    The Unruptured Intracranial Aneurysm Treatment Score as a Predictor of Aneurysm Growth or Rupture

    Background and purpose: The Unruptured Intracranial Aneurysm Treatment Score (UIATS) was built to harmonize the treatment decision making on unruptured intracranial aneurysms. Therefore, it may also function as a predictor of aneurysm progression. In this study, we aimed to assess the validity of the UIATS model to identify aneurysms at risk of growth or rupture during follow-up.Methods: We calculated the UIATS for a consecutive series of conservatively treated unruptured intracranial aneurysms, included in our prospectively kept neurovascular database. Computed tomography angiography and/or magnetic resonance angiography imaging at baseline and during follow-up was analyzed to detect aneurysm growth. We defined rupture as a cerebrospinal fluid or computed tomography-proven subarachnoid hemorrhage. We calculated the area under the receiver operator curve, sensitivity, and specificity, to determine the performance of the UIATS model.Results: We included 214 consecutive patients with 277 unruptured intracranial aneurysms. Aneurysms were followed for a median period of 1.3 years (range 0.3-11.7 years). During follow-up, 17 aneurysms enlarged (6.1%), and two aneurysms ruptured (0.7%). The UIATS model showed a sensitivity of 80% and a specificity of 44%. The area under the receiver operator curve was 0.62 (95% confidence interval 0.46-0.79).Conclusions: Our observational study involving consecutive patients with an unruptured intracranial aneurysm showed poor performance of the UIATS model to predict aneurysm growth or rupture during follow-up.</p

    Stressors and support system among parents of neonates hospitalised with systemic infections:Qualitative study in South India

    OBJECTIVE: To explore stressors and support system for families with a neonate admitted with a systemic infection.DESIGN: Qualitative study using in-depth interviews (IDIs), based on principles of grounded theory.SETTING: A busy level III neonatal unit of a tertiary care teaching hospital in coastal Karnataka, India, between May 2018 and January 2019.PARTICIPANTS: Parents and accompanying attendants of neonates admitted to the neonatal unit with one or more systemic infections.METHODS: Using purposive sampling, semi-structured IDIs were audio recorded, transcribed verbatim and a thematic analysis was performed.RESULTS: Thirty-eight participants were interviewed, lasting between 30 and 59 min. Babies' hospitalisation with sepsis was an unprecedented, sudden and overwhelming event. Stressors related to uncertainties due to the information gap inherent to the nature of illness, cultural rituals, financial constraints, barriers to bonding and others. Parents reported experiencing insomnia, gastric disturbances and fatigue. Support (emotional and/or financial) was sought from families and friends, peers, staff and religion. Availability and preference of emotional support system differed for mothers and fathers. In our context, families, peers and religion were of particular importance for reinforcing the available support system. Participant responses were shaped by clinical, cultural, financial, religious and health service contexts.CONCLUSION: Designing a family-centred care in our context needs consideration of stressors that extend beyond the immediate neonatal intensive care unit environment and interactions. Understanding the influence of the nature of illness, financial, familial and cultural contexts helps identify the families who are particularly vulnerable to stress.</p

    Injury Prediction in Competitive Runners with Machine Learning

    Purpose: Staying injury-free is a major factor for success in sports. Although injuries are difficult to forecast, novel technologies and data science applications could provide important insights. Our purpose is to use machine learning for the prediction of injuries in runners, based on detailed training logs. Methods: Prediction of injuries was evaluated on a new data set of 77 high-level middle and long distance runners, over a period of seven years. Two analytic approaches were applied. First, the training load from the previous seven days were expressed as a time series, with each day’s training being described by ten features. These features were a combination of objective data from a GPS watch (e.g., duration, distance), together with subjective data about the exertion and success of the training. Second, a training week was summarized by 22 aggregate features, and a time window of three weeks before the injury was considered. Results: A predictive system based on bagged XGBoost machine learning models, resulted in Receiver Operating Characteristic curves with average Areas Under the Curves of 0.724 and 0.678 for the day and week approach, respectively. Especially the results of the day approach reflect a reasonably high probability that our system makes correct injury predictions. Conclusions: Our machine learning-based approach predicts a sizable portion of the injuries, in particular when the model is based on training load-data in the days preceding an injury. Overall, these results demonstrate the possible merits of using machine learning to predict injuries and tailor training programs for athletes

    Exposure to violence, chronic stress, nasal DNA methylation, and atopic asthma in children

    BACKGROUND: Exposure to violence (ETV) or chronic stress may influence asthma through unclear mechanisms.METHODS: Epigenome-wide association study (EWAS) of ETV or chronic stress measures and DNA methylation in nasal epithelium from 487 Puerto Ricans aged 9-20 years who participated in the Epigenetic Variation and Childhood Asthma in Puerto Ricans study [EVA-PR]). We assessed four measures of ETV and chronic stress in children (ETV scale, gun violence, and perceived stress) and their mothers (perceived stress). Each EWAS was conducted using linear regression, with CpGs as dependent variables and the stress/violence measure as a predictor, adjusting for age, sex, the top five principal components, and SVA latent factors. We then selected the top 100 CpGs (by p value) associated with each stress/violence measure in EVA-PR and conducted a meta-analysis of the selected CpGs and atopic asthma using data from EVA-PR and two additional cohorts (Project Viva and PIAMA).RESULTS: Three CpGs (in SNN, PTPRN2, and LINC01164) were associated with maternal perceived stress or gun violence (p = 1.28-3.36 × 10-7 ), but not with atopic asthma, in EVA-PR. In a meta-analysis of three cohorts, which included the top CpGs associated with stress/violence measures in EVA-PR, 12 CpGs (in STARD3NL, SLC35F4, TSR3, CDC42SE2, KLHL25, PLCB1, BUD13, OR2B3, GALR1, TMEM196, TEAD4, and ANAPC13) were associated with atopic asthma at FDR-p &lt; .05.CONCLUSIONS: Pending confirmation in longitudinal studies, our findings suggest that nasal epithelial methylation markers associated with measures of ETV and chronic stress may be linked to atopic asthma in children and adolescents.</p

