Expression of Myogenic Regulatory Factors in the 
   Pharyngeal Muscle of the Mouse Embryos

   Morphogenesis of the pharynx, which is involved in swallowing, remains unclear. In 
the present study, the expression of myogenic regulatory factors: MyoD, Myf5, myo-
genin, and MRF4, were immunohistologically examined using mouse embryos. Myf5 
protein was expressed in the caudal pharynx at embryonic day 12 (E12), while MyoD, 
myogenin and MRF4 proteins were expressed at  E13. Subsequently, the expressions 
spread from the caudal to cranial pharynx over time by E15. Thus, muscle precursor 
cells in the pharyngeal muscles differentiate into mature myotubes during the embry-
onic stages and they spread throughout the entire pharynx by E15.

             Introduction 
 Swallowing is a movement to send ingested 
food to the stomach through the pharynx 
and esophagus. This movement starts in the 
 embryo1). Also, tongue and head movements 
start in the  embryo2). The morphogenesis of 
skeletal muscles of the areas are critical for 
these movements. The pharyngeal muscles 
are skeletal muscles involved in  swallowing3). 
Several reports have been published on skeletal 
muscles involved in oral  movements4,5). Tongue 
muscles involved in tongue movements complete 
maturation at  birth4). The masseter muscles 
remain immature at  births5). On the other 
hand, few reports have been published on the 
morphogenesis of the pharyngeal muscles. The 
timing of the morphogenesis of the pharyngeal 
muscles remains unclear. 
 The skeletal muscles are formed through 
the differentiation and maturation of muscle
precursor cells into myoblasts and myotubes. 
In these processes, myogenic regulatory factors 
(MRFs), transcription factors that are specifically 
expressed in the skeletal muscles, play an 
important  role6). The MRFs include Myogenic 
differentiation 1  (MyoD)7), Myogenic factor 5 
 (Myf5)8),  myogenin9,10), and Myogenic factor 6 
 (MRF4)11). MyoD and Myf5 are involved in the 
differentiation of muscle precursor cells into 
 myoblasts12,13,14). Myogenin is involved in the 
differentiation of myoblasts into  myotubes15.16). 
MRF4 expression is involved in myotube 
 maturationtion17). 
 In the present study, the development and 
morphogenesis of the pharyngeal muscles were 
examined along with expressions of MRFs 
proteins. 
        Materials and Methods 
 Pregnant ICR mice were purchased (Nippon 
Clea, Tokyo, Japan). ICR mice at embryonic
days  11, 12, 13, 14 and 15  (E11, E12, E13,  E14 
and E15) were used in this study. Mice in this 
study were used in accordance the Guidelines 
for Animal Experiments at Ohu University 
(2014-14). Mice were deeply anesthetized 
with dimethyl ether. Mice were euthanized to 
remove embryos. The embryos were fixed in 4% 
paraformaldehyde-phosphate buffer (pH7.4) at 
4°C for 24 hours. Then, to observe the midline 
region, paraffin blocks were made according to 
routine procedures to prepare serial sections 
with a thickness of 5 µ m in a sagittal direction. 
 For morphological observation, hematoxylin 
and eosin  (H-E) staining was conducted with 
an optical microscope  (Optiphot-2, Nikon, 
Tokyo, Japan). 
 Immunohistochemical staining was routinely 
conducted to examine the protein expressions 
of the MRFs. After blocking in 3% hydrogen 
peroxide methanol at room temperature for 10 
minutes, polyclonal rabbit anti-MyoD  (1 200, 
Bioss, Boston, MA, USA), polyclonal rabbit 
 anti-MyfS  (1 500, Bioss, Boston, MA, USA), 
monoclonal rabbit anti-myogenin  (1 200, 
Abcam, Cambridge, UK), and polyclonal rabbit 
anti-MRF4  (1 500, Bioss, Boston, MA, USA) 
antibodies were employed as primary antibodies. 
Staining was conducted at 4°C overnight. After 
washing with phosphate-buffered saline (PBS 
pH7.4), Histofine Simple Stain MAX-PO (R) 
(Nichirei Biosciences., Tokyo, Japan) was applied 
as a secondary antibody at room temperature 
for an hour. Finally, Diaminobenzidine coloring 
(Nichirei Biosciences., Tokyo, Japan) reaction 
was conducted for observation with an optical 
microscope  (Optiphot-2, Nikon, Tokyo, Japan). 
               Results 
 1.  E11 
 In the  Ell mouse, mesenchymal cells scattered 
throughout the entire pharyngeal subepithelial 
connective tissue, although they accumulated
at a higher density in the caudal pharynx (Fig. 
