理学療法士（PT）は，機能不全や歩行再建を検討する際に，観察による歩行分析（Observational gait 
analysis：OGA）を用いる．OGAは特別な機器を必要とせず評価可能であるという利点を有する一方で，
PTがその技術を獲得するためには長期のトレーニングを要する．また，多くの PTが OGAの実施に困
難感を有していることや，OGAの信頼性・妥当性の不十分さが報告されている．これらの報告を踏まえ
課題を整理すると，以下のようにまとめられる．すなわち，１）困難感をもたらす要因は明らかにされて
いないこと，２）先行研究の実験デザインの質に対する指摘が報告されていること，３）OGAの欠点を
補う観察方法やトレーニング方法が検討されていないこと，である． 
 
そこで，予備的調査では，PTを対象にアンケート調査を行い，歩行分析における困難感の程度や，そ
の要因を明らかにすることとした．歩行分析は運動学的観察と逆運動力学による推論から構成されるが，
今回は客観的に評価可能な「運動学的観察」に着目し調査を進めた．対象は，無作為に抽出したPT 3,840
名とした．アンケートは，歩行分析の困難感に関する設問と，観察の過程に関する設問から構成された．
その結果，7割を越えるPTが歩行分析に困難感を有し，その要因に観察精度が関連していることが示唆
された． 
 
第 1,２研究は，予備的調査で得られた困難感を形成する要因としての観察精度について明らかにする
ことを目的とした．本研究では PT 20名を対象に，歩行障害を呈した患者の歩行映像を観察させ，歩行
中の関節角度を評価させた．その結果，全般的に良好な信頼性を示し，OGAを臨床で用いることの一つ
の根拠を得ることができた．一方で，各関節部位にて，系統誤差が認められる部位とそうでない部位が存
在し，観察部位に応じたトレーニング方法の必要性が示唆された．また，観察する部位に焦点を絞ること
で観察精度が向上する可能性がみられ，トレーニング方法の開発に有益な情報となった．

Physical therapists (PTs) use observational gait analysis (OGA) when considering dysfunction and 
gait reconstruction. OGA has the advantage that it can be performed without special equipment. 
However, acquiring this skill requires long-term training. It has been reported that many PTs have 
difficulty with OGA and that it lacks reliability and validity.  
Based on the reports to date, the problems can be summarized as follows. In other words, 1) the 
factors causing the difficulty have not been clarified; 2) there are reports that point to the quality of 
the experimental design of previous studies; 3) observation and training methods that correct for the 
weaknesses of OGA have not been investigated. 
 
Therefore, in the first study, we conducted a questionnaire survey of PTs to clarify the level of 
difficulty in gait analysis and the factors contributing to it. Gait analysis consists of kinematic 
observations and inverse kinematic inferences. In this study, we focused our investigation on 
``kinematic observation'', which can be objectively verified. Subjects were 3,840 randomly selected 
PTs. The questionnaire consisted of 1 question about the difficulty of the overall gait analysis process 
and 12 questions specifically about the difficulty of the observation process. The results showed that 
more than 70% of the PTs had difficulties. It was also suggested that the difficulty factor was related 
to observation accuracy.  
 
The purpose of The first and second studies was to clarify observation accuracy which is a factor that 
makes the difficulty obtained in the pilot study. 20 PTs were asked to observe videotapes of patients 
with gait disorders and assess joint angles during gait. The results generally showed good reliability. 
This provides a basis for clinical use of the OGA. On the other hand, for each joint there were parts 
where systematic errors were observed and parts where they were not, suggesting the need for 
training methods that specifically target the observed parts. It was also found that focusing on the 
observed part can improve the observation accuracy, providing useful information for the 
development of training methods. 
 
We hope that the results of this study will provide fundamental data for the consideration of OGA 
observation and training methods. In addition, we believe that this will provide the findings that will 
help to solve the problems with the OGA evaluation method.

