    Effect of Health Education with Spinal Anesthesia Video to Lower Pre-Anesthesia Anxiety in Genteng General Hospital, Banyuwangi, East Java, Indonesia

    Background: Spinal anesthesia is an anesthetic method by injecting local analgesic drugs into the subarachnoid space in the lumbar region. The purpose of the study was to analyze the effect of health education with spinal anesthesia videos on anxiety in pre-anesthesia in the operating room of the Genteng Regional General Hospital Banyuwangi.Subjects and Method: A quasi experiment study was conducted in Genteng General Hospital, Banyuwangi, East Java, in August 2022. A sample of 15 patients who underwent surgery under spinal anesthesia was selected by incidental sampling. The dependent variable is anxiety. The independent variables were video educational intervention. Anxiety was measured by Anxiety Analog Scale (AAS). Other variables were collected by questionnaire. The difference test in the paired group was examined using paired t-test.Results: Anxiety level after intervention (Mean= 180; SD= 80.84) was lower than before (Mean= 305; SD= 75.12) with p<0.001.Conclusion: Video of spinal anesthesia intervention lower anxiety in pre anesthesia surgery.Keywords: anxiety, spinal video, pre anesthesia, spinal anesthesia, health educationCorrespondence: Katmini.     Meta-Analysis the Effect of Hydrotherapy on the Elevation of Motoric Functional Ability in Post Stroke Patients

    Background: Stroke is the third leading cause of death in the world after heart and cancer and the disease that causes serious and permanent disability is the first in the world. Hydrotherapy in stroke plays a role in developing, maintaining, and restoring movement and function with motor training based on an understanding of the pathophysiology, neurophysiology, kinematics and kinetics of normal motion. The purpose of this study was to analyze the effect of hydrotherapy on improving motor functional ability in post-stroke patients.Subjects and Method: This study was a meta-analysis with the following PICO, population: stroke patients. Intervention: administration of hydrotherapy. Comparison: Standard therapy. Result: motor functional improvement. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles “hydrotherapy” OR “Aquatic therapy” AND “functional enhancement” AND “post stroke”. The articles included are full-text English with a randomized control trial study design from 2002 to 2022. Article selection was carried out using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A meta-analysis of 9 studies from China, Korea, Italy, Spain and Brazil concluded that hydrotherapy has an effect on post-stroke functional improvement. Post-stroke patients who received hydrotherapy intervention had motor functional ability 0.68 units higher than those who did not receive hydrotherapy. The difference was significant (SMD= 0.68; 95% CI = 0.44 to 0.91; p=0.001).Conclusion: Hydrotherapy improves motor functional ability of post-stroke patients.Keywords: Hydrotherapy, functional enhancement, post strokeCorrespondence: Lilik Sigit Wibisono.     The Impact of the COVID-19 Pandemic on the Resistance Level of Mycobacterium Tuberculosis in Suspected Tuberculosis Patients at the Surakarta Central General Hospital

    Background: Analysis of the impact of the Covid-19 pandemic on the resistance level of Mycobacterium tuberculosis (MTb) isolates to Anti Tuberculosis Drugs (ATD) needs to be carried out considering the level of MTB resistance is an important factor in the occurrence of Multidrug-Resistant TB (MDR-TB) in Indonesia. The purpose of the study was to analyze the impact of the Covid-19 pandemic on the level of resistance of MTb isolates of patients with suspected TB in Surakarta.Subjects and Method: This research is a quantitative descriptive study with a retrospective Cohort based on the medical record data of suspected TB patients  at Surakarta Central General Hospital in 2020 – 2021. The sampling technique was random sampling with a total of 200 samples each year. Data analysis used the SPSS 21 program.Results: Characteristics of respondents with suspected TB patients who visited the Surakarta Central General Hospital in 2020 and 2021, mostly in men and occurring in productive ages 36-55 years. The most effective type of ATD against MTb isolates is Rifampicin with a mean rank of 6.00 and a p=0.035. While Ethambutol is ineffective as an ATD with a mean rank of 58.00 and a p<0.001. The results of the Wilcoxon test analysis showed  there was a significant difference in the level of resistance or sensitivity of MTb isolates in 2020 and 2021 with a p-value=0.000.Conclusion: There is a significant effect of the impact of the Covid-19 pandemic on the level of resistance or sensitivity of MTb isolates of patients with suspected TB in the Surakarta area in 2020 and 2021.Keywords: pandemic, Covid-19, resistance, MTb isolates, ATDCorrespondence: Yusup Subagio Sutanto.     Effectiveness “Kamio” on Treatment Compliance and Quality of Life Bipolar Disorder Patient in Dr. Iskak General Hospital, Tulungagung, East Java

