Muscle Shear Modulus Heterogeneity 
【目的】大腿直筋（RF）の筋損傷は近位領域での発生頻度が高い．我々はヒト生体を対象
に，他動運動に伴う RFの弾性変化は近位で大きいことを報告した．筋伸長に伴う弾性変
化は受動張力を強く反映すること（R2 = 0.99）から，RF筋内の力学特性が不均一である
可能性が示された．しかし，大腿直筋において力学分布が一様でない原因は不明である．
近年，皮膚や深筋膜といった表層組織の剥離により筋弾性が約 50％低下することや，筋
間結合等を介して筋間で張力が伝達分散される筋膜力伝達と呼ばれる現象が報告されて
おり，表層組織や筋間結合は筋弾性変化の局在特性に影響を及ぼす可能性がある．本研究
では皮膚，深筋膜および筋間結合が RFの弾性率に与える影響を筋内の複数部位で検討し
た． 
【方法】Thiel法固定献体 11体 11肢を対象とし，測定は股関節中間位において膝関節屈
曲 0˚,60˚,120˚の 3肢位で実施した．組織処理は皮膚あり，皮膚剥離，深筋膜剥離，RFと
他筋の結合剥離の 4 条件とした．超音波せん断波エラストグラフィ法を用いて各肢位で
の RFの弾性率を筋の起始と停止を結ぶ線の近位から 25％（近位部），50％（中央部），75％
（遠位部）で測定した. RF 弾性において膝角度，処理条件，部位を要因とした反復測定
三元配置分散分析および多重比較検定を用いた．また形態による影響を検討するため，皮
膚あり，膝屈曲 0˚における各部位の筋横断像を超音波 B モード法を用いて撮像し，画像
処理ソフトを用いて解剖学的横断面積（ACSA）を算出した．ACSA において部位を要因と
した反復測定一元配置分散分析を実施した． 
【結果】RF弾性における分散分析の結果，二次の交互作用（p = .16）は無いものの，処
理と部位の交互作用がみられた（p < .001）．組織処理の影響として，近位（-49.3％）お
よび中央（-22.2％）は深筋膜や筋間結合の剥離で弾性率が低下した．一方で，遠位領域
では処理による弾性変化が少なかった(-1.1％)．部位差の傾向として，生体に近い皮膚あ
り条件では近位の弾性が最も高かったが，深筋膜や筋間結合の剥離後は近位の弾性低下
に伴って傾向が逆転し，遠位の弾性率が最も高値を示した．また ACSAは部位の主効果（p 
 
 
< .001）をみとめ，近位・中央領域と比較して遠位領域で小さい横断面積であった． 
【考察】組織処理と部位の交互作用がみられたことから，RF 弾性は部位ごとに表層組織
や筋間結合から受ける影響が異なることが示唆された．近位領域や中央領域では深筋膜
や筋間結合の剥離によって弾性率が低下した一方で，遠位領域では組織処理による弾性
変化が少なかった．深筋膜は全身に渡って骨格筋を支える膜であり，今回剥離を行った大
腿深筋膜は先行研究によって近位領域で薄く，遠位領域に向かうにつれて厚くなるとい
う構造的特徴が示されている．材料力学的に同じ外力を加えた場合には薄い組織におい
てより強い力学ストレスが発生することから，深筋膜の薄い近位や中央領域では深筋膜
から受ける影響が遠位領域よりも大きかった可能性がある．筋間結合による影響として，
大腿直筋の遠位領域における筋間結合は他の大腿四頭筋（内側広筋・外側広筋・中間広筋）
のみである一方，近位領域や中央領域では股関節周囲筋である大腿筋膜張筋や縫工筋，内
転筋との結合も有する．そのため，近位・中央領域では他筋との結合が多く，擬似的な停
止部として作用することで遠位部よりも弾性率が高かった可能性がある． 
【結論】深筋膜および組織間結合が RF弾性に与える影響には筋内部位差が存在し，RF弾
性の筋内不均一性に関与している可能性が示唆された．

Introduction: Intramuscular heterogeneity exists in the shear modulus of the rectus 
femoris (RF) muscle. However, the underlying heterogeneity mechanisms are not 
entirely understood. Previous research has reported that detachment of superficial 
tissues reduces the shear modulus by 50%. The aim of this study was to examine the 
effects of the skin, deep fascia, and intermuscular connections on the shear modulus of 
the RF at multiple sites. 
Materials and Methods: Eleven donors were fixed using the Thiel method. 
Measurements were performed at 0°, 60°, and 120° knee flexion in a neutral hip 
position. Tissue processing was performed under four conditions: superficial tissue 
(CONT), skin off (SKIN), deep fascia detachment (FASC), and intermuscular 
connections detachment (ALL). The shear modulus at the proximal, central, and distal 
regions were measured using ultrasound shear wave elastography. The study was 
approved by the Sapporo Medical University Ethical Committee. 
Results: Three-way ANOVA revealed no significant interaction between treatment, 
site, and angle (P = .156), treatment and angle (P=.067), or site and angle (P=.441). 
 
 
There was a significant effect of treatment (P<.001), site (P=.010), and angle (P<.001) 
and interaction between treatment and site (P<.001). The proximal shear modulus was 
greater than the central for CONT. There were no significant differences between the 
measurement sites for SKIN. The distal shear modulus was greater than the proximal 
for FASC. The distal shear modulus was also greater than the proximal and central for 
ALL. 
Conclusions: Intramuscular regional differences that influence superficial tissue and 
intermuscular connections of RF elasticity heterogeneity were observed.

現代において、医療従事者が遺伝学の基礎知識や適切な認識を持っている必要がある
ものの、遺伝学を学ぶ機会に十分恵まれているとはいえない。歯科領域においても、遺
伝学の応用が有効的であると考えられるにも関わらず、遺伝学を学ぶ体制は整っていな
い。本研究では、1年前の 5年生時に選択科目として遺伝教育の講義を履修した歯学部生
の遺伝教育の長期的効果を確認することに併せて、履修しなかった学生との遺伝につい
ての理解・認識の違いについての調査を行うため、今後の歯科医療を担う歯学部生がど
の程度遺伝医療に興味があり、正しい知識を身に着けているのかを調査した。 
北海道大学歯学部 6年生 60名に対して遺伝に関する認識調査の協力を依頼し、対面に
て集まった 12名にアンケートを配布した。その場で 12名全員が回答し、回収を行った。
その後、COVID19 の影響により対面にての講義等が困難になったため、紙媒体から
Google フォームでのアンケートに切り替えた。60 名全員にメールにて Google フォーム
でのアンケートを送付し、期限内に 17 名より回答を得て、計 29 名より回答を得た（回
収率 48.3％）。 
  結果において、選択科目として「臨床遺伝学」を「履修した」と回答した者は 12 名
（41.4％）、「履修していない」と回答した者は 17名（58.6％）であった。遺伝に関す
る用語の理解を問う設問に対して、生物学で学ぶ知識や歯科医学にも関連する疾患につ
いては、「理解できている」と回答した者が多数であった一方、臨床遺伝医学用語や臨
床遺伝に関連する職種になどについては「聞いたことがない」、「聞いたことがあるが
意味は分からない」と回答した者が多くみられた。また、「歯科教育における遺伝教育
は十分に行われていると思う」、「遺伝性疾患のチーム医療において歯科医のかかわり
は現時点で十分であると思う」という問いに対して、「そう思わない」、「どちらかと
いえばそう思わない」と回答した者は、いずれも 80％以上であった。 
今回の研究では、アンケートに回答した歯学部生の多くが歯科教育における遺伝教育
が十分でなく、遺伝性疾患のチーム医療において、歯科医のかかわりは不十分であると
 