 1A). In the  E11 mouse, none of the MRFs was 
positive in the pharynx (Fig.  1B-  E). 
 2.  E12 
 In the  E12 mouse, the primordial hyoid bone 
was found in the cranial larynx. In addition, 
the primordial first cervical vertebra was noted 
in the dorsal pharynx. In posterior pharyngeal 
wall, the cell density was lower in the cranial 
pharynx, and was higher in the caudal pharynx 
with cell accumulation (Fig. 2A). In the  E12 
mouse,  Myf5 proteins was positive in the caudal 
pharynx relative (Fig.  2B-E). 
 3.  E13 
 In the E13 mouse, the primordial cricoid 
cartilage was found in the ventral pharynx. 
 In posterior pharyngeal wall, the cells 
accumulated around the upper end of the first 
cervical vertebrae cranially to the reference line. 
In anterior pharyngeal wall, the cell at a higher 
density in the caudal pharynx (Fig. 3A). In the 
 El3 mouse, MyoD,  Myf5, myogenin, and MRF4 
proteins were positive in posterior pharyngeal 
wall, upper end of the first cervical vertebrae 
cranially to the reference line. In anterior 
pharyngeal wall, none of the MRFs protein was 
positive in the pharynx (Fig.  3B-  EX 
 4.  E14 
 In the  E14 mouse posterior pharyngeal wall, 
the cell density was higher throughout the 
entire laryngopharynx. In anterior pharyngeal 
wall the cell density accumulated around the 
upper of the cricoid cartilage (Fig. 4A). In the 
 E14 mouse posterior pharyngeal wall, MyoD, 
 Myf5, myogenin, and MRF4 proteins were 
positive throughout the entire laryngopharynx. 
In the anterior pharyngeal wall, MyoD,  Myf5, 
myogenin, and MRF4 proteins were positive 
around the upper of the cricoid cartilage (Fig. 
 4B-E). 
 5.  E15 
 In the  E15 mouse posterior pharyngeal wall,
cells were noted even in the nasopharynx, and 
the cell density was higher throughout the 
entire pharynx. In the anterior pharyngeal 
wall, cell accumulation was higher than  E14 
mice in the upper pharynx (Fig. 5A). In the E15 
mouse, the four MRFs proteins were positive 
throughout the entire pharynx (Fig.  5B-E). The 
timing of MRFs expressions is shown in Table 1. 
              Discussion 
 In the mouse pharynx, the nasopharynx is 
located between the nasopharyngeal hiatus and
nasal cavity, and the laryngopharynx is located 
between the nasopharyngeal hiatus and rostral 
fourth of the plate of the cricoid  cartilage18). 
In the  E12 mouse, the cell density was lower 
in the cranial pharynx, and was higher in the 
caudal pharynx with cell accumulation by the 
morphological observation. Myf5 protein was 
positive. Myf5 is involved in the differentiation 
of muscle precursor cells into  myoblasts12,13,14), 
suggesting that differentiation into myoblasts 
starts at  E12 in the pharynx. In addition, 
since the expression site of Myf5 protein
corresponded to the cell accumulation site in 
the caudal pharynx, muscle precursor cells 
may accumulate in the pharynx. Myogenin is 
involved in the differentiation of myoblasts 
into  myotubes15,16), and the expression of MRF4 
implicates myotube  maturation17). It is suggested 
that myoblast differentiation coincides with 
myotube differentiation and maturation in the 
pharyngeal muscles. Although MyoD protein 
was negative at  E12, MyoD protein was positive 
at  E13. MyoD and Myf5 are involved in the 
differentiation of muscle precursor cells into 
 myoblasts12,13,14). The expression of myoD mRNA 
was later than that of Myf5 mRNA during 
mouse  development19,20,21,22). 
 At  E14 when the primordial pharyngeal
cartilage appeared, all the four proteins were 
positive in the laryngopharynx. At  E15, they 
were positive in the cranial nasopharyngeal 
region. Thus, pharyngeal cells proliferate from 
the caudal to cranial regions over time and 
also in the  MRFs- proteins positive regions, 
suggesting that the pharyngeal muscles are 
formed from the caudal to cranial regions. The 
posterior pharyngeal wall is formed earlier 
than the anterior pharyngeal wall, because 
the former shows earlier cell accumulation and 
positive reactions for MRF proteins than the 
latter. 
 The esophagus adjacent to the pharynx 
consists of the same  foregut-  derived tissues as 
the pharynx. In the esophagus of the embryo,
the mRNA expressions of MRFs are first noted 
in the cranial regions close to the pharynx and 
subsequently in the caudal  regions19). Thus, 
myogenesis in the pharynx and esophagus may 
start at and spread from their adjacent site. 
