【研究目的】 
胎生期のストレスが、成長後の認知機能、情動安定性、社会性に関わる脳領域である、
前頭前野、海馬、扁桃体の機能に異常を生じさせ、統合失調症を含む思春期の精神疾患
のリスクとなることが指摘されている。母体免疫活性化 (MIA) は、母体におけるサイ
トカイン関連の炎症反応とそれに伴うエピジェネティックな変化を惹起し、胎児の脳の
発達に変動をもたらす。その結果、生涯を通じてストレス脆弱性を高め、認知、感情、
社会的機能障害を引き起こす素因となり、知的障害、双極性障害、自閉症スペクトラム、
統合失調症を含む重篤な精神疾患の潜在的な危険因子となることが指摘されている。し
かしながら、その生物学的メカニズムは未だ不明な点が多く、根本的な治療法を目指す
開発研究が続けられている。 
脳内の興奮性/抑制性 (E/I) ニューロンのバランスの不均衡が、MIA に関連する様々
な神経精神疾患における行動異常や認知機能低下の主要なメカニズムとして提唱され
ている。また、神経細胞の増殖、生存、シナプス可塑性に関与する脳由来神経栄養因子
(Bdnf) については、その遺伝子多型が統合失調症の症状や治療効果と相関するなど、統
合失調症の発症との関連性が種々に報告されている。 
これまでの研究で、我々は、発達期ストレス脆弱性モデル動物を用いて、抗精神病薬
の投与が社会的行動の異常を改善させ、その効果発現に、血中/脳中の Bdnf の変動が伴
うことを報告した。今回、MIA 誘発統合失調症モデル動物を作成し、抗精神病薬ブロナ
ンセリン (Blon)、およびハロペリドール (Halo) を投与し、社会的行動の変化を比較解
析するとともに、統合失調症の発症との関連が推察されている大脳辺縁系領域における
パルブアルブミン (PV) 陽性 GABA (作動性) インターニューロン、E/I バランス、およ
び Bdnf mRNA (Total Bdnf と特異的転写産物 Bdnf long 3’UTR) 発現レベルの変動を解析
し、統合失調症の発症と社会性・認知機能障害に関わる脳内メカニズムの一端の解明を
試みた。 
【研究方法】   
（1）MIA 誘発統合失調症モデルラットの作成と治療薬投与 
母体ラットとして Jcl:Wistar 系妊娠ラットを購入した (Clea Japan, Ins)。モデル動物は、
胎生期 E10 から E13 にかけて、母体に poly(I:C) (ウイルス感染性を惹起する人工 2 本鎖
RNA) を腹腔内投与して作成した。正常群には Saline を投与した。出生仔は生後 30 日
で離乳させ、生後 42 日から 63 日にかけて、正常群には水を、モデル群には水 (コント
ロール)、Blon (低用量：0.01 mg/kg/day)、Blon (高用量：0.1 mg/kg/day)、Halo (0.03 mg/kg/day)
をそれぞれ経口投与した。 
（2）行動評価：社会相互作用試験 
行動解析は既報に従って実施し、一辺 70cm 四方のオープンフィールドを用意し、照度
計を用いて照度を 80 ルクスに統一した。前日に試験ラットをオープンフィールドに入
れ、30 分間の馴化試験を行った。当日は異なるケージで飼育されたパートナーラットを
用意し、試験ラットと共にオープンフィールドに入れた。10 分間にパートナーラットに
働きかけた能動的な社会的行動  (sniffing, following, climbing over or under the 
partner) が観察された回数と時間を Video Recording System を用いて測定した。 
（3）PV 陽性 (+) GABA インターニューロン数解析 
行動解析後、ラットを深麻酔・ホルマリン還流後に脳を取り出し、スライスをパラフィ
ン包埋した。背側海馬、腹側海馬、扁桃体等、統合失調症発症関連領域を含む薄切切片 
(20 µM) において、PV 用抗体 (マウス抗 PV, 1:2000; Sigma, St Louis, MO, USA) を用い
て染色後、ABC system を用いて評価を行った。 
（4）GABA/Glutamate ニューロン関連遺伝子群の発現解析と E/I バランスの測定 
背側海馬、腹側海馬、扁桃体を含む各脳組織をパンチング採取した。Total RNA を分離
取得した後、測定遺伝子の各 probe と、iScriptTM one-step RT-PCR kit for probes (Bio-
Rad Laboratories)を用い、qRT-PCR (CFX384 real time system, Bio-Rad Laboratories) 
装置を用いて解析した。E/I バランスの測定は、Vglut/Vgat mRNA 発現レべル比を、
Glutamate/GABA ニューロン活性バランス  (E/I ratio) として算出した。また、
Glutamate、GABA それぞれの神経伝達関連遺伝子の Z-score を平均化した総合的な
E/I バランスの指標として、Z-Excitation/Z-Inhibition 比を算出した。さらに、同領域
における Total Bdnf、および Bdnf long 3’UTR mRNA 発現レベルの変動について解析を
行った。 
（5）統計解析 
統計解析は、IBM SPSS Statistics (IBM, IL, USA), 24.0 と GraphPad Prism 8 (GraphPad 
Software, CA, USA) を用いて実施した。 
 
