    Efficacy and Safety of Angiopoietin-like 3 Inhibitors in Decreasing Low-density Lipoprotein Cholesterol

    Hypercholesterolemia is estimated to effect 31.7% of the United States population according to the Center for Disease Control and Prevention. The mainstay of treatment for this condition is currently statin therapy. While these medications are often effective in decreasing low-density lipoprotein cholesterol (LDL-C) to patient’s goal range, there are populations in which they are not sufficient. These populations include those who have genetic predispositions, such as familial hypercholesterolemia, or those who experience refractory hypercholesterolemia to statin medications. To combat this issue, a class of medications named Angiopoietin-like 3 (ANGPTL3) inhibitors have been created to use as an adjunct to statin therapy for hypercholesterolemia treatment. The purpose of this literature review is to evaluate the efficacy, safety, and tolerability of ANGPTL3 inhibitors as an adjunct to statin therapy in lowering LDL-C levels in those with severe hypercholesterolemia. A literature review was performed using electronic databases including PubMed, ClinicalKey, and Embase. Keywords were ANGPTL3, statin, and familial hypercholesterolemia were used, which yielded 1,935 results. These were then filtered further by clinical trials within the last 10 years, which yielded 116 results. Exclusion criteria included those that included vupanorsen (discontinued), bempedoic acid, Inclisiran, and PCSK9 inhibitor adjuncts. Eight articles were included in this literature review as well as one case study. Current literature suggests that ANGPTL3 inhibitors are effective and well tolerated in reducing LDL-C levels as an adjunct to statin therapy. This is applicable to clinical practice in treating patients with severe hypercholesterolemia in which statin therapy is not effective alone in lowering LDL-C levels to the patient’s desired goal range.https://commons.und.edu/pas-grad-posters/1333/thumbnail.jp

    The Utility of Pharmacogenetic Testing in Pain Reduction

    Chronic pain is one of the most common reasons patients seek care from primary care providers. Individualized responses to pain medications make pain difficult to treat effectively. The purpose of this literature review is to examine the utility of pharmacogenetic-guided opioid therapy across chronic, postoperative, and cancer-related pain. A structured search of PubMed and Embase yielded 151 articles, of which nine met the inclusion criteria. Findings suggest that pharmacogenetic-guided therapy may reduce opioid dosages and enhance pain control, particularly in chronic pain settings. Evidence for postoperative pain outcomes was mixed, and studies on cancer-related pain emphasized the role of genetic variability in predicting adverse effects. Despite promising results, limited testing availability and low provider adherence limit integration into primary care.     Effect of Whole-Food Plant-Based Diet on LDL Cholesterol: A Scoping Literature Review

    Background: The whole-food, plant-based (WFPB) diet includes whole grains, whole plants, unrefined sugars, beans, and legumes. The diet excludes meat, dairy, and added fats. The diet has been popularized by documentaries .The diet claims to lower total and LDL cholesterol to a safe level as both a primary and secondary mode of prevention. Purpose: Review existing studies to see whether the WFPB diet will lower LDL cholesterol in adult males over a 14 day or more duration. Methods: A scoping literature review was completed of the University of North Dakota’s library system for keyword combinations. The returned 102 studies were reviewed and reduced to 16 studies, 2 of which were meta-analyses, and 10 of which were WFPB studies versus the “American diet.” Results: The methods, findings, and results of the 16 studies were synthesized. Findings of the 10 WFPB studies were a reduction in LDL cholesterol by approximately 20 mg/dL, or 0.2-0.4 mmol/L. The remaining studies imply that a diet low in oil, high in fiber, and plant-based reduces the overall LDL of patients. Implications for practice: This scoping review suggests the hypothesis that the WFPB diet will lower LDL cholesterol in adult males is correct. Physicians, dieticians, and other healthcare practitioners should be educated to these benefits. Further studies with larger sample sizes and longer duration are needed

    Life Is Just a Classroom: Student Perceptions of Courses Incorporating Popular Culture

    Some faculty design and teach college courses that incorporate popular culture, presumably to engage and spark interest in students. The purpose of this study is to examine student perceptions of courses that incorporate popular culture. College students (N = 464) were randomly assigned to read a course description and learning objectives that incorporated popular culture in general, the specific popular cultural celebrity Taylor Swift, or no mention of popular culture (control). Incorporating popular culture generally or specific to Taylor Swift had no overall effects on topic interest in the course or likelihood to enroll. However, students with greater interest in popular culture appear to be more interested and more likely to enroll in courses incorporating popular culture. Similar findings were noted with the Taylor Swift course. Students perceived less difficulty in the course incorporating Taylor Swift compared to the other courses. Faculty who teach such courses may use these findings to inform their expectations of students enrolling in them and ensure clear expectations for academic rigor