In the esophagus, Myf5 mRNA was expressed 
first, followed by myogenin, MRF4, and MyoD 
mRNAs on the following day, suggesting that 
the esophagus is formed in the same manner as 
the pharynx. 
 The expressions of MRFs mRNA in the 
embryo differ with sites. In the trunk, Myf5 
mRNA is first expressed at E8, myogenin 
 mRNA  at E8.5, MRF4  mRNA  at  E10 and 11, and 
MyoD mRNA at  E10.5. In the skeletal muscles 
of the limb bud, Myf5 mRNA is expressed at
 E10-12, myogenin mRNA and MyoD mRNA 
at  E10.5, and MRF4 mRNA at  E1620,21,22). In the 
skeletal muscles involved in oral functions, 
MyoD mRNA, Myf5 mRNA, and myogenin 
mRNA are expressed at  Ell, and MRF4 mRNA 
at  E13 in the mouse tongue  muscles4). The 
movement of the mouse tongue starts at  E13- 
14 2), although the maturation of skeletal muscle 
was completed at birth in the tongue muscles, 
but not in the masseter  muscles5). The initiation 
of suckling by the tongue muscles differs from 
that of biting by the masseter  muscles23). Thus, 
muscle maturation may be associated with the 
muscle  functione. 
 In the present study, MRF4 protein was 
positive at  E13, indicating myotube maturation,
and spread throughout the entire pharynx at 
 E15, suggesting that all pharyngeal muscle 
tissues matured at E15 or later depending on 
the timing of swallowing in the embryo.
              Conclusion 
 The morphogenesis of the pharyngeal muscles 
starts in the proximal part of the esophagus , 
and all pharyngeal muscles mature by  E15, 
as revealed by the expression of MRF4. These 
findings may be associated with the start of 
swallowing.

化学物質曝露と健康影響
～雄性生殖機能 か らのアプ ローチ～

はじめに
世界には化学物質(医 薬品を含む)が 約3,000
万種存在するといわれている。その中で医薬品は,
開発過程で実験動物 を対象として,一 般毒性 ・遺
伝毒性 ・生殖毒性試験などが実施 され,さ らに臨
床試験 を実施することで,人 に対す る有害作用 ・
副作用に至るまで詳細な検討が行われてから,上
市 され る。一方,医 薬品以外の化学物質について
は,毒性の検討は必須ではない。 さらに,新 たな
化学物質は 日々現れているので,一 般 に使用 され
る化学物質の中で,人 の健康に影響を与える可能
性があるもの全てについて迅速に検討することは
不可能である。
本稿では,医 薬品を含む化学物質曝露が健康に
及ぼす影響について,我 々の研究成果の一部を紹
介 し,さ らに本分野の今後の研究の方向性につい
て解説する。
精子数の変化
1992年にCarlsenらによって,精 子数(濃 度)
が過去50年間に半減 したという報告がな され,そ
の論文の中では,同 時に精巣腫瘍や尿道下裂など
の男児生殖腺奇形の発生率が高 くなっていること
も述べ られた。 さらに,そ の原因の1つ として,
内分泌撹乱化学物質(い わゆる"環境ホルモ ン")