 
 
【研究成績】 
（1）統合失調症モデルの妥当性および抗精神病薬の効果の解析 
社会相互作用試験において、MIA 誘発統合失調症モデル群では、コントロール群と比
較し社会的相互作用時間の有意な短縮を認めた。また、モデル群に Blon (高用量) を投
与した群では、社会的相互作用時間の有意な改善を認めた。一方、Blon (低用量)、およ
び Halo 投与群では社会的行動の有意な変動は認めなかった。 
（2）PV (+) GABA インターニューロンと社会的行動との関連の解析 
統合失調症モデルでは背側海馬、および扁桃体領域の PV (+) GABA インターニューロ
ン数が有意に減少していた。また、両脳領域において、PV (+) GABA インターニューロ
ン密度と社会的相互作用時間との間に正の相関を認めた。さらに、Blon (高用量) 群では
PV (+) GABA インターニューロン密度の有意な改善を認めた。一方、Blon (低用量) 群
と Halo 群では有意な変動は認めなかった。 
（3）E/I バランスの変動の解析 
背側海馬において、Halo 群では E/I ratio の有意な低下を認めた。腹側海馬では、統合失
調症モデル群で上昇した E/I ratio が Blon (低用量および高用量) 群で有意に低下したが、
Halo 群では有意な変化を認めなかった。また、モデル群では、背側海馬において z-
excitation score の有意な変化は認めなかったが、Blon (低用量) 群では腹側海馬の z-
excitation score を有意に減少させた。さらに、Blon (高用量) 群、および Halo 群では扁桃
体の z-excitation score を有意に増加させた。 
（4）Bdnf の特異的転写産物の発現変化と抗精神病薬の影響 
Blon (高用量) 群では背側海馬で、Blon (低用量) 群では腹側海馬で、GABA インターニ
ューロンの樹状突起末端への局在が報告されている Bdnf 転写産物、Bdnf long 3’UTR 遺
伝子発現の有意な増加を認めた。Halo 群では有意な変動は認めなかった。一方、Total 
Bdnf 遺伝子発現量は各群を通して有意な変動はなかった。 
 
【考察】 
本研究では、poly (I:C) 投与による MIA 誘発統合失調症モデルを用いて、抗精神病薬
Blon および Halo の社会的行動への影響について検討を行った。その後、
GABA/Glutamate ニューロン関連遺伝子と、E/I バランス、さらに Bdnf 転写産物の発
現変動の解析を実施した。その結果、Blon (高用量) 群では、モデル群で減少した PV (+) 
GABA インターニューロン密度の増加作用を認め、PV (+) GABA インターニューロン密
度の変動が統合失調症における社会的行動の障害とその回復のメカニズムに関連して
いることが示唆された。また、Blon (高用量) 群では、背側海馬の PV 遺伝子発現を増加
させており、細胞密度の変動と一致する所見と考えられた。一方、Halo 群では、背側海
馬で Vgat 遺伝子発現を増加させたものの、社会的行動障害の改善効果は認められなか
った。Halo 群では扁桃体で PV 遺伝子発現を有意に減少させており、行動改善効果を認
めないこととの関連に興味がもたれた。 
 Blon は、ドパミン D2 に加え、D3 受容体、ならびにセロトニン 5HT-2A 受容体拮抗作
用を有する抗精神病薬である。先行研究では、D3 受容体拮抗作用が統合失調症患者の
社会性・認知機能障害の改善効果と関連することが報告されている。本研究では、D2 受
容体拮抗薬である Halo によって MIA 誘発の社会的行動障害に効果を認めず、PV (+) 
GABA インターニューロン密度も変化させなかったことから、Blon の効果に D3 拮抗作
用を介するメカニズムの可能性が考えられた。 
 以前の研究で、我々は、プロテオグリカン NG2 を発現する成体 SVZ 由来前駆細胞に
おける GABA インターニューロンの発生と分化が、NMDA 受容体拮抗薬 MK-801 の曝
露により障害され、その効果は Blon 投与により回復するが Halo では回復しないことを
報告した。さらなる研究が必要ではあるが、海馬のニューロン新生の促進作用は D3 受
容体シグナルの修飾に関連し、社会的認知機能の改善に寄与する可能性が推察された。 
 また、Blon (低用量) 投与は、腹側海馬で PV、GAD1/67、Vgat を含む GABA 関連遺伝
子群の発現を増加させるとともに、E/I バランスおよび z-excitation score の減少を認めた
が、社会的行動障害には影響を与えなかった。これについては、低用量の Blon の投与
が、社会性・認知機能や遂行機能よりも、情動行動に対しての改善効果を示す、との研
究報告と一致する知見であると考えられた。 
 加えて、本研究では、Blon (高用量)の投与によって、背側海馬で Bdnf の特異的転写産
物 Bdnf long-3’UTR の発現量を増加させていることを見出した。Bdnf は、GABA インタ
ーニューロンの発達を制御し、その神経伝達を調整していることが知られている。また、
Bdnf long-3’UTR は樹状突起の末端に分布し、シナプス形成に関与していることが報告
されていることから、本結果は、抗精神病薬 Blon の投与が、用量や脳領域特異的な機
序で Bdnf long-3’UTR 発現を変化させ、神経可塑性、神経新生、シナプス伝達改善効果
を発揮している可能性を示すものと考えられた。一方、Blon (低用量)の投与は、腹側海
馬の Bdnf long-3’UTR の発現量を増加させたが、腹側海馬はストレス反応に重要な領域
であり、感情や情動により関与していることが示唆されており、今回評価した社会的行
動に影響を及ぼさなかった理由の 1 つであると推察された。背側海馬での Bdnf long-
3’UTR 発現増加のメカニズムを含め、今後のさらなる研究が必要であると思われた。 
 