 
 
感じていることが明らかになった。 
歯学部のコア・カリキュラムに臨床遺伝学が加わり、歯学部生が遺伝について学ぶよ
うになることで、遺伝性疾患に関わるチームの一員として、スムーズな連携体制を構築
することが可能になると思われる。歯科医が加わった遺伝医療体制が実現することによ
って、多職種によるチームとして患者を様々な角度からサポートしていくことが可能に
なり、患者の便益につながることと考える。また、学習方法としても従来の講義形式だ
けでなく、グループ学習やロールプレイなどを導入し、効果的に遺伝教育が行われるこ
とにより、学生の知識の定着や興味の促進につながることが期待され、遺伝医療のさら
なる発展に寄与していくものと思われる。

Experiences of smoking and smoking cessation during pregnancy: Interviews with 
mothers with infants 
＜目的＞ 
 妊娠期の禁煙に関するサポート状況、喫煙及び禁煙の思いや行動に影響した出来事や
人などの妊婦を取り巻く背景を含め、どのようにたばこと向き合いながら妊娠期を過ご
していたのかに着目し、妊娠期における喫煙及び禁煙に関する体験を明らかにする。 
 
＜方法＞ 
 研究デザインは、質的記述的研究である。対象者は、妊娠前または妊娠中に喫煙してい
た経験があり、3歳未満の児をもつ母親 6名である。データ収集は、インタビューガイド
を用いた半構造化面接を実施した。インタビューの主な内容は、妊娠中の喫煙や禁煙に関
して生じた思いや自身がとった行動、思いや行動に影響した出来事や人、禁煙のサポート
状況についてである。分析方法は、まず、録音データから逐語録を作成し、妊娠期におけ
る喫煙及び禁煙に関する体験についてのコードを抽出する。次に、コードの類似性と相違
性に留意しながらサブカテゴリー、カテゴリーを作成した。本研究は、札幌医科大学倫理
委員会の審査を受け、札幌医科大学学長の承認を得て実施した(承認番号 4-1-59)。 
 
＜結果＞ 
 対象者 6名は、年齢が 20～30歳代、妊娠前に 1日 2～20本の喫煙を 2～16年続けてい
たが、妊娠期を通して禁煙していた。 
妊娠期における喫煙及び禁煙に関する体験として、111 のコード、22 のサブカテゴリ
ー、8のカテゴリーが抽出された。 
対象者は、【妊娠前は禁煙の必要性を感じなかったが、妊娠を機に禁煙を始める】【妊娠
の自覚からたばこに対する意識が変わり、禁煙の自信をもつ】ようになるが、【妊娠して
からの生活の変化に伴い、禁煙の気持ちが揺らぐ】状況もあり、【禁煙への強い意志と喫
煙の代替行動を見つけて吸いたい気持ちに対処する】ことで禁煙を続けていた。また、妊
娠期のサポートについて、【医療従事者による禁煙のサポートを受けていない】背景があ
 
 
り、【夫の存在と喫煙の配慮が大きな支え】となっている。そして、禁煙を続けていくう
ちに【禁煙による経済面、子どもの成長面でメリットを感じる】ようになり、【妊娠期以
降も禁煙を続けていきたい】と考えていた。 
 
＜考察＞ 
 妊娠前に禁煙の必要性を感じなかった対象者も、妊娠を機に禁煙を始めるため、喫煙に
よる自身の健康への影響以上に、子どもへの影響や子どもの存在は禁煙を強く動機づけ
させ、妊娠という出来事は禁煙の大きなきっかけになっていたと考える。 
妊娠期の禁煙に自信をもち、禁煙を続けていた対象者は、妊娠中にたばこを吸いたい気
持ちが表れていた。これは、妊娠中に禁煙を続けていても再喫煙のリスクがあることを示
唆し、たばこの依存性だけではなく、妊娠による様々な変化が精神状態に影響して吸いた
い気持ちが生じると考える。禁煙の気持ちが揺らいでも、妊娠中にたばこを吸わないよう
に、自分なりに努力しながら過ごしていたことが考えられる。 
妊娠期の禁煙サポートでは、妊娠後すぐに禁煙を開始していたため、医療者によるサポ
ートを受けていなかったことが推察され、夫など身近な人の存在が禁煙の大きな支えに
なると考える。 
そして、妊娠期の禁煙で感じたメリットは妊娠中の禁煙を継続させるとともに、妊娠期
以降の禁煙継続に対する前向きな気持ちにもつながると示唆する。 
 
＜結論＞ 
 妊娠前に喫煙していた妊婦は、妊娠を機に禁煙を始め、禁煙の自信をもつようになるも
のの、たばこの依存性や妊娠による様々な変化から禁煙の気持ちが揺らぎ、自分なりに吸
わないための努力をすることや身近な人の支えがあることで禁煙を続けていた。そして、
禁煙を続ける中でメリットを感じ、妊娠期以降の禁煙継続に対する前向きな気持ちが表
れるという体験があった。

Objectives: This study explored how pregnant women considered and addressed 
smoking during their pregnancy and clarified their experience of smoking and smoking 
cessation during pregnancy; influencing factors surrounding pregnant women, such as 
any support for smoking cessation during pregnancy, perceptions about smoking and 
smoking cessation, and people and events that affected pregnant women’s smoking 
behavior, were evaluated. 
 
 
 
Methods: This qualitative descriptive study included six mothers who had a history of 
smoking before or during pregnancy and had a child under the age of 3 years. Data 
were collected via a semi-structured interview using an interview guide. During the 
interview, the participants were asked about their thoughts and actions regarding 
smoking and smoking cessation during pregnancy, people and events that influenced 
them, and status of support for their smoking cessation. The analysis method required 
first creating verbatim transcripts from the recorded data and then extracting codes 
about their experiences related to smoking and smoking cessation during pregnancy. 
Next, we created subcategories and categories while paying attention to the similarities 
and differences of the codes. This study was reviewed by the ethics committee of 
Sapporo Medical University and was approved by the president of Sapporo Medical 
University (approval number 4-1-59). 
 
