    Linguistic mechanisms of coherence in aphasic and non-aphasic discourse

    Background: Coherence is the quality that distinguishes discourse from a random collection of sentences. People with aphasia have been reported to produce less-coherent discourse than non-language-impaired speakers. It is largely unclear how coherence is established in natural language and what leads to its impairment in aphasia.Aims: This paper presents a cross-methodological investigation on coherence in the discourse of Russian native speakers with and without aphasia. The purpose of this study was to examine the connection between language impairments in aphasia and different aspects of discourse coherence in order to determine the linguistic mechanisms that could be involved in establishing and maintaining it.Methods &amp; Procedures: Coherence was operationalised as a combination of four aspects: informativeness, clarity, connectedness, and understandability. Twenty participants were asked to retell the content of a short movie. The retellings were annotated using Rhetorical Structure Theory (RST), a formalistic framework for discourse-structure analysis. Next, they were evaluated for coherence on a four-point scale by trained raters. The ratings were compared between groups. A classification analysis was performed to determine whether the ratings could be predicted based on the macrolinguistic variables collected from the RST annotations and several microlinguistic variables previously linked to coherence.Results: Retellings produced by speakers with aphasia received lower ratings than those of control participants on all aspects of coherence. The results indicate that different combinations of microlinguistic and discourse-structure variables play a role in establishing each of the coherence aspects.Conclusions: Our results provided supporting evidence on coherence impairment in aphasia. Perception of a discourse as more or less coherent was associated with both microlinguistic and macrolinguistic variables, with different combinations of variables relevant for each of the aspects. Furthermore, we found that discourse structure plays an important role, especially for understandability. We speculate that pragmatic knowledge shared by interlocutors may boost the coherence of aphasic discourse.</p

    Temporal context effects are associated with cognitive status in advanced age

    The perception of temporal intervals changes during the life-span, and especially older adults demonstrate specific impairments of timing abilities. Recently, we demonstrated that timing performance and cognitive status are correlated in older adults, suggesting that timing tasks can serve as a behavioral marker for the development of dementia. Easy-to-administer and retest-capable timing tasks therefore have potential as diagnostic tools for tracking cognitive decline. However, before being tested in a clinical cohort study, a further validation and specification of the original findings is warranted. Here we introduce several modifications of the original task and investigated the effects of temporal context on time perception in older adults (&gt; 65 years) with low versus high scores in the Montreal Cognitive Assessment survey (MoCA) and a test of memory functioning. In line with our previous work, we found that temporal context effects were more pronounced with increasing memory deficits, but also that these effects are stronger for realistic compared to abstract visual stimuli. Furthermore, we show that two distinct temporal contexts influence timing behavior in separate experimental blocks, as well as in a mixed block in which both contexts are presented together. These results replicate and extend our previous findings. They demonstrate the stability of the effect for different stimulus material and show that timing tasks can reveal valuable information about the cognitive status of older adults. In the future, these findings could serve as a basis for the development of a diagnostic tool for pathological cognitive decline at an early, pre-clinical stage.</p

    The role of innovation for employment growth among firms in developing countries:Evidence from Nigeria

    This paper examines the nexus between innovation and employment growth. In contrast to most studies that employ data from technologically advanced economies, the study uses well-documented data from a leapfrogging and catching-up economy in sub-Saharan Africa. We find a positive relationship between process innovation and employment growth among Nigerian manufacturing and services firms from 2005 to 2010. Furthermore, product innovation is also positively related to employment growth in manufacturing. This study's peculiarity is that innovative firms' ability to grow rely on their operations in regional and export markets, respectively. Our study contributes to the understanding of innovation and firm development in emerging economies.</p

    Therapists' perceptions and attitudes in facial palsy rehabilitation therapy:A mixed methods study

