    Social media and the propagation of violence against women

    This article focuses on social media and the propagation of violence against women. It has been claimed that the power of the media to set an agenda, to  focus public attention on key public concerns, including gender issues, is highly significant. This implies that the media can influence what the public  thinks about. However, the foregoing is, to a large extent, in reference to the traditional or mainstream media. We live in a digital age. What about the  new media or social media? Can social media platforms (SMPs) such as Twitter, YouTube and Facebook be used to set the agenda for change in  contemporary society? Can SMPs be used to focus the attention of the public on gender issues, particularly on how to address sexual and other forms of  violence against women? Can these platforms engender social change or they are just tools for leisure or entertainment? It is in reaction to these issues  that this article uses descriptive and observational methods to investigate three social media campaigns on Twitter and Facebook – He For She, Bring  Back Our Girls and My Dress My Choice – with a view to assessing their capacity to set the public’s agenda towards combating sexual violence against  women and whether social media is an effective tool to create awareness on what is at issue. While we argue that the social media is an acknowledged  change agent which is capable of mobilizing the public to stand up against incidents of sexual violence, among other gender issues, the conclusion  reached is that in the three instances we examined, the momentum is often short-lived.&nbsp

    Communicating empathy for change: A kinesics assesment of Ojo Bakare’s “The voyage”

    The primary aim of any theatrical endeavour is to effectively communicate. Kinesics is a non-verbal communication concept which deals with the usage of  body movements and gestures for message dissemination. Kinaesthetic empathy is a kinesics theory which explores the uncanny ability of making  members of audience experience related feelings, and interpret the message being communicated through movements in a performance. “The Voyage”,  created and directed by Ojo Rasaki Bakare is an improvisational total theatre which documents the socio-political journey of Nigeria as a nation, using the  tripartite performative mediums of music, dance and drama for communication. This paper interrogates the various levels of the Nigerian socio-  political change identified through the modes of kinesic communication within the play. Through unstructured interviews conducted on randomly  selected members of the audience, this paper establishes the diverse levels of kinaesthetic empathy registered through the audience response to the  performance. The paper also examines the instructional, emotional and aesthetic values of movements employed in “The Voyage” performance, to  prescribe effective change management in Nigerian governance. The study identifies the elements of kinesic communication such as dance, mime,  gestures and dramatic movements, as extremely useful in disseminating the message of the play and engineering audience empathy towards the  management of the change mantra, as regards the Nigerian political scene. The study recommends more concerted efforts at employing kinesics for effective communication in theatrical performances

    Profil de la pathologie non traumatique du tendon d’Achille à l’écho-doppler: Profile of non-traumatic pathology of Achille’s tendon on Doppler ultrasound

    Objectif : Décrire les anomalies à l’écho-doppler du tendon d’Achille pour douleurs postérieures de la cheville.Matériel et méthodes : Etude transversale réalisée au CHU Campus de Lomé, allant de mai à octobre 2018 (période de 6 mois), incluant 57 tendons d’Achille chez 37 patients adultes se présentant pour un examen écho-Doppler avec une douleur postérieure de la cheville.Résultats : Il y avait une prédominance féminine (sex-ratio: 2,36). L’âge moyen était de 47,61 ans ± 9,86 (25 - 64 ans). Les prescripteurs étaient dominés par les internes (27,03%), les neurologues (21,62%) et les médecins généralistes (18,92%). Les pathologies tendineuses achilléennes étaient les enthésopathies (45,61% ; mécanique dans 40,35% et inflammatoire dans 5,26%), les ruptures tendineuses (21,05%) et les tendinoses (17,54% ; corporéale dans 12,28% et distale du conflit de Haglund dans 5,26%). Les autres lésions (isolées et/ou associées) étaient dominées par les enthésophytes (59,65%), la bursite (24,56%), la péri tendinopathie (10,53%) et une irrégularité de la corticale osseuse (8,77%). Un oedème et une hyper vascularisation de la graisse de Kager étaient observés dans 7,02% chacun.Conclusion : Les pathologies du tendon d’Achille étaient variées et fréquentes surtout chez la femme de la quarantaine. Les principales lésions observées étaient les enthésophytes, les bursites et les ruptures tendineuses. L’écho-Doppler a permis un bilan lésionnel précis et une distinction des atteintes mécaniques et inflammatoires. Objective: To describe Doppler ultrasound abnormalities of the Achilles tendon for posterior ankle pain. Materials and methods: Cross-sectional study carried out at the Campus Teaching Hospital of Lome, ranging from June to December 2014 (period of 6 months), including 57 Achilles tendons in 37 adult patients presenting for an ultrasound Doppler examination with posterior ankle pain.Results: There was a female predominance (sex ratio: 2.36). The mean age was 47.61 years ± 9.86 (25 - 64 years). Prescribers were dominated by interns (27.03%), neurologists (21.62%) and general practitioners (18.92%). The Achilles tendon pathologies were enthesopathies (45.61%; mechanical in 40.35% and inflammatory in 5.26%), tendon ruptures (21.05%) and tendinosis (17.54%; corporeal in 12, 28% and distal to the Haglund conflict in 5.26%). The other lesions (isolated and / or associated) were dominated by enthesophytes (59.65%), bursitis (24.56%), peritendinopathy (10.53%) and cortical bone irregularity (8, 77%). Edema and hypervascularization of Kager's fat were observed in 7.02% each.Conclusion: The pathologies of the Achilles tendon were varied and frequent, especially in women in their forties.     Profil des pathologies neuro-ophtalmologiques à l’imagerie des troubles visuels à Lomé: Profile of neuro-ophtalmological pathologies in visual disorders imaging in Lomé

