Improving Patient Flow and Operational Efficiency in Emergency Rooms using a Discrete Event Simulation Approach
Emergency rooms (ERs) are essential components of the healthcare system, but in recent times, ERs across Canada have been experiencing extended emergency room length of stay (ERLOS), patients leaving without being seen, and overcrowding. The purpose of this research is to address these issues by utilizing a discrete event simulation approach to improve patient flow and operational efficiency. We propose two policies and interventions that can help alleviate the pressure on ERs, improve operational efficiency, and reduce complications associated with delayed treatment. The first policy is an Acute Medical Unit which is hospital unit that is staffed and equipped to receive patients with acute medical illness and provide rapid assessment and treatment to emergency patients. The second policy is an On-Call Physician, a physician who is called when the number of active patients in ER exceeds twice the ER capacity and helps with the increased workload. To develop the simulation model and test the impact of proposed strategies, we use Rockwell Arena 16 and factor in real-life factors associated with ERs such as arrival rates, service times, and patient acuity levels. In addition, the impact of entry and access blocks to and from the ER is examined. An Entry Block prevents patients from accessing treatment in the ER as a result of a lack of capacity. An Access Block prevents patients from accessing a bed in the hospital itself. Both factors have a significant impact on ER operations and efficiency. Our findings indicate that the proposed intervention strategies can reduce the time patients spend waiting for treatment and the number of patients leaving without being seen. This reduces the complications associated with delayed treatment and addresses overcrowding in emergency rooms. Therefore, the proposed policies have the potential to improve patient flow and operational efficiency in ERs. These findings have significant implications for healthcare facilities as they can utilize this simulation model to test various resource planning strategies and make informed decisions to improve patient healthcare experiences
A Study of Retirement Transition and Fandom of Retired Hockey Players
The purpose of this study was to understand and explore the retirement transition of professional men’s hockey players. Specifically, the research examined the role athlete identity theory has this important period of their lives (Brewer et al., 1993). Previous research has heavily ignored studying former professional players and the sport of hockey, but while some similarities exist in the athletic transitions for all sports, the experiences of retired professional hockey players are not necessarily representative of all other sports (Andrijiw, 2010). The study showed the importance of preparing for retirement and how not doing such can lead to many difficulties (Knights et al., 2019). To fulfill this study’s purpose, a qualitative research design was constructed to study retired men’s professional hockey players. Participants (n=11) who had been retired for a for a minimum of three years were sampled. Data were collected through semi-structured interviews, taking place either over the telephone or via virtual videoconferencing (Zoom), and then analyzed using a constructivist grounded theory approach (Charmaz, 2000). Four main categories arose, and within them, multiple sub-categories. These findings highlight the difficulties that can occur during the transitional years for a recently retired athlete, and how the athletic identity plays an important role in the process. This study provides a deeper theoretical understanding of the role the athletic identity has in the experiences retired hockey players have in their transition out of professional sport. The findings of this study could lead to continued awareness of these challenges so other athletes are aware of what might be in store for them in their retirements. Further research is necessary to continue to examine the uniqueness hockey retirement, as the challenges are far greater than many athletes realize
Exploring Pedestrianism in Contemporary Streetscape Planning: A Scrutiny of the YongeTOmorrrow Initiative in Downtown Toronto
In a study of Toronto’s YongeTOmorrow plan, my thesis explains the significance of pedestrianism and the role that it plays in the planning and regulation of the urban streetscape. However, the plan under scrutiny proposes an overhaul to the streetscape that creates a reinvented pedestrianism. This is a danger to publicness, as its benefits will be limited to businesses and their middle-class consumers, whose presence and interests are prioritized. Meanwhile, street-present non-consumers will be urged to move along under the regulatory absolutism of The Safe Streets Act, 1999. In this context, efficient flow is being reshaped to privilege consumption while continuing to restrain the liberty it alleges to cultivate
The time(s) of our lives: Exploring and opening up alternative temporalities through the experience of disability
