Building an Implementation Toolkit for Stop the Bleed Training: An Evidence-Based Practice Change
No full textSchool shootings are a common and devastating problem in America and a source of traumatic death for both students and teachers. After the shooting at Sandy Hook Elementary the American College of Surgeons (ACS) developed a course called Stop the Bleed (STB) which taught bystanders how to recognize and treat uncontrollable hemorrhage. The number one cause of preventable death after trauma, such as a gunshot wound, is uncontrolled hemorrhage. By implementing this course in a school system, it is hoped that fatalities can be reduced. In 2023, a bill was passed in Utah that mandated funds be set aside for schools to purchase STB kits for every classroom. In 2024, a needs assessment performed by a prior University of Utah student determined that there was no standardized training to ensure that all school staff knew how to use these kits. A toolkit was needed to help schools implement training in their individual facilities. This project was rolled out in phases. The first phase involved training nurses, and the 2nd and 3rd phase was training schoolteachers and staff. Multiple methods were utilized to gather data for this project including surveys with qualitative and quantitative questions. There were also qualitative discussions with stakeholders such as school staff, district safety officers, school nurses and local hospital trauma outreach education offices. This project started with a pre-survey and discussions with key stakeholders. Then, a large training session with staff from a local public school district was planned. The training targeted teachers from elementary, middle and high schools. After the training was administered a postsurvey was sent to instructors to determine what barriers they discovered. This information was used to determine the best way to formulate a toolkit to help facilitate further training for staff from other school districts in the future. The training was given to 15 school nurses and over two thousand staff. Twenty-five people took the post-training assessment survey. This survey identified some key elements to the development of the toolkit. These were barriers such as communication, the appropriate instructor-to-staff ratio, difficulty coordinating with multiple people, and the need to identify an STB champion for each school. The post-training survey was also helpful in identifying key elements in the sustainability of maintaining STB training in schools, such as ensuring one person in each school was trained as an instructor and assigning yearly refresher modules to staff. Developing a STB toolkit makes implementing STB training easier for schools or large school districts. The administration of this training ensures that all school staff can use the STB equipment they have in their classrooms and are more likely to help save a life in the future

Leadership Perspectives on Sick Leave: A Quality Improvement Needs Assessment
No full textCurrent sick leave policies in Utah reveal significant disparities, with no mandated sick leave at the private, federal, or state levels. This situation has led to varying organizational practices, potentially creating misalignments between employer expectations and employee understanding regarding its use. Inconsistencies in the policy\u27s application and expectations can negatively impact workplace culture, employee well-being, and organizational performance. This quality improvement project aimed to conduct a formal needs assessment of sick leave policies in Utah businesses, concentrating on leadership perspectives, identifying best practices and areas for improvement, and developing evidence-based recommendations for policy refinement. From a business perspective, organizational leaders in Utah should acknowledge the advantages of implementing supportive sick leave policies. These policies boost job satisfaction, enhance employee retention, and decrease presenteeism and workplace injuries. By investing in transparent and considerate sick leave practices, Utah businesses can foster healthier work environments that ultimately enhance organizational performance and lower healthcare costs for all stakeholders. This needs assessment employed a qualitative design through a grounded theory methodology to explore Utah leaders\u27 perspectives on sick leave policies across various business sectors. To complete this, semistructured interviews were conducted with 17 participants from diverse organizations, analyzed using iterative coding processes to identify key themes, and developed into a SWOT analysis and recommendations. This project employed a qualitative approach using semi-structured interviews to gather data from business leaders and policy influencers. This method was selected for its ability to address key topics while allowing participants to express their perspectives freely. Interview questions were collaboratively developed and validated by individuals with similar backgrounds to enhance credibility. Purposive sampling ensured diverse representation across leadership roles and organizational sizes. Participants were recruited through professional networks, industry associations, and snowball sampling. The project was deemed exempt from ethical approval under internal quality improvement criteria by the University of Utah College of Nursing. The quality improvement project identified five key themes in Utah leaders\u27 perspectives regarding sick leave policies: balancing employee needs with organizational constraints, clearly and consistently communicating policies, adapting to context and individual needs, navigating legal and regulatory constraints, and the role of leadership in adapting policies. Leaders emphasized the importance of achieving a sustainable balance between supporting employee health and maintaining operational efficiency while acknowledging the need for clear communication, flexibility, and adaptation to specific organizational contexts. The qualitative findings and SWOT analysis highlight the necessity for Utah businesses to implement transparent, flexible, and employee-centric sick leave policies that effectively balance organizational and employee needs. Key recommendations involve enhancing communication strategies, addressing health equity concerns, and aligning policies with long-term economic advantages such as employee retention and reduced healthcare costs. Organizations can improve workplace culture, support employee wellbeing, and boost overall performance by adopting these strategies. Future research should further assess these recommendations within broader Utah contexts to identify optimal practices, emphasizing specific initiatives demonstrating universal benefits for consideration in state-level policy recommendations

This is not nystagmus because it is excitation of the SO muscle that initiates the movements (followed by a slower relaxation phase) rather than the slow phase being the culprit (as in jerk or pendular nystagmus). Since the secondary action of the SO muscle is depression, this is best appreciated in adduction where the vertical action of the muscle is maximal. These movements were purely monocular in the left eye, without right eye involvement