    Objectif : décrire des anomalies neuro-ophtalmologiques détectées au scanner et à l’IRM pour troubles visuels à Lomé.Matériels et méthodes : Etude transversale, descriptive réalisée dans les services d’imagerie médicale des CHU Sylvanus Olympio et Campus et de la clinique Autel d’Elie de janvier 2012 décembre 2016, concernant les examens de TDM et d’IRM orbitaire et/ou cérébrale des patients de tout âge, demandés pour des anomalies visuelles non-traumatiques durant la période d’étude. Les variables étudiées étaient l’âge, le sexe, le motif de la demande de l’imagerie, les anomalies détectées et leur topographie.Résultats : Au cours de l’étude, 284 examens TDM et d’IRM avaient été réalisés. L’âge moyen des patients était de 38,6 ans. Il y avait 148 (52,1%) hommes (sex ratio de 1,09). La suspicion de neuropathie optique (47%) et l’exophtalmie (30%) étaient les motifs les plus fréquents de demande d’examens d’imagerie. L’imagerie était normale dans 40% des cas. Les lésions étaient de topographie cérébrale dans 62% (n=105), le plus souvent de localisation sellaire (33,4%). Les lésions orbitaires (n=65) étaient intra coniques dans 52%. Les anomalies étaient tumorales (68,9%), vasculaires (17,6%), infectieuses (12,9%) et inflammatoire (0,6%). La pathologie tumorale était le plus retrouvée au-delà de 30ans (63,2%) et de localisation le plus souvent cérébrale (56,4%). Les lésions vasculaires (17,6%) étaient toutes ischémiques et prédominaient chez les patients de plus de 40 ans (83,3%). Les lésions infectieuses (13%) prédominaient avant 39 ans (86,3%) et le plus souvent de siège orbitaires (63,6%). La seule lésion inflammatoire (0,6%) était la sclérose en plaque (SEP) de topographie cérébrale diffuseConclusion : Les étiologies à l’imagerie des troubles visuels sont le plus souvent tumorales avec une prédominance des topographies cérébrales. The variables studied were age, sex, reason for requesting imaging, abnormalities detected and their topography.Results: During the study, 284 CT and MRI scans were performed. The mean age of the patients was 38.6 years. There were 148 (52.1%) males (sex ratio 1.09). Suspicion of optic neuropathy (47%) and exophthalmos (30%) were the most common reasons for requesting imaging tests. Imaging was normal in 40% of cases. The abnormalities were in cerebral topography in 62% (n = 105), most often of sellar localization (33.4%). Orbital abnormalities (n = 65) were intraconical in 52%. The abnormalities were tumor (68.9%), vascular (17.6%), infectious (12.9%) and inflammatory (0.6%). Tumor pathology was most often found beyond 30 years (63.2%) and most often cerebral (56.4%). Vascular lesions (17.6%) were all ischemic and predominated in patients over 40 years of age (83.3%). Infectious lesions (13%) predominated before age 39 (86.3%) and most often of orbital sites (63.6%). The only inflammatory lesion (0.6%) was multiple sclerosis (MS) of diffuse cerebral topographyConclusion: The etiologies of visual disorders in radiological are most often tumoral with a predominance of cerebral topography