Results: The six participants were in their 20s and 30s, smoked 2–20 cigarettes a day 
for 2–16 days before pregnancy, and had quit smoking throughout their pregnancy. 
Each participant was interviewed separately. We extracted 111 codes, 22 subcategories, 
and 8 categories as experiences of smoking and smoking cessation during pregnancy. 
The participants【did not feel the need to quit smoking before pregnancy but took the 
pregnancy as an opportunity to do so】;【their awareness toward cigarettes changed 
upon becoming aware of the pregnancy, and they became more confident about 
smoking cessation】, although【there were situations in which the lifestyle changes 
after pregnancy shook their confidence to quit smoking】. The participants continued 
smoking cessation by【finding a strong will to quit smoking and alternative behaviors 
to deal with the desire to smoke】. Additionally, they received【minimal support for 
smoking cessation from the medical staff】. This was in the background when they 
needed support during their pregnancy; they were assisted by the【presence of their 
husbands, and their husbands’ consideration regarding smoking was a big support】. 
As the participants continued with their smoking cessation, they【began to feel the 
benefits of smoking cessation in terms of their finances and the growth of their 
children】, and they thought that【they could to continue with smoking cessation even 
after their pregnancy】. 
 
Discussion: Even participants who did not feel the need for smoking cessation before 
pregnancy had quit smoking during pregnancy. Therefore, it is thought that rather than 
the negative effects of smoking on one’s own health, the effects of the same on children   
and presence of children strongly motivated the participants to quit smoking, and the 
 
 
event of pregnancy was a major catalyst for their smoking cessation. The participants 
who felt confident about smoking cessation during pregnancy and had continued to quit 
smoking thereafter confirmed their desire to smoke during pregnancy. This suggests 
that there was still the risk of relapse even if the participants continued their smoking 
cessation during pregnancy, and it is thought that not only smoking dependence but also 
various changes due to pregnancy affected their mental state and increased their desire 
to smoke. However, even though the participants’ feelings about smoking cessation 
wavered, they made every effort to avoid smoking during pregnancy. Smoking 
cessation support during pregnancy began immediately after the pregnancy was 
determined, and the participants indicated that there was minimal support from the 
medical staff; instead, the participants obtained their support from the presence of their 
loved ones, such as their husbands, who were a great support for smoking cessation. 
It is suggested that the benefits reported from smoking cessation during pregnancy can 
lead to the continuation of smoking cessation during the pregnancy, in addition to 
positive feelings toward the continuation of smoking cessation after pregnancy. 
 
Conclusion: Pregnant women who smoked before pregnancy began their smoking 
cessation during pregnancy and felt more confident about quitting, although their 
smoking dependence and various changes caused by their pregnancy shook their 
confidence to quit smoking. However, they continued their smoking cessation by 
making an extra effort to not smoke and receiving support from their loved ones. 
Additionally, the participants felt the benefits of continuing their smoking cessation and 
therefore had positive feelings about continuing their smoking cessation after 
pregnancy.

 呼吸筋由来の代謝受容器反射が脳循環に与える影響 
Title of the paper 
 Effects of respiratory muscle-induced metaboreflex on cerebral circulation 
研究目的：呼吸筋活動の増加は，代謝産物産生を増加させ，呼吸筋由来の筋代謝受容器反
射を誘発する．呼吸筋由来の筋代謝受容器反射の誘発は，血管運動神経活動（筋交感神経
活動）の亢進により血圧上昇を引き起こす．この血圧上昇は，活動筋の血管抵抗を増加さ
せ，血流量増加を抑制することが報告されているが，脳循環に与える影響は不明である．
よって本研究は呼吸筋由来の代謝受容器反射が，安静時および運動時の脳血流調節に与
える影響を明らかにすることを目的とした．安静時の交感神経刺激は脳血流を変化させ
ないため，呼気終末二酸化炭素分圧に変化がなければ呼吸筋由来の代謝受容器反射が安
静時の脳血流に与える影響は小さいと考えられる．一方で，有酸素性運動中の呼吸筋由来
の代謝受容器反射の亢進は，より血圧を増加させ，血管抵抗の増加や呼吸筋への血流配分
増加により脳血流の増加を抑制する可能性がある． 
方法：健康な男性 12 名を対象とした．吸気筋力の指標である最大吸気口腔内圧（PImax）
は脳血流測定と同時に測定することができない．そのため 1 日目に脳血流測定と同じタ
イミングで PImaxを測定し，急性に吸気筋力が低下しているかを確認した．その後，安静
試行と運動試行を 2 日に分けて実施した．各試行で，吸気抵抗負荷の 2 分後に脳血流を
測定する吸気抵抗負荷条件（IL 条件）と，抵抗を負荷しない呼吸の 2 分後に脳血流を測
定する条件（コントロール条件）をランダムで実施した．IL 条件では，40％–50％PImaxの
吸気抵抗呼吸を，終了基準を満たすまで行った．脳血流測定中は自由呼吸 3分後に，呼吸
筋の活動を増加させるため，等二酸化炭素過換気を 3分行った．運動試行では最高酸素摂
取量の 40％強度の下肢サイクリング運動を脳血流測定中に実施した．超音波診断装置を
用いて，内頸動脈の血管径および血流速度と，中大脳動脈の血流速度を継続的に測定し
た．各試行で，PImaxをそれぞれの条件前後に測定した．実験中は呼吸循環パラメータを継
続的に測定した．また内頸動脈の血管径と血流速度より血流量，シェアレートを算出し，
血流量と平均血圧を用いて，血管コンダクタンスを算出した.  
結果：PImaxは安静試行および運動試行ともに IL 条件でのみ介入前から低下した（両者と
も P＜0.05）．収縮期血圧は，自由呼吸中は吸気抵抗負荷の影響が有意ではなかった（吸
 
 
気抵抗負荷の主効果：P＝0.06）．しかし過換気中には，運動試行にて IL 条件がコントロ
ール条件よりも高値を示した（P＝0.04）．内頸動脈の血管コンダクタンスは，安静試行お
よび運動試行ともに IL 条件でコントロール条件よりも低値を示し，この低下は自由呼吸
中および過換気中の両方で観察された（吸気抵抗負荷の主効果：両者とも P＜0.05）．内
頸動脈の血流量は，自由呼吸中には吸気抵抗負荷の主効果を認めなかったが（P＝0.22），
過換気中には安静試行および運動試行ともに IL 条件が低値を示した（吸気抵抗負荷の主
効果：P＝0.02）． 
結論：吸気抵抗負荷による血圧上昇の傾向は，呼吸筋由来の代謝受容器反射が誘発された
ことを示した．呼吸筋由来の代謝受容器反射が亢進した場合，安静時および運動時の内頸
動脈の血管コンダクタンスは低下し，脳血管抵抗の増加を示した．さらに，呼吸筋由来の
代謝受容器反射が亢進した状態で呼吸筋仕事量を増大させると，安静時および運動時に