【結論】 
 本研究では、MIA 誘発統合失調症モデルの社会的行動障害を改善するメカニズムと
して、特に背側海馬における PV 遺伝子発現、PV (+) GABA インターニューロン密度、
および Bdnf long 3'UTR 遺伝子発現の増加が関わる可能性が明らかとなった。抗精神病
薬による胎生・発達期ストレス関連統合失調症の社会性・認知行動障害改善効果のメカ
ニズムの一端を示すものと考えられ、今後の新薬開発の一助になればと期待している。

結果の要旨 
 近年、胎生期のストレスが、認知機能、情動安定性、社会性機能に関わる、脳領域の機
能に異常を生じさせ、統合失調症等の精神疾患発症のリスクとなることが指摘されてい
る。母体免疫活性化は、胎児の脳の発達に影響を及ぼすことが報告されており、その結
果、統合失調症の危険因子となることが指摘されている。また、海馬 CA1領域の GABA
インターニューロン機能異常が疾患発症前駆期の病態生理に関連していることが示され
ており、その異常が前頭皮質および扁桃体などの他の脳領域に拡大し、精神病症状発症
への移行を誘発する可能性があると推察されている。 
このような背景のもと、本研究は、統合失調症の発症に結びつく神経分子メカニズム
を明らかにする目的で実施された。方法としては、母体免疫活性化により統合失調症モ
デルラットを作成し、その社会性行動と脳内 GABA作動性ニューロン動態に対する抗精
神病薬の影響を調べた。結果として、モデル動物にブロナンセリンまたはハロペリドー
ルを投与した群において、背側/腹側海馬および扁桃体領域の GABA 作動性ニューロン
関連遺伝子群の増加と、Glutamate作動性ニューロン関連遺伝子の変化を認めた。また、
同領域での興奮性/抑制性ニューロンバランスの変化並びに BDNF 遺伝子の有意な変動
を認めた。 
 本論文は、統合失調症の発症前駆期における病態の一端を明らかにし、創薬につなが
る知見を提供するものであり、審査員全員から博士（医学）の学位授与に値すると認め
られた。

するヘテロ耐性が特定の菌種、特定の遺伝子系統で多く認められる) 
 
 研究目的 
 コリスチン (CST) は多剤耐性グラム陰性桿菌感染症治療の最終選択肢に位置づけら
れる抗菌薬の 1 つである。CST に対する耐性機構として、CST の標的である菌体表層
リポ多糖の 4-アミノ-4-デオキシ-アラビノース (Ara4N) やホスホエタノールアミン 
(p-EtN) による修飾が知られている。Enterobacter cloacae complex (ECC) においては
CST に対してヘテロ耐性の表現型を示す菌株の分離が報告されている。しかしながら、
多くの CST 耐性株について、菌種や遺伝子系統に基づいた疫学と CST ヘテロ耐性のメ
カニズムとの関連性は十分に明らかになっていない。本研究では、ECC 臨床分離株に
おける CST ヘテロ耐性を網羅的に把握し、ECC の分子疫学的特徴とヘテロ耐性の表現
型との関連性を明らかにすることを目的とした。 
 
 研究方法   
札幌医科大学附属病院で患者検体から分離され、MALDI-TOF MS により ECC と同
定された菌株を使用した。CST の Minimum inhibitory concentrations (MIC) は微量
液体希釈法で測定し、ヘテロ耐性の表現型の検出は population analysis profiling (PAP)
法を用いた。hsp60 クラスタリング、multilocus sequence typing (MLST)、コアゲノム
解析による分子疫学的解析を行った。菌種は average nucleotide identity (ANI)を用い
て決定した。CST ヘテロ耐性メカニズムを調べるために、CST 耐性にかかわるとされ
る遺伝子の phoPQ（二成分系）、eptA（p-EtN 転移酵素）、arnT（4AraN 転移酵素） の
欠損株を作製して、それらの性状を解析した。arnT の mRNA 発現量は定量 real time 
逆転写 PCR (RT-qPCR) により測定した。 
 
