    Changes in Cortisol Levels before and after Supportive Psychotherapy in Patients with Comorbid Cervical Cancer Distress with Depression Type: Perubahan Kadar Kortisol sebelum dan sesudah Psikoterapi Suportif pada Pasien Kanker Serviks dengan Komorbiditas Distres Tipe Depresi

    Objective: To prove the success of supportive psychotherapy thatwas provided as a distress therapy on advanced cervical cancer.Knowing the prevalence of distress type of depression in patientwith epithelial cervical cancer, proving the benefits of psychotherapysupportive for distress can decreasing cortisol level in cervicalcancer patient, can assess distress thermometer score, HAM-D17score and scoring incident predictors of distress with depressiontype.Methods: There were 32 subjects from 71 advanced cervical cancerpatients had mild-moderate depression. Then randomizationblocking was performed to determine a subject who entered thetreatment group (n = 16) who got supportive psychotherapy orcontrol group who got common psychotherapy (n = 16). All ofparticipants assessed the distress level with cortisol value, distressthermometer score, and HAM-D17 score before and after they gotsupportive psychotherapy.Results: After the intervention of psychotherapy in the treatmentgroup decreased HAM-D17 score, the average decline 7.53 (SB 3.34).The mean decreasing in the control group was 3.98 (SB 2.85). Thereis a significant difference in mean reduction in HAM-D17 scores ontreatment and control groups with p = 0.003 (p <0.005). There wasdecreasing blood cortisol level in the treatment group amounted to39.43, while the control group there was a drop of 1.59. The reductionof cortisol level in the treatment group and the control has a pvalue0.302. After got supportive psychotherapy, found a decreasingthe average value of the thermometer distress in the treatmentgroup 3.02 and the control group 2.51, with a p value more than0.492.Conclusion: There were 45% of cervical cancer patients in the clinicexperiencing distress disorder with depressive type. The bloodcortisol level could be decreased by giving supportive psychotherapywith a mean decrease of 39.43 nmol/l. There was a significantreduction in the level of depression (HAM-D17 score) of 7.53 pointsand distress thermometer impairment by 3 points after givensupportive psychotherapy.     Relationship of Retinol Binding Protein Four Serum Level on Endometrial Hyperplasia and Endometrial Carcinoma: Hubungan Kadar Serum Retinol Binding Protein Empat pada Hiperplasia Endometrium dan Carcinoma Endometrium

    Abstract Objective : To determine the relationship of elevated serum retinol binding protein 4 with abnormal uterine bleeding Methods : This study was an observational quantitative with cross sectional methods, with all women who had abnormal uterine bleeding caused either by endometrial carcinoma or endometrial hyperplasia at RSUP Prof.DR.RD Kandou, and affiliation hospitals from November 2016 until April 2017. Data were analyzed With SPSS version 2.0 to see the significancy level. Results: Of 26 research subjects, 23 subjects with endomtrial hyperplasia and 3 subjects with endometrial carcinoma. From the total of 26 malignancy and hyperplasia diagnoses, 21 had IMT> 25 and 23 were diagnosed with Endometrial Hyperplasia and 3 Carcinoma Endometrium. 18 subjects had elevated serum RBP4 levels, with 15 people with endometrial hyperplasia and 3 with endometrial carcinoma. With the Fischer Exact test statistic, serum retinol binding protein 4 levels were found in both endometrial hyperplasia and endometrial carcinoma p = 1.00, meaning no significant difference for the occurrence of abnormal uterine bleeding. Conclusion: There was no significant association between serum retinol binding protein 4 between endomterium carcinoma and endometrial hyperplasia. Keywords: abnormal uterine bleeding, endometrial carcinoma, endometrial hyperplasia, serum retinol binding protein 4   Abstrak Tujuan : Mengetahui adanya hubungan peningkatan kadar serum retinol binding protein 4 pada hyperplasia endomterium dengan carcinoma endometrium Metode : Penelitian ini adalah jenis kuantitatif observasional secara potong lintang, dengan semua perempuan yang mengalami perdarahan uterus abnormal yang disebabkan oleh hiperplasia endometrium atau carcinoma endometrium di Bagian Kebidanan dan Kandungan Rumah Sakit Umum Pusat (RSUP) Prof.DR.R.D Kandou, dan RS jejaring mulai November 2016 sampai April 2017.Data dianalisa dengan SPSS versi 2.0 untuk melihat tingkat kemaknaannya. Hasil : Dari 26 subjek penelitian, 23 subjek dengan hyperplasia endomtrium dan 3 subjek dengan carcinoma endometrium. Didapatkan data penelitian dari total keganasan diagnosa  dan hiperplasia sejumlah 26 orang, sebanyak 21 orang memiliki IM T>25 dan sebanyak 23 orang didiagnosa dengan Hiperplasia Endometrium dan 3 orang karsinoma Endometrium. Didapatkan sebanyak 18 subyek penelitian mengalami peningkatan kadar serum RBP 4, dengan 15 orang yang mengalami hiperplasia endometrium dan 3 orang dengan karsinoma endometrium. Dengan uji statistik Fischer Exact test, didapatkan kadar serum retinol binding protein 4 baik pada hiperplasia endometrium dengan karsinoma endometrium p=1.00, mengartikan tidak mempunyai perbedaan bermakna untuk terjadinya perdarahan uterus abnormal. Kesimpulan : Tidak terdapat hubungan bermakna kadar serum retinol binding protein 4 antara karsinoma endomterium dengan hiperplasia endometrium.     Pregnancy Outcome in Infertility Patient with Endometriosis Cyst after Laparoscopic Cystectomy

