    Malignant Melanoma of the Penis in a Patient with a History of Prostate Brachytherapy: A Case Report and Review of Literature

    Introduction: Penile melanoma is rare and usually results in a poor prognosis. We report a case on a male who presents with penile melanoma after being treated with radiation therapy for his prostate cancer.Presentation of Case: We describe a rare case of penile melanoma discovered  3 years post radiation therapy for prostate cancer in an 84 year old gentleman. The patient presented with a dark skin lesion on the glans. It was noted in the patient’s chart that there was no prior history of melanoma or sun exposure to the genital region. Upon further work up, the patient was found to have penile melanoma without metastases. The patient opted for a partial penectomy. After routine follow up, through 7 year later, the patient continues to show no evidence of reoccurrence of melanoma or metastatic disease.Conclusion: While melanoma is common in the male gender, penile melanoma is found to be only a very small percentage of these cases. Particular interest was taken in this case due to the patient’s history of being treated with Brachytherapy I-125 seeds for his prostate cancer. Although probable cause of the melanoma is unclear, further investigation is warranted for patients who develop penile melanoma post radiation therapy for prostate cancer.

    Therapy Related Chronic Myeloid Leukemia (trCML) or non-Therapy Related Second Malignancy Chronic Myeloid Leukemia (smCML) following Diffuse Large B-Cell Lymphoma(DLBCL): A Case Report and Review of Literature

      Introduction: Second malignancy could be either previous therapy related or non-therapy related like syndromic or shared etiologic exposure. It could be either a hematological/solid malignancy following tretment for previous solid tumour or prior hematological malignanacy.Review of literature regarding secondary Chronic Myeloid Leukemia (CML) following previous active treatment for primary cancer is listed out for further understanding.Presentation of Case: We describe a 71 year old elderly male who developed Chronic Myeloid Leukemia(CML) after a period of 6 years during follow up of Stage IV Diffuse Large B-Cell Lymphoma(DLBCL) for which he received 8 cycles of R-CHOP based Chemo-immunotherapy in 2008 .Whether it is therapy related Chronic Myeloid Leukemia (trCML) following prior cytotoxic treatment or simply a non-therapy related second malignancy hronic Myeloid Leukemia(smCML) is a matter of debate.However our patient responded dramatically like denova CML to imatinib therapy.Conclusion: Therapy related CML or non therapy related second malignancy CML following DLBCL treatment is rare but responds dramatically like denova CML to imatinib therapy.     

    Screening and Optimization of Physical Parameters for Enhanced Alkaline Protease Production by Alkaliphilic Bacillus Subtilis SH2 Isolate

    The present investigations dealt with the optimization of the physical parameters for production of alkaline protease by alkaliphilic Bacillus subtilis SH-2 isolated from slaughter house soil of Warangal, Telangana State, India. Primary screening of four different samples revealed one potent isolate. Morphological and Biochemical characterization followed by Molecular signature of 16s rRNA homology confirmed that the isolate SH-2 belongs to Bacillus subtilis. Bacillus subtilis SH-2 was screened on four different reported mediums (M1213, M660, Horikoshi and Halophilic Bacillus medium) under shake culture conditions. Maximum alkaline protease production (500 EU/ml) obtained on M1213 and Horikoshi mediums. Further optimization of physical parameters by OVAT method revealed that mean generation time (41.18 min), 4% level inoculum, incubation time 72 hrs, pH 10, temperature 350C and agitation 150 rpm are ideal for enzyme production. OVAT method resulted in 2.2 fold increased production of alkaline protease production (1100 EU/ml)

    Development of Weft Knitted Heating Pads on V-bed Hand Flat Knitting Machine by Using Conductive Yarns

    In this investigation weft knitted heating pad was developed on V-bed hand flat knitting machine by using acrylic, polyester as a main yarns  and three different copernic, thermotech –N, thermaram  as conductive yarns with both all knit and inlaid insertion. Moreover time period for heating of yarn, recovery time of yarn for making weft knitted heating pad, structural comparison of different conductive yarn as copernic, thermaram ,thermotech –N  and main yarn as acrylic yarn, polyester yarn was studied. Through analysis conductive yarn that inlaid in the acrylic yarn showed satisfactory heating performance for different time periods and retained more heat rather than polyester yarn. Copernic conductive yarn and thermotech-N conductive yarn had high resistance when compared with thermaram conductive yarn that generated more heat. Acrylic yarn when used as main yarn having conductive thermotech-N yarn inlaid in its structure had produced better heating and retaining properties for the weft knitted heating pad