研究成績及び考察 
 ECC 臨床分離株 138 株のうち、38 株 (27.5%) が CST 耐性であり、その全てが PAP
でヘテロ耐性の表現型を示した。CST ヘテロ耐性の出現頻度は臨床検体の由来（呼吸
器、尿、血液、その他）の間で有意な差異は認められなかった。hsp60 クラスタリング、
コアゲノム解析、ANI の結果から、CST ヘテロ耐性株は、Enterobacter roggenkampii 
（hsp60 cluster IV）、Enterobacter kobei （cluster II）、Enterobacter chuandaensis 
(clusters III, IX)、Enterobacter cloacae subspp. (clusters XI, XII) においてはほぼす
べての株で認められた。一方、Enterobacter hormaechei subspp. (clusters VIII, VI, 
III)、Enterobacter ludwigii (cluster V) では CST ヘテロ耐性株は全く認められなかっ
た。CST ヘテロ耐性株では、CST の添加によって、arnT （4AraN 転移酵素）の mRNA
発現量の増加が CST の濃度依存的に認められ、その発現誘導は二成分系 PhoPQ を介
していることが欠損株を用いた検討で示された。CS 耐性を付与する外来遺伝子である
mcr（pEtN 転移酵素をコード）のうち、mcr-9 と-10 が 138 株の ECC のうちそれぞれ
5 株 (3.6%)、8 株 (5.6%)に検出されたが、mcr の保有株と非保有株間では CST の感受
性、PAP の結果に有意な差は認められなかった。 
 
結論 
 ECC 臨床分離株において CST ヘテロ耐性株が約 28％と多く認められ、それらは特
定の菌種および遺伝子系統に集積していた。CST の存在による二成分系 PhoPQ を介し
た arnT mRNA の発現誘導が、CST ヘテロ耐性の表現型獲得に寄与していることを明
らかにした。一方、外来の耐性遺伝子である mcr (mcr-9 と-10) の獲得は、ECC におい
ては CST 耐性への寄与は低いと考えられた。

結果の要旨 
多剤耐性グラム陰性菌感染症治療の最終選択肢のひとつに位置付けられているコリスチ
ンに対する耐性菌増加が懸念されている。本申請論文ではわが国の臨床分離株の
Enterobacter cloacae complex において、耐性率が 27.5%ですべてヘテロ耐性の表現型を
示した。ヘテロ耐性株は特定の遺伝系統、菌種で多く認められた。さらにヘテロ耐性の分
子機序を明らかにした。E. cloacae complex の臨床検査における詳細な菌種同定の意義、
ヘテロ耐性の簡易検出方法の提言が当該学術分野における本論文の意義である。審査にお
いて委員から実験の方法論、疫学データに関する他国、他地域との比較、コリスチン適正
使用とヘテロ耐性との関係性に関する質疑応答と議論が展開された。また、臨床微生物
学、臨床検査分野に関する最終試験が実施され、専攻分野に関する十分な学力を有してい
ると認められた。以上の経過から、福澤氏の本論文は博士（医学）の学位授与に値するも
のと審査委員全員の承認を得た。

Abstract
Purpose Ultrasound-guided inferior alveolar nerve block (UGIANB) is a mandibular analgesic procedure in which local 
anesthetic is injected into the pterygomandibular space (PMS). Several studies have reported the clinical efficacy of UGIANB 
for mandibular surgeries; however, its effective range has never been investigated. We performed a cadaveric study to inves-
tigate the success rate of UGIANB injections and to determine whether injected dye could stain the mandibular nerve (MN) 
trunk and its branches.
Methods We performed UGIANB on the bilateral faces of 4 Thiel-embalmed cadavers. A needle was advanced to the PMS 
under ultrasound guidance and 5 mL of dye was injected. The cadaver was dissected and inspected for the presence of dye 
in the PMS; the range of dye spread to any of the inferior alveolar nerve (IAN), lingual nerve (LN), buccal nerve (BN), 
mandibular nerve (MN), auriculotemporal nerve (ATN), or facial nerves; and for the presence of intravascular dye.
Results We performed eight UGIANB procedures on four cadavers. Dye was observed in the PMS in 7/8 injections. Stain-
ing was observed in all IAN, LN, and BNs that could be identified at dissection. No MN or auriculotemporal nerves (ATNs) 
were stained in any injections. No intravascular dye was observed in any injections.
Conclusions UGIANB can administer anesthetic into the PMS with high accuracy. UGIANB injections reached the IAN, 
LN, and BNs, but did not reach the MN or ATNs located outside the PMS. The findings of this cadaveric study indicate that 
UGIANB can provide sufficient analgesia for mandibular surgeries.

Introduction
Ultrasound-guided inferior alveolar nerve block (UGIANB) 
is an analgesic procedure used for treating severe postopera-
tive pain following mandibular surgery. The inferior alveolar 
nerve (IAN) descends from the mandibular nerve (MN), the 
third branch of the trigeminal nerve. The MN leaves the 
skull through the foramen ovale and then becomes the IAN, 
which passes through the pterygomandibular space (PMS) 
and enters the mandible. In UGIANB, local anesthetics are 
delivered to the PMS through the lateral surface of the face. 
Compared with MN block, in which the anesthetic solution 
is administered near the foramen ovale, UGIANB is a shal-
lower block. In addition, using ultrasound enables real-time 
monitoring of the block needle, visualization of important 
anatomical structures, and the spreading of local anesthetic. 
We could expect that these features of UGIANB to convey 
the advantages of a higher success rate and a lower incidence 
of serious complications such as arterial puncture compared 
with those of MN block and intraoral-approach IANB.
Several studies have reported the clinical efficacy of 
UGIANB. We previously reported a case of a teenager who 
underwent orthognathic surgery and required no postop-
erative analgesics after ultrasound-guided maxillary nerve 
block and UGIANB [1]. Kojima et al. have described the

