Brothers with Becker muscular dystrophy show discordance in skeletal muscle computed tomography findings : A case report
Becker muscular dystrophy is caused by DMD mutations and is characterized by progressive muscle atrophy. The wide variations observed in muscle atrophy progression in Becker muscular dystrophy are considered multifactorial, including differences in mutations and environmental factors. In this case, two brothers, aged 2 and 3 years, had the identical DMD mutation, confirming their Becker muscular dystrophy diagnosis. They began using handrails when ascending and descending stairs at the age of 16 due to progressive muscular weakness. Over an 18-year follow-up, the older brother consistently had high serum creatine kinase levels, significantly over median levels. Muscle computed tomography finings revealed that the older brother’s gluteus maximus and vastus femoris cross-sectional areas were only half and one-third of the younger brother’s, respectively. The mean computed tomography values of gluteus maximus and vastus femoris were significantly lower in the older brother. Our report suggests that muscle atrophy in Becker muscular dystrophy cannot be solely explained by dystrophin mutation or environmental factors
An unusual configuration of two anomalies in the extensor digitorum profundus complex in a human
The extensor digitorum profundus complex underwent degeneration of the ulnar segments during primate adaptation and evolution. This process resulted in the preservation of only the extensor pollicis longus and extensor indicis in some apes, including humans. Consequently, anatomical variations within the digitorum profundus complex in modern humans have been well-documented, with detailed reports on their frequency and patterns in previous studies. Here, we report an unusual arrangement involving two anomalies in the extensor digitorum profundus complex, identified in a 66-year-old Japanese male cadaver. In this cadaver, two accessory muscles differentiated from both the extensor pollicis longus and extensor indicis. Notably, the latter muscle featured a tendon bifurcating towards both the thumb and index fingers, referred to as the extensor pollicis et indicis communis. Under the extensor retinaculum, the tendon of the accessory extensor pollicis longus passed through an independent compartment, whereas that of the extensor pollicis et indicis communis traversed a compartment shared by the extensor indicis and the extensor digitorum communis. Both muscles were innervated by the posterior interosseous nerve. Previous studies have reported that the accessory slip of the extensor pollicis longus and extensor pollicis et indicis communis appear at frequencies of 0.6% and 0.4-1.4%, respectively. However, to the best of our knowledge, a configuration in which both appear simultaneously has not been reported. The data from this case could provide essential insights into the variations in the extensor digitorum profundus complex in humans and non-human primates

紀元前 2 千年紀の北モンゴリア西部は、東アジアの中で先駆けて青銅器文化を受容した当時の文化技術の最先端地域であったと言える。北モンゴリア西部を中心とした鹿石ヘレクスール文化の担い手は、青銅器文化に加えて、おそらく騎馬を利用した遊牧を取り入れることによって分布圏を拡大させていたと考えられる。本稿の目的は、東アジアにおいて当時の最先端の文化技術を受容していた鹿石ヘレクスール文化の担い手である個人に焦点を当て、その生活史や容貌を含めた人物像を明らかにすることにある。対象は、ヒャウル-ヒャラーチ遺跡(ゴビ・アルタイ県)の方形ヘレクスールと円形ヘレクスールからそれぞれ出土した1 号人骨と20 号人骨である。考古学的情報から伺える当時の社会背景を踏まえ、法人類学的分析によって浮かび上がった人物像と生活痕を基に復顔像を制作した
Successful perioperative preparation of a child with autism spectrum disorder in collaboration with his school for special needs education : a case report
Introduction, The incidence of autism spectrum disorder (ASD) in children is greater than 1%. Children with ASD show an increased rate of hospital contact for several reasons. Perioperative preparation for children with ASD can be challenging; therefore, obtaining information from patients’ families prior to surgery is important. However, no previous reports have described the collection of information from educational facilities. Case report, A 12 years-old male patient with ASD was referred for surgery for traumatic dislocation of the left knee joint. Before admission, we obtained valuable information from his parents regarding expected behavioral problems and coping strategies during hospitalization and from his teachers at his school for special needs education. In particular, the information obtained from teachers was specific and practical. Consequently, we could effectively conduct perioperative management based on his specific autistic characteristics. Conclusion, We report a pediatric case of ASD in which favorable perioperative management was successfully achieved by collecting information before admission from family members as well as teachers at the patient’s school for special needs education. This management may help in future hospital admissions for children with autism
