Adolescents and Young Adults (AYA) with Special Healthcare Needs Transitioning to Adult Care: A Needs Assessment (Emphasis on Rural Communities)
In 2022, the National Survey of Children\u27s Health reported that 86.6% of adolescents did not receive the services necessary to transition to adult healthcare (Data Resource Center for Child & Adolescent Health, 2022b). In Utah, 15.5% of children have special healthcare needs (Data Resource Center for Child & Adolescent Health, 2022c), and 24 out of 29 counties have a shortage of primary care physicians, with the majority of these counties being in rural locations (US Health Resources and Services Administration, 2024). A needs assessment was conducted within 73 primary care clinics to evaluate the barriers and facilitators involved in accepting AYAs with special healthcare needs. A survey was sent out to primary care providers (PCPs) across an Intermountain West state and follow-up interviews were conducted with some of the providers. A SWOT (strengths, weaknesses, opportunities, and threats) analysis was performed, and informal discussions were conducted with community partners. The needs assessment distribution included primary care providers (N=408). Of those, 11% (n=44) responded, and 5% (n=19) completed beyond the demographics section. The needs assessment participants included rural primary care providers 10% (n=2) and urban primary care providers 90% (n= 17). Clinic types included: family medicine (53%, n=10), pediatrics primary care (32%, n=6), internal medicine (16%, n=3), and med peds (0%, n=0). Barriers that were most common when PCPs try to accept AYAs with special healthcare needs were: training and/or experience gaps (54%, n=7), lack of time in appointments (39%, n=5), and lack of provider hand-off (31%, n=4). The most common barriers when trying to transfer AYAs with special healthcare needs were a lack of available adult providers (31%, n=5) and hesitancy from patients and/or family members (31%, n=5). The support necessary for the providers and their practices to care for AYAs with special healthcare needs were: a list of adult specialists who will accept care of AYAs with special healthcare needs (47%, n=9), care coordination support (47%, n=9), additional training on specific pediatric-onset chronic diseases (37%, n=7), and provider handoff/medical summaries (37%, n=7). When rural providers were asked what barriers they experience when caring for AYAs with special healthcare needs, the one rural healthcare provider\u27s response involved the challenges of reaching sub-specialists/coordinating care (50%, n=1). When rural providers were asked what potential solutions might address the discussed barriers in rural healthcare, one responded with telehealth (50%, n=1). Barriers complicate accepting and transitioning AYAs with special healthcare needs, which in turn negatively affects health outcomes for this patient population. It is vital for these barriers to be addressed to ensure there is no gap in healthcare for these vulnerable patients
Implementing a Survivorship Screening Tool in Breast Cancer Survivors: An Evidence-Based Quality Improvement Project
With advancements in cancer therapies, more patients are achieving remission. Following treatment, many of these patients struggle with ongoing physical, psychosocial, and practical concerns (Hussey et al., 2024). Providers face challenges in identifying and meeting these specialized needs while managing capacity (Love et al., 2022). The National Comprehensive Cancer Network (NCCN) recommends using standardized screening tools to identify unmet physical and psychosocial needs in cancer survivors (Sanft et al., 2023). Providers at an outpatient breast oncology clinic in Salt Lake City, UT, do not utilize a standardized tool to assess for unmet needs post-treatment, leading to potential missed identification of patients\u27 needs and missed opportunities for referrals to survivorship resources. This project aimed to implement a standardized screening protocol to improve need identification and increase referrals. A quality improvement project was designed to improve the quality and quantity of screening for unmet needs in breast cancer survivors. The NCCN Survivor Assessment Survey was adapted to fit clinic needs and implemented with four providers over six weeks. Pre- and post-implementation surveys assessed provider screening practices, attitudes, and knowledge of survivorship resources. Education sessions with involved staff reviewed the screening tool, workflow, and available resources. Weekly provider feedback provided opportunities for adjustments to the protocol as needed. Screening and referral rates were analyzed and collected from the electronic health record (EHR). Data from survey responses were compiled and analyzed. Implementing a standardized screening protocol increased screening rates, identification of needs, and referrals to survivorship resources. 92% (n=123) of patients screened reported at least one unmet need, and 71% (n=88) of those patients were referred to survivorship resources, compared to 43% (n=77) before implementation. All providers (n=4) found the tool easy to use and integrate into their clinical workflow. 75% (n=3) of providers were satisfied with the tool, with 25% (n=1) neutral. Providers noted improved need identification and patient provider communication. Implementation of a standardized screening tool to assess for unmet needs in breast cancer survivors is effective and increases referrals to survivorship resources. The next steps include integrating the tool into the EHR for streamlined delivery and expanding it to other clinics and diagnosis groups within the organization
Bridging the Gap: An Evidence-Based Quality Improvement Initiative to Increase Palliative Care Referrals in Primary Care
