    Insuline resistance and insuline sensibility in patients with obstructive sleep apnea

    Obstructive sleep apnea (OSA), also referred to as obstructive sleep apnea-hypopnea (OSAH), is a sleep disorder that involves cessation or significant decrease in airflow in the presence of breathing effort. It is the most common type of sleep disordered breathing (SDB) and is characterized by recurrent episodes of upper airway collapse during sleep. These episodes are associated with recurrent oxyhemoglobin desaturations and arousals from sleep. Despite being a common disease, OSAS is underrecognized by most primary care physicians; an estimated 80% of Americans with OSAS are not diagnosed. Apnea may occur hundreds of times nightly, 1-2 times per minute, in patients with severe OSA, and it is often accompanied by wide swings in heart rate, a precipitous decrease in oxygen saturation, and brief electroencephalographic (EEG) arousals concomitant with loud breathing sounds as a bolus of air is exhaled when the airway reopens. The cardinal symptoms of sleep apnea include the "3 S s": S noring, S leepiness, and S ignificant-other report of sleep apnea episodes. Recent studies suggest that OSA increases the risk of developing insulin resistance and type 2 diabetes. The aim of the present study was to assess whether obstructive sleep apnea is a risk factor for insulin resistance, using surrogate estimates of insulin-mediated glucose uptake. We studied a population of 174 (122 males) subjects evaluated in our Sleep Lab for the suspect of OSA. All subjects underwent a standard nocturnal polysomnography (Compumedics S-Series). The HOMA index, an index of insulin resistance and the QUICKI index, an index of insulin sensitivity, were calculated from the values of fasting glucose and insulin obtained in the morning. The percentage of patients with iper-trigliceridemia was significantly higher in patients with OSAS than in controls (P 2.4) was higher in OSAS patients than in controls (P < 00.5), as well as BMI values were higher in patients with OSAS than in controls (P < 00.5) . The results showed that the risk factors for insulin resistance (HOMA > 2:45 ) were predominantly the BMI (OR 2.4, 95 % CI 1.3-4.6 , P <0.001) , OSA (OR 4.0, 95 % CI 1.6 -9.7 , P <0.001) and hypertension (OR 2.3 , 95% CI 1.2-4.3 , P<0.001). Revealed no correlation with sex , age, hypercholesterolemia and ipertrigligeridemia . We did a multiple regression in which the OSAS (OR 2.7, 95 % CI 1.2-3.70 , P <0.05) and BMI (OR 2.3, 95% CI 0.29-2.70 , P <0.05), but not hypertension (OR 1.8, 95% CI 0.8-6.0, P <0.8) were independent risk factors for insulin resistance. From the correlation between insulin levels and OSAS we got a positive relationship between insulin and AHI (r = 0:32, P <0.001) and insulin levels and BMI (r = 0:43, P <0.001)

    Una delle tematiche in ascesa nell attuale dibattito nazionale e internazionale è la relazione tra innovazioni tecnologiche e processo educativo, in particolare all interno del contesto scolastico. Viviamo un momento di transizione culturale che richiede un radicale cambiamento dell intero universo formativo. Tali trasformazioni investono la sfera culturale incidendo sulle pratiche di costruzione della conoscenza. I nuovi media offrono l opportunità agli studenti di costruire, acquisire e produrre nuove conoscenze e non di subirle passivamente, tutto ciò implica non solo una modifica organizzativa del sistema di educazione formale, ma altresì un cambiamento del ruolo del docente che diventa un mediatore, un regista che per giungere agli obiettivi necessari per la formazione del singolo non può esimersi dalla conoscenza dei nuovi strumenti multimediali utilizzati dai discenti nella loro quotidianità. Aprirsi al nuovo significa, dunque, avvicinarsi al mondo degli alunni utilizzando degli strumenti che possono permettere di attivare percorsi di conoscenza partecipata più stimolanti e motivanti. Alla luce di tali considerazioni, l interrogativo dal quale prende avvio la ricerca è quello di comprendere se sia possibile educare attraverso la rete. L idea seminale della ricerca nasce dall osservazione di un progetto Comenius di ricerca-azione denominato SMILEY (Social Mindedness in LEarning community) che ha come obiettivo la trasformazione delle differenze tra gli studenti in risorse per giungere all integrazione tra pari. Tutto ciò viene posto in atto attraverso una piattaforma e-learning e un Serious Game denominato Your Town. Seguendo tutte le fasi del progetto dalla costruzione di un questionario, alla somministrazione, fino all analisi dei dati, è stata prodotta una ricerca empirica che ha utilizzato i riscontri empirici del suddetto progetto al fine di fornire una cornice interpretativa solida all esperienza di ricerca. Lo studio di caso oggetto del presente lavoro è stato posto in essere attraverso un approccio di tipo integrato ovverosia quali-quantitativo, sono stati utilizzati, dunque, strumenti di tipo standard e non-standard al fine di analizzare i dati raccolti. Quello che è emerso dalla ricerca è che in una scuola intesa come comunità di apprendimento i nuovi media possono divenire un utile strumento per rendere la prassi educativa più motivante e coinvolgente per gli studenti, ma la semplice introduzione dello strumento multimediale non può determinare il cambiamento sociale e culturale dell istituzione scolastica. L utilizzo delle tecnologie dell informazione e della comunicazione non può sostituirsi alle pratiche tradizionali e alle interazioni tra i soggetti, ma può fornire un utile stimolo per costruire ambienti di apprendimento partecipativi che possano coinvolgere e motivare gli studenti, orientando la loro capacità decisionale