    Vomissements incoercibles de l’enfant révélant à postériori une sténose caustique du pylore : aspects cliniques et en imagerie: Uncontrollable vomiting in the child revealing a posteriori caustic pyloric stenosis: clinical and imaging aspects

    Objectif : Décrire les aspects épidémiologiques, cliniques et en imagerie médicale au cours de vomissements incoercibles du jeune enfant après ingestion accidentelle de potasse.Matériels et méthodes : Il s’agissait d’une étude rétrospective mono-centrique de février 2017 – janvier 2019, concernant de jeunes enfants ayant bénéficié d’une échographie abdominale et/ou d’un transit gastroduodénal pour vomissements incoercibles et dont la cause s’est avérée être une ingestion accidentelle de potasse.Résultats : Au total 20 enfants de 18,55 mois d’âge moyen [11-48 mois], étaient reçus pour des vomissements incoercibles évoluant en moyenne depuis 30 jours. L’errance diagnostique dans un environnement médical pédiatrique dominé par les affections infectieuses pulmonaires et gastro – intestinales aboutissait dans 60% des cas à un état de déshydratation et de dénutrition sévères des patients. L’échographie abdominale prescrite dans 13/20 cas, en ultime recours du bilan étiologique retrouvait une importante stase gastrique, une région pylorique affinée avec de rares et faibles passages gastroduodénaux. L’ensemble évoquait une sténose non hypertrophique du pylore. Dans huit cas, les résultats du TOGD complémentaire s’accordaient à ceux de l’échographie. L’antécédent d’ingestion de potasse était incriminé a posteriori.Conclusion : Devant des vomissements incoercibles s’intégrant en réalité dans un tableau d’« occlusion digestive haute » du jeune enfant, l’imagerie médicale élimine les causes extrinsèques ou endoluminales duodéno-pyloriques et permet d’évoquer une sténose non hypertrophique qui sera corrélée à l’antécédent manqué d’ingestion de potasse utilisée dans les ménages pour la fabrication artisanale de savon. Objective: Describe epidemiological, clinical aspects, including the contribution of medical imaging during incoercible vomiting of young children after accidental ingestion of potash.Materials and methods: This was a single-centre retrospective study from February 2017 - January 2019, concerning young children who had undergone abdominal ultrasound and/or gastro-duodenal transit in the aftermath of incoercible vomiting after accidental potash ingestion.Results: A total of 20 patients, around 18.55 months old, were seen for incoercible vomiting, which had progressed on average for 30 days. Diagnostic wandering in a pediatric medical environment dominated by infectious pulmonary and gastrointestinal diseases resulted in 60% of patients being severely dehydrated and undernourished. Abdominal ultrasound prescribed in 13/20 cases, as a last resort for the etiological assessment found significant gastric stasis, a refined pyloric region with rare and weak gastroduodenal passages. All suggested nonhypertrophic pyloric stenosis. In eight cases, the results of the complementary GDT were consistent with those of the ultrasound. The antecedent of ingestion of potash was incriminated posteriori.Conclusion: Faced with incoercible vomiting actually fitting into a picture of "upper digestive occlusion" of the young child, the medical imaging testified to the absence of extrinsic and endoluminal pyloric obstacles. It thus made it possible to evoke a non-hypertrophic stenosis which will be correlated with the missed history of ingestion of potash used in households for the manufacture of soap

    Facteurs pronostiques de l’hématome cérébral spontané à la tomodensitométrie: Prognostic factors of spontaneous cerebral hematoma on CT-scan in patient with hemorrhagic stroke