理学療法士は対象者の最適な運動パターンの習得を促す役割があり，動作の再獲得において
言語指導は欠かす事のできない手段である．言語指導は簡潔に提示することが推奨されている
ため，言語指導の中でも短い運動命令の形式で与えられる「言語強化」が有効と考えられるが，
動作指導における言語強化の効果について検証されていない．よって，本論では「言語強化の
効果の解明」を主な狙いとした． 
 
動作指導を想定した言語強化に関する研究は皆無に等しいため，まずは動作指導場面におけ
る言語指導の使用状況の整理を目的に観察的研究を行った．その結果，最も使用頻度の高かっ
た言語指導は「言語強化」であり，また「立ち上がり動作」で頻繁に使用されていた．したが
って，本実験では言語強化の効果検証を行う課題として立ち上がり動作を採用し，離殿のタイ
ミングの指導に着目することとした． 
 
 第 1実験では，理学療法士が立ち上がり動作に対して与える言語強化の特徴を探索的に検証
することを目的とした．事前に運動量戦略と安定戦略のモデル映像を一つずつ撮影した。14名
の理学療法士にモデル映像を視聴させ，立ち上がり動作の離殿を促す言語強化のタイミングを
分析した．その結果，動作戦略によらず，体幹前傾運動範囲の 85%付近で言語強化を与えられ
ることが明らかとなった． 
 
 第2実験では，健常成人の離殿タイミングの指導における言語強化の効果検証を目的とした．
20名の健常成人を運動量戦略誘導群と安定戦略誘導群に割り付け，第1実験で得られたデータ
をもとに一定のタイミングで言語強化を与える 10 試行，その後言語強化を与えない 3 試行を
実施した．その結果，言語強化を与えない試行において離殿と言語強化のタイミングが近づく
ことが明らかとなった．したがって，離殿を促す言語強化を一定に与えることで離殿のタイミ
ングを学習し，学習した離殿タイミングは言語強化を与えない試行で再現されることが示唆さ
れた．

Physical therapists have a role in facilitating the acquisition of optimal movement patterns. Verbal 
guidance is an indispensable tool for motion reacquisition. Verbal reinforcement in the form of short 
motor commands is regarded as effective. We recommend that verbal guidance be given in short 
sentences. Nevertheless, verbal reinforcement’s effectiveness for motion guidance has not been verified. 
Therefore, the main aim of this study was clarification of verbal reinforcement effects. 
 
 No report of the relevant literature describes a study of verbal reinforcement in the context of 
motion guidance. Therefore, we conducted an observational study to ascertain the actual situation related 
to verbal guidance use in motion guidance situations. Results show verbal reinforcement as the most 
frequently used verbal guidance, and as frequently used for the sit-to-stand task. Therefore, the 
sit-to-stand task was adopted to verify verbal reinforcement effectiveness, with emphasis on guidance for 
the timing of the seat-off . 
 
 The first experiment was conducted to verify characteristics of verbal reinforcement given by 
physical therapists for seat-off guidance. Before the experiment, model videos of the sit-to-stand task for 
each momentum and stabilization strategy were recorded. Then 14 physical therapists were asked to 
watch the model videos and to analyze the timing of verbal reinforcement to encourage the seat-off 
during the sit-to-stand task. The findings revealed that verbal reinforcement was given for around 85% of 
the forward trunk inclination range of motion, irrespective of the motion strategy. 
 
 The second experiment was aimed at verifying verbal reinforcement effectiveness for seat-off 
timing guidance for healthy adults. After 20 healthy adults were allocated to a momentum strategy 
guidance group and to a stabilization strategy guidance group, they completed 10 trials of verbal 
reinforcement given at a certain timing based on data obtained from the first experiment, followed by 3 
trials without verbal reinforcement. Results showed that the time lag between the seat-off and verbal 
reinforcement was shorter in trials without verbal reinforcement. The findings suggest that subjects can 
learn the seat-off timing in trials with constant verbal reinforcement, and that the learned seat-off timing is 
reproducible in trials without verbal reinforcement.