Purpose: Increased respiratory muscle activation leads to enhanced metabolite 
production and induces muscle metaboreflex. Respiratory muscle-induced 
metaboreflex elevates blood pressure by increasing sympathetic vasomotor outflow 
(i.e., muscle sympathetic nerve activity). Consequently, elevated blood pressure 
increases peripheral vascular resistance and attenuates increased blood flow in active 
limbs; however, its effect on cerebral circulation remains unclear. Therefore, this study 
aimed to elucidate the effects of respiratory muscle-induced metaboreflex on the 
regulation of cerebral circulation at rest and exercise. As resting cerebral blood flow is 
maintained after sympathetic stimulation, we hypothesized that, unless the end-tidal 
pressure of carbon dioxide changes that respiratory muscle-induced metaboreflex does 
not change resting cerebral blood flow. In contrast, enhanced respiratory muscle-
induced metaboreflex during aerobic exercise may accelerate the pressor response and 
inhibit an increase in cerebral blood flow by increasing cerebrovascular resistance, 
resulting in preferential blood distribution to the respiratory muscles. 
Methods: A total of 12 healthy men were included in this study. Maximal inspiratory 
mouth pressure (PImax), as the index of inspiratory muscle strength, cannot be 
simultaneously measured with cerebral blood flow. Therefore, we measured PImax at the 
same timing as cerebral blood flow measurement and confirm whether the subject’s 
inspiratory muscle strength was reduced at the first visit. Then, rest and exercise trials 
 
 
were conducted on two separate days. In each trial, cerebral blood flow was measured 
2 min after inspiratory loading breathing (IL condition) and non-loading breathing 
(control condition) in random order. In the IL condition, 40%–50% PImax inspiratory 
resistance breathing was performed till exhaustion. During cerebral blood flow 
measurements, participants breathed spontaneously for 3 min, followed by a 3-min 
isocapnic hyperventilation to increase respiratory muscle activity. In the exercise trial, 
cycling exercise was performed at 40% of the peak oxygen uptake. The diameter and 
blood velocity of the internal carotid artery (ICA) and mean blood velocity of the 
middle cerebral artery were measured by Doppler ultrasound. In each trial, PImax was 
measured before and after performing each condition. Cardiorespiratory parameters 
were continuously measured during the experiment. The blood flow and shear rate of 
the ICA were calculated from the diameter and blood velocity. ICA conductance was 
calculated as blood flow divided by mean arterial pressure. 
Results: PImax decreased from pre-intervention levels only in the IL condition in rest 
and exercise trials (P<0.05 for both). Systolic blood pressure did not change 
significantly due to inspiratory loading during spontaneous breathing (main effect of 
IL: P=0.06). However, when isocapnic hyperventilation is used to increase the work of 
the respiratory muscles, SBP during exercise was higher in the IL condition than in the 
control condition (P=0.04). During both spontaneous breathing and isocapnic 
hyperventilation, ICA conductance was lower in the IL condition than in the control 
condition in rest and exercise trials (main effect of IL: P<0.05 for both). ICA blood flow 
showed no significant effects on inspiratory loading during spontaneous breathing 
(P=0.22) but was lower during isocapnic hyperventilation in the IL condition in both 
rest and exercise trials (main effect of IL: P=0.02). 
Conclusion: The tendency of elevated blood pressure due to inspiratory loading 
indicates respiratory muscle-induced metaboreflex was elicited. Respiratory muscle-
induced metaboreflex reduced ICA conductance both at rest and during exercise, 
indicating increased cerebrovascular resistance. Furthermore, when the work of 
breathing is increased in the presence of enhanced respiratory muscle-induced 
metaboreflex, both ICA conductance and blood flow decreased at rest and during 
exercise. These results suggest that respiratory muscle-induced metaboreflex increases 
cerebrovascular resistance at rest and during exercise and decreases cerebral blood flow 
as respiratory muscle work increases.

【目的】協働筋切除（SA）は，代償性過負荷により実験動物において筋肥大を誘導
する方法として広く用いられてきた．一方，近年，SA では，短期間で急激な筋肥大
が生じる反面，単位断面積当たりの張力である固有張力が低下することが明らかと
なり，生理学的な適応範囲を超えた過用モデルとして認識され始めている．そこで，
本研究では，過用による筋機能低下のメカニズムを明らかにするために，ラット SA
モデルにおいて，固有張力低下の原因を探った． 
【方法】13-15 週齢の Wistar 系雄性ラット 6 匹を用い，右後肢を無処置の対照群
（CNT），左後肢を SA を行う SA 群（SA）とした．麻酔下にて下腿三頭筋のうち腓腹
筋とヒラメ筋の遠位約 1/3 を外科的に全切除した．SA 処置 2 週間後に，足底筋を
採取し，その後の実験に供した．  
【結果】SA 処置後の足底筋では，筋重量および筋線維横断面積が増大した．一方，
電気刺激誘発性の固有張力が著しく低下した．また，物理的スキンドファイバーに
おいて，カフェイン誘発性張力に変化は認められなかったが，脱分極誘発性張力お
よび最大 Ca2+誘発性張力が低下した．さらに，これらの機能障害は，小胞体（ER）
ストレス関連タンパク質（Grp78, Grp94, PDI，Ero1），脂質過酸化物（4-HNE）付
加タンパク質ならびにユビキチン化タンパク質の増加と，三つ組タンパク質（DHPR，
RyR，STAC3，JP1）の減少を伴っていた． 
 
 
【考察】物理的スキンドファイバーを用いた検討により，SA 後の固有張力低下の
要因は，DHPR と RyR の連関不全および筋原線維機能の低下であることが示された．
DHPR と RyR の機能連関には，DHPR および RyR 自体に加え，関連タンパク質である
STAC3 および JP1 が重要な役割を果たすことが報告されている．本研究の結果，SA
後の骨格筋では，これらすべての三つ組タンパク質の減少が認められたことから，
三つ組構造の破綻が固有張力低下の要因であると考えられる．また，SA による三
つ組タンパク質の減少は，ユビキチン化タンパク質の発現量の増加を伴っていた．
そのため，SA では，主要なタンパク分解経路であるユビキチンプロテアソーム系
の活性化が，三つ組タンパク質の減少を引き起こすと考えられる．活性酸素種（ROS）
は，ユビキチンプロテアソーム系を介したタンパク分解を促進することが示されて
いる．本研究において，脂質過酸化の指標である 4-HNE の発現量が増加したことか
ら，SA によるユビキチンプロテアソーム系の活性化への酸化ストレスの関与が示
唆される．また，SA 後の筋では，ER ストレスの上昇により，ROS 生成に関与する
PDI および Ero1 の発現量が増大したことから，ER ストレス由来の酸化ストレスが
誘導されると考えらえる．また，これにより，三つ組タンパク質だけでなく，筋原
線維タンパク質の退行性変化が引き起こされることで，筋原線維機能の低下が生じ
ることが推察される． 
【結論】SA による骨格筋の機能障害は，DHPR と RyR の連関不全ならびに筋原線維
の機能低下を伴い，そのメカニズムには，ER ストレスを引き金とした酸化ストレ
スの上昇およびそれに伴う筋タンパク質の退行性変化が関与することが示唆され
た．本知見は，将来的に，過用による筋機能低下に対する予防・治療法の確立に寄
与することが期待される．

