The Effects of Food Intake & Physical Activity on Resting Metabolic Rate
Resting metabolic rate (RMR) measures the energy a person requires to sustain vital body function while at rest. RMR is the most considerable element of total daily energy expenditure (McArdle et al., 2015). RMR varies depending on gender, age, weight, fat-free body mass (FFM), and physical fitness level (McMurray et al., 2014). During this study, RMR was measured indirectly by measuring the volume of oxygen consumption (VO2), the volume of carbon dioxide production (VCO2) (McMurray et al., 2014). The proper protocol for measuring RMR was to have the subject complete an overnight fast and refrain from working 12-18 hours before the test (McArdle et al., 2015). This protocol ensures an accurate RMR measurement because recent food intake and recently active skeletal muscle can cause an increase in RMR (Speakman & Selman, 2003). However, this study deviated from this protocol to examine the impact of eating and exercising on the measured RMR value compared to the value from an estimation equation, for each subject. The Cunningham equation and the Harris-Benedict equation were used for the prediction values. The drive to understand the physiological changes that occur when one has recently eaten and participated in physically activity, inspired the idea to see how big an impact those aspects had on RMR. Often known as the “rest and digest” stage, the parasympathetic nervous system would normally be in a total resting stage for the RMR test. However, in this lab, the subjects were currently “digesting”, and their skeletal muscle is recovering, causing the subjects to not truly be in a resting state to test RMR. The purpose of this study was to compare subjects measured resting metabolic rate (RMR) results, after breaking standard preparation protocol, to the equation based estimated kcal/day values. The hypothesis of this lab is that the measured RMR level of each subject, both male and female, would have a significant increase of the RMR value when compared to the predicted RMR. Medication Reconciliation: Protecting the Geriatric Population
This project was designed to address the issue of medication errors that occur during the transition from an acute care setting to an assisted living setting. Geriatric adults are often supported by a complement of medical specialists and subspecialists. As a result, reconciling medications following a hospital discharge can be challenging. To further complicate the problem, oftentimes, the primary care provider is not highly engaged with the specialists and subspecialists. The World Health Organization (WHO) has prioritized this issue through the introduction of the Medication Without Harm initiative. This initiative is focused on reducing adverse drug reactions (ADRs) and adverse drug events (ADEs) resulting from improper reconciliation during transitions of care. In an effort to reduce the risk of medication-related harm to the geriatric population residing in an assisted living environment, this project presents a standard operating procedure (SOP) with supporting documents and tools designed to identify medication discrepancies resulting from transitions in care. This is supplemented with a specific communication tool for primary care providers to assist in the clarification and reconciliation of transitional medication orders
Enhancing the Accuracy and Effectiveness of the National Institute of Health Stroke Scale in a Rural Emergency Departmennt
A rural facility in the foothills of North Carolina found a need for process improvement regarding the emergency department and completion of the NIHSS. The DNP student and the Stroke Coordinator along with the Quality Improvement Coordinator agreed with the need for education of staff concerning accurate assessment and documentation of the NIHSS. The DNP project implemented additional training for the emergency department nurses and paramedics in the form of face-to-face classes with a PowerPoint. A pre-and post-survey was conducted to gauge the comfort level of staff with the NIHSS and stroke processes of the rural emergency department. The Stroke Coordinator collected data to assess the effectiveness of the DNP project. Results showed that completion of admission NIHSS charting increased from 63% to 80% after implementing the education for completion of admission NIHSS. Resources were given to the Stroke Coordinator to continue education of rural emergency department staff
Changing the Ways Bedside Nursing Staff Think About ‘All Things Urinary Catheters
The purpose of this project was to bring attention to the importance of the bedside nursing staff’s back to the basics approach to nurse-led interventions to help reduce the number of increasing hospital-acquired infections (HAI), specifically catheter-associated urinary tract infections (CAUTI). The motivation to address this problem was due to the bedside nursing staff under-utilizing best practice interventions to prevent insertions and re-insertions of urinary catheters. A high number of urinary catheter insertions and re-insertions increases the risk of a CAUTI. Evidence-based interventions should be reinforced and utilized by bedside nursing staff to prevent insertions and re-insertions. Evidence-based nurse-led practices such as bladder scanning (to assess urinary bladder volumes), encouraging fluids by mouth or intravenous (if applicable), and mobilizing the patient frequently and as soon as possible post-surgery (Simsek & Sureyya, 2017). Methods used to address this quality improvement project are education via poster board presentation and pre and post-test questionnaires to evaluate the knowledge level of the bedside nursing staff. Nursing staff includes registered nurses (RN), licensed practical nurses (LPN), and certified nursing assistants (CNAs). The nursing staff plays an integral part in the care, maintenance, and reduction of catheter-associated urinary tract infections (CAUTIs). Education of best practices and supportive measures to decrease catheter-associated insertions and re-insertions to reduce CAUTI rates were discussed. Reinforcing and re-educating on evidence-based nurse-led practices will continue to be the most critical interventions to improve patient outcomes. (Parker et al., 2017)
A Mixed Methodology Study of How Neuroscience and Social-Emotional Learning Impact Students’ Academic Achievement and Behavior
This study is a mixed-method case study using a qualitative and quantitative study structure. The intent of this study was to measure the impact of neuroscience or brain-based social-emotional (SEL) skills and strategies on students who have been identified with disruptive behavior and/or trauma. The impact is evaluated through the Collaborative for Academic, Social, and Emotional Learning (CASEL framework of five competencies (CASEL, 2020). The SEL program used in this study was the Second Step program. There were 33 students who were in an intervention pull-out group. Eight teachers and eight parents participated in the study. Quantitative data were gathered from a 10-item questionnaire distributed to the parents and teachers. Qualitative data were gathered through one-on-one interviews with the classroom teachers. They were asked five open-ended questions. The findings of this study suggested the younger a student was when identified with disruptive behavior and/or trauma and received SEL intervention, the greater the likelihood of positive change in behavior. This study suggests that when students are given SEL tools and are able to practice them, they may learn to make better decisions which range from personal to collaborative situations. Students who exhibit SEL skills, may be able to attend better to instruction, which in turn may result in an increase in academic performance. With SEL skills and strategies, students may be able to understand their emotions and use the skills and strategies to make changes in how they react
