    p53 Gene Codon 72 Polymorphisms among Cervical Carcinoma Patients

    Objective: To identify the association between p53 gene codon 72 polymorphism and cervical carcinoma. Method: An analytic observational study with case-control design, from November 2013 until March 2014 in the Department of Obstetrics and Gynecology and Microbiology Laboratory Faculty of Medicine, Sriwijaya University, Dr. Moh. Hoesin Hospital Palembang. Result: In total there are 100 samples analyzed consisting of 50 subjects and 50 control groups. Genotype distribution in subject group are 54% Arg/Arg, 42% Pro/Arg and 4% Pro/Pro, and in control group are 36% Arg/Arg, 46% Pro/Arg and 18% Pro/Pro. Arg/Arg genotype is at risk of cervical carcinoma 6.7 times higher compared with Pro/Pro genotype (p=0.013; OR 6.75; 95% CI 1.34-34.94). Arg allele in the p53 gene codon 72 increase the risk of cervical carcinoma 2.6 times more than Pro allele. Conclusion: Proline mutation to Arginine in gene p53 P72R is one of the risk factor for cervical carcinoma. Keywords: arginine, cervical carcinoma, gene p53 codon 72, polymorphism, prolin

    Pereira Suture: an Alternative Compression Suture to Treat Uterine Atony

    Objective: To evaluate the use of Pereira suture as a conservative surgical management in postpartum hemorrhage due to uterine atony. Methods: The study was performed in the Department of Obstetrics and Gynecology, Temanggung General Hospital, Central Java, Indonesia. Data was retrieved retrospectively from medical records, from January 2011 until December 2013. Results: Pereira suture was done in four cases of uterine atony with failed conservative management, and the procedure was found to be successful in all cases. Conclusion: Pereira suture is an alternative surgical procedure for the treatment of uterine atony after failed conservative management.     Effectiveness of Pelvic Organ Prolapse Surgery in Women with Depressive Symptoms and Decreased Quality of Life

    Objective: To know the effectiveness of pelvic organ prolapse surgery in decreasing depressive symptoms (based on PHQ9) and improving quality of life in women with pelvic organ prolapse. Methods: This experimental study without control is conducted at the Obstetrics and Gynecology Department of Dr. Mohammad Hoesin Hospital Palembang/Faculty of Medicine Sriwijaya University, from October 03 2012 until May 31, 2014. Data containing selfadministrated questionnaire about depressive symptoms (PHQ9) and quality of life (PFIQ and PFDI) were recorded. Questionnaire was performed before and six months after surgery. Sample included 26 women with pelvic organ prolapse seeking pelvic organ prolapse surgery, which qualified the inclusion criteria. Data were analyzed using Chi Square and Fisher Exact test. Data analysis was done using SPSS 18.0. Results: According to paired T test there is a significant difference between mean PHQ9 score before (6.69±3.80) and 6 months after surgery (1.96±1.75)(p=0.001). Total PFIQ score decreased from 17.15±9.39 to 2.88±4.01 with 14.27±5.38 reduction. PFDI score before surgery were 29.85±15.73 and decreased to 11.50±10.99, with a reduction of 18.35±4.74. Conclusion: There was significant reduction in depressive symptoms and improved quality of life in women with prolapse after surgery, compared to before surgery.     PPARy Expression in Eutopic and Ectopic Endometrium of Reproductive Age Women with Endometriosis

    Objective: To evaluate the expression of PPARy receptor and to compare its expression in eutopic and ectopic endometrium in women with endometriosis. Method: This is a cross sectional study. Ten female subjects with endometriosis that underwent laparoscopy or laparotomy who fulfilled the inclusion criteria were recruited by consecutive sampling. Two samples were taken, eutopic endometrium and ectopic endometrium from endometriosis cyst wall during surgery of each subject. PPARy expression was examined by two-step RT-qPCR. Our data was statistically examined using the paired t-test and Pearson’s correlation test. Result: PPARy was found to be expressed in eutopic and ectopic endometrium of women with endometriosis using the RT-qPCR method. The expression of PPARy was not statistically different in eutopic and ectopic endometrium (1.16 relative fold vs 1.25 relative fold; p=0.26). By Pearson’s correlation there was a weak positive correlation between PPARy expression of eutopic and ectopic endometrium (r=0.16). Conclusion: PPARy was detected by two-step RT-qPCR in eutopic and ectopic endometrium of women with endometriosis. Semiquantification of PPARy expression showed that there was no significant difference between PPARy expression in eutopic and ectopic endometrium of women with endometriosis. There was a weak positive correlation between PPARy expression in eutopic and ectopic endometrium of women with endometriosis.     Effect of Smoking on Advanced Stage Cervical Cancer Patient Survival