    Objective: To investigate about the rate of pregnancy in womenwho had undergone laparoscopic cystectomy.Methods: This was a retrospective study. Data were taken from themedical records of patients with infertility in Fatmawati Hospital,Jakarta, Indonesia. Data then analyze to know is there anyassociation between age, infertility duration, bilaterality of the cyst,tubal patensy, r-AFS stage with pregnancy rate.Results: A total of 64 subjects were recruited in this study. Therewere 23 subjects (35.9%) that got pregnant within one year afterundergoing laparoscopic procedure. Those who were 35 years oldor less had a greater chance to get pregnant (p = 0.01, OR = 6.75),duration of infertility ï‚£ 3 years had a greater chance to getpregnant with OR = 3.2 and p value = 0.032, r-AFS stage II and IIIhad a greater chance to get pregnant to with (p = 0.04, OR = 3.25and 4.25).Conclusion: The pregnancy rate after laparoscopic procedure is35.9% in this study.       Objective: To analyze factors associated with anxiety in postmenopausal women.   Methods: This study was descriptive analytic with cross-sectional approach conducted in 228 postmenopausal women that aged 50-64 years old in coastal areas of Kendari City. Samples were taken by simple random sampling. The technique of collecting data used questionnaires and the Taylor Manifest Anxiety Scale (T-MAS). Data were analyzed using Chi-square test with a significance value p ï‚£ 0.05.   Results: The results showed that 188 respondents (82.4%) had. anxiety. Its most experienced anxiety was aged 50-54 years old (37.7%), low education (96.0%), as housewife (62.7%), low income (91.2%) and lived with a partner (54.4%). There was correlation between anxiety with age (p=0.016), education level (p = 0.009), income (p = 0.011), and residence status (p = 0.029). There was no correlation between anxiety with occupation (p = 0.351).   Conclusion: There was a correlation between anxiety in postmenopausal women in coastal areas with age, education level, income, and residence status.   Keywords: age, anxiety, education level, income, postmenopausal women, residence statu

