    Sublingual Hematoma Secondary to Mandibular Trauma Mimicking Ludwig’s Angina: A Clinical Conundrum

    Background Sublingual hematoma, or pseudo-Ludwig’s angina, is a rare but potentially life-threatening condition that mimics Ludwig’s angina. Prompt differentiation between hemorrhagic and infectious etiologies of floor of mouth swelling is crucial for appropriate airway and therapeutic management (Baharloo et al., 2020; Cheng et al., 2021; Marin et al., 2020).   Case report We report the case of a 19-year-old male who sustained mandibular trauma in a motor vehicle accident. He presented with progressive submental swelling, muffled voice, and elevation of the tongue, raising suspicion for Ludwig’s angina. Imaging revealed mandibular symphysis and condylar fractures with sublingual hematoma, but no evidence of abscess or gas formation. Flexible endoscopy showed posterior tongue displacement without laryngeal involvement. The patient was managed conservatively with corticosteroids, antifibrinolytics, and antibiotics. Airway intervention was not required. Definitive surgical fixation of mandibular fractures was performed after clinical stabilization. The hematoma resolved completely without complication.   Discussion This case highlights the diagnostic challenge in distinguishing pseudo-Ludwig’s angina from true deep neck infections. While both conditions may present with similar clinical findings, imaging and endoscopy play vital roles in guiding management (Alamoudi & Hariri, 2022; Brotfain et al., 2012). Unlike Ludwig’s angina, sublingual hematomas often respond to conservative therapy and rarely necessitate surgical airway intervention unless rapidly expanding or causing obstruction (Lovallo et al., 2013).   Conclusion Sublingual hematoma secondary to mandibular trauma should be considered in the differential diagnosis of floor-of-mouth swelling. Early imaging and interdisciplinary evaluation are essential to avoid misdiagnosis and unnecessary invasive procedures.

    Recovery High School: Feasibility of Co-Produced, Peer-Based Mental Health Support for Adolescents

    Background: Adolescents accessing mental health services face barriers, from long waitlists to lack of age-appropriate programming. Low-barrier early mental health intervention is needed, and the Recovery College model could be an effective approach. Objective: We explored the feasibility of a Recovery College model in the adolescent population, termed Recovery High School, and ascertained the mental health and support needs of this population to inform its development. Method: Information was obtained via semi-structured interviews with mental health clinicians and analysis of data from a mental health hospital’s adolescent waitlist. Themes and trends among adolescents seeking mental health care were developed. Results: Interviews with adolescent clinicians (N=5) yielded nine themes and considerations for Recovery High School. Mental health concerns such as trauma, social issues and emotional dysregulation were identified, and related course topics were suggested. Challenges and logistical considerations such as scheduling and age of students in courses were identified. Caregiver needs included connection and system navigation. Data from the outpatient waitlist revealed mood and anxiety disorders to be the most prevalent issue facing adolescents. Conclusion: Peer-based co-produced educational courses - Recovery High School, was well supported by adolescent clinicians. With some modifications to the adult model, Recovery College could be developed for adolescents and their caregivers

    How Vernacular Photography Can Reconstruct the Objectification of Women in Art

    Women have been the subject of art for centuries, often portrayed as objects of desire for the male gaze. This objectification has led to the erasure and misrepresentation of women in history, stripping them of their values and agency. However, vernacular photography offers a counter-narrative, allowing artists to reconstruct the way women are viewed in art. This paper examines Erika DeFreitas\u27 exhibition it\u27s because of the shimmer, the verge, and the yet, which utilizes vernacular photography to highlight women\u27s issues and challenge stereotypes. Through her work, DeFreitas desexualizes women and highlights their voices, showcasing their agency through acts of labor and care. By analyzing DeFreitas\u27 work, such as very strongly may be sincerely fainting, this paper demonstrates how vernacular photography can deconstruct women\u27s objectification in art, providing a more accurate and truthful representation of womanhood. This research contributes to the ongoing conversation about the importance of reconfiguring the way women are perceived in art, allowing them to be seen as more than just objects of desire

    Prenatal triptan use for migraine and child neurodevelopment: A systematic review

    Objective: Triptans are a commonly used medication for acute migraine relief and can cross the placental barrier in pregnancy. We conducted a systematic review to summarize and evaluate published literature examining the association between prenatal triptan use for migraine and child neurodevelopmental outcomes. Methods: Search terms for “pregnancy” and “triptans” were used to search Medline (Ovid), Embase (Ovid), and CINAHL (EBSCO) for relevant studies published from database inception to August 2023. Original research studies examining the relationship between triptan use for migraine during pregnancy and any child neurodevelopmental outcomes were included. An adapted version of the Systematic Assessment of Quality in Observational Research (SAQOR) tool was used for quality assessment. We conducted a narrative synthesis of eligible studies. Results: Our search strategy retrieved 1407 articles, of which five met inclusion criteria for the narrative synthesis. All studies used data from the Norwegian Mother and Child Cohort Study (MoBa), used parental self-report to measure triptan use during pregnancy, and were of moderate quality. Outcomes examined varied across studies and included externalising and internalising behaviours, communication, psychomotor function, temperament, and attention-deficit/hyperactivity disorder. Two studies reported an association between prenatal triptan use and externalising behaviours in children up to three years old, but no associations with other neurodevelopmental outcomes or in older children were found. All studies accounted for confounding by indication for migraine, but residual confounding by migraine severity was possible. Conclusion: Prenatal triptan exposure may be associated with an increased risk of externalising behaviours in children aged 1.5-3 years. Future studies outside the MoBa cohort are needed to assess the internal and external validity of these findings. Consideration of individual triptans as distinct exposures and the use of measures of in utero triptan exposure that are less susceptible to recall bias than retrospective self-report are important directions for future research
