    Classic Antiphospholipid and Antiphosphatidylserine Antibody Profile in Suspected Antiphospholipid Antibody Syndrome Patient

    Objective: To compare the classic examination results of antiphospholipid (aPS) and antiphospatidylserine (aPL) antibody profile to establish the diagnosis from suspected antiphospholipid antibody syndrome (APS) patient in order to state the subsequent treatment strategies. Method: This descriptive cross-sectional study design was conducted at outpatient clinics of Dr. Cipto Mangunkusumo Hospital (RSCM) from January to December 2015. The laboratory test was held in Clinical Pathology Laboratory RSCM/Faculty of Medicine Universitas Indonesia (FKUI) and in corporation with Prodia laboratory. Result: All of normal patients did not have positive result in any laboratory examination (Lupus Anti-coagulant (LA), anticardiolipin (aCL), anti-?2 glycoprotein I (anti-?2GPI), and aPS). In patient suspected APS, 11 (37.1%) patients had positive aCL, 7 (25.9%) patients had positive anti-?2GPI, and 11 (37.1%) patients had positive aPS. The most positive cross laboratory examination was between aCL and aPS (25.9%). In this study, we found the most positive test result was aCL and aPS (62.9%). From this study, suspected APS patient who had negative result in classic laboratory examination, but showing the positive result in aPS was in 5 (18.5%) patients. Conclusion: All normal pregnant patients do not have any positive classic examination and aPS. Meanwhile, in patients with suspected APS, less than 20% patients show positive result of aPS with negative result in classic laboratory examination.     Postradical Hysterectomy Survival Rate in Early Stage Cervical Cancer Patients

    Objective: Determining the survival rate of early stage cervical cancer patients after radical hysterectomy. Method: A retrospective cohort study was conducted on 24 earlystage cervical cancer patients who had performed radical hysterectomy in Prof. dr. R.D. Kandou Hospital Manado during the period between January 2008 and December 2010. We used Kaplan-Meier methods to observe the survival rate. Result: The mean age of patients in this study was 47.8 (33-63) years old with a median of 45 years old. The largest proportion was less than 50 years old (66.7%) and stage IIA cervical cancer (66.7%) as the severity of cancer. Most histopathology type was the squamous cell carcinoma (50.0%). About 70.8% patients did not have lymph nodes metastasis and 62.5% patients did not receive adjuvant therapy. This study revealed that 1-year, 2-year, 3-year, 4-year, and 5-year survival rate were 100.0%, 100.0%, 95.8%, 83.0%, 70.8%; respectively.     Theurapeutic Response of Neoadjuvant Chemotherapy between Platinum and Ifosfamide Combination and Platinum, Vincristine and Bleomycin Combination in Cervical Carcinoma Stage IB2

    Objective: To evaluate the theurapeutic response and acute toxicity of neoadjuvant chemotherapy between the combination of Platinum and Ifosfamide, and the combination of Platinum, Vincristine and Bleomycin in Cervical Carcinoma Stage IB2 and then continued with radical hysterectomy and pelvic lymphadenectomy. Method: Thirteen samples received neoadjuvant chemotherapy of Platinum and Ifosfamide and 17 samples received neoadjuvant chemotherapy of Platinum, Vincristine and Bleomycin, after receiving the neoadjuvant chemotherapy, clinically complete response samples underwent radical hysterectomy and pelvic lymphadenectomy (PI VS PVB = 3 VS 1). Histopathology examination was performed to evaluate the presence of malignant viable cells at the cervix, pelvic lymph node metastasis and parametrium metastasis. Acute toxicity evaluation was performed based on gastrointestinal, genitourinarius and hematology sign and symptom. Result: Theurapeutic response of PI is 1.12 higher than PVB (p>0.05). Subanalysis of group response of PI is 1.962 higher than PVB. PI and PVB have the same risk to have pelvic lymph node metastasis, but not parametrial metastasis. There were no differences in terms of the risk of gastrointestinal, genitourinarius and hematologic toxicity between PI and PVB. Conclusion: There was no statistical difference in clinical and pathological response, and also in acute toxicity between the two combination (p>0.05).     Impact of Ethinyl Estradiol to Human Telomerase Reverse Transcriptase Activity on Complete Hydatidiform Mole Culture