At its most rudimentary level, the inscription praises the king and glorifies his commander. However, a closer reading demonstrates the poet playing with the conventions of his time. One of the techniques used to enhance the power of the ruler was to represent the commanders as replicas of their king. The author turns this mechanism into the inscription’s poetic motif.     KiDS-1000 Cosmology:Cosmic shear constraints and comparison between two point statistics

    We present cosmological constraints from a cosmic shear analysis of the fourth data release of the Kilo-Degree Survey (KiDS-1000), doubling the survey area with nine-band optical and near-infrared photometry with respect to previous KiDS analyses. Adopting a spatially flat Λ\LambdaCDM model, we find S8=σ8(Ωm/0.3)0.5=0.7590.021+0.024S_8 = \sigma_8 (\Omega_{\rm m}/0.3)^{0.5} = 0.759^{+0.024}_{-0.021} for our fiducial analysis, which is in 3σ3\sigma tension with the prediction of the Planck Legacy analysis of the cosmic microwave background. We compare our fiducial COSEBIs (Complete Orthogonal Sets of E/B-Integrals) analysis with complementary analyses of the two-point shear correlation function and band power spectra, finding results to be in excellent agreement. We investigate the sensitivity of all three statistics to a number of measurement, astrophysical, and modelling systematics, finding our S8S_8 constraints to be robust and dominated by statistical errors. Our cosmological analysis of different divisions of the data pass the Bayesian internal consistency tests, with the exception of the second tomographic bin. As this bin encompasses low redshift galaxies, carrying insignificant levels of cosmological information, we find that our results are unchanged by the inclusion or exclusion of this sample

    Reducing the Gap Between Pro-Environmental Disposition and Behavior:The Role of Feeling Power

    Environmental issues are some of the most pressing threats the world is facing nowadays. In this context, motivating individual pro-environmental behavior becomes highly relevant. One strategy is to harness people’s pro-environmental dispositions (e.g., biospheric values, proenvironmental attitudes). Although acknowledging the need to behave pro-environmentally lies at the core of these dispositions, the extent to which they are reflected in day-to-day proenvironmental practices fluctuates to a great extent. How to bridge this gap between dispositions and behaviors in pro-environmentalism? This research tests a novel psychological solution, that is, to heighten subjective feelings of power. Power depicts people’s control over their own and others’ outcomes. Two studies (total N = 338, with n = 200 in Study 1 and n = 138 in Study 2) manipulated people’s situational sense of high versus low power (by recalling and writing about relevant incidents), measured pro-environmental dispositions (biospheric values in Studies 1 and 2; attitude toward a specific environmental cause in Study 2), and examined their effects on proenvironmental behaviors (spending time on environmental persuasion in Study 1 and spending money on environmental donation in Study 2). Overall, both studies revealed that proenvironmental dispositions predicted pro-environmental behaviors, but only when the actorswere prompted to experience a high instead of a low sense of power. The findings illuminate power as an important and viable communication tactic—to orient people toward their dispositions and practice what they preach in pro-environmentalism

    Cognitive behavioral therapy for prolonged grief in children and adolescents:A randomized clinical trial

    Objective: Prolonged Grief Disorder (PGD) was newly included in ICD-11 and resembles Persistent Complex Bereavement Disorder (PCBD) newly included in DSM-5. In adults, PGD can be successfully treated using cognitive behavioral therapy (CBT). There is no evidence yet that CBT interventions can successfully alleviate PGD in children and adolescents. The goal of this randomized clinical trial was to examine the effects of CBT for PGD in children and adolescents (named “CBT Grief-Help”) in comparison with the effects of non-directive supportive counselling. Methods: One hundred and thirty four children and adolescents with PGD (aged M=13.10 (SD=2.84, range 8-18) years, bereaved M=37.79 (SD=36.23, range 4-188) months earlier) were randomized to receive either CBT Grief-Help (n=74) or supportive counselling (n=60). Both treatment conditions encompassed nine individual sessions with children/adolescents paralleled by five sessions counselling with parents/caretakers. Children/adolescents completed measures of PGD, depression, and posttraumatic stress disorder (PTSD), and their parents/caretakers completed measures of their children’s problem behavior, before treatment, immediately after treatment, and three-, six-, and twelve-months following treatment.Results: Both treatments yielded moderate to large effect sizes across PGD and most other outcome measures. Compared to supportive counseling, CBT Grief-Help resulted in significantly greater reductions PGD-symptoms at all post-treatment assessments, and more successfully alleviated depression, PTSD, and internalizing problems six- and twelve-months following treatment. Conclusions: PGD and accompanying symptoms in bereaved children and adolescents can be effectively treated using CBT interventions. The superior long-term effects of CBT Grief-Help relative to supportive counselling suggest that this treatment successfully harnesses children and adolescents to the challenges faced after loss