    Background: Facial palsy rehabilitation therapy plays an essential role in treating facial palsy.Purpose: This study aimed to gain insight into therapists' perceptions and attitudes toward facial palsy rehabilitation therapy and to examine whether therapists could be categorized into distinct groups based on these attitudes and perceptions.Methods: Thirteen semi-structured, in-depth interviews were conducted in a purposive sample of therapists. Interviews were analyzed using thematic analysis. Next, a questionnaire containing questions about therapists' characteristics and perceptions and attitudes toward facial palsy rehabilitation therapy was sent to all facial palsy rehabilitation therapists in the Netherlands and Flanders (n = 292). Latent class analysis (LCA) was performed to identify and analyze distinct groups of therapists.Results: Seven themes were derived from the interviews: treatment goals, therapy content, indications, measurement instruments, factors influencing success, emotional support, and cooperation with colleagues. The questionnaire was filled out by 127 therapists. A 2-group structure consisting of a positive class and a negative class was found to fit the questionnaire data best. No distinction could be made regarding therapists' characteristics.Conclusion: Considerable variation in stated treatment practices was present among therapists. Therapists could be classified into 2 groups. This study raises several hypotheses that require further study.</p

    Tic disorders in children and adolescents:does the clinical presentation differ in males and females? A report by the EMTICS group

    Tic disorders have a strong male predominance, with a male-to-female ratio of 4:1 in Tourette syndrome (TS) and 2:1 in persistent tic disorders. In other neurodevelopmental conditions, such as autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD), the disparity in sex distribution has been partially related to differences in symptom presentation between males and females. In tic disorders, however, little research has been conducted on this topic, probably due to the limited access to large samples with a significant proportion of females. The aim of this study was to describe sex differences in the clinical presentation of tic disorders in children and adolescents in one of the largest pediatric samples with TS/persistent tic disorders (n = 709, 23.3% females) recruited as part of the European Multicenter Tics in Children Study (EMTICS). Validated measures assessed the severity of tics and comorbid psychiatric symptoms. Using mixed-effect models, we found that sex had a significant influence on the severity of tics, ADHD symptoms, ASD symptoms, and emotional problems. Males had more severe symptoms than females, except for emotional problems. We also observed a statistically significant interaction between sex and age on the severity of tics and compulsions, with females showing higher symptom severity with increasing age than males. These findings indicate that the clinical presentation of TS/persistent tic disorders varies with sex. Males seem to exhibit a more noticeable pattern of clinical symptoms at a younger age that may contribute to their earlier detection in comparison to females.</p

    A longitudinal qualitative study to explore and optimize self-management in mild to end stage chronic kidney disease patients with limited health literacy:perspectives of patients and health care professionals

    ObjectivesLimited Health literacy (LHL) is associated with faster kidney deterioration. Health care professionals (HCPs) promote self-management to maintain kidney function, which is difficult for patients with LHL. Evidence lacks on perceived barriers and best strategies to optimize their self-management. Our study aims to explore experiences with and barriers for self-management from the perspectives of LHL patients and HCPs to identify strategies to optimize self-management.MethodsWe performed a longitudinal qualitative study with semi-structured in-depth interviews and focus group discussions among CKD patients and LHL (n=24) and HCPs (n=37) from general practices and hospitals.ResultsFour themes arose among patients: 1) CKD elusiveness, 2) suboptimal intake of knowledge 3) not taking a front-seat role, and 4) maintaining change. Among HCPs, three themes emerged: 1) not recognizing HL problems, 2) lacking effective strategies, and 3) health care barriers.ConclusionWe suggest three routes to optimize self-management: providing earlier information, applying person-centred strategies to maintain changes, and improving competencies of HCPs.Practice implicationsHCPs need to explain CKD self-management better to prevent kidney deterioration. New interventions, based on behavioural approaches, are needed to optimize self-management. HCPs need training to improve recognition and support of LHL patients

