DESIGN, SYNTHESIS, AND ANALYSIS OF PAIRED COILED-COIL PEPTIDIC MOLECULAR BUILDING BLOCKS USED FOR LINEARLY CONTROLLED SELF-ASSEMBLY OF α-HELICAL COILED-COIL HETERODIMER PEPTIDE PAIRS
Molecular building blocks are fundamental to biological synthesis and processes and have been utilized in advanced materials, drugs and drug delivery systems, and biotechnology. Proteins have been used as molecular building blocks for the construction of complex, well-ordered structures. Coiled-coil protein domains are essential subunits used for the oligomerization of protein complexes, gene expression, and structural elements of biological materials. The synthesis and assembly of proteins utilizing coiled-coil motifs are of great scientific interest due to their potential applications in disease treatment, biomechanical motors, nanoscale delivery systems, etc. However, assembling protein complexes with specific morphology is still challenging because the controllability of the protein association is complicated by multiple interactions between a diverse array of amino acids.
Initial progress toward the design, synthesis, and characterization of paired coiled-coil peptidic molecular building blocks that can self-assemble with a high degree of controllability are presented herein. Eight unique 32-residue peptides were synthesized, each with one modified residue to covalently crosslink two peptides via a 1,3-dipolar cycloaddition click reaction. Each peptide was expected to fold into α-helical coiled-coil heterodimer peptide pairs upon association with its complementary pair. Furthermore, each crosslinked MBB was designed to exhibit controlled self-assembly with its specific complementary MBB. This design strategy allowed the intermolecular interactions of the peptide to control self-assembly of multiple MBBs via association through the sidechains of the heptadic sequences. Mass spectroscopy, size-exclusion chromatography (SEC), and HPLC analysis show successful synthesis and purification of individual peptides. Additional chromatographic data show successful association of dimeric coiled coils, synthesis of crosslinked MBBs, and self-assembly of the crosslinked MBBs

To use or not to use oxygen for MI Benchmark Study
It is a common practice for emergency personnel to administer oxygen for a patient diagnosed with a myocardial infarction. It is a well-known practice to place the patient on supplemental oxygen even if they are not hypoxic. The use of supplemental oxygen has been a cornerstone in the treatment of myocardial infarction. The rationale behind the administration of oxygen therapy is to increase oxygen delivery to the myocardium, therefore limiting the infarct size and future complications (Hofmann et al., 2017). The use of supplemental oxygen on normoxic patients has been challenged due to the possibility of causing further injury to the myocardium. Above normal oxygen levels in the blood can cause coronary vasoconstriction and increase the production of reactive oxygen species, potentially contributing to reperfusion injury (Hofmann et al., 2017). Acute myocardial infarction is 1 of the 5 most expensive causes of hospitalization, any complications increase the total cost and can lead to re-admissions or recurrent emergency department visits. Under the Hospital Readmissions Reduction Programs (HRRP), Medicare will not pay for a re-admission within 30 days of the patient being discharged, meaning that hospitals will pay the expenses generated. It is important to minimize complications and be more cautious in the administration of supplement oxygen
Nursing Students\u27 Moral Courage Development Through Incivility Simulation Education
Objectives: The purpose of this study was to determine if the use of an incivility simulation intervention and incivility educational module would increase nursing students’ perceived moral courage more than an incivility educational module alone.
Hypotheses: Nursing students’ moral courage scores would increase more in response to an incivility simulation intervention and educational module than for those who received the incivility educational module intervention alone.
Methods: The NLN Jeffries Simulation Theory framed the proposed intervention using a quasi-experimental pre-test-post-test comparison group design. A convenience sample of 66 students was utilized from one university across two classes. The Moral Courage Scale for Physicians was administered to all participants before and after the interventions.
Results: Students that participated in an online educational module plus a simulation activity had a significant increase in pretest/post-test moral courage scores. An insufficient sample for the comparison group prohibited the ability to compare outcome measures for the intervention vs comparison groups.
Conclusion: Uncivil simulation interventions combined with an incivility educational module are useful for developing nursing students’ moral courage when faced with uncivil behaviors in the clinical environment
Regional Anesthesia in Outpatient Total Joint Arthroplasty
Within the past five years, outpatient total joint arthroplasties have increased three-fold due to the change in insurance companies’ reimbursement plans. After continuous literature research collection, support for spinal anesthesia was higher than compared to general anesthesia (GA) for outpatient total joint arthroplasty procedures. Healthy patient who receives regional anesthesia showed decreased post-operative pain, increased rehabilitation, and, most of all, a decrease in facility stays (Macfarlane et al., 2009). This has allowed healthy patients who meet physician, anesthesia, and facility requirements to transition from hospitals to ambulatory surgery centers for total joint replacement procedures. With this new transition comes change for anesthesia guidelines to provide better patient outcomes for these specific patients. This evidence can allow facilities to transition this information into clinical practice to implement a policy change as well as a practice change for a new anesthesia guideline.
