    Outcomes of Impella 5.5-supported percutaneous coronary intervention in patients with cardiogenic shock.

    Objectives. The use of a microaxial flow pump (mAFP) to support high-risk percutaneous coronary intervention (HRPCI) in patients with cardiogenic shock (CS) is well established. A high-flow, surgically implanted mAFP (Impella 5.5, Abiomed/Johnson & Johnson MedTech) is increasingly used in patients with CS, yet little is known about the outcomes in HRPCI. The authors aimed to describe the outcomes, patient and procedural characteristics of Impella 5.5-supported HRPCI in a high-volume center. Methods. The authors identified all adult patients who underwent Impella 5.5-supported percutaneous coronary intervention (PCI) at Providence St. Vincent Medical Center from January 1, 2021, to December 31, 2024. Patient demographics and clinical data were abstracted from the medical record. Categorical data were reported as frequency and percentage. Continuous variables represented by intervals were summarized using mean and SD or median and IQR. Results. Of the 34 patients identified, 31 were male, and the mean age was 62.9 (± 9.7) years. All patients had a maximal CS stage of C-E. The mean left ventricular ejection fraction was 24.3%. Unprotected left main was treated in 50% of the study cohort, and 94% of the patients required multivessel PCI. The mean SYNTAX score was 35.9 (± 10.8) and the residual SYNTAX score was 7.4 (± 5.9). Major adverse cardiac and cerebrovascular events occurred in 41% of the patients. The 90-day all-cause mortality was 38%. Conclusions. Impella 5.5 is feasible to support HRPCI in patients with severe multivessel coronary artery disease, severe heart failure, and CS with acceptable short- and intermediate-term patient outcomes

    Characteristics of patients with liver tumors deemed ineligible for enrollment on Children\u27s Oncology Group trial AHEP1531: An opportunity to expand inclusion criteria and improve outcome.

    BACKGROUND: Specific patients with hepatoblastoma (HB) and hepatocellular carcinoma (HCC) do not meet eligibility criteria for Children\u27s Oncology Group (COG) trials, limiting an understanding of how comorbidities affect the outcome. We define such a population for future-focused care improvements. METHODS: A questionnaire was sent to COG institutional principal investigators to obtain anonymized data regarding patients with a liver tumor diagnosis not enrolled on AHEP1531 due to ineligibility by trial criteria or other reasons (excluding parent/patient preference). RESULTS: Responses received for 55 patients (38 HB, 4 HCC, and 13 not reported) included 12 excluded from further analysis due to various factors, including lack of additional information. Five patients were eligible but not enrolled due to pandemic-related staffing issues (n = 1) or the best interest of the patient (physician preference, n = 1; risk for poor tolerance of chemotherapy, n = 1; not specified, n = 2). The remaining 38 patients included 1 ineligible due to a prior malignancy, 9 due to performance status, and 4 due to timing requirements. Thirty-five of 38 patients were ineligible due to organ function criteria (pulmonary/oxygen requirement, n = 25; renal, n = 10; liver, n = 4; cardiac, n = 3). Seven patients were ineligible due to two or more organ function criteria. Twenty-five of 38 patients were reported to have a preexisting condition including 4 patients with trisomy 18. CONCLUSIONS: Patients with HB, and potentially HCC, who are ineligible for COG trials are primarily patients with an oxygen requirement and/or renal insufficiency, often associated with preexisting congenital conditions. Such patients would benefit from future studies to improve outcomes

    The Association of Bone Mineral Density with Distal Junctional Kyphosis and Distal Junctional Failure After Multilevel Spine Fusions: A Systematic Review and Meta-Analysis.

    BACKGROUND: Distal junctional kyphosis (DJK) and distal junctional failure (DJF) are known complications of adult multilevel spinal fusion surgery. Previous literature has extensively investigated proximal junctional kyphosis and proximal junctional failure, but DJK and DJF are relatively understudied. This study investigates the association between bone mineral density (BMD) and DJK/DJF via a systematic review and meta-analysis. METHODS: A literature search was conducted across PubMed, Cochrane, Web of Science, Embase, and Scopus to find studies reporting DJK, DJF, and BMD. A 12-month minimum follow-up and radiographic biomarker for BMD (Hounsfield units [HU] or a T-score) individually reported for each patient type were required for inclusion. Studies that did not report individualized biomarkers but provided averaged estimates of the effect of BMD on DJK/DJF development were used for systematic review. RESULTS: Our search yielded 12 unique studies with 1094 patients, of which 5 studies with a total of 519 patients were suitable for comparison by meta-analysis. Patients who developed DJK/DJF had significantly lower HUs (113.17 ± 33.86) than patients who did not develop DJK/DJF (142.51 ± 41.39). No significant difference was found with regard to dual-energy X-ray absorptiometry measurements, age, or body mass index between patients who did and did not develop DJK/DJF. CONCLUSIONS: Patients who developed DJK/DJF had significantly lower computed tomography-measured HU as compared to those without DJK/DJF. Our findings highlight the potential importance of BMD evaluation with computed tomography prior to multilevel spine fusion surgery, though further research would be helpful to evaluate the significance of dual-energy X-ray absorptiometry-based BMD measurements on DJK/DJF development