The purpose of this qualitative study was to understand how some relate to time atypically and how, precisely through that relation, they help to disclose possibilities for alternative ways of being more generally. Both the COVID-19 pandemic as well as increasing threats of climate change have engendered an appreciation of the precariousness of our existence; in short, these crises have illuminated the inevitability of an uncertain future both immediately and in the long term. Despite the loom, the ways in which we live our lives in the dominant culture of Western society reflects a linear, future oriented temporality where able-bodied citizens often strive for progress and advancement, transformation, and ultimately mastery of the environment. Other temporalities exist, however, such as those shared by people whose bodily experiences construct their social realities in unconventional ways. Often it is their diagnosis that puts them out of line both with developmental time and fundamentally the neoliberal ethos of a productive life. This study strove to disrupt the domination of linear time and opposingly argued that these alternative relationships with time may be more well suited to the precarious nature of our lives. Guided by Rosemarie Garland-Thomson’s (2012) bioethical assertion that we might want to conserve rather than eliminate disability, I sought to uncover alternative temporalities in the hopes of disclosing their generative potential. Four memoirs written by parents of children with diverse disabilities were used as qualitative data to convey the experience of disability, temporalities and alternative ways of being in the world. STEAM Pedagogy to Promote 21st Century Skills: A Poetry Unit Plan for Grade 9 Ontario English Classrooms
The purpose of this research project was to create a handbook for Ontario-based English teachers interested in incorporating STEAM-based pedagogy (science, technology, engineering, arts, and math) to promote students’ 21st century skills. The handbook is a Grade 9 poetry unit plan that meets the curriculum expectations of Ontario’s Grade 9 English course, ENL1W, as well as curriculum expectations in other Ontario Grade 9 courses across the STEAM disciplines. A review of curriculum documents, along with a comprehensive literature review on STEAM pedagogy revealed a gap regarding translating the theory of interdisciplinary integration into practice in secondary subject areas, where courses are typically taught as discrete subjects. Therefore, this handbook was created to address this literature gap by providing high school teachers with a hands-on resource they may use to implement an integrated, STEAM-based unit. The unit plan was reviewed by Ontario English teachers and found to be helpful in both teaching and assessing English, STEAM subjects, and 21st century skills. Future research projects may build on this work by creating similar resources for other grade levels or disciplines, and exploring their impact on teaching and learning experiences
Letter from John Trumbull to Lord Sidmouth, June 1813
John Trumbull (1756-1843) was an American painter and architect who was known for his historical paintings of the American Revolution. During the revolution he served as an aide to George Washington and became a colonel. In 1794 he went to London as John Jay’s secretary during the peace negotiations with Great Britain following the American Revolution. He remained in London for 10 years, and then continued to travel between the U.S. and London for his painting career. In 1817 he was commissioned by the U.S. Congress to paint scenes from the American Revolution in the Rotunda of the Capitol at Washington, D.C. These paintings were based on smaller versions that he had painted in the 1780s and 1790s.
Henry Addington, or Lord Sidmouth (1757-1844), was a British politician who served as Prime Minister from 1801 to 1804. He became a member of the House of Commons in 1784 and became speaker in 1789. He remained speaker during the time that Trumbull was working as John Jay’s secretary in London and the two may have become friendly at that time. Addington served as home secretary from 1812 to 1822.A letter from John Trumbull to Lord Sidmouth, dated June 22, 1813. The letter requests a visit with Lord Sidmouth. Trumbull notes that he is the former Secretary to John Jay and asks for “the honour of a few minutes conversation”. An attached sheet contains Lord Sidmouth’s reply, dated June 24, 1813. He requests that Trumbull visit him at the Home Office the following day at half past twelve. The reverse side of this letter contains notes that Trumbull made about their conversation, much of it concerning the War of 1812. Some of his notes follow:
“conversation turned on the destruction of small towns on the Chesapeake. I expressed my high satisfaction that the officers & troops in Canada, instead of retaliating the threats held out in Gen’l Hull’s proclamation, of giving no quarters & the conduct of the commanding officer of Fort Niagara, in firing hot balls upon the Village of Newark—had conducted the war with humanity and restrained the savages in a great degree from their usual habits of indiscriminate destruction & burning…His [Lordship] made many professions of gen’l good will to the U.S.—regretted the war which had been forced upon Great Britain by the unreasonable pretensions of the American Government
Unfamiliar face matching ability predicts the slope of face learning.