    Giudicare l'amministrazione è amministrare? Profili evolutivi dell'azione di adempimento

    L innovazione legislativa - consistente nel riconoscimento al giudice amministrativo del potere di condannare la P.A. ad un facere (e, segnatamente, al rilascio del provvedimento amministrativo richiesto dal privato cittadino) - si colloca in un percorso giurisprudenziale e dottrinario che, sotto l influenza del diritto europeo, ha costantemente eroso gli antichi privilegi e le cittadelle di immunità di cui la P.A. ha sempre goduto. L interesse del privato, sotteso all eliminazione dell atto illegittimo e soltanto indirettamente tutelato con l istituzione della Quarta sezione del Consiglio di Stato, ha progressivamente assunto natura di situazione giuridica sostanziale meritevole di protezione al pari del diritto soggettivo. Il risarcimento del danno per lesione degli interessi legittimi pretensivi, la recente introduzione dell azione di adempimento, i profili di ammissibilità dell azione di accertamento nonché gli ampi poteri cautelari del giudice amministrativo (strumenti ancor più ampi di quelli concessi al giudice civile) sono soltanto alcuni dei momenti del discorso dottrinario, giurisprudenziale e legislativo che conclude per il riconoscimento della piena ed effettiva tutela dell interesse legittimo. In particolare, l introduzione dell azione di condanna al rilascio di un determinato provvedimento amministrativo si è tradotto in un cambiamento dell incisività del sindacato giurisdizionale sulle attività e le valutazioni compiute dalla P.A. nel perseguimento dell interesse pubblico assegnato alle sue cure. Non a caso anche nell ordinamento tedesco - ove l azione di adempimento pubblicistica ha avuto la sua primigenia elaborazione - il controllo giurisdizionale sulla discrezionalità amministrativa si è fatto particolarmente penetrante . Si è consapevoli, però, che la tutela dell interesse legittimo non possa essere interamente sovrapposta alla tutela del diritto soggettivo: il controllo giurisdizionale del potere amministrativo non può che avvenire secondo forme e strumenti che tengano conto sia della particolare natura del soggetto controllato (che opera per la cura di un interesse generale) sia del peculiare rapporto che si instaura tra cittadino e pubblici poteri. Così, il tema dell ampliamento dei poteri del giudice amministrativo finisce per involgere questioni a rilevanza costituzionale (e non di carattere meramente processuale) perché incidenti direttamente sui limiti che incontrano i poteri dello Stato tra loro e nei confronti del cittadino, quindi sui contenuti di diritti essenziali che connotano la nostra forma di Stato