    Objectif : déterminer à la tomodensitométrie (TDM) les facteurs pronostiques de l’hématome intracérébral de l’accident vasculaire hémorragique (AVCH).Matériels et méthodes : étude prospective analytique couvrant une période de 06 mois, portant sur 98 patients hospitalisés au service de Neurologie du CHU campus de Lomé, victimes d’un AVCH, avec un hématome intra parenchymateux confirmé au scanner cérébral. Les paramètres étudiés concernaient l’âge, le sexe, les signes cliniques, la topographie, le volume de l’hématome, les engagements, le pronostic vital et la durée du séjour hospitalier.Résultats : 98 cas d’AVCH avec un hématome intracérébral étaient colligés. L’âge moyen était de 53,04±10,9 ans et le sex-ratio de 1. L’HTA était le principal facteur de risque (40,89%). Les signes cliniques étaient dominés par le déficit moteur avec 35 cas (35,7%). 41 cas (41,8%) de décès étaient enregistrés. Parmi les patients décédés, l’hématome était sous cortical dans 75,6% des cas, le volume de l’hématome était supérieur à 30cm3 dans 63,4% des cas et dans 70,7% des cas, la déviation de la faux du cerveau était supérieure à 10mm avec une différence statistiquement significative. La durée moyenne d’hospitalisation des patients décédés ou survivants n’avait pas de relation significative avec la topographie, ni le volume de l’hématome, ni de l’engagement.Conclusion : Le volume et la topographie des lésions, associés à l’engagement seraient des facteurs du pronostic vital de l’hématome intracérébral à la TDM. Objective: to determine the prognostic factors of cerebral heamatoma on ct-scan in patients with hemorrhagic strokeMaterials and Methods: it was a prospective analytical study of 06 months duration, including 98 patients admitted in the Neurology ward of the Lomé campus university teaching hospital for ct-scan proved cerebral hematoma in hemorrhagic stroke (HS). The studied variables were age, sex, clinical signs, topography and volume of hematoma, mass-effet, mortality and hospitalisation duration.Results : 98 patients with cerebral hematoma were enrolled. The mean age was 53,04±10,9 years with 1.0 sex-ratio. HTA was the main risk factor (40.89%). Motor deficit (35.7%) was the most frequent clinical. The mortality rate was 41.8% (41 cases). Among dead patients, the hematoma was subcortical in 75.6%,with a volume greater than 30 cm3 in 634%, and 70.7% had a midline deviation of more than 10mm with a statiscally significant difference. The average duration of hospitalisation of dead patients or surviving patients had no significant relationship with the topography, the volume of the hematoma, or the brain midline deviation.Conclusion: The volume and topography, associated with brain midline displacement could be the prognostics factors for brain hematoma on ct-scan

    Concordance diagnostique clinique – imagerie –chirurgie dans les urgences abdomino-pelviennes à l’hôpital de Sikasso: Clinical, medical imaging and surgical diagnostic concordence in abdomino-pelvic emergencies at the Sikasso hospital