    Background: Bipolar Disorder is a chronic disease with various variations in the course of the disease that are often unknown, misdiagnosed and even when diagnosed are often not adequately treated. The aim of the study was to analyze the effectiveness of KamiO on quality of life in Bipolar Disorder patients at dr. Iskak Tulungagung Hospital.Subjects and Method: Observational study with randomized control trial was conducted at dr. Iskak General Hospital, Tulungagung, East Java, from May 25 to August 25, 2022. The study population were 160 patients with bipolar disorder. A 114 sample was selected using the simple random sampling divided into (1) 57 sample in the treatment group and (2) 57 sample in control group. In this study, the independent variable was KAMIO. The dependent variable is medication adherence and quality of life. While the measuring instrument used is a questionnaire. Data analysis using Mann Whitney test.Results: Patients in the intervention group more adherent to visit medical treatment (Mean= 63.00) than control group (Mean= 52.00), with p= 0.039. Quality of life in bipolar disorder patients in the intervention group (Mean= 72.25) was higher than in the control group (Mean= 42.75) and it was statistically significant (p<0.001).Conclusion: Quality of life in the intervention group (kamio cards) is higher than in the control group.Keywords: kamio effectiveness, treatment adherence, quality of life, bipolarCorrespondence: Katmini.     Effectiveness of Acupuncture Therapy to Reduce Pain in Patients with Upper Trapezius Myofascial Pain Syndrome: A Meta-Analysis

    Background: Upper trapezius myofascial pain syndrome is a muscle pain condition associated with sensory, motoric, or autonomic functions and generated by trigger point arising from hardened and stiff taut band of muscle fiber while being touched as well as palpated. One of the interventions to reduce the pain is by using acupuncture therapy. The study aims to analyze and estimate the effectiveness magnitude of acupuncture therapy toward the pain reduction in patients with upper trapezius myofascial pain syndrome.Subjects and Method: This was a systematic review and meta-analysis study using PRISMA diagrams. The article search was conducted based on the eligibility criteria using the PICO Model. population: patients with upper trapezius myofascial pain syndrome. Intervention: acupuncture therapy. Comparison: placebo acupuncture. Outcome: reduced pain. The articles used were from 6 databases, namely Pubmed, Google Scholar, Science Direct, Scopus, Spinger Link, and Hindawi Key words for article search were “acupuncture” OR “acupuncture therapy” AND “upper trapezius pain” OR “upper trapezius myofascial pain syndrome” OR “upper trapezius MPS” AND “Randomized Con¬trolled Trial” OR “RCT”. Inclusion criteria used of the study were full text articles with Randomized Controlled Trial study design, measure of association used was Mean SD. Articles were analyzed using the Review Manager 5.3 application.Results: The 9 articles reviewed were from Taiwan, China, UK, Turkey, Brazil, and Iran. The study result indicated that acupuncture therapy was effective in reducing pain in upper trapezius myofascial pain syndrome. Upper trapezius myofascial pain syndrome that obtained acupuncture therapy had average score of pain -1.16 units lower than placebo acupuncture (SMD= -1.16; CI 95%= -1.93 to -0.39; p= 0.003).Conclusion: Acupuncture therapy was effective in reducing pain in upper trapezius myofascial pain syndrome.Keywords: acupuncture, pain, upper trapezius myofascial pain syndrome, randomized controlled trial, meta-analysis.Correspondence: Niken Putri Eka Saraspuri.     Meta-Analysis Effectiveness of Massage on Spasticity in Children with Cerebral Palsy