[OBJECTIVE] Synergist ablation (SA) is an experimental procedure for the 
 
 
induction of hypertrophy, whereas it causes a decrease in specific force (i.e., 
force per cross-sectional area). Here, we investigated the mechanisms behind the 
reduction in specific force in the compensatory hypertrophied muscles. 
[METHODS] Male Wistar rat had unilateral surgical ablation of gastrocnemius 
and soleus muscles to induce the compensatory hypertrophy in the plantaris 
muscles. Two weeks after SA, plantaris muscle was dissected from each animal 
and used for physiological and biochemical analyses.  
[RESULTS] SA significantly increased the mean fiber cross- sectional area. In 
contrast, both the depolarization-induced and the Ca2+-activated maximum 
specific force, but not the caffeine-induced force, were markedly reduced in 
mechanically skinned fibers from the SA group. These deleterious functional 
alterations were accompanied by increases in the amount of ER stress proteins 
(i.e., Grp78, Grp94, PDI and Ero1), ubiquitin-conjugated proteins, and lipid 
peroxidation (i.e., 4-HNE), and decrease in the amount of triad protein (i.e., 
DHPR, RyR, STAC3, JP1) in SA muscles.  
[DISCUSSION] By using the mechanical skinned fiber technique, we revealed 
that SA-induced muscle weakness is due to both disturbance in DHPR-RyR 
coupling and impaired force generating capacity in myofibrils . DHPR-RyR 
coupling requires the expression of DHPR, RyR, STAC3, and JP1. Thus, the 
decreased amount of all these triad protein accounts for the reduction in specific 
force production in skeletal muscle from SA group. Notably, the reduction in the 
triad protein was associated with increased amount of ubiquitinated proteins in 
SA group, suggesting that the activation of ubiquitin-proteasome system (UPS) 
causes the degradation of these proteins in compensatory overloaded muscles. 
Previous study has shown reactive oxygen species (ROS) to promote UPS-
 
 
mediated protein degradation. Given that the amount of 4-HNE, an indicator of 
lipid peroxidation, was increased by SA treatment, increased oxidative stress 
may be involved in the activation of UPS in chronically overloaded muscle. 
Furthermore, ROS are generated as a byproduct of oxidative protein folding in 
the ER, which is mediated by ER protein PDI and Ero1. In the stressed ER, 
accumulation of misfolded proteins perpetuates ROS production, as a result of 
persistent protein refolding attempts. Thus, increased amount of PDI and Ero1 
suggests the ER stress as one of the causative factors of oxidative stress in SA-
treated skeletal muscle. The activation of ER/oxidative stress may lead not only 
to the reduction in triad proteins but also to the myofibrillar dysfunction, because 
Ca2+-activated myofibrillar force production has been shown to be depressed 
when skinned fiber is exposed to ROS.  
[CONCLUSION] SA causes intrinsic contractile dysfunction due to the 
impaired excitation-contraction coupling and force-generating capacity. This is 
likely to be induced by degenerative alterations of triad and myofibrillar 
proteins, which result from increased oxidative stress triggered by ER stress 
pathway. These data may contribute to the development of prevention and 
treatment of overuse-induced muscle dysfunction.

 A study on the kinematic characteristics of hand writing using an underlay with an uneven surface 
研究背景 
不器用さを有する児の運筆活動への支援として，様々に工夫された筆記具や補助具が
市販されている．多くは筆記具把持を補助するためのものが多いが，近年，下敷きに細か
いドットを施し，書字や描画時に固有感覚のフィードバックを生じさせ運筆動作の改善
を目的とした道具（以下，ざらざら下敷き）も開発されている．しかし，その道具を使う
事でどのような効果があるのかという点については，客観的な検証は十分に行われてい
ない．  
研究目的 
本研究では健常成人を対象に，ざらざら下敷きを使用した描画時に生じる，運筆の正確
性の変化や体幹，肩甲帯周囲筋の筋活動の特徴を明らかにすることを目的とする．その結
果からざらざら下敷きが描画，書字機能を向上させる支援方法になりうるか考察する． 
方法 
①対象は健常成人男性 34名とし，運筆動作が右利きの者を対象とした．神経学的疾患，
眼科的疾患，動作を行うのに困難な整形学的疾患を有する者は除外した． 
②データ収集の方法／機器 
1)本実験手続き 
下敷き表面に 0.6ｍｍピッチで 0.05ｍｍ程度のドット配置がなされた「ざらざら下敷
き」（商品名：魔法のざらざら下じき，株式会社オフィスサニー）と通常の下敷きの 2種
類を使用した．課題は，Ａ３用紙に印刷された横線（40㎝ 2本線を 3mmの間隔で印刷）・
斜線(40㎝ 2本線を 3mmの間隔で右上がり 40°で印刷)・円（直径 28㎝と 27.4㎝を同心
円上に描いた図形を印刷）とし，罫線からはみ出ないように描画してもらった． 
2)測定 
測定時，動作遂行中の様子はデジタルビデオカメラで被験者の右側方から映像を撮影
し記録した．筋活動はテレマイオ DTS EM－801 を使用し体幹，肩甲帯周囲筋について表面
 