    The Association between Menstrual Disorder and Work Disturbance among Employees

    Objective: To investigate the association between menstrualdisorders including each menstrual cycle disorder, durationand volume of menstrual bleeding disorder, menstrual intervaldisorder, and another disorder that related to menstruationagainst disruption of work among employees.Methods: This study used a cross sectional design. Samplerandomization with winpepi® software for Windows 7®. Dataanalysis using SPSS 24® software for Windows 7®.Results: A total of 150 subjects were recruited in this study. Theprevalence of menstrual disorder was 87%, menstrual cycledisorder 3%. Menstrual volume disorder 31%, 15% menstrualabnormalities, 83% menstrual pain disorder and premenstrualsyndrome 71%. Prevalence of work disturbance was 49% formild disturbance, 47% for moderate disturbance and severework disturbance by 4%. There was a relationship betweenmenstrual volume disorder, hypermenorrhoea, and menstrual /dysmenorrhoea pain to occupational disruption among theemployee at RSCM (p <0.001). The submission of the proposedleaves due to menstrual disorder in the proposed permit for1 day by 73%.Conclusion: The prevalence of severe work disturbance due tomenstruation is not high but can lead to disruption in runningjobs and activities.     The Association of Creactive Protein Levels in Second Trimester of Pregnancy with Preeclampsia

    Objective: To determine C-reactive protein in second trimesterof pregnancy women who preeclamptic and non-preeclampticwomen, and to determine the relationship between the level ofC-Reactive protein of trimester two pregnancy and preeclampsiaoccurrence.Methods: This was a prospective study conducted at the policlinicsof network of academic hospitals of the Department of Obstetricsand Gynecology of Faculty of Medicine, Universitas Hasanuddin andpoliclinic of child and mother, and some Public Health Centers inMakassar city from September 2015 - April 2016. The C-reactiveprotein 115 level of pregnant women in pregnancy age of 24-28weeks was checked, whether the subjects underwent preeclampsiauntil the childbirth process. Statistics analysis used Fisher’s exacttest and Mann Whitney test.Results: The result indicate that 9 subjects (7.8%) developedpreeclampsia and 106 subjects did not become preeclampsia. Thelevel of C-Reactive protein increased in preeclampsia group comparedto non preeclampsia group i.e 5.05  1.153 : 3.36  0.265,but statistically the result is not significant (value p>0.05).Conclusion: The average score of C-Reactive protein of preeclampsiagroup is 33.5% higher than non-preeclampsia group,even though these results cannot be used as the score to predictthe preeclampsia occurrence.Keywords: C-reactive protein, preeclampsia, second trimesterpregnanc

    Oral versus Vaginal Misoprostol for Labour Induction : A Comparative Study

    Objective: To compare the efficacy and safety of hourly titrated oralmisoprostol in solution (OMS) with vaginal misoprostol (PV) forlabor induction.Methods: Randomized Controlled Trial (RCT), double blind-add onthe study was conducted from January-November 2016 in deliveryward of Moh. Hoesin general hospital. Women 30 weeks of gestationwith an unfavorable cervix (Bishop score ï‚£6) and an indicationfor labor induction were randomly assigned to receive titrated oralor vaginal misoprostol. The OMS group received a basal unit of20 ml misoprostol solution (1 ï­g/ml) every 1 hour for four dosesand then were titrated against individual uterine response. In theabsence of regular uterine contractions, the dose was increased to40 ml hourly for four doses and then 60 ml for four doses. Thevaginal group received 25 ï­g every 4 hours until attaining a morefavorable cervix for three doses. All the subjects received amylumplacebo. In labor within 12 hours was the primary outcome.Results: A total of 30 women were enrolled in this study. One subjectin the OMS group was dropped out due to eclamptic seizure. Theaverage interval from induction until in labour in OMS group was5.753.14 hour and 6.604.46 hour in PV group (p = 0.56). In labourstage was achieved within 12 hours in 14 women (100%) in OMSgroup and 14 women (93.3%) in PV group (p = 1.00). Vaginaldelivery was achieved within 24 hours in 13 women (92.9%) inOMS group and 15 women (100%) in PV group. The incidence ofuterine hyperstimulation/ tachysystolic was 7.1% in OMS groupcompared with 13.3% in PV group. Fetal distress was found only 1case (7.1%) in OMS group. There was no difference in the maternaland neonatal outcome of labor in both the groups.Conclusion: Oral titrated in solution, and vaginal route ofadministration of misoprostol for induction of labour areequally effective and safe.[Indones J Obstet Gynecol 2018; 6-2: 89-97]Keywords: hourly titrated oral misoprostol in solution, oral misoprostol,randomized controlled trial, vaginal misoprosto