    Objective: To prove the effect of ethinyl estradiol as an activator on human Telomerase Reverse Transcriptase (hTERT). Method: The experimental study was conducted in vitro by using culture of complete hydatidiform mole trophoblast cell. We exposed the culture to ethinyl estradiol in varied doses and measured the concentration of hTERT through RT-PCR quantitative. There were 40 specimens as control group and 20 specimens exposed to ethinyl estradiol in different doses (10, 20, 40 and 80 mcg) as experimental group. The activity of hTERT was measured by RT-PCR and the concentration of it was assessed by ELISA. We analyzed the variables using ANOVA, Turkey post hoc and Pearson correlation test. Result: In control group, the concentration of hTERT was not detected. Meanwhile, the concentration among different doses of ethynil estradiol (10, 20, 40, 80 mcg) was 113,117.5; 114,507.6; 102,193.9; 127.546.1 amoles/ml, respectively. Among experimental group, they were significantly different both using F test (ANOVA) (p=0.001) and Turkey post hoc test (p=0.005). The correlation among group was 0.84 which meant higher level of ethinyl estradiol was correlated with higher activity of hTERT. Conclusion: Ethinyl estradiol impacts to the increase of hTERT activity on complete hydatidiform mole cell culture.     Tampilan integrin ανβ3 endometrium pada wanita infertil dengan penyakit radang panggul subklinik

    Tujuan: Diketahuinya tampilan integrin ανβ3 pada wanita infertil dengan penyakit radang panggul (PRP). Rancangan/rumusan data: Penelitian bersifat klinis retrospektif. Bahan dan cara kerja: Penelitian dilakukan pada 52 orang pasien infertilitas dengan penyakit radang panggul subklinik yang datang ke klinik Yasmin RSCM. Dilakukan biopsi endometrium pada hari ke 20- 24 siklus haid. Hasil biopsi dianalisa dengan pemeriksaan imunositokimia di Makmal terpadu RSCM-FKUI. Hasil pewarnaan dievaluasi untuk mendapatkan HSCORE. Hasil: Didapatkan 26 sampel (50%) dengan intensitas lemah, 16 sampel (30,7%) dengan intensitas sedang, 5 sampel (9,6%) dengan intensitas kuat dan 3 sampel (5,7%) dengan intensitas sangat kuat. Kesimpulan: Intensitas tampilan integrin endometrium yang rendah pada 50% pasien infertilitas dengan penyakit radang panggul subklinik, menunjukkan kemungkinan adanya kaitan sebab-akibat antara penyakit radang panggul dan kejadian infertilitas.     Pengaruh Isoflavon Terhadap Profil Lipid pada Perempuan Menopause/pascamenopause