    Background: Children with spastic cerebral palsy generally experience spasticity in the upper and lower extremities. Spasticity is stiffness that occurs due to damage to the central nervous system caused during the child's growth and development from prenatal to postnatal. Spasticity can be controlled with intervention. The purpose of this study is to analyze, and estimate the effectiveness of massage for spasticity in children with cerebral palsy.Subjects and Method: This study uses a meta-analysis study design with PICO as follows: P= children with cerebral palsy, I= massage, C= non massage, O= Modified Ashworth Scale (MAS). The article search process was carried out between 2000-2020 using databases from PubMed, Google Scholar, PEDro, NCBI. Based on the database, there were 9 articles that met the inclusion criteria. This study involved 817 articles. The analysis was performed using Review Manager software (RevMan 5.3).Results A total of 9 Randomized Control Trial (RCT) studies from Europe, America, Asia and Africa were selected for systematic review and meta-analysis. COVID-19 patients receiving Ivermectin therapy decreased the duration of hospital stay by 1.52 units compared to standard therapy and this result was statistically significant (SMD= -1.52; 95% CI= -2.34 to -0.70; p= 0.003).Conclusion: Ivermectin can reduce the duration of hospital stay for COVID-19 patients.Keywords: Massage, cerebral palsy, Modified Ashworth Scale, meta analisisCorrespondence: Salma Muazarroh.     Effect of Virgin Coconut Oil Administration to Increase HDL, Decrease LDL and IL-6 in Hypercholesterol Male Wistar Rats

    Background: Hypercholesterol is a change in blood lipid profile levels, from rising cholesterol levels that can be triggered by frequent consumption of fatty foods. Hypercholesterol causes adipose tissue and macrophages to release inflammatory cytokines, then adipose cells release IL-6 and spur the formation of CRP. This study aimed to determine the effect of Virgin coconut oil (VCO) on changes in HDL, LDL, and IL-6 levels in hyper cholesterol mice.Subjects and Method: This randomized controlled trial was carried out at the PSPG UGM Nutrition Laboratory in August 2021. The total number of samples used was 20 tails and 4 spare tails, namely 24 male Wistar strain rats. Sampling using simple random sampling. The independent variables in this study were the dose of VCO 0.9 ml/200g BW/day, and 0.45 ml/200g BW/day. The dependent variables in this study were HDL, LDL, and IL-6 cholesterol levels in male Wistar rats. The data in the analysis used a normality test with the Shapiro-Wilk test and a data homogeneity test with the Levane test. HDL dan LDL levels were measured using a lysis reagent, while IL-6 was measured using the ELISA method.Results: Average LDL and HDL levels are highest in the P1 group compared to the P2, K0, and K1 groups. The One Way Anova test on HDL levels showed a significant difference between groups with p<0.001. One Way Anova test results on LDL and IL-6 levels showed significant differences between the groups (p<0.001) and (p= 0.004).Conclusion: Administering VCO at a dose of 0.9 mL/ 200 g BB/day 0.45 mL/200 g BB/day, and 86.4 mg/200 g can increase HDL and IL-6 levels in male rats Wistar strain with hyper cholesterol and lowered LDL levels by the same dose in male mice with hyper cholesterol.Keywords: hyper cholesterol, HDL, LDL, IL-6Correspondence: Meli Maulidia, Magister Ilmu Biomedik, Universitas Islam Sultan Agung Semarang.     The Efficacy of Aquatic Therapy for Pain in Knee Osteoarthritis: A Systematic Review and Meta-Analysis

    Background: Osteoarthritis (OA) is a joint disease that affects adults to the elderly in which the incidence and prevalence of OA will increase with age. One of the interventions to reduce pain for improving the quality of life in patients with knee osteoarthritis is by providing aquatic therapy. This study aims to determine the effectiveness of aquatic therapy for pain in patients with knee osteoarthritis.Subjects and Method: It was a metaanalysis study using PICO. P: patients with knee osteoarthritis. I: aquatic therapy. C: nonaquatic therapy. O: reduced pain. The articles used in this study were obtained from several databases namely Google Scholar, PubMed, NCBI, Science Direct, and Springer Link. The keywords used were "osteoarthritis knee" OR "OA knee" AND "aquatic" OR "aquatic physiotherapy" OR "aquatic therapy" OR "aquatic exercise" OR "hydrotherapy" AND "quality of life" OR "QOL" AND "pain" and "RCT" or "randomized control trial". The article included in this study was a fulltext article with a Randomized Controlled Trial study design. The Review Manager 5.3 application was used to analyze articles. Results from the metaanalysis were reported using the PRISMA flow diagram.Results: A total of 9 articles that had been analyzed were from Spain, Thailand, Brazil, Australia, New Zealand, Korea, and Italy.     Effect of Zingiber officinale Rhizome Powder on Fasting Blood Sugar Levels and HbA1c in Type 2 Diabetes Mellitus Patients: A Meta-Analysis