We provide the first examination of individual differences in the efficiency of face learning. Investigating individual differences in face learning can illuminate potential mechanisms and provide greater understanding of why certain individuals might be more efficient face learners. Participants completed two unfamiliar face matching tasks and a learning task in which learning was assessed after viewing 1, 3, 6, and 9 images of to-be-learned identities. Individual differences in the slope of face learning (i.e., increases in sensitivity to identity) were predicted by the ability to discriminate between matched (same-identity) vs. mismatched (different-identity) pairs of wholly unfamiliar faces. A Dual Process Signal Detection model showed that three parameters increased with learning: Familiarity (an unconscious type of memory that varies in strength), recollection-old (conscious recognition of a learned identity), and recollection-new (conscious/confident rejection of novel identities). Good (vs. poor) matchers had higher Recollection-Old scores throughout learning and showed a steeper increase in Recollection-New. We conclude that good matchers are better able to capitalize on exposure to within-person variability in appearance, an effect that is attributable to their conscious memory for both learned and novel faces. These results have applied implications and will inform contemporary and traditional models of face identification.The Brock Library Open Access Publishing Fun
Using Persistent Homology for Topological Analysis of Protein Interaction Network of Candida Antarctica Lipase B Molecular Dynamic Simulation Model
In this work, we aim to examine the activity of one of the most efficient and commonly
used lipases, Candida Antarctica Lipase B (CalB), from the perspective of multiple
computational techniques. To this end, we first conduct a series of Molecular Dynam-
ics Simulations on CalB in different conditions to analyze the conformational changes
of the protein and probe its unusual high-temperature activity. Next, we build the
protein interaction network of amino acids for CalB to study pairwise interactions
between amino acids (nodes) and probe the protein in terms of statistical features of
links’ distribution. Finally, we employ an algebraic topology-based method to study
the protein interaction network from a broader perspective. The ”Persistent Homol-
ogy (PH) method” is then presented as a way to exceed pairwise interactions and
examine protein networks in terms of patterns of interaction between the nodes. Per-
sistent Homology studies the evolution of the protein interaction network’s topologi-
cal features (homology groups) in different states. Employing topological analysis, we
compare the active form of CalB at high temperatures to its inactive states to account
for possible topological contributions to the protein functionality. By discovering a
prominent 1-dimensional hole in the active form of the protein, we highlight the role
of higher-order interaction patterns in the network. Moreover, using the evolution of
topological features, we study topological changes in protein networks and show the
decline in the total number of 1-dimensional features as the protein loses activity and
compactness over time. Accordingly, we propose that the protein’s general conforma-
tional changes and three-dimensional structure are not the only facets contributing
to its active state. Instead, we suggest examining the topology of the protein inter-
action network, referred to as different dimensional holes of the networks, as a higher
dimensional analysis should be used to account for protein functionality. Hence, in
this work, we desire to present that one needs to consider topological features acting
as patterns of interaction between the components to study, examine or predict the
folding of polypeptide chains into active structures
Intercultural Competence Development: Exploring International Graduate Student Leaders' Journeys through Contemplation and Mindfulness-Based Inquiry
Given the globalization of education, intercultural competence (IC) is a pivotal learning outcome in student mobility and internationalization. While scholars conceptualize IC as the ability to communicate effectively and appropriately in cross-cultural contexts, few studies have explored international graduate student leaders’ IC development and how they perceive IC in their transitional journeys, and most research demonstrates IC development mainly through Western-centric frameworks. This study thus explored, through a non-dominant cultural lens, the IC development of international graduate student leaders to deepen the understanding of their lived experiences. Inspired by Thich Nhat Hanh’s philosophy of Engaged Buddhism, the study scaffolded an innovative contemplation and mindfulness-based inquiry and developed the Lotus theoretical framework combining Mezirow’s (1978) transformative learning theory and Drake’s (2010) story model to explore participants’ lived experiences. Eight international graduate student leaders from two Ontario institutions attended the semi-structured interview and two focus groups in the form of a Story Circle (Deardorff, 2020c). Findings depicted new narratives that challenged a deficit view about international students and revealed (a) participants’ perceptions about IC, (b) their journeys of perspective transformation, (c) their engagement in the meaning-making processes, and (d) the role of IC in leadership development. The study’s results offer insight into life in transition and how these students make meaning of their lives through leadership engagement and IC development. Such insights could raise awareness among different stakeholders about the meaning and importance of IC and may enable institutional leaders and practitioners to develop more effective programs and a more inclusive environment for all
