    NECK PAIN - RISK FACTORS AND PROPHYLAXIS

    Indtroduction: Neck pain is one of the most common complaints in physical therapy practice. It is a widespread condition and it rates second in frequency of patient complaints of the musculoskeletal system after low back pain. Neck pain is the fourth leading cause of disability. That makes it a significant medico-social problem. First of all this condition leads to absence from work anddisability which is associated with huge losses for the economy and for the healthcare system. Secondly, it causes reduced quality of life and worsening of the mental health of the people affected. This necessitates new research for therapeutic approaches, timely rehabilitation and methods of prophylaxis,in order to achieve full recovery and prevent relapses and complications.Goal: The aim of this article is to review the most common causes of neck pain and introduce treatment regimens valuable to the medicinal practice of the physical therapy specialist.Materials and methods: Review of the literatureResults: Risk factors leading to neck pain should be easily recognized and clinical decision must be made. This will ensure all causes treatment and choosing correct physical therapy modalities. Detailed comprehension of the risk factors and accurate timely diagnosis of patients with neck pain provide the opportunity for well-planned complex treatment with the inclusion of key physical modalities.Conclusion: Preventive measures related to building habits for correct posture and behavior when performing daily work activities, as well as striving for a healthy lifestyle, regular inclusion of physical exercises and preventive courses with physical therapy, are a successful strategy for managing neck pain

    The latest findings in neuroscience prove some essential facts about neural data processing. Modern narratology proves how important stories are not only in our daily lives, but also as a means of learning. Stories reveal everyday actions, emotions, but also the overall cultural context in which they were created. The use of narrative forms (movies, mininovels, commercials...) in the learning of languages is based on the understanding of our consciousness as essentially empathic, flexible and a priori narrative. In this way, language learning can be very effective, transforming from a difficult process into an entertaining and exciting activity. Engaging with stories provides opportunities for a pragmatic approach to the foreign language. It stimulates the imagination and emphasizes the connections between language, thought, and culture. Commercials and film narratives make emotional references to human longings, eternal dreams, and also to a range of modern day problems and pains. They also provide rich material for a variety of language exercises as well as for the analysis of social and cultural codes

    The discipline cricket, until recently unknown to Bulgaria, has its traditions at Plovdiv Medical University. The beginning was founded in 2015 year by the student Kiran Dasan – he was the main contributor, now a doctor at London Kings Hospital. The game of Cricket is dynamic, attractive and easily attracts supporters. At the moment, more than 90 students are engaged in this sport at MU–Plovdiv, and the best of them are formed in 6 teams and are part of the University cricket league. Along with technical, tactical and mental preparation, the requirements of a good cricketer also imply excellent physical abilities. In the present complex of exercises, we have developed those to improve the flexibility of the athletes, which, although it is not a basic quality for this type of sport, has its contribution to prevent injuries, improve agility, and hence for faster and more accurate mastering of the performance technique

    The Higher Commercial School in Varna is founded in 1920. It is the second oldest high school and the first commercial high school in Bulgaria. Its university curricula played a pioneering role for the establishment of language teaching in higher business education in Bulgaria. Significant place in the first university curricula was reserved for the language teaching. Distinguished linguists who had graduated abroad were recruited to teach foreign languages. Two of them were elected for rectors. The aim of this paper is to comment upon the role of the foreign language teaching in the university curricula and to popularize the names and work of the first rectors at the Higher Commercial School in Varna

    ОТРАЖЕНИЕ НА ПАНДЕМИЯТА ОТ COVID-19 ВЪРХУ СОЦИАЛНАТА РАБОТА ПРИ ХОРА С УВРЕЖДАНИЯ

    Темата на настоящия доклад е обусловена от появилата се у нас през 2020 г. и действаща до началото на месец май 2023 г. пандемия от COVID–19, когато от СЗО статутът на пандемия е отменен, която се отрази върху целия обществен живот в Република България, засягайки в немалка степен и социалната работа при хората с увреждания в страната. Разгледани са някои аспекти на отражението на пандемията в центровете за работа с хора с увреждания.Цел: Да се проучат някои отражения на пандемията от COVID–19 върху социалната работа при хора с увреждания.Материал и методи: Проведено е анкетно проучване сред 40 социални работници в 5 центъра за работа с хора с увреждания от град Сливен относно дейността им в условията на пандемията от COVID–19. Резултатите са обработени статистически.Резултати и обсъждане: Социалните работници не се влияят в много голяма степен (92%), от това, че човек, когото познават е болен от COVID–19 и биха го избрали за приятел. Все пак за 8% от тях приятелството с болен от COVID–19 е нежелателно. Много голяма част от тях (82%) могат да убедят потребителите на предлаганата от тях социална услуга, да се придържат към наложените ограничения в условия на пандемия, но 18% не са се справили с убеждението в спазване на ограниченията. Още повече от тях (26%) не могат да убедят потребителите на предлаганата от тях социална услуга, да се придържат към задължителните изисквания за тяхното поведение в условия на пандемия. За 74% от социалните работници при ежедневната им работа в центровете това не е проблем

    The present article aims to classify the mistakes made by first year students of Pharmacy, Medicine and Dental medicine in the course of Latin language and medical terminology. The terminological competence in the field of medical science is important for the students both for their further training and their future professional development as medical specialists.The difficulties faced by first-year students in mastering the terminological basis are manifold: the complexity of Latin grammar, the specificity of the artificially created terminological language, the lack of philological attitude of the learners, etc. Among the most common grammatical challenges are mastering the principles of term formation in anatomical, clinical and pharmaceutical terminology, as well as basic grammatical rules.We do a systematization of the mistakes made by the first year students as evidenced by the midterm tests and final exams on the Blackboard electronic platform. Their analysis is the key to revealing the specific difficulties in learning Latin medical terminology as well as to identifying mechanisms for solving the problems and achieving effective results in learning process

