No full textThe irritable bowel syndrom (IBS) is a common gastrointestinal disorder with bloating, abdominal pain, diarrhoea and/or obstruction and winds as typical symptoms. These symptoms often result in a reduced quality of life of the patients. IBS has a multifactorial pathogenesis, often associated with psychological aspects like stress. One therapeutic approach between various others is a nutritional therapy, and the FODMAP- reduced diet is based on the knowlegde of bloating properties of different foods. The acronym "FODMAP" stands for "Fermentable Oligosaccharides, Disaccharides, Monosaccharides And Polyols" that are predominantly in the small intestine unabsorbable short-chain carbohydrates. During the fast fermentation process in the large intestine of these substrates the intestinal microflora produces relatively large amounts of gas. Further, these substrates have a high water-binding capacity, leading to a higher bulk volume. These effects are characteristic for FODMAPs, but in general only RDS patients suffer from severe gastrointestinal symptoms after the ingestion of FODMAP-rich foods. The aim of this literature review was to evaluate the value of a FODMAP-reduced diet as a therapeutic option to reduce IBS symptoms. Therefore a total of 13 studies investigating the effects of a FODMAP-reduced diet per se or compared to a control group were analysed. Over all studies, the FODMAP-reduced diet resulted in a clinically significant reduction of the score on the IBS symptom severity scale (IBS-SSS) of 117 points between beginning and end of the studies. The results in the FODMAP groups were superior compared to the smaller reduction of 52 points in the control groups. Compared with not dietary-led interventions like yoga and gut-associated hypnotherapy, the results obtained by the FODMAP-reduced diet were equal.