が取 り沙汰 されたことか ら,この論文は世界的に
注 目を集めたのである。
私は前任の帝京大学医学部泌尿器科在勤中に,
科学研究費および旧科学技術庁振興調整費の支援
を受けて,全 国,400名余 りを対象にして精液調
査を行い,(1)年齢で区切 ると,若 い年代 ほど精
子数が少ないこと,(2)地域差が存在す ること,
(3)都内近郊在住者の20代に絞 ると,1980年代
に比較 して2000年前後では精子数減少な ど,精
液性状の悪化が認められることを報告 している。
現在では,経 年的にヒ ト精液性状は全般 に悪化 し
ていること,精子濃度には人種差が存在するとい
うことで,コ ンセンサスが得 られている。
これらの結果をより精度の確かなものにするに
は,永 年にわたって同 じ年代の男性の精液検査 を
し続けるとい う地味な仕事が必要である。本分野
ではデンマークの結果がよく引用 され るが,彼 の
地では20歳の徴兵検査時に精液検査が一般健康
診断の一部として組み込まれているとのことであ
る。 しか し,わが国ではこのよ うな地味な研究に
は公的研究費 も付かなくなっているので,わ が国
の男性生殖機能変化に関する基礎的調査研究は止
まった状態にある。
雄性生殖毒性研究
生殖はその英名であるreproductionが示す と
おり,生物の最 も基本的かつ重要な機能である子
孫をつ くる過程であり,そ こに何 らかの異変が起
こるとその種の存続に直接影響する。 このように,
生殖過程に及ぼす影響を検討することは重要なの
で,毒 性試験のなかには 「生殖毒性」 という項 目
が存在 している。そこでは,受 精,性 周期,受 胎
能,分 娩の異常,産 仔の形態異常などの機能異常
などが検討 されている。これ らは,主 として雌性
生殖過程に関する事項である。これは,サ リドマ
イ ド事件の反省か ら,胎児への影響を検討するこ
とが始まったことに起因 している。
一方,生 殖 はもちろん,雌 だけでは成立せず,
雄の生殖能に関する検討もすべ きであるとの議論
が30年ほ ど前か らお こり,成 獣曝露による精子
形成能や精子受胎能への影響が検討項 目に選ばれ
るようになった。 しかし,次世代影響に関 しては,
妊娠する雌と異な り,精子は受精 させて,遺 伝子
を与 えるだけで役割が終わると考えられているた
めか,医 薬品の開発時に検討 されることはなく,
その他の人に係わる化学物質において も今のとこ
ろ検討 されていない。
医薬品と精液性状
医薬品や健康食品,サ プリメン トの服用による
生殖への影響は,検 討 される必要がある分野では
あるが,毒 性を除いては十分な検討が行われてい
るとはいえない。
前任校在勤中の自験例では,不 妊外来 を受診 し
た患者 さんに,服 薬歴を尋ねたところ,一 例目は
クローン病 に対す るスルファサラジンを服用中,
二例 目は受診10年前に胃がんの手術 を受け,現
在,5-FU服 用中という例 があった。 この2薬
はともに,精 子性状を悪化させることが添付文書
またはインタビューフォーム(IF)に記載 されて
いるが,主 治医にま現病治療 を優先 して投薬 したも
のである。ともに主治医 と連絡をとったが,前 者
はクローン病治療を優先 したので服薬 を継続 し,
精液性状は改善 しなかったが,後 者では服薬中止
により精液性状が回復 した。
上述の5-FUの よ うに抗がん剤を服用中とい
うことであれば,造 精機能障害が起 こることは容
易に推測可能であるが,そ の他の薬剤については
すぐにはわからない場合 も多い。 したがって,必
要に応 じて,添 付文書だけでな くインタビュー
フォーム(IF)まで遡って調べ ることが必要 となる。
最近の例では,男 性型発毛治療薬であるプロペシ
ア(finasteride)は,IFでは毒性試験で投与後の
雄 ラットを繁殖 に用いるとその産仔が減少す ると
いう記載があるが,添 付文書には記載 されていな
い。 これ らは,ド ラッグインフォメーション活動
の一環であるので,薬 剤師に依頼す ることで情報
が得 られる。
次世代影響から先制医療へ
化学物質等の健康影響 に関 して,過 去20年間
程を顧みた場合,デ ィーゼル排ガス,内 分泌かく
乱化学物質(環 境 ホルモン),PM2.5(微少粒子
状物質)などが大 きな社会問題 となって きた。我々
は,こ れらの物質の影響に対 してその初期から研
究 に取組み,今 までにデ ィーゼル排 ガス曝露
PM2.5やその本体であるナノ粒子曝露により,成
獣曝露時だけでなく,胎仔期曝露で生 まれた雄が
成獣になってから精子形成能が低下すること,神
経系の発達が障害 を受けること,さ らに化学物質
曝露が性決定機構に影響 を及ぼすことなどを報告
している。
上記の私たちの研究のみならず,親(雌 雄を問
わず)に 化学物質曝露などを行ったあと,正常雌
と交配してその産仔を得て,飼 育す ると,産仔が
