Para-aortic and Meningitis Tuberculosis: A Case Report
Background: In 2018, Indonesia was in third place as the country with the highest incidence of tuberculosis (TB) in the world. In addition to pulmonary TB, extrapulmonary TB cases are also quite large. Lymphadenitis of the aortic TB can cause a fatal risk if not treated immediately. This study aimed to explore the para-aortic and meningitis tuberculosis case in Klaten Hospital, Central Java.Case presentation: There was a 24-year-old woman with complaints of missing chest pain for 2 months, fear of seeing light (photophobia), and decreased consciousness. The patient was a referral from Klaten Hospital with a diagnosis of a mediastinal tumor. The chest radiograph shows a picture of homogeneous opacity in the anterior mediastinum. Bronchoscopy results show compression stenosis in 1/3 distal and blunt carina. After a sternotomy, it was obtained pus (pus) and tissue granuloma in the area of the aorta. After the rapid molecular test was carried out, the results showed that M tuberculosis detected. The results of histopathology of anatomical pathology show epitheloid tubercle and Datia Langhans cells that suggest an infection with M. tuberculosis. Therapy was done by giving a standard anti-tuberculosis drug, namely Rifampicin 450 mg, INH 300 mg, Ethambutol 1000 mg, and Pyrazinamide 1000 mg. The patient's condition improved marked by weight gain in 2 months.Conclusion: In the case of pulmonary masses (mediastinal tumors), we need to be aware of the possibility of cases of TB lymphadenitis because Indonesia is a country with a high prevalence of TB. Left Femoral Artery Occlusion due to Neglected Closed Fracture Distal Femur: A Case Report
Background: Distal femur fractures must always be considered for vascular disorders given the high risk. The incidence rate of femoral fractures is 1/10000 patients per year with the incidence rate of vascular injury around 0.10% -2.00%. Salter-Harris (SH) fracture classification is used in cases of fractures involving the growth plate (physeal plate) in children. Volkmann's ischemic contracture is caused by severe injury to the deep tissue and nerves. If tissue pressure is equal to or more than the perfusion pressure of the capillary, the microcirculation will be disrupted and ischemic occur. Normal myosit cells will be replaced by fibroblasts that tend to contract.Subjects and Method: A 10 years old boy had a traffic accident while riding a motorcycle 6 days before the patient came to the hospital. The patient could not walk, feeling pain and swelling on the left knee, the patient didn’t bring to the hospital immediately after the accident. On physical examination, there was angulation in the distal left femur, there was tenderness on palpation and unable to do dorsiflexion or moving the fingers of the left foot but still could feel touch stimulation on the pedis.Results: In our patient case with the diagnosis of left femoral artery occlusion et causa closed fracture distal femur (Salter-Harris type II) managed by surgery to release the femoral artery with internal fixation to the distal femur using Kirschner wire (minimally invasive) and massage to the femoral artery give optimal results.Conclusion: Surgery to release the femoral artery with internal fixation to the distal femur using Kirschner wire (minimally invasive) and massage to the femoral artery give an optimal result.Keywords: Salter-Harris fracture, femoral artery, Kirschner wire, Volkmann's ischaemic contractureCorrespondence: Pamudji Utomo. Effectiveness of Infant Massage on Strengthening Bonding and Improving Sleep Quality
Background: Baby massage is the oldest and most popular touch therapy known to mankind, which is also the art of health care and treatment that has been practiced for centuries. This study aims to determine the effect of infant massage on bonding attachments between mother and baby and the quality of infant sleep.Subjects and Method: This was experimental study with Randomized Controlled Trial (RCT) design conducted in five health centers in Ponorogo Regency, East Java in March to April 2019. The total sample was 120 mothers and infants aged 3 to 6 months divided between massage groups (treatment) and not massage (control) based on The dependent variable of this study was bonding attachment and sleep quality. The independent variable in this study was infant massage. Data collection was using questionnaires and checklists. Pulse Pressure Initial and Arterial Pressure Mean as A Prognostic Death Information in Acute Myocardial Infarction Patients