postoperative analgesic effect of UGIANB in a retrospective 
observational study [2]. However, the effective range of the 
local anesthetic has not been investigated. In this cadaveric 
study, we performed UGIANB on Thiel-embalmed cadavers 
and investigated the success rate and the extent of dye spread 
following UGIANB.
Methods
This study was approved by the Ethics Committee of Sap-
poro Medical University, Japan (No. 2-1-33). Four Thiel-
embalmed cadavers (four females) without deformities or 
defects of the head and neck were provided by the Depart-
ment of Anatomy, Sapporo Medical University School of 
Medicine. UGIANB to patients was not included in the study 
protocol (No. 2-1-33). US images and photos in Fig. 5 were 
taken from a patient with pain in the mandibular region after 
obtaining written informed consent.
The IAN enters the mandible after passing through the 
PMS, which is a cleft surrounded by the mandible laterally 
and the pterygoid muscles medially [3]. The IAN, lingual 
nerve (LN), buccal nerve (BN), and maxillary artery are 
located within the PMS [4, 5]. We injected blue or green 
aqueous paint (four blue, four green) dissolved in saline into 
the PMS under ultrasound guidance, as follows. The cadaver 
was placed in the supine position. A 6–15 MHz high-fre-
quency linear probe (Sonosite S II and Edge II; Sonosite, 
Bothell, USA) was applied to the lateral surface of the face, 
positioned transversely and caudad to the zygomatic arch. 
To enable clear visualization of the PMS, we opened the 
mouth widely.
All injections were performed by the same anesthesiolo-
gist with accumulated experience of > 50 UGIANB proce-
dures (SK). A 22-G 5 cm echogenic needle (Ultraplex 360; 
B. Braun, Melsungen, Germany) was inserted from the 
cranial side of the probe using the out-of-plane technique 
(Fig. 1). We administered a small amount of dye when the 
needle tip exceeded the masseter muscle. If the entire lateral 
pterygoid muscle posterior to the PMS was pushed down, we 
considered it as an intra-PMS dose and administered a total 
of 5 mL of dye (Fig. 2). At least 15 min after dye admin-
istration, the cadaver was carefully dissected. Dissections 
were performed by three oral surgeons (TT, KN, HD) and 
one dentist (DO), each with more than 8 years of clinical 
experience, one of whom has performed > 200 gnathoplasty 
surgeries (HD). After the skin incision, the facial nerve (FN) 
was identified entering the medial parotid gland. The zygo-
matic arch and masseter muscle were removed. The mandi-
ble was cut cephalic to the mandibular foramen. The IAN, 
LN, and BN were identified on the surface of the lateral and 
medial pterygoid muscles, and the pterygoid muscles were 
then removed. The foramen ovale was identified, followed by 
the auriculotemporal nerve (ATN) and the MN. The evalu-
ation outcomes were as follows.
(1) Presence/absence of dye in the PMS,
(2) Dye staining of the IAN, LN, BN, ATN, MN, and FN,
(3) Presence/absence of intravascular dye.
Results
Eight UGIANB procedures were performed on the bilateral 
faces of four cadavers. The findings of the extent of dye 
spread into the PMS and around the nerves are presented 
in Fig. 3a.
In 7/8 injections, dye was successfully injected into the 
PMS. In the unsuccessful case, poor ultrasound visualiza-
tion of the PMS prevented administration of the dye. Of the 
seven successful injections into the PMS, the IAN and LN 
were stained in 7/7, and staining was seen in 5/5 BNs that 
could be identified at dissection. No staining was observed 
in the ATN or MN, which lie outside the PMS (Fig. 3b, c).
We identified dye staining of the FN behind the parotid 
gland in one injection. No intravascular dye was observed 
for any injection.
Discussion
The results of the present study confirmed that local anes-
thetic could be administered into the PMS with high accu-
racy by UGIANB, and revealed that 5 mL of dye injected

