Synthesis of Pyrazole and Thiophene Derivatives Together with their Antitumor Evaluations
No full textThe reaction of cyclohexanone, cyanoacetylhydrazine and elemental sulfur gave the 4,5,6,7-tetrahydrobenzo[b]thiophene derivative 3. The latter compound was used as the key starting material for the synthesis of pyrazoles, thiazole derivatives. The newly synthesized products were screening against the three cancer cell lines namely the human tumor cell lines, MCF-7 (breast adenocarcinoma), NCI-H460 (non-small cell lung cancer), and SF-268 (CNS cancer) and the human normal cell line the normal fibroblast WI 38. The results showed that compounds 7a and 19c were the most potent compounds among the tested compounds

Since the discloser of its structure in 1969, (Z)-(-)-14-methyl-8-hexadecen-l-ol and methyl (E)-(-)-14-methyl-8-hexadecenoate as the sex pheromones have been the subject of intense study by the synthetic organic community. Case Report: Psychotic Depression with Pseudodementia Misdiagnosed as Dementia Remitted with TCA
No full textEpidemiological research has indicated that elderly patients with depression are at increased risk of subsequent dementia including Alzheimer’s disease. In addition, several studies have also shown that depressive pseudodementia may well develop into irreversible dementia. It is a matter of course, however, that all depressive patients with pseudodementia do not progress to irreversible dementia. The present case is an elderly woman with psychotic depression who was misdiagnosed with dementia because of poor response to serotonin selective reuptake inhibitor (SSRI) and serotonin and norepinephrine reuptake inhibitor (SNRI) as well as a manifestation of moderate cognitive impairment. She was remitted with pharmacotherapy by tricyclic antidepressant (TCA), with no cognitive problems. Psychiatrists should take care not to give too much weight to certain evidence that pseudodementia is a strong predictor of dementia, and should observe individual depressive patients carefully and treat them in any possible way including with TCA or electroconvulsive therapy

In 2014, The Commonwealth Fund ranked the US last among the developed countries including Canada and Australia; however we were the first in expenditure. Medicare and Health accounts for 27% of the Federal Budget, and the annual US Health care spending by households, businesses, and the government is due to reach over 3 trillion dollars in 2015. This article reviews and suggests cost containment strategies, the pros and cons of consolidation of health care, and the future of computerized medicine

No full textBackground: CosmoFer® solution for injection contains stable aqueous solution of iron(III)-hydroxide dextran complex, which is equivalent to the physiological ferritin.CosmoFer® has been used in the Hemodialysis Department/HDD/ at the Sofia St. Anna Hospital since June 2012.Objective: Topresent the use of the medication in the routine practice in the Department.Materials and Methods: The use of the medication in the routine practice for a period of 1 year and 4 months in 63 chroniodialysis patients was presented. Clinical cases with manifestations of adverse effects were reported.Results and conclusions: CosmoFer® showed very good effects when used once or twice per month of renal anemia and chronic kidney disease (CKD) patients.The clinical cases presented are indicative of patients with severe clinical status and polymorbid patients.Comfort and safe administration are achieved in slow intravenous infusion of CosmoFer®dissolved in 100ml physiological solution
Comparison between Utilization of Botox Versus Fluticasone Furoate in Patients with Intermittent Allergic Rhinitis: A Randomized, Controlled Trial
No full textBackground: Allergic rhinitis is a common disease affects about 5 to 22% from general population. Botulinum toxin is a neurotoxic protein assumed to lead to symptomatic control of intermittent allergic rhinitis. Objectives: To compare between utilization of botulinum toxin type A versus fluticasone furoate in treatment of intermittent allergic rhinitis.Patients and Methods: A randomized clinical controlled trial study created on seventy two adult patients with intermittent allergic rhinitis divided into two groups; Group B (BTX-A) and Group F (fluticasone furoate).Results: After one month the mean intensity of nasal symptoms according to VAS among the Group B (BTX-A) sneezing was 2.41 while 2.16 in Group F (fluticasone furoate). The difference is not statistically significant.Conclusions: Intranasal injection of Botulinum A(BTX-A) is a highly effective, safe, and simple procedure, with a long-lasting symptomatic relief for patients with intermittent allergic rhinitis. It as a might be used as an alternative treatment for fluticasone furoate nasal spray
Update on Vestibulotoxicity: How can we Recognize and Prevent it
No full textDrugs and other substances may cause irreversible damage to the inner ear. The term vestibulotoxicity refers to the toxic effect on the peripheral vestibular system and is usually caused by damage to the hair cells of the semicircular canals and otolithic organs. The devastating consequence of vestibulotoxicity is bilateral peripheral vestibular loss, including the phenomenon of oscillopsia. Parenterally administered streptomycin and gentamicin are the most common cause of vestibulotoxicity. There are few descriptions of other potentially vestibulotoxic drugs and substances, including cisplatin, solvents, organophosphate poisoning and others. Analysis of the literature shows weak evidence for true vestibulotoxicity. Unilateral vestibulotoxicity has been described after the use of ear drops containing gentamicin and, interestingly, also after parenteral use of gentamicin.We here present an updated review of vestibulotoxicity, and suggest the appropriate investigation for suspected unilateral and bilateral vestibular loss
Nevus of Ota: Effective Treatment by Laser
No full textThe nevus of Ota is a dermal nevus characterized by bluish pigmentation in the distribution of the first and second division of trigeminal nerve. A total of ten cases were included in our study, detailed history and complete clinical examination were done for all patients. Most of the patients had lesions at birth and the majority was females. The treatment was done by Q-Switched Nd:YAG laser 1,064 nm and fractional 2940nm Er:YAG laser. 34% show excellent results, 53% show good results, while 13% show poor results.Objective: The aim of this study is to explore the long lasting results of Q-Switched laser in association with fractional laser for treatment of nevus of ota

Effect of Tobacco Restriction on Length of Hospital Stay of Psychiatric Patients
No full textPurpose: The objective of the study was to compare the length of hospital stay of smokers, former smokers, and nonsmokers admitted to the psychiatric unit before and after an increase in the severity of smoking restriction.Design and Methods: A cross-sectional study was conducted with 270 patients (smokers, former smokers, and nonsmokers) hospitalized in a psychiatric ward of a general hospital. They were divided into two groups: (1) 101 patients allowed one cigarette per hour (from 8:00 a.m. to 10:00 p.m.) and (2) 169 patients whose restriction was changed to eight cigarettes/day.Findings: The smokers hospitalized after the change in the patients’ access to cigarettes presented shorter length of hospitalization (13.8 days) than with smokers hospitalized and allowed one cigarette per hour (17.1 days). Regardless of the restriction in place, the smokers’ average length of hospital stay (14.9 days) is shorter than the average for former smokers (17.7 days) and for nonsmokers (17.6 days).Practice Implications: The restriction on smoking may be an important tool of care in psychiatric services; however, the way it is implanted, the support resources available, and the training of nurses to enable them to deal with patients during the course of implementation must be revised