    Epidemiology of combined clavicle and rib fractures:a systematic review

    Purpose The aim of this systematic review was to provide an overview of the incidence of combined clavicle and rib fractures and the association between these two injuries. Methods A systematic literature search was performed in the MEDLINE, EMBASE, and CENTRAL databases on the 14(th) of August 2020. Outcome measures were incidence, hospital length of stay (HLOS), intensive care unit admission and length of stay (ILOS), duration of mechanical ventilation (DMV), mortality, chest tube duration, Constant-Murley score, union and complications. Results Seven studies with a total of 71,572 patients were included, comprising five studies on epidemiology and two studies on treatment. Among blunt chest trauma patients, 18.6% had concomitant clavicle and rib fractures. The incidence of rib fractures in polytrauma patients with clavicle fractures was 56-60.6% versus 29% in patients without clavicle fractures. Vice versa, 14-18.8% of patients with multiple rib fractures had concomitant clavicle fractures compared to 7.1% in patients without multiple rib fractures. One study reported no complications after fixation of both injuries. Another study on treatment, reported shorter ILOS and less complications among operatively versus conservatively treated patients (5.4 +/- 1.5 versus 21 +/- 13.6 days). Conclusion Clavicle fractures and rib fractures are closely related in polytrauma patients and almost a fifth of all blunt chest trauma patients sustain both injuries. Definitive conclusions could not be drawn on treatment of the combined injury. Future research should further investigate indications and benefits of operative treatment of this injury.</p

    Anger Responses in Adolescents:Relationship with Punishment and Reward Sensitivity

    A substantial proportion of youth with anxiety disorders shows comorbid behavioral (anger) problems. Such comorbid profile is associated with low treatment effectiveness and negative (longterm) outcomes. This study was therefore designed to examine trait factors that may promote anger responding in adolescents. By presenting participants (N = 158, mean age = 15.7, 56% female) with a series of common anger-eliciting situations, we tested whether high reward sensitivity would be associated with anger via perceived non-reward, and high punishment sensitivity via perceived threat. In line with the hypotheses, an indirect effect of reward sensitivity on anger was found via perceived non-reward, and an indirect effect of punishment sensitivity on anger via perceived threat. The latter association also had an indirect effect via perceived non-reward. High punishment and reward sensitivity may thus set adolescents at risk for developing (comorbid) anger problems via heightened threat and non-reward perceptions.</p

    Development of an Inquiry Stance? Perceptions of Preservice Teachers and Teacher Educators Toward Preservice Teacher Inquiry in Dutch Primary Teacher Education

    This study aims to gain insight into the perceived purpose and value of preservice teacher inquiry in Dutch primary teacher education by teacher educators and preservice teachers at the undergraduate level; it also assesses the implementation of teaching and learning activities, and learning outcomes associated with teacher inquiry. In the Netherlands, inquiry competence in primary teacher education develops over a 4-year period, resulting in students' completion of capstone projects using practitioner inquiry. The authors combine a survey with focus groups of teacher educators and preservice teachers from eight institutes. They find differences between preservice teachers' perceptions of the implementation of inquiry competence and teacher educators' visions and perceptions of such implementation. All participants, students and educators, believe inquiry to be valuable and perceive learning outcomes of inquiry to be enriching, yet about half of the preservice teachers do not to expect to undertake inquiry in their future teaching jobs.</p

    Integrating cultural and biological perspectives on long-term human-walrus (Odobenus rosmarus rosmarus) interactions across the North Atlantic

    The hunting of marine mammals as a source of subsistence, trade, and commercial revenue has formed an important part of human cultures across the North Atlantic. One important prey species has been the Atlantic walrus (Odobenus rosmarus rosmarus), sought after for meat, skin, blubber, ivory, and bone. Unfortunately, biological studies of current walrus populations and studies across the humanities and social sciences into past use and hunting of walruses, have been poorly integrated. Disciplinary boundaries have left a gap in understanding the reciprocal effects of human-walrus interactions. Emerging interdisciplinary methods offer new opportunities to write the historical ecology of Atlantic walruses. The integration of methods such as ancient DNA, isotopes, past population modelling, zooarchaeological assemblages, and ethnographic interviews can now be used to answer previously intractable questions. For example, how has walrus hunting shaped and been influenced by changes in human settlement and trade, what have been the cumulative impacts on walrus populations, the extent of anthropogenic selective pressures or the effect of changing hunting regimes on particular populations of walruses? New, collaborative research approaches applied to the wealth of Arctic archaeological faunal remains already housed in museum collections offer a unique chance to explore the past dynamics of human-animal interactions.<br/