into the PMS stained the nerves located within the PMS 
(IAN, LN, BN) but not those located outside it (MN, ATN). 
This is the first human cadaveric study to investigate the 
extent of dye spread into the PMS, by either UGIANB or 
intraoral IANB.
Intraoral IANB is a common procedure in dentistry [5] in 
which a needle is advanced blindly through the oral mucosa 
until the PMS is reached. It carries the risk of direct damage 
to nerves and vessels, and has a reported incidence of blood 
aspiration of 2.9–22% [6]. The success rate of intraoral 
IANB is 75–85% [4, 7], and it cannot be performed in 
patients who are unable to open the mouth fully. Kojima 
et al. reported a case in which UGIANB was performed on 
a patient with a mouth-opening disorder. Following the pro-
cedure, this patient was able to open their mouth enough 
to allow dental procedures to be performed [8]. In general, 
opening the mouth during UGIANB lowers the coronoid 
process of the mandible and facilitates ultrasound visualiza-
tion of the PMS (Fig. 4) [9].
Ultrasound enables observation of the block needle, 
important anatomical structures, and the spread of local 
anesthetic in real time, and may thus lead to increased suc-
cess rates and fewer complications. Compared with MN 
block, UGIANB is performed at a more shallow position in 
the infratemporal fossa [10] (Fig. 5a, c), which may decrease 
the incidence of arterial puncture. According to several stud-
ies [4, 5], the maxillary artery and the inferior alveolar artery 
are located within the PMS. However, Iwanaga et al. [5] 
reported that the artery that can be visualized between the 
mandibular coronoid process and the condylar process by 
ultrasound during UGIANB is considered to be the maxil-
lary artery. In the present study, the PMS containing the 
maxillary artery could be identified on the ultrasound image 
as the space between the deep part of the masseter and the 
lateral pterygoid muscle (Fig. 5a). However, this area also 
includes the temporalis tendon that attaches to the coronoid 
processes [9, 11]. It is difficult to distinguish between the 
PMS and the temporalis tendon using ultrasound equipment, 
and it is also difficult to accurately advance the needle tip 
into the PMS. To perform UGIANB safely, local anesthetic 
should be administered following the ‘pop’ that occurs when 
the needle tip crosses the masseter, with confirmation by 
ultrasound (Fig. 5b). In this study, we performed UGIANB 
in the same way, and if we could obtain an image in which 
the entire lateral pterygoid muscle was pushed down, we 
administered dye at that point. As a result, the dye was accu-
rately administered within the PMS. Advancing the needle 
too far can lead to accidental puncture of the maxillary 
artery. In our study, dye successfully reached the PMS in 
7/8 (87.5%) UGIANB injections, which is a higher success 
rate than for common intraoral IANB, and the complica-
tion of intravascular administration was not observed. The 
quality of the ultrasound image was insufficient to visualize 
the PMS in one case, which prevented injection. However, 
pulsation of the maxillary artery is a useful marker for iden-
tifying the PMS in a living person (Fig. 5a).
UGIANB is a compartment block for administering local 
anesthetic into the PMS, and the dosage has been previously 
reported as 3–5 mL [1, 2, 8, 12].In the present study, 5 mL 
of dye stained the IAN, LN, and BN enough to enable iden-
tification at dissection in all successful injections; therefore, 
we consider that this volume was sufficient. An additional 
finding of the study was that dye administered into the PMS 
did not spread to nerves outside the PMS. Because the PMS 
is a compartment enclosed by fascia and the mandible [4], 
and as fascial integrity is retained in Thiel-embalmed cadav-
ers [13], we consider that dye was unlikely to spread through 
the fascia of the PMS.
We were unable to identify the BN in the first two injec-
tions for the reason that we removed the masseter before 
identifying this nerve, which attaches behind the masseter. 
From the third injection onwards, we removed the masse-
ter after identifying BN, and we could observe dye-stained 
BN in the PMS. The FN passes between the parotid gland

and the masseter. We observed dye-stained FN behind the 
parotid gland in one injection, which suggests that some 
dye was administered at a shallower position than the PMS.
This study has the following limitation. Thiel-embalmed 
cadavers have excellent physical and functional properties 
for performing ultrasound-guided regional anesthesia [13]; 
however, the results of cadaveric studies do not necessarily 
reflect the results in living subjects [14, 15]. Nevertheless, 
in the present study, consistent staining patterns were seen 
for nerves in the PMS and for nerves outside the PMS. 
It is important to conduct further investigation of local 
anesthetic spread and the analgesic effect of UGIANB in 
clinical studies with living patients.
The IAN, LN, and BN provide sensory innervation to 
the mandible, mandibular teeth, buccal mucosa, gingiva, 
and anterior two-thirds of the tongue. In this cadaveric 
study, we demonstrated that UGIANB could provide suf-
ficient analgesia for mandibular surgeries. In conclusion, 
UGIANB could administer anesthetics into the PMS with 
high accuracy. The injections reached the IAN, LN, and 
BN in almost all cases, but did not reach the MN or ATN 
located outside the PMS.

脂肪組織における間葉系細胞の細胞老化は糖尿病性創傷の病的変化を促進する 
 
研究目的 
糖尿病性潰瘍は難治性潰瘍であり、有効な治療法の開発が国際的に望まれている。潰
瘍部に起こる慢性炎症の制御が新たな治療法開発の鍵と推測されるものの、未解明な点
も多い。慢性炎症の原因の一つとして着目されている細胞老化は、もともと細胞にダメ
ージが加わったとき、異常な細胞増殖を止めるためのがん抑制機構として知られる。老
化細胞は、自らの増殖を止めるとサイトカインやケモカインなど様々な因子の分泌現象
(細胞老化関連分泌形質 Senescence Associated Secretory Phenotype: SASP)を起こす。
この SASPによって免疫細胞が誘導され、老化細胞自身は貪食されて組織のクリアラン
スを促すことで、組織の修復・再生に至る。しかし、慢性炎症において老化細胞がクリ
アランスされず蓄積することで病態を引き起こすことが数多く報告され、組織修復を促
す細胞老化と慢性炎症を誘導する細胞老化の違いは何なのか、その謎の解明が重要視さ
れている。 
糖尿病患者では全身の脂肪組織に慢性炎症が起こっており、多様な炎症性サイトカイ
ンが過剰に発現し、創傷治癒を遷延させることが既に知られている。糖尿病性潰瘍の成
因に、脂肪組織における細胞老化の異常性との関係性が強く示唆されるものの、既存研
究のほとんどは潰瘍表面および皮膚のみを対象として解析されているため、皮膚の深部
にある脂肪組織との関係性は解明されていない。本研究の目的は、皮膚の創傷治癒過程
における脂肪組織の細胞老化を明らかにし、難治とされる糖尿病性潰瘍の新しい治療タ
ーゲットを探索することである。 
 