その成長に伴って種々の病態 を示すことが,最 近
報告 されるようになって きた。
最近の疫学研究では,妊 婦がダイエッ トなどで
低栄養状態にあると,出 生児は低体重児とな り,
その子は将来的に “肥満 ”“糖尿病 ”“高血圧 ”な
どの発症 リスクが高 くなるとい う報告がある。 さ
らに,高 齢父親の子は母親の年齢に係わらず,自
閉症 リスクが上昇するという報告 もある。
この考 えは,以 前 は,「次世代影響」の一部 と
いわれてきたが,最 近の研究成果から,「受精時,
胎芽期,胎 児期の子宮内及び乳幼児期の望 ましく
ない環境がエピゲノム変化を起 こし,それが疾病
素因となり,出生後の環境 との相互作用によって
疾病が発症する。生活習慣病等の多因子疾患はこ
の2段 階 を経 て発症 す る。」 とい う 「DOHaD
(DevelopmentalOriginsofHealthand
Disease)」学説と呼ばれるようになっている。 こ
れは,次 世代の安全 と健康のためには,出 生前の
親の段階から健康維持に努める必要があることを
示唆するものであり,今後の健康問題を考える上
で,新しい切 り口を与えるものとして 「先制医療」
という考え方が提案 されている。
おわりに
化学物質は医薬品を含めて,服 用(投 与〉によ
り人に何 らかの影響が生 じるが,そ の詳細な機序
や正 しい評価方法はまだわからないというのが実
情である。
本稿が,診 療上,化 学物質に曝露 される可能性
が高い歯科医師の皆様 に対 して何 らかの情報提供
になれば幸いである。
本論文の要旨は第60回奥羽大学歯学会(平成27年11月
14日郡山市)特別講演において発表した。

前突 症患者 の理想 とす る顔貌 の評価

【目 的】 骨格性 下顎前突症 患者 と矯 正医 の理想
とす る顔 貌の認識 の差異 を検 討す ること。
【対 象】骨格性下顎 前突症 と診 断 され た39名(男
性19名,女 性20名)を 対 象 と し,男 女 それ ぞれ を
Long-face群(男性10名,女 性10名),Normal-
face群(男性9名,女 性10名)に 分類 した。比 較
対象 は,平 均 臨床 経験21.4年の 日本 矯正 歯科 学会
認定医10名とした。
【方 法 】VIVID910(KONICA MINOLTA)を用
いて,骨 格 性下顎 前突症患 者の咬 頭嵌合位 の顔 貌
を撮影 した。得 られたデ ー タをポ リゴン編集 ソフ
トで立体構築 され たデ ー タへ と変換 し,三 次元顔
貌画像 を作成 した。 この三 次元顔貌 画像上 に6つ
の 可 動 点(Ls,Stm,Li,Sm,Pogs,Mes)を
設定 し,水 平的,垂 直的 に 自由 に変 形 させ,患 者
に は自己理想顔 貌 を,矯 正 医 には各患者 の理 想顔
貌の描画 を指示 した。両群 間の統 計学的解析 には
Mann-Whitney U-testを用い た。
【結 果】 矯正 医群 と比較 して,患 者 は全 ての群
で,Ls,Stmを前方 方向へ描 画 した。矯正 医群 は,
Long-face群の男 性 と比 較 して,Mesを 上 方 へ
描 画 した が,Normal-face群と矯 正 医群 で は有
意 差 は 認 め ら れ な か っ た。Normal-face群,
Long-face群の女 性 は,矯 正 医 群 よ り も有 意 に
Mesを上方へ描 画 した。
【結 論】骨 格性 下顎前突症 患者 自身 と矯正 医 に
よ る理想顔 貌 を比較 した結 果 以下 の ことが示 唆
され た。① 男性患者,女 性患 者 と も,上 唇部 の前
後的 な認識 が矯正 医 と異 な る。②男性 患者,女 性
患者とも,下顎の前後的位置関係は矯止医と同 じ
認識である。③男性患者は顔の長 さに関 して寛容
であるが,女 性患者は矯正医よりも短い顔貌を理
想 とする。④骨格性下顎前突症患者の治療の際は,
顔貌形態や性差の影響を考慮 した治療計画の立案
が必要である。

9)オ トガイ部における舌孔の出現部位 と顎舌
骨筋 との位置関係

【緒 言】舌孔の有無が内出血に関連 している場
合がある。舌孔に関しての研究は行われているが
周囲の軟組織 との関係についての報告はなく,実
際の出現部位を特定できる研究はほとんどない。
そこで,顎 舌骨筋付着部に対す る垂直的位置関係
で把握す ることにより,舌孔の出現部位 と周囲軟
組織 との関連の推測が可能 となると考え検討を
行った。
【材料および方法】奥羽大学倫理審査委員会の承
認(承 認番号105号)を得た後,奥 羽大学実習用
遺体25体において顎舌骨筋 を剖出 し,下 顎骨へ
の付着部にマーキング したのちに,歯 科用コーン
ビームCTを 撮影 した。得 られた画像 データか ら
舌孔の出現部位と顎舌骨筋付着部を観察 した。
顎舌骨筋線を垂直的基準とし,上 方を舌下隙,
下方を顎下隙に分類 した。 さらに,顎 舌骨筋線の
