Frontal electroencephalogram based drug, sex, and age independent sedation level prediction using non-linear machine learning algorithms
Brain monitors which track quantitative electroencephalogram (EEG) signatures to monitor sedation levels are drug and patient specific. There is a need for robust sedation level monitoring systems to accurately track sedation levels across all drug classes, sex and age groups. Forty-four quantitative features estimated from a pooled dataset of 204 EEG recordings from 66 healthy adult volunteers who received either propofol, dexmedetomidine, or sevoflurane (all with and without remifentanil) were used in a machine learning based automated system to estimate the depth of sedation. Model training and evaluation were performed using leave-one-out cross validation methodology. We trained four machine learning models to predict sedation levels and evaluated the influence of remifentanil, age, and sex on the prediction performance. The area under the receiver-operator characteristic curve (AUC) was used to assess the performance of the prediction model. The ensemble tree with bagging outperformed other machine learning models and predicted sedation levels with an AUC = 0.88 (0.81-0.90). There were significant differences in the prediction probability of the automated systems when trained and tested across different age groups and sex. The performance of the EEG based sedation level prediction system is drug, sex, and age specific. Nonlinear machine-learning models using quantitative EEG features can accurately predict sedation levels. The results obtained in this study may provide a useful reference for developing next generation EEG based sedation level prediction systems using advanced machine learning algorithms.</p
Philosophy of education in a new key:On radicalization and violent extremism
This collective paper on radicalization and violent extremism part of the 'Philosophy of education in a new key' initiative by Educational Philosophy and Theory brings together some of the leading contemporary scholars writing on the most pressing epistemological, ethical, political and educational issues facing post-9/11 scholarship on radicalization and violent extremism. Its overall aim is to move beyond the 'conventional wisdom' associated with this area of scholarly research best represented by its many slogans, metaphors and other thought-terminating cliches. By providing conceptual lenses on issues previously compartmentalized primarily [or even exclusively] in security and intelligence studies or at the fringes of scholarly interest, radicalization and violent extremism turn out to be much more complex than 'radicalization studies' has been eager to acknowledge.</p
Emotion dynamics in children and adolescents:A meta-analytic and descriptive review
Theories on children and adolescents emotion dynamics were reviewed using data from 102 ecological momentary assessment studies with 19.928 participants and 689 estimates. We examined age-graded differences in emotional intensity, variability, instability, inertia, differentiation, and augmentation/blunting. Outcomes included positive versus negative affect scales, discrete emotions (anger, sadness, anxiety, and happiness), and we compared samples of youth with or without mental or physiological problems. Multi-level models showed more variable positive affect and sadness in adolescents compared to children, and more intense negative affect. Our additional descriptive review suggests a decrease in instability of positive and negative emotions from early to late adolescence. Mental health problems were associated with more variable and less intense positive affect, and more intense anxiety and heightened sadness variability. These results suggest systematic changes in emotion dynamics throughout childhood and adolescence, but the supporting literature proved to be limited, fragmented, and based on heterogeneous concepts and methodology
Community Health Center Efficiency. The Impact of Organization Design and Local Context:The Case of Indonesia
Background The decentralization of the Indonesian healthcare system, launched in the year 2000, allowed the authorities of local community health centers (CHCs) to tailor their services to the needs of their clients. Many observers see this as an opportunity to increase CHC efficiency. Building on the Context Design Performance Framework, this paper assesses the extent to which efficiency variations between CHCs can be explained by the degree of fit between their organizational design characteristics and aspects of the communities in which they are embedded. Methods Data envelopment analysis (DEA) was applied to construct a measure of CHC efficiency for a sample of 598 CHCs in 2011, drawn from a publicly available Ministry of Health (MoH) dataset. Tobit regression analysis was applied to assess the impact of organization design and community characteristics and their interplay on efficiency. ResultsLarge variations in CHC efficiency were discovered, suggesting that not all CHCs are equally capable of finding the optimal design to operate most efficiently. A significant inverted U-shape relationship was found for the organization design-efficiency link: efficiency is highest for CHCs with 1-2 horizontal units and decreases for CHCs exceeding or not reaching this number. No significant association was found between community characteristics (proportion of poor people, remote location of CHC) and CHC efficiency. Conclusion Organizational design matters for CHC efficiency, but no evidence was found for the hypothesis that a better fit between community characteristics and CHC design increases efficiency. A potential reason for this might be that CHC management’s main design challenge is how to cope with the scarce availability of well-trained health personnel
Session-based concurrency, declaratively
Session-based concurrency is a type-based approach to the analysis of message-passing programs. These programs may be specified in an operational or declarative style: the former defines how interactions are properly structured; the latter defines governing conditions for correct interactions. In this paper, we study rigorous relationships between operational and declarative models of session-based concurrency. We develop a correct encoding of session 휋-calculus processes into the linear concurrent constraint calculus (횕회회), a declarative model of concurrency based on partial information (constraints). We exploit session types to ensure that our encoding satisfies precise correctness properties and that it offers a sound basis on which operational and declarative requirements can be jointly specified and reasoned about. We demonstrate the applicability of our results by using our encoding in the specification of realistic communication patterns with time and contextual information