研究方法 
2 型糖尿病マウス(Leprdb/db)と非糖尿病マウス(Leprdb/+)の背部に開放創を作成し、2
日後、8 日後で組織を採取し、PCR および免疫組織化学法を用いて、細胞老化関連遺
伝子の発現量解析および細胞老化を呈する細胞の同定を行った。脂肪組織が皮膚創傷
治癒に与える影響を検討するため、蛍光色素 DiI でラベルした糖尿病マウスの皮下脂
肪組織を非糖尿病マウスの背部皮下に移植した群(疾患群)と、非糖尿病マウスの皮下脂
肪組織を非糖尿病マウスに移植した群(コントロール群)のにおいて創傷治癒過程およ
 
 
 
び脂肪由来細胞の局在を解析した。さらに、糖尿病マウスおよび非糖尿病マウスの皮
下組織が産生する SASP 因子の違いを解析するため、脂肪組織を培養し、培養上清の
SASP因子を抗体アレイ解析を行った。次いで、SASPを含む培養上清でマウス線維芽
細胞を培養し、創傷治癒アッセイを行った。最後に、糖尿病患者および非糖尿病患者か
ら得られた組織検体を用い、免疫組織化学法を用い解析を行った。 
 
研究成績及び考察 
開放創作成後 2日、8日における皮膚、皮下脂肪組織における細胞老化関連因子の発
現量を解析したところ、非糖尿病マウスの皮下脂肪組織において、開放創作成後 2日で
細胞老化関連遺伝子が一過性に増加したが、糖尿尿マウスでは開放創作成後 2日におけ
る一過性の細胞老化関連遺伝子の発現増加は認められなかった。次に、組織学的解析に
よって、非糖尿病マウスの皮下脂肪組織で一過性に老化する細胞が間葉系細胞であるこ
とを明らかにした。さらに、非糖尿病マウスでは老化した間葉系細胞が開放創作成後 8
日で減少するのに対し、糖尿尿マウスでは開放創作成後 8日の時点でも老化した間葉系
細胞が蓄積することを明らかにした。次に、皮下脂肪組織が創傷治癒の遅延に関与する
かどうかを検討するため、糖尿病/非糖尿病マウスの皮下脂肪組織をそれぞれ、非糖尿病
マウスに移植したところ、糖尿病マウスの皮下脂肪組織を移植された群で、有意に創傷
治癒が遅延することがわかった。その時、脂肪由来細胞は創傷治癒過程の真皮にはほと
んど存在しないことから、液性因子によって創傷治癒を制御している可能性を見出し
た。そこで、脂肪組織が分泌する SASP 因子の違いを糖尿病/非糖尿病マウスでそれぞ
れ解析したところ、SASP因子の発現パターンの違いを認めた。さらにそれらの SASP
因子を含む培地で線維芽細胞を培養し、創傷治癒アッセイを行ったところ、糖尿病マウ
ス由来の SASPは創傷治癒を抑制することを in vitroで明らかにした。最後に、ヒト検
体を用いた解析によって、糖尿病患者において、創傷形成後の経過日数が多いほど、老
化間葉系細胞の割合が増加することを明らかにした。 
 
結論 
本研究は、正常な創傷治癒では皮下脂肪組織における間葉系細胞の一過性の細胞老化と
それに伴う SASP の分泌が創傷治癒を促す一方で、糖尿病では皮下脂肪組織に老化した
間葉系細胞が蓄積し、創傷治癒を阻害する SASPを分泌することで少々治癒を遷延させ
る可能性をマウスを用いた実験で明らかにした。この病態は糖尿病性潰瘍患者において
も同様であり、糖尿病性潰瘍の病態に細胞老化が影響していることが強く推察された。
これらの結果から、皮下脂肪組織に存在する間葉系細胞の細胞老化制御が、糖尿病性潰
瘍の新しい治療アプローチになる可能性を示した。

結果の要旨 
 
本研究は、正常な創傷治癒では皮下脂肪組織における間葉系細胞の一過性の細胞老化
とそれに伴う Senescence associated secretory phenotypes (SASP)因子の産生によって
創傷治癒を促す。一方で、糖尿病では皮下脂肪組織に老化した間葉系細胞が蓄積し、そ
の SASP は創傷治癒を阻害する可能性を、糖尿病モデルマウスを用いた実験で明らかに
した。この病態は糖尿病性潰瘍患者においても同様であり、糖尿病性潰瘍の病態に細胞
老化が影響していることが強く推察された。これらの結果から、皮下脂肪組織に存在す
る間葉系細胞の細胞老化制御が、糖尿病性潰瘍の新しい治療アプローチになる可能性を
示した。 
 