    Background: Diabetes mellitus (DM) type 2 is the most common type of diabetes caused by insulin resistance and relative deficiency of insulin secretion. Several studies have shown inconsistent results regarding the effect of Zingiber officinale powder in patients with type 2 DM. As a result of these inconsistencies, researchers are compelled to prove the effect of Zingiber officinale powder on fasting blood sugar (GDP) and HbA1c levels in type 2 DM patients through a meta-analysis study.Subject and Method: This study is a meta-analysis of a number of randomized controlled trials. The articles were obtained from the PubMed, Science Direct, Springerlink and Google Scholar databases published in 2012-2022. The search articles were carried out by considering the feasibility of the criteria determined using the PICO model. Population: type 2 DM patients, Intervention: Zingiber officinale rhizome powder, Comparison: placebo, Outcome: GDP and HbA1c levels. The keywords used are “Zingiber officinale” OR “Zingiber” OR “Z. officinale” OR “Ginger” AND “Fasting blood glucose” AND “HbA1c” OR “A1c” OR “Glicemic” OR “Glucose” OR “Diabetes Mellitus” OR “type 2 diabetes mellitus” AND “Randomized Controlled Trial”. Article selection with PRISMA flow diagram. Analysis of articles using the Revman 5.3 application.Results: A total of 11 articles were reviewed in this study. Analysis of 11 articles showed that administration of Zingiber officinale rhizome powder could significantly reduce GDP levels in type 2 DM patients (Standardized Mean Difference (SMD)= -0.79; 95% CI= -1.32 to -0.26; p= 0.004). Analysis of 10 articles showed that administration of Zingiber officinale rhizome powder could significantly reduce HbA1c levels (SMD = -0.38; 95% CI= -0.55 to -0.22; p<0.001).Conclusion: Zingiber officinale rhizome powder can significantly reduce GDP and HbA1c levels in type 2 DM patients.Keywords: Zingiber officinale, diabetes mellitus tipe 2, GDP, HbA1cCorrespondence: Fajar Novianto.     Multiple Segments Thoracal Spinal Canal Stenosis Caused by Ossification of Ligamentum Flavum: A Case Report

    Background: Thoracal spinal canal stenosis (SCS) that refers to multiple segments of the thoracic spine is a peculiar clinical condition. Various factors can cause thoracic SCS, including ossification of the ligamentum flavum (OLF). The aim of this study is to explain about the diagnosis for multiple segment thoracal spinal canal stenosis and management properly to prevent further complications.Case Presentation: A 58-year-old male came to Orthopaedic Outpatient Clinic in Prof. I.G.N.G Ngoerah Hospital, complaining back pain since one year prior to admission. Pain aggravated when he was bending his back. The patient was diagnosed with Thoracal Spinal Canal Stenosis Th 8-9, Th9-10, Th10-11. Patient underwent decompression, stabilization, fusion and biopsy surgery.Results: Patient underwent decompression, stabilization, fusion and biopsy surgery at RSUP Prof. I.G.N.G Ngoerah. In durante operation, already done laminectomy and eight pieces of pedicle screw was applied in Th8-Th11 (55mm x 40 mm Monoaxial on right side Th8, 55mm x 40 mm Polyaxial on left side Th8; 55mm x 40 mm Monoaxial on right side Th9, 55mm x 35 mm Monoaxial on left side Th9; 55mm x 40 mm Monoaxial on right and left side Th10; 55mm x 40 mm Monoaxial on right and left side Th11). There were no complications found in this patient after 4th day following surgery.Conclusion: The goal of surgical intervention for SCS is decompression by removing the calcific areas that are responsible for the spinal canal narrowing and subsequent cord compression. Although the diagnosis and choice of intervention can be difficult, early detection and management are critical to postoperative success.
