Identification of macroelements and microelements in the leaves of the Synadenium grantii Hook used as medicinal plant in the Brazil
Introduction: The Synadenium grantii Hook species is a medicinal plant of the Euphorbiaceae family and is popularly known in the Midwest regions of Brazil as leitosinha or janaúba. All the parts of Synadenium grantii Hook as stem, latex, leaves and flowers are used for the treatments of various disease, but no elemental composition has been done.
Objective: The aim of present work was to measure the macroelements (K, Mg, Mo and P) and microelements (Cr, Cu, Fe, Mn, Mo, Zn, Si, Ni, Co, Cd and Al) in the leaves of medicinal plant Synadenium grantii Hook used in the treatment of diseases by rural and urban communities of the Campo Grande, State of Mato Grosso do Sul, Brazil.
Method: The chemical digestions of samples were prepared utilizing HNO3 and H2O2 and microwave digestion system Speedwave Berghof, Germany. After digestion, the concentrations of the elements in leaves were determined by technique of Inductively Coupled Plasma – Optical Emission Spectrometer (ICP-OES) with a Dual Plasma (Thermo Scientific – iCAP 6000 Series)
Results: Results demonstrated that the Synadenium grantii leaves are source of K but aren't the best source of Na and P. Its leaves are rich in elements such as Mg, Cr, Cu, Fe, Mn, Mo and Zn. The leaves of Synadenium grantii showed a high content of Mg values tolerable upper intake level. However, Fe, Na, P, Mn, Mo and Zn are below the values tolerable upper intake level. The leaves of this plants have elements as Ni, Cu, Cr, Cd above the permissible limit set by WHO and FAO.
Conclusions: The gaps in knowledge about the level of contents in the Synadenium grantii was completed in this work and would serve as a tool for deciding the dosage of prepared from this plant with medicinal and nutritional purposes. Brazil should create legal standards for the use of medicinal plants based on internationally practiced requirements.

Patient Safety Culture In Health Organizations: Scoping Review
Objective: identify and map the forms to evaluate the patient safety culture in health organizations.
Method: scoping review, developed based on the method proposed by the Joanna Briggs Institute. Data collection occurred in June 2016 in 15 international databases. Descriptive statistics was used for data analysis.
Results: the sample consisted of 75 publications. Among the eleven instruments identified to evaluate the safety culture, the Hospital Survey on Patient Safety Culture questionnaire and the Safety Attitudes Questionnaire had the greatest international repercussion, since they were adapted, validated and used in different continents and contexts.
Conclusion: The synthesis of the instruments enabled clarifying their characteristics, and how they can be important tools to support and accompany changes in the safety culture over time.

Breast Cancer Mortality In Brazil: Correlation With Human Development Index
Background: Mortality from breast cancer decreased in high-income countries, while countries with middle and low incomes as Brazil still has upward trend. However, large geographical variations among the federal units are observed in the country. The aim of the study was to evaluate the trend of specific mortality from breast cancer in women over 20 years old years among different states of Brazil from 1996 to 2012.
Methods and Findings: Ecological study, using linear regression model for temporal analysis of specific mortality coefficient from malignant neoplasm of breast. We also checked the degree of its correlation with the HDI for the states of Brazil during the stated period. There was an increase in the specific mortality rate for malignant neoplasm of the breast in order of 33%, with range from 23.2 to 30.8 / 100,000 inhabitants. The states with the highest human development HDI in 2010, showed the largest specific mortality rates of breast cancer.
Conclusion: Taking the trends of mortality from cancer an important role, this study confirms the need for improvements in mammography coverage, following radiological lesions suspected and access to appropriate therapy.
 