垂直的位置の把握のため,正 中部におけるオ トガ
イ棘 と下顎下縁の距離(AC)と,各 部における
顎舌骨筋付着部と下顎下縁の距離(BC)を計測 し,
その比率(BC/AC)を求めた。近遠心的基準 と
して,オ トガイ棘外側縁とオ トガイ孔近前縁の間
を2等分 し,オ トガイ棘側およびオ トガイ孔側 と
して,舌 孔の出現部位 を分類 した。
【結 果】観察 された舌孔の全数は35個であり,
舌下隙15個中オ トガイ棘側で5個(33.3%)オト
ガイ孔側で10個(66.7%)であり,顎下隙20個中,
オ トガイ棘側で1個(5.0%),オトガイ孔側で19
個(95.0%)であった。顎舌骨筋線の垂直的位置
関係(BC/AC)は,オトガイ棘外側 縁では,ほ
ぼ中央(52.0%)であるのに対 し,オ トガイ孔前
縁では上方(79.2%)に位置 し,それ らの中央部
ではほぼ中間の高さ(66.6%)であった。
【考 察】このたびの検討では,オ トガイ棘側で
は舌下隙,オ トガイ孔側では顎下隙に舌孔が多く
開口していた。顎舌骨筋線の垂直的位置は,正 中
付近では下顎下縁からオ トガイ棘までの距離の中
央 に位置 し,小 臼歯部ではより上方に存在 してい
たことから,歯の喪失に伴 う形態変化の影響が少
ないオ トガイ棘をランドマークとした顎舌骨筋線
の垂直的位置の推測が,顎 骨手術に伴 う術後の内
出血の術前診断に有用であると考えられる。
【結 語】今回私たちは,オ トガイ部における舌
孔の出現部位 と顎舌骨筋 との位置関係について検
討を行 ったので報告 した。

17)総義歯に不満がある患者に対 しインプラン
トオーバーデンチャー(IOD)を実施 した
2症例

【緒 言】通常の義歯に対 して種々の不満のある
患者に対してはインプラン ト治療が極めて有効で
ある。 しか しボーンアンカー ドブリッジでは高額
な費用やメインテナンスの煩雑 さなどの欠点があ
る。10Dはその欠点 を回避 し,患 者の満足度 も
得ることが出来る治療 として有効 と考 えられ る。
今回,IODを実施 した2症 例の詳細 を報告する。
①47歳の男性
初診:H23/5
主訴:入 れ歯をやめてインプラン トにしたい。
現病歴:以 前かかりつけ歯科で歯周病が原因で
抜歯後,義 歯を装着 したが,外 れやすいことが気
になる。
②68歳の女性
初診:H25/12
主訴:義 歯の不適
現病歴:以 前より義歯の不適および開口時の脱
離が気になっていた。
【治療経過】
①33と43を抜歯 後に上下顎両側2・4部 にイン
フ゜ラン ト埋入(ブ ローネマル クシステムMarkⅢ
グルービー φ4×11.5mm4本)
5か月の待期期間後に2次 手術を行い,IOD(ド
ルダーバータイプ)を 装着 した。
②上顎義歯の調整後,上 顎両側2・4部 にインプ
ラン ト埋入(ブ ローネマルクシステムMarkⅢグ
ルービー φ4×10mm4本)
4か月の待期期間後に2次 手術 を行い,IOD(ド
ルダーバータイフ゜ )を装着 した。
【考 察】2症 例共 に,SPT開始後の検査 でイ
ンプラン ト周囲に透過像を認めたため,患 者に状
況 を説明 しSPTを継 続 して いる。Goodacreら
(2003)は,下顎IODおよび上顎固定性ブ リッジ
に比べ,上 顎IODの インプラン ト喪失率 は有意
に高いことを報告 した。患者の不良なコンプライ
アンス,喫 煙,イ ンプラン ト埋入後に旧義歯 を使
用 して患部を圧迫 したこと,ドル ダーバー使用に
よるインプラン ト周囲組織の清掃が困難であるこ
と,が インプラン ト周囲炎の関連因子と考 えられ
た。
【結 語】上顎IODは インプラン ト周囲炎に罹
患す る確率が高いため,下 顎10Dに 比較 して,
患者ごとの リスク評価と管理を徹底する必要があ
る。

省力化について考える

労力の省力化 を考 える上 でクローズア ップで きる話題 が,矢 沢永吉の 「やっちゃえ 日
産」 とくる 「セ レナ」 のプ ロパ イ ロッ トシステ ムで ある。車線,車 間,速 度,停 止の ア
シス トは運転者の労力が省力 され るメ リッ トもある反面,使 い方 によっては安全性 も危
惧 され る。実際,ア メリカにおいて,シ ステム的には 日産 のそれ よ りも省力度合 いが大
きいオ ー トパ イロ ッ トシステ ムを導入 してい る 「テス ラ　 モデルS」 で事故 が起 き死者
も出てい る。ただ これは ヒ トが運転 して いた トレー ラー との事故で ある。 この事故 につ
いて判 断が難 しいのは,「テ スラ」 に乗 っていた ヒ トが システ ムを解除 して いれ ば回避
で きたか も しれない し,む しろ トレー ラーもオ ー トパ イロッ トだったな ら起 きなかった
か もしれ ない とい うことであ る。現 時点で商品化 されてい る自動化 した機械 は整備 され
た環境下で,不 測 の事態 が起 きなけれ ば,ヒ トが操作 す るよ りも誤操作は少ない可能性