Background: Arterial blood pressure is an easily get variable, including systolic blood pressure, diastolic blood pressure, mean arterial pressure (MAP) and pulse pressure (PP). Although the variable blood pressure has clinical importance in many cardiovascular diseases, the variable that has become the best predictor in clinical practice cannot be determined. The purpose of this study is to determine the prognostic value of PP and MAP at the start of hospitalization in patients with acute myocardial infarction (IMA).Subjects and Method: This was a retrospective cohort study conducted at cardiovascular intensive care unit, Dr. Moewardi Hospital, Surakarta, Central Java. A sample of 150 IMA patients from July 2013 to December 2013 was selected for this study. Blood pressure was measured the first time the patient arrives at the emergency unit to determine the value of PP and MAP. Then the patient was observed by looking at mortality during hospitalization as a final result. Basic characteristics data were analyzed using logistic regression and displayed in quartile form. The Kaplan-Meier curve was used to see mortality in each quartile. Cox proportional regression analysis was used to see the magnitude of the risk of variable PP, MAP and other variables on mortality.Results: The highest mortality was found in the first quartile group of PP (OR= 1.81; p<0.001) and MAP (OR= 1.69; p<0.001). The Kaplan-Meier curve showed the first quartile of the two groups had the lowest survival, while the third highest quartile (p <0.001). The results of the cox analysis showed a decrease in mortality risk of 0.49 per increase in PP by 10 mmHg (95% CI= 0.07 to 1.00; p= 0.044) and 0.31 each increase in MAP by 10 mmHg (95% CI= 0.09 to 0.53; p= 0.003).Conclusions: Blood pressure measurement can produce two variables as predictors of mortality in IMA patients, namely PP and MAP. Effects of Pineapple Juice and Ginger Drink for Relieving Primary Dysmenorrhea Pain among Adolescents
Background: Primary dysmenorrhea is characterized by low abdominal pain occurring during menstruation. It is negatively impact on all aspects of life of adolescents such as increased school absteeinsm, emotional disorders, impaired social interaction and productivity. The herbal drink which they can self-prepared is common choice as home remedies to relieve the pain. This study aimed to examine the effect of pineapple juice and ginger drink in providing pain relief to the adolescent with primary dysmenorrhea.Subjects and Method: This was a randomized trial study conducted at three high schools in Surakarta, Central Java, Indonesia, from January to April 2018. A sample of 39 female students aged 15-17 years was selected for this study, divided into three groups. The treatment was given for 3 days from first day of menstruation for 3 menstruation cycles. The pain score were measured by numeric rating scale (NRS). The data were analyzed by Mann-Whitney.Results: Pain score in pineapple juice group (mean= 2.25; SD= 1.79) and ginger drink group (mean= 3.63; SD= 2.02) were lower than control group (mean= 5.64; SD= 1.25) and it was statistically significant.Conclusion: Pineapple juice and ginger drink effective to reduce pain of primary dysmenorrhea among female adolescent.Keywords: Dysmenorrhea, pain, pineapple juice, ginger drink, female adolescentsCorrespondence: Dita Wrisnijati. The Effects of Tamoxifen and Chemotherapy after Surgery on the Recurrence and Survival of Breast Cancer Patients with Positive Hormone Receptor: A Meta-Analysis
Background: Breast cancer is the most common type of cancer suffered by women in the world. The combination of tamoxifen and chemotherapy significantly reduces the risk of recurrence and mortality in breast cancer patients. This study aimed to analyze how effective the combination of tamoxifen and chemotherapy after surgery to reduce the risk of recurrence and mortality in breast cancer patients with positive hormone receptor.Subjects and Method: This study used systematic review and meta-analysis. The researcher collected articles from the Pubmed journal database. The subjects of the study were women with breast cancer after surgery. The dependent variable was recurrence and survival (mortality). The independent variables were tamoxifen and chemotherapy. Data were analyzed based on the fixed and random effects model using RevMan 5 software.Results: There were 3 articles with 3,761 women who were involved in the analysis process. The combination of tamoxifen and chemotherapy could reduce the risk of recurrence in women with positive hormone receptor (ER+) breast cancer (HR=0.68; 95%CI=0.58 to 0.80; p=0.001). The combination of tamoxifen and chemotherapy showed weak and non-significant decreasing trend in reducing the risk of mortality in women with ER+breast cancer (HR=0.87; 95%CI=0.73 to 1.03; p= 0.11).Conclusion: The combination of tamoxifen and chemotherapy in women with ER+ breast cancer is effective in reducing the risk of recurrence. Association between Carbohydrate, Vitamin C, Vitamin E, and HBA1C Level