 
筋電図を記録した．  
さらに描画の正確性の評価として，マップメジャーを使用して，罫線からはみ出した描
画の距離を測定した． 
3）データ分析／解析方法 
分析は運筆の正確性として罫線に重なった・はみ出した線の距離，筋活動としては各測
定筋の平均％MVCを求めた．統計分析としては各描画でのざらざら下敷き条件と下敷き条
件のはみ出し距離，各測定筋の平均％MVCの平均値を算出し，ｔ検定を用い有意水準 5％
で比較した． 
結果 
各描画での下敷き条件とざらざら下敷き条件のはみ出し距離では，横線描画は 5.7 ㎝
と 3.9㎝，斜線描画では 10.3㎝と 8.4㎝，円描画では 16.3㎝と 15.6㎝であり，横線と
斜線描画においてざらざら下敷条件ではみ出し距離が有意に短いという結果であった（横
線：t(33）=5.01 p<.001，斜線：t(33)=2.61 p＝0.007）．また，各測定筋の％MVCの条件
間での比較では，斜線描画における右側腹直筋の％MVCがざらざら下敷き条件（ｔ(33)＝
2.19，ｐ＝0.017），円描画では右側僧帽筋中部の％MVCがざらざら下敷き条件 (ｔ(33)＝
2.11，ｐ＝0.02) で有意に高かいという結果であった．  
考察 
下敷き条件間のはみ出し距離の比較から，ざらざら下敷きを使用することで直線描画
時に運筆の正確性が改善されたことが明らかになった．これは，ざらざら下敷きの表面に
施される細かいドットが運筆動作に抵抗を加え固有感覚のフィードバックを促進し，運
動方向がより意識されやすくなったため，運筆の正確性が高くなった可能性が示唆され
る．また，下敷き条件間の測定筋の％MVCの比較では，ざらざら下敷きは斜線課題で右側
腹直筋，円課題で僧帽筋中部の筋活動を促進していた．僧帽筋中部や腹直筋に対して運筆
時の筋活動が増加したことから，描画課題による筋の働きや動作方向によっては上肢操
作や姿勢の安定性ための体幹，肩甲帯周囲筋の筋活動が促通される可能性があることを
示唆された．本研究結果から，ざらざら下敷きは運筆を開始し始める幼児や不器用さがみ
られる児への環境的な支援方法の一つになる可能性があると考えられた．

Abstract   
Background 
Various writing instruments and aids are commercially available to support writing 
activities children who need assistance grasping writing utensils. Recently, however, 
a tool with fine dots on the underlay has been developed to generate intrinsic sensory 
feedback during writing and drawing and to improve writing movements (hereafter 
 
 
referred to as “rough underlay”). However, an objective verification of the effectiveness 
of these tools has not been sufficiently conducted. 
Aim 
The purpose of this study is to clarify the characteristics of changes in stroke accuracy 
and the activities of the trunk and periscapular muscles of the shoulder girdle in 
healthy adult participants when drawing with a rough underlay. The results will be 
used to examine whether rough underlay can be used as a support method to improve 
the participants’ drawing and writing functions. 
Method 
1.The participants were 34 healthy male adults who were right-handed in writing, 
excluding those with neurological, ophthalmological, or orthopedic diseases that made 
it difficult for them to perform hand movements.  
2. Data collection methods/equipment 
1) Procedure 
Two types of underlays were used: a “rough underlay” (product name: Mahono 
Zarazara Shitajiki, Office Sunny Inc.) with dots of about 0.05 mm at a pitch of 0.6 mm 
on the surface of the underlay, and a normal underlay. The participants were asked to 
draw horizontal lines (two 40-cm lines printed at 3-mm intervals), diagonal lines (two 
40-cm lines printed at 3-mm intervals at 40° to the right), and circles (figures drawn 
on a concentric circle with diameters of 28 cm and 27.4 cm) printed on A3 paper 
without protruding out of the ruled lines. 
2) Measure 
A digital video camera was used to record the participants’ movements from the right 
side during the measurement. Muscle activity was recorded by surface 
electromyography of the trunk and periscapular muscles using a TELEMYO DTS EM-
801. In addition, to evaluate drawing accuracy, we used a map measure to measure 
the distance of drawings that deviated from the ruled lines. 
3) Data analysis/analysis methods 
The distance of the line overlapping or overhanging the ruled line was determined as 
the accuracy of the stroke, and the average % Maximal voluntary contraction 
(hereafter referred to as “%MVC”) of each measured muscle was determined as muscle 
activity. For statistical analysis, the average values of the overhang distance and 
average % MVC of each measured muscle were calculated, and these values were 
compared between the rough underlay and normal underlay conditions in each 
drawing using a t-test. Statistical significance was set at p< 0.05". 
Results 
 
 
The average overhang distances for the normal and rough underlay conditions for each 
drawing were 5.7 cm and 3.9 cm, respectively, for the horizontal line drawing, 10.3 cm 
and 8.4 cm for the diagonal line drawing, and 16.3 cm and 15.6 cm for the circle 
drawing. The overhang distance was significantly shorter in the rough underlay 
condition for the horizontal and diagonal line drawings (horizontal line: t [33] = 5.01, 
p < .001; diagonal line: t [33] = 2.61, p = 0.007). A comparison of the % MVC of each 
measured muscle between the conditions showed that the % MVC of the right rectus 
abdominis muscle in the diagonal drawing was significantly higher in the rough 
underlay condition (t [33] = 2.19, p = 0.017). In the circle drawing, the % MVC of the 
middle right trapezius muscle was significantly higher in the rough underlay condition 
(t [33] = 2.11, p = 0.02). 
Interpretation 
A comparison of the overhang distance between the underlay conditions revealed that 
the use of a rough underlay improved stroke accuracy when drawing straight lines. 
This suggests that the fine dots on the surface of the rough underlay add resistance to 
the stroke motion and promote proprioceptive feedback, making the user more aware 
of the direction of movement, resulting in improved stroke accuracy. A comparison of 
the % MVC of the measured muscles between the underlay conditions showed that the 
rough underlay enhanced the activity of the right rectus abdominis muscle in the 
diagonal task and that of the central trapezius muscle in the circular task. The 
increased activity of the mid trapezius and rectus abdominis muscles during drawing 
may stimulate the trunk and periscapular muscles for upper limb manipulation and 
postural stability, depending on the muscle action and direction of movement. The 
results of this study suggest that the rough underlay may be an effective 
environmental support method for young children who are beginning to write and for 
those who show clumsiness.

 研究目的 
 本研究の目的は、汎用されている手指消毒薬中有効成分における開封後の安定性およ
び消毒効果の変動について明らかにすることである。 
 
 研究方法   
本研究は、札幌医科大学附属病院において設置および保管されていた、手指消毒薬 12
件において、その有効成分であるグルコン酸クロルヘキシジン  (Chlorhexidine 
gluconate: CHG)およびエタノール濃度、消毒効果の変動について解析した観察研究で
ある。手指消毒薬には CHGおよびエタノールを含有するジェル状製剤である「ヒビス
コール S ジェル」を使用した。比較対象には、開封直後の手指消毒薬を用いた。CHG
は高速液体クロマトグラフィー (High performance liquid chromatography: HPLC)
法、エタノールはガスクロマトグラフィー (Gas chromatography: GC)法にて測定した。
設定した分析条件については精度、直線性を検討し、標準品と手指消毒薬との検出ピー
クの比較を行い、有効成分の定量性を検証した。各消毒薬の容器重量、保管されていた
箇所の温度および湿度を計測した。消毒効果については、Escherichia coli ATCC25922, 
Staphylococcus aureus ATCC25923, Pseudomonas aeruginosa ATCC27853 および
Candida albicans ATCC10231を用い、ASTM International が提示する ASTM E-2315
に基づく Time-Kill試験を実施した。 
 
