Trends and Variations in Tracheal Intubation for Acute Respiratory Failure in the United States
The 62nd Annual Medical Student Research Forum at UT Southwestern Medical Center (Tuesday, January 30, 2024, 3-6 p.m., D1.700 Lecture Hall)BACKGROUND: Acute respiratory failure (ARF) is a critical medical emergency with increasing mortality rates and hospitalizations in the United States (US). Tracheal intubation (TI) is often required to provide mechanical ventilation in these patients. However, recent evidence shows that TI in critically ill patients is associated with substantial morbidity and mortality, and hence, understanding trends and variations in the practice of TI in critically ill patients with ARF is crucial for improving patient outcomes and healthcare practices.
HYPOTHESIS: The study aims to evaluate the frequency of TIs in ARF patients and to assess trends and variations in TI use across US hospitals. We hypothesize a declining trend in TIs among ARF patients and significant variability in TI utilization across US hospitals.
METHODS: In our retrospective cohort study, we utilized the National Inpatient Sample (NIS) database from the HCUP. Patient selection relied on ICD 10th Revision codes to identify critically ill adults aged 18 and above who underwent TI and were diagnosed with ARF (2016-2020). We also extracted patient demographic and hospitalization details from the database.
RESULTS: From 2016-2020, 2,531,420 patients were admitted to US hospitals with ARF, and of these, 522,746 underwent TI (26.02%). The mean age was 62 years, 44.4% were women, and the in-hospital mortality was 32.8% (95% CI, 32.6%-33.0%). The mortality among patients receiving TI increased significantly from 30.6% (95% CI, 30.1%-31.1%) in 2016 to 37.8% (95% CI, 37.3%-38.3%) in 2020 (p<.0001). However, the percentage of ARF-related hospitalizations receiving TI decreased from 23.9% (95% CI, 23.4% - 24.4%) in 2016 to 18.9% (95% CI, 18.5% - 19.3%) in 2020 (p<.0001).
CONCLUSION: We found a decline in TI use for patients with ARF across hospitals in the United States. This could be due to the increased use of alternative techniques to manage ARF, such as non-invasive ventilation and high-flow nasal cannula.Southwestern Medical Foundatio

From Student Project to Published Manuscript: Improving Methodological Quality in Student-initiated Reviews
Health sciences students at our university frequently undertake "systematized" or systematic review-lite projects as part of their course or program requirements. These projects often become the stepping stone to publication for students and their advisors. Librarians are frequently consulted to support these student-initiated projects, before publication or after an initial publication rejection. Our team has developed a set of best practices to improve the methodological quality of such student-initiated projects as they prepare for publication. Not Even in Your Wildest Dreams: The Realities of a Forced Weeding Project
An institutional administrative decision to relocate an IT data center to the journal stacks resulted in the loss of one-third of our print serials. Working with space requirements, decisions were made regarding which titles to withdraw. Seizing on the opportunity and logistical arrangements for the disposal of the print volumes, we expanded our weeding project to monographs on an unaffected floor. This poster describes how an academic health sciences library withdrew over 30,000 items, shifted the entire print journal collection, and began to weed and shift the print book collection
Social Support and Health Information Sharing on Reddit: A Qualitative Analysis of Infertility Patient Experiences
Individuals experiencing infertility often experience a lack of support, leading them to seek community with others experiencing infertility. There is currently a lack of understanding about infertility information behaviors, including finding community and information on Reddit. This study examined infertility-related online discussion on an infertility subreddit to understand Reddit users' infertility and treatment experience. The researchers collected comments of "TREATMENT Community Thread" posts from r/infertility from January to March in 2023 using a Python script through Pullpush Reddit API. Manual coding methods were applied by three coders to identify major themes of the comments on treatment-related posts using a modified version of a theoretical framework developed by studying the Instagram infertility community. Results indicate that Reddit users often share their own personal medical experience as a means of providing advice, and social support is a key theme of enhancing the experiences of individuals in the subreddit. For communities centered around sensitive and alienating health conditions such as infertility, Reddit can provide a place for social support in addition to asking and providing advice. Health sciences librarians have an opportunity to more holistically consider consumer health sources as well as educate communities about how to evaluate information shared in online communities

Efficient and Intelligent Radiotherapy Planning and Adaptation
Treatment planning--inverse planning on volumetric tomography images--is the foundation of modern radiotherapy. Contouring planning structures and optimizing plan dose distributions are the two most important components of treatment planning. Treatment planning happens in two stages. The initial treatment plan is first designed before the commencement of treatment course. With the on-line images acquired before each fraction of treatment, treatment plan may be adapted off-line/on-line accounting for inter/intra-fractional changes. This dissertation presents innovative research to improve the efficiency and intelligence of treatment planning, with a special focus on its application in on-line adaptive planning.
Four standalone, but related, techniques are developed in this dissertation.
* Registration-guided on-line image segmentation. This technique targets the segmentation of on-line images. Based on the previous methods for on-line image contouring such as registration-based contour adaptation and deep learning-based image segmentation, we proposed a technique that can combine the merits of individual method.
* Volumetric dose extension and isodose tuning. Interactive plan dose tuning is essential to on-line plan adaptation. We developed a dose painting algorithm that can output volumetric dose distribution from two isodose surfaces in real time. Then we applied the algorithm to interactive plan dose tuning, which allows for tuning volumetric dose by dragging isodose lines.
* Fluence map prediction. Plan optimization is the central part of inverse planning. We proposed a deep learning-based fluence map prediction method, to achieve inverse planning without optimization. Compared to fluence map optimization, fluence map prediction is as accurate and much faster because of its nature as a direct inference calculation.
* On-line proton range verification. Proton therapy is sensitive to motion and anatomy variation. For the previously proposed range-guided proton therapy strategy, we conducted an end-to-end Monte-Carlo simulation of the on-line proton measurement process. The key parameters of range measurement, such as the mapping from measurement result to range and measurement uncertainty, are obtained by simulation.
Clinical data has been used to train and evaluate each technique. The results show the feasibility, generalizability, as well as limitations of the developed techniques. Future directions for on-line planning were discussed
