    Is POCUS the New Stethoscope for the Critical Care PA?

    ABSTRACTThe objective of this paper is to emphasize the vitality of POCUS (Point-of-care ultrasound) and the practical bedside application for diagnosing critical diseases in critical care, specifically critical respiratory illnesses. A literature review search was conducted using PubMed, and identified 5 journals to substantiate the purpose of the paper. The results showed that proper training in POCUS is crucial for practicing critical care PAs (Physician Associates/Assistants), but the type of training is still dependent on the credentialing that the hospital or practice requires. POCUS is not the ultimate authority, but proper utilization can decrease the need for advanced imaging, such as a CT scan (Computed Tomography). POCUS is an initial and noninvasive tool that allows PAs to gather real time, reliable, and reproducible information on acute respiratory illnesses such as pulmonary edema, pneumothorax, and lung effusion. These three respiratory diseases can be diagnosed through the BLUE Protocol, a fast algorithm (\u3c3 \u3eminutes) that provides an analysis of the pulmonary system and the veins within the lungs. The protocol identifies the presence or absence of acute respiratory failure through identifying lung sliding, artifact (A lines/B lines), alveolar consolidation, and/or pleural effusion. With proper training and education in ultrasonography, and specifically the BLUE protocol, the cause of a critical care patient’s acute respiratory failure can be rapidly identified. The implications of these findings highlight the importance of increasing POCUS education among critical care PAs and the need for guidelines both in POCUS education and in diagnosing critical respiratory illnesses.     Surviving the Angry Patient

    Urgent care medicine balances clinical efficiency and patient satisfaction. Providers are expected to see 40-60 patients during a 12-hour shift, provide timely and appropriate medical care, document comprehensively, all while maintaining a patient satisfaction score of 70% or higher. Difficult patient encounters are inevitable in a specialty that thrives on patient satisfaction and throughput. Encountering a problematic patient can derail the rest of the day and demands empathy and patience. All of which makes it hard to meet metrics. This article examines how difficult patient encounters contribute to provider burnout, lowers patient satisfaction, and increases the risk of litigation. Additionally, identifying interventions and mitigation strategies that the urgent care team can influence to improve the encounter for the provider and patient alike. A structured literature review was conducted using peer-reviewed articles from PubMed and Google Scholar. Inclusion criteria were set to “urgent care”, “difficult patient”, “satisfaction score”, “angry patient”, “time management”, and “patient experience.” The article limitations applied were free full-text and published dates of the last five years. Long wait times can influence patient aggression, increasing the risk of litigation. Enhancing operational efficiency by decreasing wait times at all levels of care can lead to higher patient satisfaction. Enhancing personal connection to humanize the patient, rather than approaching each visit as a chief complaint. Prioritizing communication to include patient discharge and follow-up instructions can improve the patient-provider interaction. Mitigation methods enhance provider resilience, thereby reducing the impact of difficult patient encounters. There are very few publications and randomized trials involving the urgent care setting. Most involve the emergency room or specialty clinics. There is not enough time to discover why a patient is difficult, and there is no universal method to mitigate the effects of these encounters. Therefore, using a multi-factorial approach by applying multiple methods outlined in this article to each patient may improve the patient\u27s and provider’s Urgent Care experience

    This research aims to examine how obesity affects work productivity and performance and how companies can support their employees diagnosed with obesity. Obesity is a growing global health concern that has significant implications for decreased workplace productivity and performance. Obesity is considered a chronic complex disease often associated with hypertension (HTN), type-II diabetes mellitus (T2DM), and heart disease that can lead to overall increases in healthcare costs for employers through increased absenteeism, presenteeism, and decreased efficiency. A systemic literature search was completed using PubMed, Cochrane Library, and Google Scholar, identifying multiple journal articles about obesity and productivity in the workplace setting. This paper explores how obesity affects work performance and productivity, including physical and cognitive impairments, increased healthcare costs, and workplace discrimination. Additionally, the paper discusses employer-led interventions, wellness events, and programs that employers can use to enhance employee health and mitigate obesity in the workplace. Since obesity can be a contentious topic, companies will face challenges when implementing obesity-related educational programs. Other challenges employers encounter are medication availability and treatment costs. However, if companies can find a way to work through these challenges, they can support their employees by providing education and possible onsite treatments for obesity. By implementing a structured wellness program, employers can enhance employee health, improve productivity and performance, and reduce healthcare costs, ultimately fostering a more effective workplace

    Nightmare Blueprint: How Personality Types and Anxiety Levels Influence Enjoyment of the Horror Genre