    But : nous avons initié cette étude dans le but de déterminer les performances des examens d’imagerie dans le diagnostic des urgences abdomino-pelviennes.Patients et méthode : Il s’agit d’une étude prospective descriptive de 6 mois allant du 4 février au 4 Aout 2019, portant sur 110 patients consentants, hospitalisés à l’hôpital de Sikasso ayant bénéficiés un examen d’imagerie pour urgence abdomino-pelvienne.Résultat : Elle a concerné 110 patients consentants présentant une symptomatologie abdomino-pelvienne aiguë. La moyenne d’âge était de 32,47 ans. Le sexe masculin a été le plus représenté soit un sex-ratio de 1,44. L’étiologie évoquée la plus fréquente a été l’appendicite soit 34,5%. Les diagnostics suspectés par les médecins traitants ont été confirmé par l’imagerie dans 60% des cas. Une différence statistiquement significative (p < 10-3), (Odds ratio = 23,778, IC = ([7,097 - 79,668]) pour l’imagerie de produire un diagnostic abdomino-pelvien exact par rapport à la clinique a été retrouvé. La concordance entre le diagnostic clinique et le diagnostic final s’est révélé corrélé à l’examen demandé avec une (p = 0,020). Nos patients reçus pour urgence abdomino-pelvienne ont pour les 3/5ème bénéficié d’un traitement chirurgical.Conclusion : La démarche de la prise en charge des patients en urgence nécessite un examen clinique bien réalisé qui oriente le choix de l’examen d’imagerie et l’imagerie permet de redresser la suspicion clinique. Malgré ces bons résultats des progrès restent à faire en matière de prescription des examens radiologiques concernant surtout les renseignements. Purpose: We initiated this study with the aim of determining the performance of imaging examinations in the diagnosis of abdomino-pelvic emergencies.Patients and method: This was a 6-month descriptive prospective study from February 4 to August 4, 2019, involving 110 consenting patients hospitalized at the Sikasso hospital who underwent an imaging examination for an abdominal emergency pelvic.Results: It concerned 110 patients with acute abdomino-pelvic symptoms. The average age was 32.47 years. The male sex was the most represented, a sex ratio of 1.44. The most frequent aetiology mentioned was appendicitis, at 34.5%. Diagnoses suspected by attending physicians were confirmed by imaging in 60% of cases. A statistically significant difference (p <10-3, Odds ratio = 23.778, CI = ([7.097 - 79.668]) for the imagining to produce an accurate abdomino-pelvic diagnosis compared to clinic has been demonstrated. The agreement between clinical suspicion, imaging and postoperative diagnosis was found to be correlated with the requested examination (p = 0.020). Our patients received for abdomino-pelvic emergency have for 3 / 5ths benefited from surgical treatment.Conclusion: The approach to the management of emergency patients requires a well-performed clinical examination which guides the choice of imaging examination and helps to redress clinical suspicion. Despite these good results, progress remains to be made in terms of the prescription of radiological examinations, especially concerning information

    Objectif : Déterminer les aspects et le pronostic scintigraphiques des métastases osseuses diagnostiquées en 15 années de pratique médicale nucléaire dans notre milieu.Patients et Méthodes : Il s’agissait d’une étude rétrospective incluant 1278 scintigraphies osseuses réalisées dans le cadre d’un cancer entre 2004 et 2019. Les données épidémiologiques, cliniques et les résultats des scintigraphies étaient colligés à partir des dossiers de médecine nucléaire. Toutes les scintigraphies avaient été faites au Tc99m-MDP sur une gamma-caméra Siemens®, par un balayage corps entier suivi si besoin d’acquisitions statiques et/ou tomoscintigraphiques. Le pronostic scintigraphique était déterminé selon les classifications EOD et SWOG. Les données ainsi compilées ont été analysées grâce au logiciel SPSS® 21.0 avec un seuil de significativité de p<0,05.Résultats : L’âge moyen des patients était de 55,9 ans (15-92 ans), la tranche des 50-70 ans étant la plus représentée (39,36%). Les principaux cancers étaient ceux de la prostate (49,68%) et du sein (31,77%), avec une évolution moyenne de 25,7 mois depuis le diagnostic. Les scintigraphies typiques de métastases osseuses représentaient 40,77% des cas et dans 9,79% de ces cas, la métastase révélait l’existence de la tumeur. Les principales localisations étaient le thorax (79,27%), le rachis (78,31%) et le pelvis (71,01%). Dans 26,49% des cas typiques, il n’existait aucun signe clinique d’appel osseux. Selon les classifications EOD et SWOG, 70,06 et 66,22% des cas avaient un mauvais pronostic, le cancer de la prostate étant le plus péjoratif et celui du col ayant le meilleur pronostic osseux (p=0,0014).Conclusion : Les métastases osseuses sont présentes dans près de la moitié des cas de cancers explorés dans notre milieu, le thorax, le rachis et le pelvis en étant les sièges préférentiels. Près d’un cancer sur dix est révélé par une métastase osseuse et plus d’un quart de patients n’ont pas de signes cliniques d’appel. Le pronostic scintigraphique est péjoratif dans plus de 2/3 des cas. Nous recommandons donc une plus grande sensibilisation des cliniciens pour un bilan systématique de l’extension osseuse dès le diagnostic du cancer. Objective: To determine the scintigraphic features and prognosis of bone metastases diagnosed in 15 years of nuclear medical practice in our setting.Patients and Methods: It was a 15-year retrospective study including 1278 radionuclide bone scans performed from 2004 to 2019. Epidemiological, clinical data and scintigraphy results were collected from nuclear medicine files. All 99mTc-MDP whole-body, static and/or SPECT acquisitions were realized on the same Siemens® gamma-camera. The scintigraphic prognosis was determined according to the EOD and SWOG classifications. Compiled data were analyzed using IBM SPSS® 21.0 software with a significant p-value <0.05.Results: The mean age of the patients was 55.9 years (15-92 years), the 50-70 age group being the most represented (39.36%). The main cancers explored were those of prostate (49.68%) and breast (31.77%), with an average duration of 25.7 months since the diagnosis. Typical scans of bone metastases accounted for 40.77% and the presence of bone metastases revealed an unknown cancer in 9.79%. The preferential sites of bone metastases were the chest (79.27%), the spine (78.31%) and the pelvis (71.01%). In 26.49% of typical cases, there was no clinical sign of bone disease. According to the EOD and SWOG’s classification, 70.06 and 66.22% of cases had a poor prognosis, prostate cancer being the most pejorative and cervical cancer having the best bone prognosis (p=0.0014).Conclusion: Bone metastases are present in nearly half of the cancer cases explored in our setting, the axial bones, mainly in chest, spine and pelvis being their preferential sites. Almost ten percent of cancers are revealed by bone metastases and more than a quarter of patients have no clinical signs. The scintigraphic prognosis is poor in more than 2/3 of cases. We therefore recommend a greater awareness among clinicians for a systematic assessment of bone extension as soon as cancer is diagnosed