    Immediate placement of dental implants in non vascularised iliac graft after resection of anterior mandible : A case report

    This case report presents a combination of surgical and prosthetic rehabilitation applied to a case in postsurgical reconstructed mandible. We report a patient suffering from desmoplastic ameloblastoma of mandible, who underwent Enbloc resection and reconstruction with iliac bone graft with simultaneous placement of dental implant in anterior mandible. Two dental implants were placed at both ends of the graft. At five years follow up, favourable osseointegration with healthy peri-implant tissue was reported

    Primary fallopian tube carcinoma is a rare gynecological cancer. Loco regionally advanced disease is clinically difficult to differentiate from similar stage ovarian and primary peritoneal carcinomas. Early stage fallopian tube tumors, despite having unique clinical presentation are provisionally diagnosed as an ovarian carcinoma due to rarity and symptom overlap with these more common gynecological cancers.     Successful Surgical Treatment of a Patient with a Solitary Asymptomatic Cardiac Metastasis from Breast Cancer, Identified by Elevated Tumor Markers and Circulating Tumor Cells

    Indroduction: Cardiac metastases are a not infrequent autopsy finding in patients dying of metastatic cancer, but are less commonly diagnosed during life (1). Although the autopsy incidence of cardiac metastases ranges may be as high as 25%, solitary cardiac metastases in the absence of metastatic involvement of other organs are rare (2).Case Presentation: We report here the case of a 66-year old woman with a history of bilateral breast cancer, where a solitary metastasis in the right atrium was successfully resected.Conclusion: In the absence of any symptoms or clinical findings on physical examination, the presence of metastatic disease was first suggested by the detection of elevated tumor markers and circulating tumor cells during routine follow up after treatment for early stage breast cancer.

    Right Upper-lower Bilobectomy for Double Primary Non-small Cell Lung Cancer

    Introduction: Right upper-lower bilobectomy is rarely performed for non-small cell lung cancer. The prevention of middle lobe torsion is an important aspect of this procedure.Presentation of case: The patient was 68-year-old man. He had double primary Stage1B non-small cell lung cancer, and requiring right upper-lower bilobectomy. The residual middle lobe was rotated approximately 90° in spite of sutured to the diaphragm. This event leads to pooling of sputum and the subsequent development of bacterial pneumonia in the middle lobe. Moreover, acute respiratory distress syndrome developed in the left lung arising secondarily to bacterial pneumonia. The patient died on postoperative day 42 due to respiratory failure.Conclusion: Fixation of the residual middle lobe is important during right upper-lower bilobectomy

    Chronic Myelogenous Leukemia Presenting with Oculomotor Nerve Palsy: A Case Report and Review of Literatures

    Introduction: Chronic myelogenous leukemia (CML), the most common sub-type of leukemia, usually presents with anorexia, fatigue, weight loss, splenomegaly, infection, and bleeding diasthesis. Cranial nerve palsy is a rare occurrence with myelo-proliferative disorder. Though prior literatures reported cranial neuropathies with different myelo-proliferative disorders, commonly acute leukemias and lymphomas, this is the first case of CML presenting with oculomotor nerve palsy.Presentation of case: We report a 55-year old African American male presented in emergency department with features of third cranial nerve palsy, including unilateral ptosis, diplopia, and downward-outward deviation of eyeball without involvement of pupil. Clinical suspicion of CML, based on preliminary routine laboratory investigation, was confirmed by bone marrow biopsy and genetic testing of marrow aspirate. Patient was treated with Imatinib and regularly followed up with complete blood count.Conclusion: CML may present with lone cranial nerve palsy as the first clinical finding. Hence, we emphasize to consider leukemia as one of the possible differential diagnoses in suspected patients presenting with isolated oculomotor palsy, if no other cause is evident