    Objective: The researchers aim to investigate the relationship between smoking habit and other factors as prognostic factors of cervical cancer. Method: We performed a retrospective and prospective cohort study with subjects that are stage IIB-IVB cervical cancer patients in Dr. Cipto Mangunkusumo Hospital followed up from August 2009 to April 2014. The subjects’medical records were reviewed, and patients were interviewed about their current condition by telephone. Questions asked include smoking habit, spouse’s smoking habit, and mortality status. Patients that could not be contacted by phone were excluded from the study. Statistical analysis was done using Stata 10. Result: Out of 390 cervical cancer patients stage IIB-IVB in 2009, there were 270 patients (69.2%) that were included in the inclusion criteria. Most of the patients are 40-59 years old (82.2%) and are nonsmokers (91.8%). The most frequent clinicopathological characteristic is IIIB (63.3%) and squamous cell carcinoma (71.9%). The 5- year survival rate is 22.6%. There is no statistical significance between advanced stage cervical cancer survival with the patients’ or patients’ husbands’ smoking habit. Conclusion: In our study, smoking habits do not aggravate survival rate of advanced stage cervical cancer patients but further research must be done with more sample. Stage, and tumor size both by physical examination and ultrasound can be used as the prognostic factor.     Factors Affecting Selection of Contraceptive Methods and Its Length of Use

    Objective: To evaluate the factors related to the selection of contraceptive methods and length of use on acceptors in Raden Saleh Clinic and Fatmawati General Hospital. Method: The study was a prospective observational study designs. The factors that we observed include the family planning factor (wife age, number of desired children and infertility), subjective factors (side effect experience of contraception, support from the husband/ family, and religion), objective factors (medical disorders, person helping to select contraception, family planning service centre and availability of contraceptives) and the level of motivation (level of education). All the clients who will receive contraception and meet the criteria for the research were interviewed and given questionnaires until the desired sample size is achieved. The study was conducted at the Raden Saleh Clinic and Fatmawati General Hospital. We then conducted follow-up at 3 and 6 months after the use of the contraceptive methods to assess the length of use. Result: The total number of subjects was 151 people. The average age of respondents was 31 years old, with the contraception options being IUD (67.1%), implants (17.8%), sterilization (7.2%), injectable contraception (3.9%), and oral contraception (3.9%). From all the factors studied, only the number of desired children were found to affect the selection of contraceptive methods significantly (p=0.008) in Fatmawati General Hospital, while in Raden Saleh Clinic all of the factors did not affect the selection of contraceptive method (p>0.05). We also found that all of these factors do not have a significant relationship to the length of contraceptive use (p>0.05). There were 6.6% of subjects (n=10) who switched the type of contraception. From all of them, there was a trend of switching from oral contraceptive to injectable method (33.3%) and from IUD to injectable method (66.7%). Conclusion: Only the number of desired children has an effect on the selection of contraceptive methods in Fatmawati General Hospital, while in Raden Saleh Clinic all of the factors studied do not affect in selection of contraceptive methods. Family planning factors, subjective factors, objective factors and motivation levels have no effect on the length of contraceptive use by clients at Raden Saleh Clinic and Fatmawati General Hospital.     Objective: To determine the association between BRCA1 gene Q356R (1186A=G) polymorphism and epithelial ovarian cancer incidence. Methods: This study is an observational analytic study with casecontrol study design. All patients diagnosed with epithelial ovarian cancer that were treated in the outpatient clinic and inpatient ward of the Department of Obstetrics and Gynecology, Dr. Mohammad Hoesin Hospital, Palembang who met the inclusion criteria were included in this study. DNA extraction was performed on blood samples, followed by PCR-RFLP process. Results: We obtained the genotype distribution of BRCA1 Q356R (1186A=G) polymorphisms to be QQ genotype (wild-type) on all 50 subjects in the case group (100%) and 50 control subjects (100%). Similarly, all BRCA1 alleles have the Q allele. The results of this study found no polymorphism of the BRCA1 Q356R (1186A=G) in the ovarian cancer and control groups. Conclusion: Polymorphism of BRCA1 gene Q356R (1186A=G) was not significantly associated with epithelial ovarian cancer incidence.     Fascin Expression as Prognostic Factor for Survival in Advanced Epithelial Ovarian Carcinoma

    Objective: To evaluate fascin expression as a prognostic factor and its correlation with survival and clinicopathologic factors (degree of differentiation and stage) in epithelial ovarian carcinoma. Methods: This study is prognostic study with historical cohort design. Fascin was analyzed in paraffin block sections of 33 advanced stage ovarian carcinoma patients using immunohistochemistry. Fascin expression was tested for its correlation with overall survival as well as with grade and stage of the cancer. Results: In this study, fascin expression has no correlation with survival. In the period of 17-22 months, samples with high fascin expression had a HR of 1.59 (95% CI=0.38-6.67, p=0.449), but in the period of 17-23 months, both groups had comparable HR. In the period of more than 23 months, samples with high expression of fascin had a better HR of 0.40 (95% CI=0.04-4.38, p=0.449). No significant correlation was found between fascin expression with grade (OR=2.08, 95% CI=0.44-9.84, p=0.442) and stage (OR=2.70, 95% CI=0.39-18.96, p=0.360). Conclusion: In this study, there was no correlation between fascin expression and survival, and also no correlation between fascin, grade and stage. Further study with a larger, more homogenous sample, analyzing confounding factors is needed.