    The Effects of the Shared Repertoire and Voluntary Action Characteristics of the Community of Practice on Knowledge Sharing at Kenya Revenue Authority

    Rationale of Study – Knowledge-sharing practices can be used to improve the current practices of the Authority. This study investigated the effects of the shared repertoire and voluntary action characteristics of a community of practice (CoP) on knowledge sharing in the Kenya Revenue Authority.Methodology – A descriptive survey research design was adopted. Both qualitative and quantitative methods were employed to collect data. Data was obtained by administering semi-structured questionnaires. The target population was 1099 KRA employees, including middle-level managers and lower-level staff based at Times Tower in Nairobi. A stratified random sampling technique was employed to create a sample size of 92 respondents, ensuring equal representation of KRA employees.Findings – The ANOVA results implied a significant relationship between shared repertoire and voluntary action with knowledge sharing at Kenya Revenue Authority. The coefficients of determination revealed that voluntary action had a more significant effect over shared repertoire on knowledge sharing in KRA.Implications – This study can help KRA to implement working CoPs, improve knowledge sharing and enhance customer-focused tax administration. The study recommends that there should be specific artefacts only shared with department members and that KRA employees should belong to a professional body beyond the department's requirements.Originality – This study fills a gap in recent research on the effects of the shared repertoire and voluntary action of a community of practice on knowledge sharing

    Determinants of Research Productivity of Academic Staff in Nigerian Research Institutes

    Rationale of Study – Research productivity is paramount to career advancement and prestige of academic staff of research institutes in Nigeria. In spite  of its importance, it has been observed that the level of research productivity of academic staff in Nigeria is low. This study, therefore, investigated the  information literacy skills, emotional intelligence and demographic factors as determinants of research productivity of academic staff in research  institutes in Nigeria. Methodology – This study adopted survey research design. The total enumeration method was used. Validated questionnaire was used to gather data  from a population size of 782 academic staff in the twelve research institutes. A 78- percent response rate was achieved. SPSS version 20 was used to  analyse the collected data. Findings – The findings revealed that information literacy skills, emotional intelligence and demographic factors had significant relationships with  research productivity of the respondents. In addition, information literacy skills, emotional intelligence significantly determined research productivity of  the respondents. More importantly, information literacy skills, emotional intelligence and demographic factors significantly determined research  productivity of the respondents. Implications – The study recommends, among other things, that training on information literacy skills and emotional intelligence be conducted for  academic staff. Additionally, the management of the research institutes should consider demographic factors of the academic staff when formulating  and implementing policies on research productivity. Originality – Few studies have been done on the influence of information literacy skills, emotional intelligence and demographic factors on research  productivity of academic staff in Nigerian research institutes