研究成績及び考察 
 対象の手指消毒薬の開封後経過日数は、44 日から最大 921 日 (中央値：180 日)であ
った。CHGおよびエタノールの測定のため設定した HPLCおよび GCの分析条件の妥
当性を検証した結果、併行精度はそれぞれ 4.91%および 3.02%、室内再現精度は 5.46%
および 5.23%であった。また、CHG およびエタノールの標準品を希釈し、検量線を作
成した結果、良好な直線性が得られ、標準品と消毒薬分析時のピークも一致していた。
これらの結果より、設定した分析条件は CHGおよびエタノールの定量分析に十分な性
能を有していることが確認された。各試料の有効成分濃度は、開封直後の手指消毒薬を
100%とすると、開封後 921日経過した試料においても、CHGおよびエタノールともに
96.0%と濃度の減少は認められなかった。また、各有効成分濃度と容器重量、温度およ
び湿度に関連性は認められなかった。Time-Kill 試験の結果、すべての試料において、
菌の発育は認められなかった。したがって、市販の手指消毒剤は、開封後ある程度の日
数経過後も使用可能であることが明らかとなった。CHG は消毒剤の中でも比較的汚染
されやすく、自施設での消毒液の調整、詰め替えによるボトル継続使用に起因する細菌
汚染事例も報告されている。本研究で使用した手指消毒液はエタノール含有溶液であ
り、CHG のみの溶液と比較し細菌汚染の可能性が低い。実際に Time-Kill 試験で菌の
増殖が認められないことから、手指消毒薬を使用期限内に適切に使用し、使用後のボト
ルを廃棄することで、細菌汚染を防ぐことが可能である。また、現在、手指消毒薬は液
体、ジェル、泡状など様々なタイプのものが使用されているが、本研究対象とした手指
消毒薬以外の製品についても、CHG とエタノールを含むジェル状製剤であれば、本研
究と同様の結果を得られると考えられる。 
 
結論 
手指消毒剤は、各施設が独自に設定した開封後使用期限を超過しても廃棄する必要は
ないことが明らかとなった。使用頻度を踏まえた効率的な使用により、手指消毒剤の不
必要な廃棄を減らすことが可能である。

手指消毒薬における有効成分の開封後安定性 
 
結果の要旨 
 本研究は、開封後の手指消毒薬において、消毒効果を有する成分であるグルコン酸ク
ロルヘキシジン（以下 CHG）、エタノール濃度および実際の消毒効果の変動について検
討した。有効成分分析において設定した各条件は適切であることが確認された。また、
手指消毒薬中の CHG、エタノール濃度ともに開封後経過日数に伴う、減少は認められな
かった。さらに、CHGおよびエタノール濃度おいて、容器重量、保管温度および保管湿
度との相関性を検証した結果、CHG、エタノールいずれも各変数との相関性は低く、有
意差も認められなかった。実際の消毒効果について、大腸菌、黄色ブドウ球菌、緑膿菌、
Candida albicans の標準菌株を使用し、ASTM-2315-03 に基づく Time-Kill 試験を行
い、評価した。その結果、今回検討した 12件の手指消毒薬において、いずれの菌種に対
しても、開封直後の消毒薬と比較し、消毒効果の減弱を認めなかった。本研究は、開封
後の消毒薬において、日数経過後も有効成分濃度および消毒効果が減少しないことを明
らかにした。 
本研究の成果は、医療資源の有効活用に寄与可能と考えられ、博士（医学）の学位授与
に値すると審査委員全員より評価された。

 研究目的 
Chimeric Antigen Receptor (以下 CAR)を遺伝子導入した T 細胞(以下 CAR-T 細
胞)は、任意の標的抗原を発現する細胞を攻撃するよう設計できる。CD19を標的とし
た CAR-T細胞は、難治性 B細胞性白血病に劇的な効果をもたらした。肉腫に発現す
る抗原を標的とした CAR-T細胞も開発が試みられているが、臨床成績は芳しくない。 
我々は、がん精巣抗原である DNAJB8 が理想的な標的抗原の候補と考えてきた。
DNAJB8はがん幹細胞の核に高発現し、正常組織では精巣のみに発現がみられる。ま
た、造腫瘍能、薬剤耐性能に関わっており、これを標的とすることで効率的に腫瘍を
拒絶できる可能性がある。一般に細胞内タンパク質に由来するペプチドは、ヒト白血
球抗原(以下HLA)によって細胞表面に提示されるが、我々はこれまでに HLA-A24分
子上に提示される DNAJB8 由来ペプチド (DNAJB8_143)を同定し、HLA-
A24/DNAJB8_143複合体を認識する細胞傷害性 T細胞の抗腫瘍効果を報告した。さ
らに、同複合体を認識し、DNAJB8 特異的 T 細胞受容体に類似した特異性と高い親
和性を示す人工抗体(B10)を樹立した。 
本研究の目的は、B10 抗体を用いて DNAJB8 を標的とする CAR を作成し、その
CARを遺伝子導入した CAR-T細胞による抗腫瘍効果を検討することである。 
 
 研究方法   
1. CAR-T細胞の作成 
抗 HLA-A24/DNAJB8_143 複合体抗体(B10)に、CD28 共刺激因子、CD3z シグナ
ルドメインを結合させた第 2 世代型 CAR(B10-28z CAR)を遺伝子組み換え操作によ
り作成する。これをレトロウイルスベクターに導入し、T細胞に感染させることで、
CAR-T細胞を作成する。ヒト末梢血由来 T細胞に遺伝子導入を行い、CAR発現効率
をフローサイトメーターにて評価する。 
 
 
2. 標的細胞の用意 
DNAJB8ペプチドをパルスした抗原提示細胞(T2-A24細胞株)、および DNAJB8発
現が確認されている腎細胞癌細胞株(CAKI-1)を陽性コントロールの標的細胞として
用いる。骨肉腫細胞株(KIKU, Saos-2, HOS)の DNAJB8 発現を評価し、発現の認め
られるものを標的細胞として用いる。 
3. In vitroでの抗腫瘍効果の検討 
B10 CAR-T細胞を各標的細胞と共培養し、産生されたサイトカインを ELISpotア
ッセイにより検出する。T細胞活性化マーカーの評価、サイトカイン産生評価をフロ
ーサイトメーターにて実施する。細胞障害活性を LDH 法、インピーダンス法にて測
定する。段階希釈法を用いて標的細胞上の DNAJB8 ペプチド量を減少させたときの
B10 CAR-T 細胞の反応を評価する。DNAJB8 ペプチドの 1 アミノ酸置換体を合成
し、B10 CAR-T細胞の抗原認識特異性を評価する。 
4. In vivoでの抗腫瘍効果の検討 
免疫不全マウスに腎細胞がん細胞株、肉腫細胞株を移植し腫瘍形成を確かめる。腫
瘍が形成された個体に、CAR-T細胞を静脈投与し、抗腫瘍効果を検討する。 
 