Early palliative care enhances quality of life, alleviates symptoms, and supports holistic care. However, primary care physicians (PCPs) often fail to inform patients about the benefits of palliative care, with uninsured, non-English-speaking, and immigrant patients facing the most significant disparities in referrals. Early palliative care improves symptom management, quality of life, and reduces healthcare costs. Despite patients being more likely to engage in palliative care when referred by their PCP, referral rates remain low, particularly among immigrant populations. Barriers include limited PCP training, misconceptions about palliative care, and discomfort in discussing it. The challenges that preclude PCPs from referring to PC are exacerbated at an urban family practice clinic where 90% of patients lack commercial insurance, introducing cultural and financial complexities. A new electronic health record (EHR) referral system was implemented at the project site; however, PCPs lack sufficient training in culturally sensitive communication, identifying palliative candidates, and using the referral system. This quality improvement project aimed to enhance PCP awareness and increase palliative care referrals. The Johns Hopkins Evidence-Based Practice model guided this quality improvement initiative, along with several Plan-Do-Study-Act (PDSA) cycles which allowed for multiple rounds of interventions. PCP awareness was assessed through a pre-intervention survey, and referral data was compared to the period 12 months prior to account for seasonal variation. Data tracked total referrals, uninsured patients, non-English speakers, immigrant patients, and those meeting Center to Advance Palliative Care (CAPC) criteria for palliative care referrals. A stakeholder-developed toolkit promoted cultural humility, raised awareness, and streamlined referrals. Toolkit resources, including the CAPC webpage and cultural humility guides, were accessible via QR codes in high-visibility areas. Feedback was collected biweekly for refinement. Additionally, Medicare and Medicaid-friendly referral options were integrated into the clinic\u27s EHR to ensure patient accessibility. Findings indicated that 879 active clinic patients had at least one palliative care-appropriate diagnosis, and 253 had two or more diagnoses per CAPC guidelines. Despite these large numbers, PCPs had not made any palliative care referrals had been made by PCPs in the past year. Following the implementation of the toolkit, six referrals were made (Figure 2). Furthermore, there was an improvement in provider awareness of PC (44.44% following the toolkit vs. 25% prior). An increase in palliative care referrals occurred, revealing previously unrecognized barriers to patient communication and access, particularly among uninsured, non-English-speaking, and immigrant populations. Implementing A Melanoma Risk Screening Tool at a Primary Care Clinic: A Quality Improvement Initiative
Melanoma, the deadliest form of skin cancer, presents a significant and growing public health challenge. Early detection is crucial, yet many primary care clinics lack standardized protocols for assessing melanoma risk, leading to missed opportunities for early detection and education. Utah leads the nation in both melanoma incidence and mortality rates due to high elevation, year-round outdoor activities, and a predominantly fair-skinned population. At a primary care clinic in Orem, Utah, the absence of standardized screening practices with a validated risk assessment tool resulted in missed opportunities for early detection and initiation of follow-up actions. Multiple barriers compounded this problem, including time constraints, lack of confidence in skin assessments, and competing priorities. This quality improvement initiative implemented a validated risk screening tool in a primary care clinic for six weeks to improve early melanoma screening and standardize follow-up actions for high-risk patients. Pre-implementation interviews and surveys established baseline practices, while post-implementation surveys assessed usability, feasibility, satisfaction, and sustainability. Weekly site visits incorporated Plan-Do-Study-Act cycles for continuous improvement. Interventions The project occurred in two phases. Phase one included a 30-minute staff training session with a PowerPoint presentation, creating a laminated workflow diagram, and assembling of melanoma prevention education packets using American Academy of Dermatology materials. Phase two involved integrating the Self-Assessment of Melanoma Risk Score (SAMScore) questionnaire into clinical workflow for six weeks, with medical assistants administering it to the first five eligible patients per provider daily. The SAMScore form included a structured checkbox system for documenting patient consent, risk level determination, follow-up actions, and confirmation of education packet distribution. The clinic transitioned from performing no melanoma risk screening to 140 completed screenings during the six-week implementation. The patient population screened was predominantly female (59.3%), with ages ranging from 18 to 86 years (mean: 49.3 years). Of those screened, 45.2% were identified as high-risk. Follow-up actions were documented for all high-risk patients, with 96.4% receiving dermatology referrals and 3.6% receiving full-body examinations. Education packets were distributed to over 91% of all screened patients. Staff surveys reported high levels of usability (83.3%), feasibility (83.3%), satisfaction (100%), and sustainability (100%), with time constraints identified as the primary implementation barrier. Implementing the SAMScore questionnaire proved valuable for standardizing melanoma risk assessment and initiating follow-up actions in primary care settings. With its paper-based structure, minimal costs, and straightforward design, the project could readily adapt to other primary care settings, particularly in regions with high melanoma rates and limited dermatologist access. Future improvements could include expanding to more clinics, integrating with electronic health records, and creating outcome-tracking systems

Jorge Fierro discusses his childhood growing up in Mexico and his family. Fierro is an entrepreneur and owner of Rico Brands, which distributes ready to eat Mexican food for supermarkets. He mentions that he was considered the "black sheep" of his family because his life did not revolve around Catholic traditions. Fierro talks about his struggles going to school and learning later in life that he had a learning disability. He moved to America after deciding to quit his schooling, and started doing manual labor and worked as a sheep herder in Wyoming. Fierro talks about his determination to become successful and learn English. He was homeless for a time in Salt Lake City and was aided by programs at the Rescue Mission in 1985. Fierro details his philosophy of developing natural food products with no preservatives, and his approach to building his company and developing it over time. He discusses his feelings about the benefits of immigration, and immigrants being able to build opportunities for themselves and other people