Use Of Mandibular Advancement Devices For Obstructive Sleep Apnoea Treatment In Adults
ABSTRACT
Introduction: This article is based on Clinical Guidelines for obstructive sleep apnoea (OSA) established by a taskforce coordinated by the Brazilian Sleep Association.
Objective: The aim of this article is to evaluate the available scientific evidence regarding the efficacy, adherence and safety of using mandibular advancement devices (MAD) as a therapeutic course for treating obstructive sleep apnoea in adult patients.
Method: Active searches were performed in the PubMed/MEDLINE, EMBASE, Scielo/LILACS and Cochrane Library databases. Methodological aspects were used to rank the levels of evidence according to the criteria of the Centre for Evidence-Based Medicine at Oxford.
Results: Mandibular advancement devices offer the best results for patients with primary snoring, upper airway resistance syndrome and mild or moderate OSA (Levels of Evidence I and II). Patients seem to exhibit greater adherence to oral appliances (MAD) than to continuous positive airway pressure (CPAP) devices, although the latter are more effective in controlling OSA (Level of Evidence I). The long-term side effects most observed after the use of MADs are related to changes in the mandibular and dental positions (Levels of Evidence I and II).
Conclusion: MAD constitute a therapeutic alternative for OSA and promote favourable results with good efficacy and adherence to treatment. Side effects can arise in the short, medium or long term. Patients must be informed about the possible occurrence of these adverse effects, and the orthodontist must be able to manage any side effects that occur due to the use of these devices
Clinical and Hematological Evaluation of Patients with Sickle Cell Anemia Before and After Four Years of Using Hydroxyurea
Objective: Evaluating clinical and hematological-clinical parameters of patients with sickle cell anemia (SCA) before and after four years of using hydroxyurea (HU).
Method: A retrospective cohort study implementing a quantitative, descriptive and analytical approach developed in two public teaching hospitals located in the Central-West region of Brazil, from November 2010 to October 2011. Data collection was performed through medical records of 32 patients with SCA to assess clinical and hematological parameters before and after HU treatment. The study was approved by the UFMS Ethics Committee under protocol number 1890/2010.
Results: All of the 32 patients were homozygous with a mean age in the prescription of hydroxyurea of 19.72±7.58 years, an initial dose of 15.59±4.27 mg/kg/day, and 22.48±5.35 mg/kg/day in the fourth year of treatment. Regarding the use of HU, average values of some hematological parameters presented a significant difference in the fourth year compared to the mean values prior to HU use, such as fetal hemoglobin (14.49±7.52%), red blood cells (2.54±0.38x1012/L), hematocrit (25.30±4.03%) and hemoglobin (9.22±3.34g/dL).
Conclusion: Treatment with hydroxyurea showed a significant increase in fetal hemoglobin levels, increased hemoglobin, hematocrit and average corpuscular hemoglobin concentration, with reduced episodes of pain, infection and acute chest syndrome in such a way as to reaffirm its efficiency in treating these patients.

Evaluation Of The Quality Of Assistance In An Pre-Hospital Mobile Emergency Service
Background: to evaluate the quality of the assistance of a Mobile Emergency Care Service and to compare the evaluation of professionals according to training and level of education.
Methods and Findings: this is a cross-sectional study, with a Donabedian theoretical reference. The sample was all the professionals of a Mobile Emergency Service and data collection performed by a previously validated quality assessment instrument. Some aspects of the service structure were evaluated negatively, and the process was evaluated in a positive way, in general. When comparing the evaluation of quality according to professional category and education, it was verified that the indicators of structure and process were considered appropriate by the professionals of average level and inadequate by the personnel of superior level.
Conclusion: the professionals identified flaws in the service structure, which can impact the assistance provided. The level of education influenced the evaluation of the quality of the service.

Automobile Accidents Attended by Mobile Emergency Care Service
Background: Automobile accidents are increasing every day and are becoming a serious public health problem due to the high morbidity and mortality rate. The goal of the current study was to characterise the traffic accidents attended by the Mobile Emergency Care Service (MECS) in Ibiara, PB.
Methods: This exploratory, descriptive, documentary study adopted a quantitative approach and analysis of data. The population consisted of all victims of traffic accidents attended by MECS in the city of Ibiara, PB, from June 2015 to June 2016. The following variables were studied: age, gender, time and day of the week the accident occurred, nature of the incident, substance ingested by the victim, wearing personal protective equipment (PPE) or not, the anatomical lesions on the victim and the body regions hit during the accident.
Results: The sample consisted of 49 accident victims, and the majority (81.6%) were male, predominately 30 to 59 years. Events occurring at night (63.3%) and during the week (65.3%) predominated. The most frequent type of accident was motorcycle fall (71.4%), with almost half having consumed alcohol (46.9%) and most not wearing PPE (77.6%). The regions of the body most affected were the lower limbs (67.3%) and upper limbs (53.1%).
Conclusion: The main factor/cause of these accidents was due to imprudence and violation of traffic laws, highlighting the need to invest in traffic education policies, to direct campaigns for the prevention of traffic accidents, as well as expand the surveillance of traffic laws by authorities
Nursing care for children with gastrostomy: integrative review
Objective: To analyze national and international scientific production on nursing care for children with gastrostomy.
Method: Integrative review conducted in the databases Latin American and Caribbean Health Sciences (LILACS), Scientific Electronic Library Online (SCIELO),Medical Literature Analysis and Retrieval System Online (MEDLINE) and Cumulative Index to Nursing and Allied Health Literature (CINAHL) without temporal delimitation of publications. The following Health Sciences Descriptors (DeCS) and Medical Subject Headings (MeSH) were used: gastrostomia/gastrostomy/cuidado da criança/children care/enfermagem/nursing. Twenty references were found in LILACS, four in SciELO, 116 in MEDLINE and 167 in CINAHL. The studies found in the LILACS and SciELO did not meet the inclusion criteria. Three articles were selected from MEDLINE and 02 from CINAHL; thus, 05 articles composed the sample of the study.
Results: We observed that gastrostomy brought about satisfaction to caregivers despite post-surgery complications. Improvements brought about by the use of the device and reduction of complications related to feeding difficulties, weight gain, decreased vomiting, abdominal distention and fatigue of the patient throughout the day were mentioned. It was also noted that caregivers in the studies felt fear and insecurity at decision-making on the insertion of gastrostomy, showing that the information for the care of these patients was a common concern among them.
Conclusion: The study allows us to conclude that the care provided by nurses to children with gastrostomy and family contributes to increase knowledge, thus facilitating decision-making during the management of the device. The use of educational technologies are to be highlighted as a strategy for promoting care during hospitalization and preparing parents to take care of the gastrostomy after hospital discharge.
 