研究成績及び考察 
  ヒト末梢血由来 T細胞への遺伝子導入プロトコルを新規確立した。B10 CAR遺伝
子は効率よく T 細胞に導入され、CAR 分子を発現した。作成した B10 CAR-T 細胞
は、DNAJB8 ペプチドをパルスした T2-A24 細胞、および HLA-A24 陽性 DNAJB8
陽性の腎細胞がん細胞株、骨肉腫細胞株をそれぞれ認識・活性化し、サイトカインを
産生した。また、これらの細胞に対し細胞傷害性活性を示した。B10 CAR-T 細胞は
HLA-A24 / DNAJB8複合体に対し高い親和性と特異性を示した。CAKI-1、KIKUを
それぞれ移植した免疫不全マウスモデルにおいて、B10 CAR-T 細胞の静脈投与は抗
腫瘍効果を発揮した。一部マウスでは腫瘍の完全寛解が認められた。 
  B10 CAR-T細胞は、標的細胞上のHLA-A24分子上に提示された DNAJB8由来ペ
プチドを、高い親和性と特異性をもって認識・活性化し、強力な細胞傷害活性を発揮
すると考えられる。また、DNAJB8はがん精巣抗原であるため、これまでに報告され
ている他の抗原を認識する CAR-T 細胞と比較して正常組織を傷害する可能性が低
く、より安全性が高いと考えられる。 
 
結論 
  がん幹細胞抗原 DNAJB8 を標的とした CAR-T 細胞療法は骨肉腫をはじめとした
HLA-A24陽性 DNAJB8陽性の腫瘍に対し有効と考えられる。

活性化し、サイトカインを産生した。また、これらの細胞に対し細胞傷害性活性を示した。
B10 CAR-T 細胞は HLA-A24 / DNAJB8 複合体に対し高い親和性と特異性を示した。
CAKI-1、KIKU をそれぞれ移植した免疫不全マウスモデルにおいて、B10 CAR-T 細胞の
静脈投与は抗腫瘍効果を発揮した。一部マウスでは腫瘍の完全寛解が認められた。B10 
CAR-T 細胞療法は骨肉腫をはじめとした HLA-A24 陽性 DNAJB8 陽性の腫瘍に対し有効
と考えられ、将来的な臨床応用につながると期待される。 
 
 以上の研究・質疑応答の結果を踏まえ、4名の審査員から本論文は医学博士の論文に値す

 研究目的 
Histone Deacetylase(HDAC)阻害剤はエピジェニックに作用する薬剤で、ヒストンの
アセチル化を亢進し、クロマチン構造の弛緩を介して発現抑制された遺伝子の発現を促
進させることで、様々な癌細胞に対して抗腫瘍効果を示すことが報告されている。今回、
我々は HDAC 阻害剤による肺腺癌に対する抗腫瘍効果のメカニズムを癌細胞の増殖、
浸潤および遊走に密接な関与が認められるタイト結合分子 lipolysis-stimulated 
lipoprotein receptor (LSR) および claudin-2に焦点を当て、解明し治療に繋げる。 
 
 研究方法   
 ヒト剖検材料、手術材料より分離培養した初代培養癌細胞および肺腺癌細胞株を用い
て、ヒストン脱アセチル化酵素阻害剤による肺腺癌に対する抗腫瘍効果のメカニズムを
解明する。 
具体的な研究方法として、まずヒト剖検材料を用いて、前癌病変(AAH)を含めた肺腺癌
におけるタイト結合分子、特に今回焦点を当てた LSR と claudin-2 の発現および局在
の変化を形態学的悪性度とともに比較検討する。次に肺腺癌細胞株(A549)を用いて、
siRNAを処置して、タイト結合分子の発現変化における癌細胞の増殖、浸潤および遊走
への影響を調べる。次に HDAC 阻害剤を用いて、タイト結合分子の発現調節と抗腫瘍
効果の関係性の検討を行う。 
また手術検体より採取した正常肺組織を用いて、正常初代培養肺上皮細胞を作成し、肺
腺癌細胞株と同様の実験を行い、HDAC阻害剤の正常細胞への影響を検討する。 
 
研究成績及び考察 
 ヒト剖検材料を用いた免疫組織染色において、肺腺癌組織では悪性度とともに LSRの
発現低下および細胞増殖に関与するタイト結合分子 claudin-2の高発現がみられた。 
また肺腺癌細胞株(A549)においても LSR のノックダウンで claudin-2 の発現誘導が認
められた。 
次に A549細胞において、HDAC阻害剤を処置すると、LSRの発現が増加し、claudin-
2 の発現が低下した。また同時に A549 細胞の増殖、浸潤および遊走能を低下させるこ
とがわかった。 
正常初代培養肺上皮細胞では、HDAC阻害剤を処置すると、LSRおよび cludin-2を含
む様々なタイト結合分子の発現を変化させたが、その発現変化は上皮バリア機能には影
響を及ぼさないことがわかった。 
 
結論 
肺腺癌細胞株に対して、種々の HDAC阻害剤による処置を行うことにより、LSRおよ
び claudin-2の発現の変化や局在の変化が起こることが確認され、HDAC阻害剤の抗腫
瘍効果のメカニズムの一端にこれらの細胞接着分子が関連していることが示唆された。

結果の要旨 
本研究では、肺腺癌で抗腫瘍効果を示すことがわかっている HDAC 阻害剤の作用機序お
よび正常細胞への影響の解明を目的とした。研究方法としては、肺腺癌細胞株である
A549 細胞と、手術組織から分離培養した初代培養ヒト肺上皮細胞に、HDAC 阻害剤で
ある TSAと JNJを処置し、PCR、Western blot、cell cycle assay、cell migration assay、
FITC-dextranバリア機能解析、ミトコンドリア呼吸能解析、免疫組織染色や免疫細胞染
色、レーザー顕微鏡や位相差顕微鏡での細胞形態観察などの方法で比較検討した。結果、
肺腺癌細胞株に HDAC阻害剤である TSA及び JNJを処置すると、細胞遊走に関与して
いる 3 細胞間タイト結合分子 LSR の発現を誘導し、細胞増殖亢進に関与している
claudin-2の発現の低下を認め、その結果、細胞増殖・遊走能は低下した。正常細胞にお
いては、HDAC 阻害剤はタイト結合分子の発現を変化させたが、バリア機能解析では上
皮バリアは破綻していないことがわかった。以上から肺腺癌において、HDAC阻害剤が
タイト結合分子を介して抗腫瘍効果を示すことが考えられた。審査委員一同は、本研究
で得られた成果を評価し、申請者が博士（医学）の学位授与に値すると判定した。