が高い と推察 され る。実 際に自動車 との レベル は違 うものの,我が家 にい る 「ル ンバ号」
は環境 を整備 してお くと確実 に仕事 を熟 して くる。つ まり現 時点で は自動化 された機械
を利用す る場 合,ヒ トがその機械 の特性 とその場 面 を勘案 して用 いる必要があ るとい う
ことで ある。
さて,歯 科関連 について はど うか。やは り省力化の筆 頭は 「CAD/CAM」になるだろ
う。 ヒ トが行 う金属,セ ラ ミックスの加工操作 を短縮 で きる。 スキャニ ング,色 調,細
部の形態修 正な どは ヒ トの手 が必要 となるが,機 械 が作業 を している間,他 の省略で き
ない作業に傾 注で きることは大 きなメ リッ トが ある。経済 的にはど うか とい う意 見 もあ
るだろ うが,そ こに関 して はや り方次第で あろ うと私 は考 えてい る。 この領域 は 「3D
プ リンタ」 も含め,今 後 さらに発展 して い く事が推察 され る。他の領域で はどうか。 印
象材の練和,麻 酔,根 管形成 などの領域 で も省力化 を して くれ る機械 がある。 しか し,
これ らと 「CAD/CAM」では省力の度合 いが異 なる。 そこはラボサ イ ドかチ ェアサ イ ド
かの違い ともい える。歯科 の場合,修 復物 は全てオー ダーメ イ ドとなり,画 一 された も
の を大量生産 す るわけではないため,ヒ トの手が随所 に取 り入れ られ るものの,修 復物
はや り直 しの利 く 「物」 であ る。作業の対象が不測な動 きをす るヒ トとな るとそ うはい
かない。 ヒ トに使 用す る機械 は安全で壊れ に くく,誤 操作 が生 じず,不 測の事態 に対応
で きなければな らない。 となる と省 力化 自体の割合 は少な くなるが,構 造が簡素 でロー
テ クな ものを使用 す る方が確実 で安全 だった りす る訳で ある。医科で は 「ダヴ ィンチ」
とい うロボ ッ トアーム と3D映 像 を駆使 す る手術用機械が ある。 これ は 「アメ リカ軍」
が戦場 で負傷 した兵士 を手術す るのに,名 医が現地に居な くて も,イ ンターネ ッ ト通信
を通 じて行 うことがで きるよ うに との思惑 か ら開発 され たもので ある。 「戦争 は発明 の
母」 などと言 われ るが,こ れ もその一端で あろう。現実 には患者のす ぐ脇 の操作台で術
者が操作 を行 うものであ る。 だがこの場合,開 発 コンセプ トに通 じる有 用性 は生 か され
てはいない。自身 でメスを握 って助手 を手足 の ように使 うの とどこが違 うのだろ う?と
思 った りもす る。 しか し,ヒ トの腕 は2本 しかないが,「ダヴ ィンチ」 のア ームは複数
本 ある。技能 が高度で あれ ば,他 人で あり,ス キル も異 なる複数の助手 に指示す るより
も,上 手 に手術 がで きるか も しれ ない。 また,手 術 には術者のほかに補助作業者が一人
いれば良いよ うで ある。前述 と意味合 いが若干異 なるが,こ れ も省力化 である。 しか し
なが ら,予 想 され たようにある程 度の確率 で トラブル は発生 しているよ うで ある。その
時に自分 が患者で ない ことを祈 るだけであ る。や は り構造が複雑 でハ イテクな機械 を導
入す ることは長所 も,短 所 もある。い くら高度化 しようとも機械 を過信せず あくまで も
便利 な道具 と して ヒ トは扱 うべ きであ る。 どれ だけ科学が進 もうともヒ トの研 鐙が最優
先で ある。
今後,歯 科 での省力化 を考 え ると,ラ ボサ イ ドでは さらに省 力化 を促進 した機械 が開
発,導 入 され るのは想像 に難 しくはない。 ただ,市 場規模,過 酷 な口腔内環境 での耐久
性,生 体 安全性,審 美性 までク リアす る材質 まで考 えると,他 業種で優れた ものが開発
されて も,歯 科領域 で即座 に実践導入 されない ことは容 易に想像がつ く。 しか し,技 工
士におけ る離職率,在 職者の年齢構成,入 学者 数の減少 などを考慮す ると,こ の問題が
近い将来,修 復物の供給 に影 を落 とすのは間違 いない。歯科医師 自体 が簡便 に操作,製
作 で きる高度 に省力化 した機械,そ れに付 随 した材料の 開発 を行 うことは急務で あろう。
一方,チ ェアサイ ドで は既 存の機械(器 械)の 特性 を生 か しつつ,完 全 な自動で はない
電動化,高 性能化,材 質改善が図 られてい くのではないか と考 えられ る。
私 も歯科理工学 と銘 打つ ところに所属す る以上 は,何 か しら歯科臨床 に貢献で きるも
のを開発 したい と思 ってい る。将 来,「プ ロフェ ッシ ョナル」 か 「情熱大陸」 に出演 す
る予定で ある!?