    Tujuan: Untuk menentukan efek perubahan profil lipid pada perempuan menopause/pascamenopause yang diberi fitoestrogen (isoflavon). Tempat: Poliklinik Menopause RSCM/FKUI Jakarta. Rancangan/rumusan data: Uji klinis cara tersamar ganda dengan desain paralel tanpa matching. Bahan dan cara kerja: Selama kurun waktu April 2005 - September 2005, terkumpul 48 orang perempuan menopause/pascamenopause berusia < 65 tahun yang sehat, telah mengalami henti haid minimal selama 1 tahun dengan kadar FSH ≥ 30 mIU/L. Setelah dilakukan randomisasi sederhana dengan tabel random dan tersamar ganda, subjek penelitian dibagi dua kelompok, kelompok pertama diberi obat fitoestrogen isoflavon oral 100 mg/hari (2 x 50 mg), kelompok kedua diberi plasebo oral. Sebelum minum obat, diperiksa kadar lipid serum berupa kolesterol total, LDL kolesterol, HDL kolesterol dan trigliserida. Pemeriksaan kadar lipid serum kedua diperiksa setelah minum obat selama 12 minggu untuk melihat perubahan profil lipid setelah minum obat. Hasil: Dari 48 orang yang ikut penelitian, 2 orang dikeluarkan dari penelitian. Ditemukan kenaikan rerata kadar kolesterol total serum setelah 12 minggu pada kelompok obat sebesar -17,21 mg/dl (8,1%), tetapi secara statistik tidak bermakna. Pada kelompok plasebo sebesar -33,04 mg/dl (15%), secara statistik tidak bermakna. Pada kelompok obat ditemukan kenaikan kadar LDL sebesar -20,43 mg/dl, lebih sedikit dibanding kelompok plasebo sebesar -30,92 dan secara statistik bermakna setelah minum obat. Ditemukan penurunan kadar rerata HDL sebesar 3,99 mg/dl, secara statistik tidak bermakna pada kelompok obat setelah 12 minggu. Pada kelompok plasebo ditemukan kenaikan sebesar 1,40 dan tidak bermakna secara statistik. Pada kedua kelompok obat dan plasebo terdapat kenaikan rerata kadar trigliserida (secara statistik tidak bermakna) yaitu berturut-turut sebesar 3,54 mg/dl dan 4,16 mg/dl. Kesimpulan: Tidak ditemukan perubahan profil lipid pada pemberian isoflavon 100 mg/hari selama 12 minggu pada perempuan menopause/ pascamenopause yang sehat.     Operasi Mikro Rekanalisasi Tuba Per Laparoskopi (Laporan Kasus)

    Tujuan: Untuk mengevaluasi hasil luaran dari prosedur operasi mikro rekanalisasi anastomosis tuba fallopii dengan laparoskopi. Tempat: Klinik Raden Saleh, Divisi Kesehatan Reproduksi, Departemen Obstetri dan Ginekologi, FKUI/RSUPNCM. Bahan dan cara kerja: Dilaporkan 3 kasus. Kasus pertama: Ny. A, 38 tahun, P3A0 (anak terkecil 3 tahun), sterilisasi 3 tahun, menikah 1 x, usia suami 41 tahun, ingin punya anak lagi karena sosial ekonomi membaik. Kasus kedua: Ny. I, 44 tahun, P3A0 (anak hidup 1), riwayat bekas seksio 1 x, nikah 1 x, usia suami 48 tahun, ingin punya anak lagi karena dua anak terakhir meninggal. Kasus ketiga: Ny. S, 40 tahun, P3A0 (anak terkecil 8 tahun), sterilisasi 8 tahun lalu, cerai dengan suami pertama 5 tahun lalu, menikah lagi 2 tahun lalu, ingin punya anak lagi karena desakan suami kedua, telah gagal menjalani IVF satu tahun yang lalu. Hasil: Rekanalisasi tuba dengan laparoskopi telah dilakukan pada ketiga pasien. Pada pasien pertama dan kedua dilakukan rekanalisasi pada kedua tuba dengan hasil pascaoperasi kedua tuba paten. Pada pasien ketiga tuba kanan oklusi diproksimal, sehingga hanya pada tuba kiri yang dilakukan rekanalisasi dengan hasil tuba kiri paten. Kesimpulan: Pada ketiga kasus rekanalisasi tuba dilakukan karena keinginan kuat dari masing-masing pasangan suami istri dengan alasan yang berbeda. Pada kasus-kasus yang diseleksi laparoskopi dengan teknik khusus memakai instrumen mikro KOH (dengan diameter alat masing-masing 2,5 mm) dapat ditawarkan untuk rekanalisasi tuba. Seleksi pasien dan teknik operasi yang baik adalah faktor kunci untuk mencapai angka kehamilan yang memuaskan. Hasil luaran pada ketiga kasus hanya dinilai pada tahap patensi tuba, sedangkan hasil luaran kehamilan belum dapat dinilai karena laporan ini dibuat baru 7 bulan berjalan.
