D. No significant different in CRP level between vitamin D group (p=0.441), but there was significant different in JADAS-27 (p=0.001). The mean of CRP and JADAS-27 were found highest in vitamin D deficiency group. Vitamin D serum level was negatively correlate with CRP (p=0.021, r=-0.452) and JADAS-27 (p=0.001 r=-0.595).CONCLUSION: Low level of vitamin D in JIA patients was inversely related to higher CRP and disease activity,suggesting that vitamin D supplementation could be havepotential role in JIA treatment.KEYWORDS: vitamin D, CRP, disease activity,JADAS-27, JI