2)骨格系分類による日本人小児の舌圧 と顎顔
面形態 との関係

【目 的】小児期の不正咬合の原因には,舌癖や
口腔周囲筋の機能といった環境的要因が深 く関与
してお り,口腔周囲筋の機能的診断および治療効
果の客観的評価が求められている。 しか しながら,
舌の機能に対する評価基準はない。そこで本研究
では,小 児の舌圧を測定することで舌機能を定量
的に評価 し,顎顔面形態 との関連性 を明らかにす
ることを目的とした。
【資料および方法】奥羽大学歯学部附属病院矯正
歯科 を受診 した患児81名(男児28名,女児53名),
平均 年齢9.09±1.46歳,Hellmanの咬合 発育段
階によるⅢAか らⅢCを 選択 し,バ ルーン式舌
圧測定器(JMS舌圧測定器〉による最大舌圧お
よび嚥下時舌圧 を測定 した。また,口 唇閉鎖力測
定器(LIP　DE　CUM　LDC-110R)を用いて最大
口唇閉鎖力を測定した。初回検査時の側面頭部X
線規格写真によるセファロ分析,お よび非接触三
次元形状計測器(Vivid910)を用いて歯列模型 か
ら口腔容積を算出し顎顔面形態を評価 した。被験
者をSkeletal　I群(2°≦ANB≦4°),Skeletal
Ⅱ群(ANB>4°),SkeletalⅢ群(ANB<2°)
の3群 に分け,舌 圧と口唇閉鎖力,顎 顔面形態 と
の関連性 について統計学的検討を行った。
【結果 および考察】最大舌圧は,SkeletalⅡ群
と比較 しSkeletalI群,SkeletalⅢ群で有意 に
大 き く,嚥 下 時舌圧 はSkeletalⅡ群 と比較 し
SkeletalⅢ群で有意に大 きな値 を示 した。最大
口唇閉鎖力 は,skeletalI群,SkeletalⅡ群 と
比 較 しSkeletalⅢ群 で 有 意 に 大 き か っ た。
SkeletalⅢ群 におけるL1-MPは,SkeletalⅡ群
と比較 し有意に小 さく,下顎中切歯の舌側傾斜 を
示 した。 これは,嚥 下時に舌が上方に挙上す るた
め下顎中切歯に加わる舌圧は小さく,口唇圧が大
きいことにより舌側傾斜を示すと考えられた。 ま
た,最 大舌圧と嚥下時舌圧,嚥 下時舌圧 と口腔容
積との問に正の相関が認められた。
【結 論】小児期における最大舌圧,嚥 下時舌圧
および最大口唇閉鎖力ともに,前 後的な骨格系分
類と深 く関連 してお り,SkeletalⅡ群 と比較 し
SkeletalⅢ群で有意に大きいことが示 された。ま
た,最 大舌圧と嚥下時舌圧に正の相関を認め,こ
れらは舌の機能評価に有用であると考え られ た。
さらに,嚥 下時舌圧 と口腔容積との問に正の相関
が認め られたことから,口蓋形成 に舌の機能圧の
関与が示唆 された。今後我々は,本 研究で得 られ
た知見を矯正歯科治療における患児の舌の機能評
価に反映 させていきたいと考えている。
