Endodontic treatment of premolars with atypical root canal anatomy. Case reports
The main purpose of the endodontic treatment is to completely remove all of the necrotic and infected tissues from the root canal. The mandibular premolars usually present with a simple root canal configuration—one root and one root canal in it, but cases of mandibular premolars with additional root canals and severe curvatures of the root canals have been reported in the literature. Proper diagnosis and analyses of the preoperative condition are considered very important for the long-term success of the root canal treatment. The aim of this article is to present two cases of mandibular premolars presenting with more complex root canal system anatomy
Innovative ways to detect and monitor vestibular deficiencies
Background. Vestibular dysfunction, which encompasses the presence of nystagmus, dizziness, vertigo symptoms, needs to be assessed in a timely manner, especially in situations when rapid diagnostic decisions are to be taken. Dizziness, as a symptom, is increasingly commonly diagnosed nowadays, even in younger patients. Videonystagmography (VNG) is one of the most widely used methods to record the presence or absence of nystagmus. The implementation and deepening of digital diagnostic workflow in otology and neurotology by including digitalization methods and telemedicine aids the diagnostic process and costs less.Methods. Scientific databases – PubMed, SCOPUS and MEDLINE, were targeted as sources to gather the data needed to execute the review of literature. Information from scientific books on the thematic, brochures, leaflets was added. Authors used the following keywords: nystagmus, vertigo, smartphone, with time period set 2014-2024. The aim of the following review article is to point out the usefulness of certain innovative methods in nystagmus detecting and monitoring, including the usage of Artificial intelligence (AI) methodology and telemedicine, which can intensify the patients’ role in the process of diagnostics and rehabilitation of vestibular disorders.Results and discussion. Clinicians nowadays focus and maintain the course of putting timely diagnosis, based on profound diagnostic measures. They include a number of digital methods, including smartphones and telemedicine.Conclusions. The onset of the appearance of vestibular disorders according to authors’ data is starting at an earlier age than before. The latter necessitates adequate treatment plan and vestibular rehabilitation execution in order to restore the lost quality of life of patients affected. Prevention and prophylaxis of vestibular disorders are not neglected. It is evident that clinicians in the field of otorhinolaryngology, as colleagues from other medical fields, are trying to deepen the digitalization of the treatment workflow by including artificial intelligence methods and telemedicine, which provide rapidity, accuracy and cost savings
Re-Interpretation Of Structure-Function Correlations And Inhibitions Of Mammalian Cyclooxygenase Isozymes With Murburn Concept
As per the acclaimed classical mechanism, cyclooxygenase (COX) supposedly has two functional sites: an aqueous phase exposed peroxidase (perox) site and a lipid-phase buried cyclooxygenase (cyclox) site. Peroxidative reaction of heme-Fe at perox site forms a high-potential Compound I species, which supposedly generates a tyrosine radical within the cyclox site via an electron-tunneling mechanism. Thereafter, two molecules of triplet oxygen are purportedly inserted into the bound lipid substrate at cyclox site to yield PGG2. Subsequently, PGG2 is moved to perox site where it is converted to PGH2. Herein, we surveyed diverse COX proteins and tested out in silico binding interactions with various substrates and inhibitors. We found that our ligand-docking results and significant experimental reports in literature support a DRS (diffusible reactive species)-based murburn mechanism, wherein the actual “oxygen-insertion reaction step” may occur beyond the physical constraints of the deep-seated active-site (wherein Fe-O or TyrO species directly interact with the substrates’ C-atom). Further, our interpretations are also supported by the earlier reports that question the wisdom of using “induced fit” type mechanism for fitting the protein’s crystal-structure, by invoking extensive “conformation-change” based explanations.