Thus, it is recommended that a regional anesthesia guideline implementation program for outpatient arthroplasty procedures be implemented in the ambulatory surgery center. This will occur over five steps such as the following: a 6-week trial with data collection; data collection and interpretation of data that is placed into a report; stakeholder presentation to secure buy-in from leadership, surgeons, and anesthesiologists; policy change and implementation of new guideline; dissemination of new guideline through educational in-services, classes, and skill check-off for anesthesiologists, staff, and surgeons. Lastly, the evaluation plan will be based on measured outcomes, interviews with staff and physicians, and patient satisfaction rating
Using Debriefs to Reduce Nursing Burnout in Nurses Caring for the Oncology Population
Nurses are a critical part of healthcare and make up the largest section of healthcare professionals in the United States. According to the World Health Organization (2020), there are approximately 29 million nurses and midwives globally. The increasing need for nurses is not a new discovery. According to the American Association of College of Nurses, more registered nursing jobs will be available through 2022 than any other profession in the United States. The number of nurses leaving the workforce each year has been growing steadily from around 40,000 in 2010 to nearly 80,000 in 2020. Nursing burnout is a known issue contributing to nursing turnover and the quality of patient care including metrics such as readmission rates, increased urinary tract infections, and surgical site infections. The persistent question of how to reduce burnout in nursing is an important one whose answer could solve significant problems in healthcare.
Focus groups and other similar interventions have been shown to improve feelings of burnout, coping, and stress. In the oncology population, end of life is a common theme. With nurses at the frontlines, feelings of distress are carried day in and day out. Interdisciplinary debriefs after difficult end of life situation that include healthcare workers from different specialties could decrease feelings of burnout.
The hospital resources necessary complete this project include a team from the Office of Performance Improvement, adequate staffing guarantees to relieve nurses during the debriefs, and support from the interdisciplinary team in the critical care unit, and support from executive nursing leadership. The resources needed to complete the project are minimal and will not require extensive expenditure.
Nurses strive to provide the best care to their patients. According to U.S. Census Bureau, by 2030, the number of residents in the United States aged 65 and over is projected to be 73.1 million, which means there will be an increased need for nursing care. It is necessary to retain nurses by assisting with professional wellbeing and job satisfaction
Retention Drivers Post-Pandemic
The Covid pandemic influenced the world in many ways that we still witness a need for recovery. One significant concern that remains apparent is the culture change within the healthcare system. This study is directed to cover only one aspect of this culture change but remains to be a very significant topic for the nursing workforce (AHC Media, 2021). Even though we have managed to find a downward trend of Covid cases, the pandemic has devastated nursing retention to well established facilities. There is currently an exodus of nurses in high acuity departments or simply bedside nursing entirely. This causes a development of additional problematic concerns. Nurses are currently burnout and feel unsupported by their employers causing a substantial staffing shortage (Forde-Johnston & Stoermer, 2022). With consideration of the literature assessed and the presence of the standing dilemma a benchmark study was generated to assist in quality improvement. The question remains what would be beneficial to invest time and budget to implement a positive culture change. The benchmark study is designed to answer the question, (P) Do nurses in acute care settings find (I) organizations that offer retention drivers (C) compared to organizations that do not offer retention drivers (O) effectively help staff retention and morale (T) after the effects of Covid in healthcare today
EXPLORING THE USE OF PIOX SYSTEM FOR TREATMENT OF ENDOCRINE DISRUPTING COMPOUNDS
In this thesis, the Photon Initiated Oxidation (PIOx) system was investigated for its possible use in the treatment of Endocrine Disrupting Compounds (EDCs). It was theorized that the unique design of the system, namely the multiple wavelengths of UV light and the micronfoam would improve removal rates of the test contaminant when compared to other systems. 17β-estradiol was chosen as the test contaminant due to the compounds known sensitivity to ozone exposure. Two methods of testing were used, the first to test an idealized homogenous concentration (method one) and the second to mimic the sudden influx of contaminant that would be common in a water and wastewater treatment environment (method two). Method one showed degradation to below level of detection within 15 minutes. Method two showed a reduction trend but with 60-80% eliminated by the end of 15 minutes. While the PIOx system did not perform above expectations, its simplicity and compact form-factor allows the system to be applicable in a significant number of situations where other AOPs would be too expensive or complex to implement. Continued research is required into the PIOx systems capabilities and its application to low budget and rural environments
Commending rather than condemning: Moral elevation and stigma for male veterans with military sexual trauma
Background
Using an experimental study, we examined the link between state moral elevation and stigmatic beliefs surrounding male veterans with military sexual trauma (MST). Methods
Undergraduate students were presented with a video or written narrative of a male veteran self-disclosing how they struggled with and overcame MST (n = 292). Participants completed measures regarding trait and demographic characteristics at baseline, then measures immediately after the disclosure stimulus to assess immediate elevation and stigma-related reactions. Results
Results suggest state-level elevation in response to a veteran self-disclosing their experience with MST was negatively correlated with harmful stigmatic beliefs about MST. A greater predisposition to experience elevation and PTSD symptoms were linked with stronger elevation responses to the stimulus. Conclusion
Findings support the need for further exploration of elevation and its potential to impact public stigma for male veterans with MST