    La ricerca indaga la storia delle comunità italiane emigrate in Belgio a seguito degli accordi bilaterali firmati dall Italia e Belgio nel 1946, accordi che prevedevano lo scambio di manodopera non qualificata per l estrazione mineraria con materie prime, in particolare carbone. Le comunità italiane emigrate hanno vissuto una prima fase d isolamento, complicata dal disinteresse della società belga che spesso sfociava in atteggiamenti razzisti. L isolamento nelle zone minerarie era voluto dalle società carbonifere, per non attirare l attenzione della società e dei sindacati sulle pessime condizioni di lavoro e di vita dei migranti. A partire dai primi anni cinquanta, il ricongiungimento familiare ha permesso alcuni miglioramenti nella vita dei migranti e ha stabilizzato il fenomeno migratorio. La catastrofe di Marcinelle, nel 1956, attira l attenzione pubblica sulla realtà mineraria. Sebbene le condizioni lavorative restino invariate, la catastrofe permette l inizio di un percorso di solidarietà e integrazione tra la società ospite e gli immigrati. La crisi del settore carbonifero causa rivolte sindacali e sposta buona parte dell emigrazione nei centri urbani, le miniere sono destinate a essere chiuse tra la fine degli anni settanta e gli anni ottanta. Gli italiani immigrati in Belgio hanno raggiunto l uguaglianza formale ma soffrono di alcuni gap causati dalle difficoltà sociali tipiche delle comunità migranti. La ricerca si occupa di individuare la differenza narrativa tra la storia dell evento e come questo sia stato percepito dalle comunità migranti, cercando di ricostruire le tappe del percorso migratorio, i momenti sui quali si è basata la costruzione di un identità collettiva e una memoria comune che hanno permesso ai migranti di percepirsi come un unico gruppo compatto. La ricerca è condotta con la metodologia della storia orale. In particolare sono stati intervistati circa quaranta emigrati, tra ex-minatori, figli, mogli ed emigrati di seconda generazione. Le interviste aperte hanno permesso di ricostruire le emozioni e i ricordi sia individuali che collettivi. Dalla ricerca si evince che esiste una differenza narrativa tra l evento e la sua percezione nella comunità, che origina delle discrasie temporali. Gli emigrati costruiscono una narrazione formale e collettiva della loro esperienza che vede come momenti salienti la catastrofe di Marcinelle, il conflitto generazionale e la vittoria ai mondiali del 1982

    Heuristic algorithms for Roman Domination Problem

    Una funzione di Dominazione Romana su un grafo G è una funzione di copertura che assegna ai vertici del grafo uno tra i valori (0, 1, 2) con l'unico vincolo che ogni vertice con valore 0 abbia almeno un vicino con valore 2. Il peso di una funzione di Dominazione Romana è la somma di tutti i valori assegnati e il numero di Dominazione Romana di un grafo G è definito come il minimo tra tutte le funzioni di Dominazione Romana su G. Dopo un'introduzione ai concetti base che ruotano attorno alla Dominazione Romana, viene studiato il problema per due particolari classe di grafi, cioè grafi a griglia e i bipartiti. Per i grafi a griglia vengono prodotti degli schemi di copertura ottimali per griglie di qualsiasi dimensione e inoltre vengono migliorati i limiti superiori e inferiori noti del numero di Dominazione Romana. Per quanto riguarda i grafi bipartiti viene proposto un approccio che partendo da un insieme di vertex cover del grafo produce una funzione di Dominazione Romana in tempo polinomiale. Nel prosieguo della dissertazione vengono mostrati vari approcci euristici per qualsiasi classe di grafo. In particolare viene prodotto un algoritmo euristico che in tempo polinomiale calcola una copertura e un numero di Dominazione Romana che rientra nei limiti teorici noti introducendo un nuovo parametro associato ai vertici del grafo. Infine una delle varianti della stessa euristica è stata implementata su architettura CUDA permettendo la parallelizzazione del calcolo su GPU e saranno mostrate le strutture dati utilizzate e le problematiche riscontrate

    Resilienza regionale: una prospettiva econometrica

    Three chapters constitute the main structure of this contribution. Chapter I reviews selected theoretical and empirical approaches dealing with regional evolution in order to identify recent developments and extensions incorporating spatial econometrics techniques. Chapter II investigates transient and permanent asymmetric effects of national-wide recessions across Italian regions during the last thirty years, by proposing the recent resilience framework as an helpful synthesis. Chapter III studies the determinants of the uneven cross-regional behaviour during crises and recoveries, by presenting two complementary econometric models, namely a linear vector error correction (VECM) model and a non-linear smooth-transition autoregressive (STAR) specification. Some of the main results here obtained are: regions within the same country differ in terms of both shock-absorption and post-recession pattern; the broad impact of a common shock shall take into account temporary and persistent effects; differences in recessions and recoveries among areas can be motivated by some elements such as industrial structure, export propensity, human and civic capital, and financial constraints. Moreover, the presence of spatial interdependencies and neighbouring interactions can play a relevant role. Moving from some of the results here presented, the desirable next step should be addressed towards a deeper analysis of the determinants of regional heterogeneity during recessions and recoveries, cross-country comparisons, the development of a more structured theoretical and empirical background, the assessment of the place-specific impact of countercyclical policies. These and other questions are left for future research