Background: Diabetes is now a disease that is a major concern both globally and regionally and is the leading cause of death in most countries. HbA1c levels are used to measure long-term glucose levels in patients with type 1 and 2 Diabetes Mellitus (DM). Restrictions on carbohydrate diets have the greatest effect on decreasing blood glucose levels. Nutritional interventions with the provision of vitamin C and vitamin E can reduce inflammation and oxidative stress as a strategy to prevent the occurrence of DM. The purpose of this study was to determine the relationship of carbohydrate intake, vitamin C and vitamin E with HbA1c levels.Subjects and Method: This was a cross-sectional study conducted at Prodia Surakarta Clinic, Surakarta, Central Java, Indonesia. A sample of 150 study subjects was selected by fixed disease sampling. The dependent variable was HbA1c level. The independent variables were carbohydrate and vitamin E. The data were collected by questionnaire and 24-hour food recall. The data were analyzed by multiple logistic regression.Results: High carbohydrate intake increased HbA1c level (OR= 2.84; 95% CI= 0.72 to 11.12; p= 0.133), while high intake of vitamin E lowered HbA1c level (OR= 0.12; 95% CI= 0.02 to 0.64; p = 0.012).Conclusion: High carbohydrate intake increases HbA1c level, while high intake of vitamin E decreases HbA1c level.Keywords: diabetes, HbA1c, karbohidrat, vitamin C.Vitamin ECorrespondence: Dwi Astuti Dian Andarwati. Factors Associated with Late Detection of Breast Cancer: Application of Health Belief Model Theory
Background: Primary prevention of breast cancer is still not available, so efforts to promote early detection continue to be the major focus in fighting breast cancer. Since early detection is associated with decreased mortality, it is important to minimize delays in detection and diagnosis. The purpose of this study was to examine factors associated with late detection of breast cancer in Surakarta, Central Java, using Health Belief Model.Subjects and Method: A cross sectional study was carried out at Dr. Moewardi Hospital, Surakarta, from October to December 2018. A sample of 200 breast cancer patients was selected by fixed disease sampling. The dependent variable was late detection of breast cancer. The independent variables were perceived susceptibility, threat, benefit, barrier, self-efficacy, family support, breast cancer detection, and family income. The data were collected by questionnaire and analyzed by a multiple logistic regression.Results: Late detection of breast cancer decreased with perceived susceptibility (b= -1.49; 95% CI= -2.55 to -0.42; p= 0.006), perceived threat b= -1.87; 95% CI= -3.05 to -0.69; p= 0.002), perceived benefit (b= -3.27; 95% CI= -4.54 to -2.00; p<0.001), self-efficacy (b= -1.49; 95% CI= -2.52 to -0.46; p= 0.004), family support (b= -1.44; 95% CI= -2.49 to -0.39; p= 0.007), early breast cancer detection (b=-2.21; 95% CI= -3.33 to -1.09; p<0.001), and income (b= -1.75; 95% CI= -2.92 to -0.59; p= 0.003). It increased with perceived barrier (b= 1.64; 95% CI= 0.49 to 2.80; p= 0.005).Conclusion: Late detection of breast cancer decreases with perceived susceptibility, perceived threat, perceived benefit, self-efficacy, family support, early breast cancer detection, and income. The Potential Effect of Thymol from Black Seed Extract (Nigella Sativa) on Inhibiting the Growth of Candida albicans (Laboratory Studies in Leucorrhea Culture)
Background: Leucorrhea is a complaint of reproductive health problems that are often experienced, 83% of cases are caused by Candida albicans. Candida albicans resistance to fluconazole (2.9%) and itraconazole (5.4%), so that antifungals from plant screening are safer and able to inhibit the growth of Candida albicans. This study aimed to explain the potential of thymol compounds in black cumin seed extract (Nigella sativa) various concentrations can inhibit the growth of Candida albicans.Subject and Method: This was a randomized controlled trial. The study sample was Candida albicans culture divided into 5 groups. The dependent variable was the growth of Candida albicans. The independent variable was thymol compounds of black cumin seed extract. The growth of Candida albicans was measured by calipers. The data were analyzed by one way ANOVA and post hoct.Results: Thymol 12.5% lowered the growth of Candida albicans 9.20 (mean= -9.20; p<0.001).Conclusion: Thymol compounds of black cumin seed extract has the potential as an alternative treatment for leucorrhea.Keywords: thymol, black seeds, Candida albicansCorrespondence:Nurul Aziza Ath Thaariq. A 25 Years Old Female with Meloheostosis: A Case Report
Background: Melorheostosis is a rare sclerosing bone dysplasia characterized by its classic radiographic feature of flowing/dripping candle wax. It usually affects one limb, more often the lower extremity, and rarely the axial skeleton. The etiology remains unknown and various theories have been proposed to explain the pathogenesis of this disease. It often has an insidious onset in early adult life. Symptoms include pain, oedema, and limitation of joint movement.Case presentation: A patient aged 25 year old male complaint of pain on the left lower extremity for the last 3 years. There is muscle atrophy at left extremity, bowing of the thigh, awelling at the foot. There is palpable mass at the left bottom, solid consistency and fixed. There are also limitation ROM of hip, knee and ankle. From the radiography examination showed inhomogen thickening of the periosteal cortical and endosteal, dripping candle wax appearance, and sclerotic lesion at soft tissue.Conculsion: X-ray is a sufficient method for diagnosis of melorheostosis.Other imaging techniques are essential for decision about therapeutic intervention (CT or MRI). Laboratory findings are usually in physiological range.