以上の研究結果および質疑応答を踏まえ、博士（医学）の学位授与を審査委員全員に

 研究目的 
  慢性炎症性皮膚疾患は、表皮ケラチノサイト、免疫細胞など様々な細胞が関与し、 
複雑な病態を形成する。IL-9はアトピー性皮膚炎や乾癬において、病態への関与が示唆
されているが、その役割については明らかになっていない。本研究では、表皮特異的 IL-
9受容体欠損マウスを用いて、慢性皮膚炎症における、表皮ケラチノサイトへの IL-9の
役割を明らかにすることを目的とした。                                        
 
 研究方法   
  表皮特異的 IL-9受容体欠損マウスにイミキモドを塗布して皮膚炎を誘導し、皮膚 
症状の重症度スコア、炎症部位の皮膚を用いた免疫組織化学染色による炎症細胞浸潤の 
程度、定量 PCR検査による遺伝子発現を評価し、コントロールマウスと比較した。イミ 
キモド塗布後の耳介表皮を用いて RNAシークエンス解析を行い、表皮特異的 IL-9受容 
体欠損マウスとコントロールマウスの遺伝子発現を比較検討した。細胞への機能的役割 
については、骨髄由来のマスト細胞や耳介皮膚の細胞を用いて、ELISA、chemotaxis 
assay、CBA assayにて検討した。                                 
 
研究成績及び考察 
  野生型マウスの定常状態の皮膚では、免疫組織化学染色にて、IL-9受容体は表皮基 
底層に限局して発現していたが、イミキモドによる皮膚炎誘導後は、IL-9受容体は表皮 
ケラチノサイト全体に発現が増加した。炎症部皮膚では IL-9はマスト細胞に発現して 
いた。表皮特異的 IL-9受容体欠損マウスでは、コントロールマウスと比較して、皮膚 
炎重症度スコアが低下し、病理組織学的所見では、表皮の肥厚が軽減し、浸潤するマス 
ト細胞数が有意に減少した。皮膚炎誘導後の表皮特異的 IL-9受容体欠損マウスとコン 
トロールマウスの耳介表皮を用いた RNAシークエンス解析では、表皮特異的 IL-9受容 
体欠損マウスにおいて、神経ペプチドであるペプチド YYの発現が低下していた。ペプ 
チド YYの刺激により、マウス骨髄由来のマスト細胞からの IL-9の産生が増加し、 
Chemotaxis assayではペプチド YYの刺激によりマスト細胞の走化性が亢進した。マウ 
スの耳介表皮を用いた ELISAにて、IL-9刺激によりペプチド YYの産生が増加した。 
以上のことから、皮膚炎症において、IL-9はマスト細胞から産生され、表皮に作用し 
てペプチド YY の産生を促し、マスト細胞を誘導してさらに皮膚炎を悪化させる可能性
が示唆された。 
 
結論 
  IL-9とペプチド YYは、ケラチノサイトとマスト細胞に作用し、相互に影響を与え

研究目的 
  遺伝性乳癌卵巣癌症候群（HBOC）の診断目的に BRCA1/2遺伝学的検査を検討す
る際，乳癌・卵巣癌の家族歴を考慮することは一般的だが，その他の HBOC関連癌
である前立腺癌・膵癌・皮膚癌（悪性黒色腫）の家族歴については，聴取されていな
いことが多い．そこで，本研究では HBOC診断における乳癌・卵巣癌以外の家族歴
聴取の意義について検証することを目的とする．                              
研究方法   
  2010 年 10 月～2021 年 9 月に北海道がんセンターで BRCA1/2 遺伝学的検査を受
検した 945例中，乳癌既発症で家族歴の記録のある，HBOC と診断された患者 123
例とその他の患者 669例の家族歴を比較した．調査対象の家族歴は，HBOC 関連癌
である乳癌・卵巣癌・前立腺癌・膵癌・皮膚癌と，日本人集団で発症の多い大腸癌・
胃癌・肝癌・肺癌・子宮癌，およびその他癌とした． 
研究成績及び考察 
  BRCA2 病的バリアント陽性症例では，陰性例と比較して，前立腺癌・膵癌・皮膚
癌の家族歴が有意に多い結果となった．それぞれ，平均 0.34/0.21/0.07人．陰性例： 
0.09/0.10/0.01 人．BRCA1 病的バリアント陽性例：0.05 /0.13/0.03 人． (すべて
p<0.05)  
結論 
  BRCA1/2 遺伝学的検査を考慮する際に，乳癌・卵巣癌以外の関連癌の家族歴も聴
取することで，検査を受けるべき患者の取りこぼしが少なくなる可能性が示唆され
た．特に，皮膚癌と HBOC との関連は，日本での報告はなく，今後さらなる検討が
必要と思われる．

結果の要旨 
 
BRCA1/2 遺伝学的検査を考慮する際に、乳癌・卵巣癌以外の関連癌の家族歴も聴取する
ことで、検査を受けるべき患者の取りこぼしが少なくなる可能性が示唆された。特に、
皮膚癌と HBOC との関連は、日本での報告はなく、今後の検討課題として示された。 
上記研究結果について、当該学生より説明があり、質疑応答が行われた。各審査委員か
らの質問に対し、適切に回答しており、問題なく審査会を終了した。