Training of Professionals from the Family Health Strategy for Psychosocial Care for the Elderly
Background: Mental disorders of the elderly constitute a public health problem due to their high prevalence, shortage of specialized services offered in Brazil, difficulties of access by the population and deficiency in the training of professionals of the Family Health Strategy for the identification, receptiveness and psychosocial assistance to the elderly.
Objectives: To analyze the training of professionals of the Family Health Strategy on psychosocial care for the elderly in the context of the Psychosocial Care Network – RAPS (Rede de Atenção Psicossocial), and to discuss how professional training influences the care provided to the elderly.
Methodology: Descriptive, qualitative study carried out with 31 professionals, 13 physicians and 18 nurses, who work at the Family Health Strategy of the city of Picos, Piauí, Brazil. The data were collected in January 2016, through a semi-structured interview guide, processed by the IRAMUTEQ software and analyzed by means of the Descending Hierarchical Classification.
Results: The results were presented in three segments, namely: 1. The practice of professionals from the Family Health Strategy in psychosocial care in the family context; 2. Training of specialized professionals, in the attention to the elderly, in the Family Health Strategy; 3. The Psychosocial Attention Network in the care of elderly users of alcohol and other drugs;
Conclusion: Health professionals have difficulties in dealing with the elderly with mental disorders in basic care. In order to facilitate access to specialized health services and to develop actions for social reintegration, prevention and harm reduction, it is necessary to implement a policy of ongoing training and education for health professionals to improve care for the elderly.

Educational Contributions Of The Nurse In The Prevention Of Sepsis In The Unit Of Intensive Therapy: Integrative Review
Sepsis is defined as a syndrome of inflammatory response, motivated by an aggressive agent, associated with systemic infection, the early approach of the infectious agent, both in the direction of diagnosis and in the control of infectious focus, are fundamental for the good evolution of the patient. This pathology has been considered a global health problem, affecting a large number of people and causing high rates of morbidity and mortality.
OBJECTIVE: The study aimed to investigate how nurses contribute in an educational way in the prevention of complications caused by sepsis in an intensive care unit.
METHOD: The research was carried out through an electronic search of scientific papers published in the databases LILACS, SCIELO and PUBMED, portal of the virtual library in health. Other materials were located in scientific journals available on university websites and articles related to the subject. The initial evaluation of the material occurred through the reading of the abstracts, in order to select those that met the objectives of the study, with this, 37 articles were selected. In carrying out the research it was possible to verify that the recognition of sepsis by nursing professionals is mainly identified through the changes in the signs and symptoms that are observed through the provision of care to these critical patients.
RESULTS: It has been shown that even with all the advances in the production of knowledge about the pathophysiology and the treatment of sepsis, it is still possible to find several difficulties for the proper handling in the diagnosis and in the ideal treatment for each particular case.
CONCLUSION: The professional nurse should always be updating and seeking new knowledge for a fast, safe and effective action, in conjunction with the entire health team to promote quality and resolutive care for the patient, especially those affected by sepsis