    Germanium Nanostructures for Efficient Light Harvesting Devices

    The growing World energy demand is setting new challenges toward the use of alternative and green resources as well as for the development of more efficient and low-power consuming devices. Thanks to their unique optical properties, group IV (such as: Si, Ge, C) nanostructures (NS) show promising applications for cheap multi-junction solar cells and, in general, for efficient energy-tunable light harvesting devices. Among them, Ge reveals interesting optical properties due to its quasi-direct bandgap and larger absorption coefficient that make it intrinsically more suitable than Si for what concerns light harvesting applications. Moreover, the larger exciton Bohr radius of Ge (~24 nm) with respect to Si, gives the chance to easily tune the optical properties of Ge NSs by varying their size. However, the properties of Ge NS depends not only by size through quantum confinement effects, but many other parameters can concur in controlling their optical behavior, especially for what concerns the optical bandgap. Discerning the role of these parameters and controlling their effects in the light absorption process contains not only a fundamental research theme, but represents a key-factor toward the implementation of Ge NS in any type of light harvesting device. For this reason, this thesis reports a detailed study on the synthesis, structural and optical properties of Ge nanostructures (quantum well, QW, or quantum dots, QDs) embedded in a dielectric matrix as well as the investigations of photo-conduction properties in prototypal light harvesting devices employing Ge NSs. Although the optical behavior of a single amorphous Ge QW can be fully modeled within the quantum confinement effect theory, this situation dramatically changes for a 3-dimensional confinement, as in an ensemble of QDs. In this last case, the effects of quantum confinement can be hidden or weakened by other parameters, such as: QD spacing and distribution, type and quality of the hosting matrix and abundance of defects related to the synthesis technique used. For this reason, we will investigate in detail the synthesis and optical properties of Ge QDs embedded in SiO2 or Si3N4 matrices, grown after thermal annealing of Ge-rich films synthesized by co-sputtering deposition; plasma enhanced chemical vapour deposition (PECVD) and ion implantation. We will give evidences of the strength of quantum confinement effects occurring in these systems as well as discerning the contributions coming from other concomitant effects. Finally, we will demonstrate that Ge nanostructures can be effectively used as active absorber and conductive medium in light harvesting devices. In particular, we will report on the spectral response of metal-insulating-semiconductor (MIS) photodetectors employing a single amorphous Ge QW or a packed array of Ge QDs as active light sensitizer and conductive medium. Both types of NSs have a fundamental role on the performances of these prototypal devices, demonstrating the large potentiality of such nanostructures for the development of high efficiency photodetectors and low cost solar cells

    Background: Parkinson s disease (PD) is characterized by motor fluctuations through the day due to L-dopa treatment. The wearing-off phenomenon is defined as a predictable, clinically relevant motor fluctuation which is routinely identify using clinical scales as well as self-reported questionnaires. Few studies have evaluated accuracy of such tools in identifying wearing-off using a waking-day monitoring study approach of patients motor status. Objectives: to assess diagnostic accuracy of clinical scales as well as self-reported questionnaires compared to an objective proposed tool obtained by a 12-h motor status monitoring in PD patients. Methods: Twenty-two PD patients were enrolled in the study. According to the item 36 of the UPDRS-ME part IV, N=11 patients were classified as fluctuating and N=11 as stable. All patients were clinically evaluated for 12 hours every 2 hours during the day and a Wearing Off Index (WOI) was computed to detect motor fluctuations. Patients also were tested using a self reported questionnaire (WOQ-19) and a 12-hours self diary (NOMO) to identify subjective perceptions of motor and non-motor fluctuations. Results: A WOI score between 2 to 3 was able to detect a wearing-off defined by the UPDRS, with a sensitivity of 90.9% and a specificity of 63.6%. WOQ-19 was less accurate to detect a wearing-off defined by the UPDRS, with a sensitivity and specificity of 90.9 % and 27.3%, respectively. When WOI was used as gold standard, UPDRS-IV item 36 revealed a sensitivity of. 71.4 % and a specificity of 87.5 % to detect the wearing-off, while the WOQ-19 presented a sensitivity of 85.7% and specificity of 25%. According to NOMO diary, fluctuating patients showed a greater percentage of non-motor fluctuations, in particular anxiety and depression (90% and 70%, respectively) even if fluctuating non motor symptoms were also identify in stable patients. Conclusions: our study provided some evidences about accuracy of commonly screening tools to detect wearing-off in clinical practice. A waking day monitoring approach, even if non routinelyapplicable in clinical practice, may be useful to accurately assess changes in motor status through the day due to changes in pharmacological regimen