    Why do some people enjoy being scared while others avoid fear at any cost? This study investigated how individual differences in personality, anxiety, and morbid curiosity contribute to the enjoyment of horror. Drawing on frameworks such as Mood Management Theory and Threat Simulation, this research explored psychological and emotional responses to five horror subgenres. Thirty-five participants completed assessments on personality, morbid curiosity, and anxiety and enjoyment before and after watching clips. Results revealed that high negative emotionality was associated with significantly heightened anxiety responses, while low-anxiety individuals reported increased enjoyment. Familiarity with horror had an effect with lower anxiety and increased enjoyment, suggesting potential desensitization. Contrary to prior research, morbid curiosity did not emerge as a significant effect. These findings underlined the psychological complexities of horror consumption, framed it as both a form of emotional exploration and a reflection of individual identity

    Synergy of Stress: Examining the Effects of Proinflammatory Stimuli on C8-D1a Astrocytes

    Neuroinflammation is a complex synergy of different cells within the central nervous system. Astrocytes play a key role in the maintenance of neural functioning, but take a special part in the inflammatory process as well. By releasing pro-inflammatory modulators like cytokines, chemokines, and reactive oxygen species, astrocytes modulate the inflammatory response in the brain. Astrocytes can be activated into this proinflammatory state in response to pathogen derived molecules such as bacterial lipopolysaccharides (LPS). Lipopolysaccharides are glycoproteins derived from gram negative bacteria and commonly used to stimulate cells in vitro to produce an inflammatory response. To look at the effects of LPS on astrocyte inflammation, C8-D1a neonatal murine astrocytes were stimulated dose responsively from 0-1000ng/mL. No significant change in nitric oxide production as measured by the Greiss reaction or in the pro-inflammatory cytokine TNF-alpha was observed through multiple trials. These results are inconsistent within the published literature on astrocyte inflammation, thus more exploration is necessary to deduce the true effects of bacterial stimulation on astrocyte inflammatory responses

The information gathered was taken from previous studies on bankruptcy and then used to compare the different reasons why small and large businesses file for bankruptcy each year. I am looking into whether or not the causes of bankruptcy differ based on a company’s size. While researching, I found no articles comparing small and large companies, so this study will incorporate the small and the large business factors together to help small business owners, people interested in bankruptcy, and large business owners understand the similarities and differences. This study was conducted to help small and large businesses decrease the likelihood of bankruptcy each year. One of the main reasons why these businesses file for bankruptcy is due to a lack of knowledge, and with this paper, the owners of these businesses can learn and not make the same mistakes others have previously made

    F1\u27s Marketing Drive: Cracking the Code to Capturing the U.S. Market

    Formula 1 (F1) has historically faced challenges establishing itself in the American sports market dominated by NASCAR and IndyCar. F1 has been long perceived as a European-centric sport, which has led to struggles with inconsistent U.S.-based Grand Prix events, premium ticket pricing, and a lack of American drivers. Liberty Media\u27s acquisition in 2017 marked a turning point, introducing innovative marketing strategies like digital engagement, new U.S. events, and the Netflix series Drive to Survive. These efforts have driven tremendous growth, with increased viewership and attendance for U.S. fans. However, while F1 averages 1.1 million U.S. viewers per race, this remains far below NASCAR\u27s 3.85 million. Economic analysis reveals that high ticket prices, such as $1,617 for Las Vegas, limit broader accessibility compared to NASCAR\u27s affordable rates. Results show that while F1 has gained traction, sustained growth will include strategic adjustments to counter NASCAR\u27s domination of viewer attention in the American market. This paper argues that F1\u27s expansion in the U.S. hinges on leveraging significant aspects of identity-based fan loyalty and attachment. Strategic efforts like forging American cultural bonds, making it more accessible, and optimizing race weekend experiences have the potential to solidify relations with U.S. fandom. By targeting those areas, Formula 1 is able to vie with NASCAR\u27s hold on the market and be a fixture among the sports presence in America

    This study examines India’s strategic plan to become one of the top three global economies by 2050 through data-driven analysis and comparisons of economic indicators from 2024 to 2050, utilizing data from trusted sources such as the Reserve Bank of India and NITI Aayog. We analyze India’s economic growth in relation to other major emerging economies, focusing on technology, manufacturing, and service sectors. Findings suggest that with strategic policy reforms, investment in innovation, and enhanced infrastructure, India can achieve significant GDP and productivity growth. This article concludes with actionable policy recommendations for local and global stakeholders