    Conversions In Minimally Invasive Colorectal Resections. Risk Assessment And Analysis Of Perioperative Results

    INTRODUCTION: Since the first announced laparoscopic colorectal resections in the 1990s, this approach has become widely accepted due to its advantages and better perioperative results with long-term ones being comparable to those of open surgery. Nowadays, the minimally invasive approach is accepted as a gold standard in many centers. Despite this, there are situations which require conversion to open surgery during surgery. Conversion is related to loss of the potential benefits of the minimally invasive approach and is accompanied by a higher risk of complications. According to some publications, the conversion risk is up to 30%. Factors related to higher conversion risk are disease, patient, level of competence of the surgeon, and others. The risk factors should be identified before the surgery. Comparison of the results from open surgery with those of conversions can give the answer whether it is appropriate to begin every elective colorectal resection as minimally invasive or conversion might have poorer results than the open approach. AIM: The aim of this article is to conduct a comparative analysis of the perioperative results in a group of 31 conversions during 183 minimally invasive colorectal resections to 102 open resections. We aim to define and create a model of conversion risk factors based on preoperative data. MATERIALS AND METHODS: Based on retrospective study of 285 patients with elective colorectal resections divided into 3 groups—102 patients with open approach, 152 minimally invasive, and 31 conversions, perioperative results were reviewed. All the methods for clinical assessment were applied. Statistical analysis was performed with IBM SPSS Statistics 25.0; MedCalc Version 19.6.3, Excel Office 2021. The sensitivity and specificity of contrast-enhanced CT were used for preoperative assessment of stage T4 colorectal cancer. RESULTS: The conversion level in the group of minimally invasive resections was 16.93%. Perioperative mortality rate in converted patients was 0% compared to 1.96% in the open surgery group and 1.31% in the minimally invasive one. A higher morbidity rate was registered in conversion group—34.4%. The compli-cations were more severe according to Clavien-Dindo scale. Conversions were related to a longer hospital stay—8.58 days, compared to open surgery—7.61 days; longer operative time, and higher blood loss. Despite this, the harvested lymph nodes were more—15.88 for converted patients, 13.34 for the minimal-ly invasive group, and 11.11 for the open surgery patients. Factors with higher conversion rate were identified: male gender, comorbidity ≥3, age ≥78, BMI ≥30 kg/m2, palpable formation, CT T4 for colon cancer, MRI T4 for rectum, personal experience of the surgeon <38 minimally invasive resections, previous open surgery. CONCLUSION: Cases of locally advanced colorectal cancer, obese patients, previous major open surgery, and surgeons without a completed learning curve are factors related to a higher risk of conversion. Converted patients had poor perioperative results than open surgery patients

    6MWT in Patients after Coronary Artery Stenting

    The 6-minute walk test (6MWT) is an easily reproducible test that measures the body's response to submaximal exercise. The test is widely applicable. The aim of the present report is to analyze the results of the 6MWT performed in patients after coronary artery stenting. The study included 30 patients, divided into two groups of 15 each, according to the kinesitherapy they received. In the control group, we administered standard kinesitherapy to patients with cardiovascular diseases. Meanwhile, in the experimental group, we implemented a specialized kinesitherapeutic program that included isometric and resistive exercises.Results. Mean values and standard deviations of distance traveled, oxygen saturation, arterial pressure, and heart rate were calculated. In the control group, the distance traveled showed an improvement of 78.5 m, while in the experimental group, it improved by 83.5 m. We observed a smoother increase in blood pressure in both groups after the therapy. Oxygen saturation in the experimental group was 94.27 at the beginning of the study and 94.60 by the end, and in the control group it was 94.67 (at the beginning) and 94.53 (by the end). The degree of dyspnea on the Borg scale in the patients of the control group at the beginning of the study was 2, and then 0.4. In the experimental group, the degree of dyspnea was 1.1 before kinesitherapy and decreased to 0.17 after kinesitherapy.Conclusion. The 6-minute walking test is a simple method for evaluating the body's response to everyday stresses. The analysis of the test results before and after kinesitherapy indicates an improvement in the endurance of the participants, signifying an enhancement in their quality of life

    Impact Of Marine Pollution On Human Health

                The greatest threat to the oceans and seas is related to human activities leading to increasing pollution from various sources. This pollution is considered to be the major environmental cause of diseases, responsible for around 9 million premature deaths per year, with a range of direct and indirect consequences for human health. The state of the European seas, including the Black Sea, is considered poor.Purpose: To research the pollution of the Black Sea and its specific pollutants and to analyse and systematise the possible consequences for human health.Material and Methods: The study analysed reports from the European environment agency and the Bulletin of the State of the water in the Black Sea basin management area, as well as publications from the scientific databases PubMed, Scopus and Web of Science. The primary focus is on the health risks associated with major pollutants in European seas and the Black Sea.Results and discussion: The Helsinki Commission identifies pollutants as hazardous substances that can harm the marine environment. They are toxic, persistent and prone to bioaccumulation. Heavy metals, organotin compounds, harmful synthetic substances, polybrominated diphenyl ether and petroleum products are major pollutants. These substances accumulate in the food chain and represent a potential risk to human health if they reach high levels.Conclusion: The European seas, including the Black Sea, are suffering from severe environmental impacts due to toxic and persistent pollutants. Approximately 91% of the Black Sea is contaminated with toxic metals, industrial chemicals and plastic waste. An integrated approach is needed to tackle these challenges, as well as raising awareness of the risks of marine pollution and human health