    Aminergic systems in sleep disorders of neurological diseases

    INTRODUCTION: Dopamine is a catecholaminergic neurotransmitter. A variety of sleep disorders implicate a dysfunction in the dopaminergic system, particularly REM Behaviour Disorder (RBD), Restless Leg Syndrome (RLS) and Periodic Limb Movements during Sleep (PLMS). PLMS involve stereotyped, rhythmic extension movements of the big toe and dorsiflexion of the foot during sleep. PLMS is particularly frequent in Restless Legs Syndrome (RLS) and other sleep disorders. Few studies have documented a pathological PLMS index in patients with Epilepsy. AIMS: The aims of this study were to evaluate the possibility of an association between Epilepsy and PLMS and to define the polysomnographic characteristics of PLMS. Given the dopaminergic nature of PLMS, the association between PLMS and Epilepsy might have represented an additional, indirect evidence of the involvement of dopamine in the pathogenesis of Epilepsy. Another aim was to evaluate the influence of some dopamine agonists commonly used in the treatment of PLMS on the PLMS pattern of some of the patients constituting our sample, as well as any impact these might have on epileptiform discharges. MATERIALS AND METHOD: We selected all patients referred to our neurological clinic from 2008 to 2012, who had previously been diagnosed with epilepsy based on the ILAE criteria, who showed no signs of Sleep Apnea, RLS, RBD and other degenerative neurologic conditions and who were not being treated with antidopaminergic and/or antidepressant drugs. All patients underwent nocturnal video-polisomnographic recordings . We selected patients who had a PLM index considered as pathological (PLMI>5). Subsequently, 5 patients were randomly selected among those with Epilepsy who had a PLMS index > 5. All 5 patients underwent other two nocturnal video-polisomnographic recordings (before and after administration of a dopamine agonist drug, Pramipexole 0.18 mg). In both recordings, PLMS and epileptiform discharges were also counted and analysed. RESULTS: Eighty-five patients who had been previously diagnosed with Epilepsy met the inclusion criteria for this study. Of these, 17/85 (20%) patients had a pathological PLM index (> 5). Of the 85 patients in our sample, 12/17 (70.6%), were diagnosed with Temporal Lobe Epilepsy, 2/17 (11.8%) with Frontal Lobe Epilepsy and 3/17 (17.6%) with Juvenile Myoclonic Epilepsy. PLMS registered in all 17 patients exhibited a mean PLMS index of 18.78 corresponding to the presence of a periodic nocturnal myoclonus of medium entity. In 58.8% PLMS appeared to be mainly distributed during the first half of the night and in 66.7% PLMS occurred during N2. The analysis of the 5 patients who underwent two VPSG (before and after administration of a dopamine agonist drug) showed a close to 50% reduction of epileptiform discharges in patients with PLMS and Epilepsy after administration of a dopamine agonist, namely Pramipexole. CONCLUSIONS: The study confirms the presence of PLMS in patients with Epilepsy. In our sample, the frequency of PLMS episodes stood at 23%. These movements mainly occurred during the first half of the night, in connection with the N2 stage, with a severity ranging from mild to moderate. They therefore had characteristics and distributions which made them more similar to the PLMS described during RLS than to those associated to neurodegenerative conditions. In addition, the PLMS in our sample were more frequent in patients with Temporal Lobe Epilepsy. Finally, considering the dopaminergic nature of PLMS, their occurrence in association with Epilepsy may serve to strengthen the role of dopamine in the pathophysiology of this disorder. This could have significant repercussions even in the therapeutic field, opening up new possibilities through the employment of dopamine agonists which may well find use in the treatment of some forms of Epilepsy