Teachers’ personal experiences of sexual initiation motivating their sexuality education messages in secondary schools in Kampala, Uganda
Little is known about how teachers’ personal sexual experiences influence their motivations and practices when teaching sexuality education. Cultural schema theory was used to explore teachers’ personal experience of the onset of sexual activity and explain how sexuality education teaching is influenced by such experiences. In-depth interviews were conducted with 40 secondary school teachers in Kampala, Uganda. The interviews were transcribed verbatim and analysed using principles of grounded theory. Findings show that while teachers’ personal experience of sexual initiation did not directly align with the content of their messages, due to the centrality and evocative function of these schemas these experiences strengthened teachers’ motivation to teach sexuality education because they enabled them to empathise with students. The study concludes that teachers’ personal experiences of sexual initiation provide intrinsic motivation for teaching sexuality education. The inclusion of dialogues and activities which encourage self-reflexivity in teacher education and training may improve the delivery of sex, sexuality and relationships education
Comparison of renal region, cerebral and peripheral oxygenation for predicting postoperative renal impairment after CABG
Patients undergoing coronary artery bypass grafting (CABG) are at risk of developing postoperative renal impairment, amongst others caused by renal ischemia and hypoxia. Intra-operative monitoring of renal region tissue oxygenation (SrtO(2)) might be a useful tool to detect renal hypoxia and predict postoperative renal impairment. Therefore, the aim of this study was to assess the ability of intra-operative SrtO(2) to predict postoperative renal impairment, defined as an increase of serum creatinine concentrations of > 10% from individual baseline, and compare this with the predictive abilities of peripheral and cerebral tissue oxygenation (SptO(2) and SctO(2), respectively) and renal specific tissue deoxygenation. Forty-one patients undergoing elective CABG were included. Near-infrared spectroscopy (NIRS) was used to measure renal region, peripheral (thenar muscle) and cerebral tissue oxygenation during surgery. Renal region specific tissue deoxygenation was defined as a proportionally larger decrease in SrtO(2) than SptO(2). ROC analyses were used to compare predictive abilities. We did not observe an association between tissue oxygenation measured in the renal region and cerebral oxygenation and postoperative renal impairment in this small retrospective study. In contrast, SptO(2) decrease > 10% from baseline was a reasonable predictor with an AUROC of 0.767 (95%CI 0.619 to 0.14; p = 0.010). Tissue oxygenation of the renal region, although non-invasively and continuously available, cannot be used in adults to predict postoperative renal impairment after CABG. Instead, peripheral tissue deoxygenation was able to predict postoperative renal impairment, suggesting that SptO(2) provides a better indication of 'general' tissue oxygenation status. Bronchial gene expression signature associated with rate of subsequent FEV1 decline in individuals with and at risk of COPD
BACKGROUND: COPD is characterised by progressive lung function decline. Leveraging prior work demonstrating bronchial airway COPD-associated gene expression alterations, we sought to determine if there are alterations associated with differences in the rate of FEV1 decline.METHODS: We examined gene expression among ever smokers with and without COPD who at baseline had bronchial brushings profiled by Affymetrix microarrays and had longitudinal lung function measurements (n=134; mean follow-up=6.38±2.48 years). Gene expression profiles associated with the rate of FEV1 decline were identified by linear modelling.RESULTS: Expression differences in 171 genes were associated with rate of FEV1 decline (false discovery rate <0.05). The FEV1 decline signature was replicated in an independent dataset of bronchial biopsies from patients with COPD (n=46; p=0.018; mean follow-up=6.76±1.32 years). Genes elevated in individuals with more rapid FEV1 decline are significantly enriched among the genes altered by modulation of XBP1 in two independent datasets (Gene Set Enrichment Analysis (GSEA) p<0.05) and are enriched in mucin-related genes (GSEA p<0.05).CONCLUSION: We have identified and replicated an airway gene expression signature associated with the rate of FEV1 decline. Aspects of this signature are related to increased expression of XBP1-regulated genes, a transcription factor involved in the unfolded protein response, and genes related to mucin production. Collectively, these data suggest that molecular processes related to the rate of FEV1 decline can be detected in airway epithelium, identify a possible indicator of FEV1 decline and make it possible to detect, in an early phase, ever smokers with and without COPD most at risk of rapid FEV1 decline.</p
Societal acceptance increases Muslim-Gay identity integration for highly religious individuals… but only when the ingroup status is stable
Reconciling religious beliefs with a sexual minority identity can be challenging. After coming out, many gay men report to renounce their religious identity or experience increased identity conflict between their religious and sexual identities. Giving up one’s own identity or identity conflict are known to predict negative wellbeing, and it is important to find ways to reduce this conflict and increase identity integration. In this experiment, we conceptualized identity integration as a social creativity strategy, and we examined whether societal acceptance (vs rejection) and ingroup experience (e.g., whether gay community feels stability or improvement about their status) would alter one’s level of Muslim-gay identity integration. We found that Muslim-gay identity integration was highest among highly religious gay men when societal acceptance was present and ingroup experience was stable. Overall, we discuss our findings by drawing parallels between social identity theory and bicultural identity integration framework, and provide implications to increase identity integration for those with multiple conflicting identities
