Comparative study on quality of life among married and non- married women living with HIV/ AIDS
p. 65 - 75Human immunodeficiency virus (HIV) infection is a serious health problem among married and non-married women. It has become a health problem for women of all ages. Antiretroviral therapies introduced in the mid 1990s have allowed many women with HIV infection to live long~ before progressing to AIDS, and to have longer survival time following an AIDS diagnosis. Then the issue of quality 0£.life becomes important for these women. The progression of HIV disease most often results in a decreasing quality of life.
While examining quality of life among women living with HIV/AIDS, limited attention has been paid to study addressing the issue of whether there is a difference in the levels of quality of life between married and non-married women living, with HIV/AIDS. This study will assess and compare the levels of quality of life in married and non-married women living with HIV/AIDS. Random systematic sampling of 114 women living with HIV/ AIDS who participated in the study were-administered well structured questionnaires by the palliative care nurses. Quality of life was evaluated using World Health Organization's Quality of Life HIV short form (WHOQOL HIV BREF) instrument.
The data reported in this study suggest that there is no significance difference with regard to quality of life between married and non-married women. The independent t test revealed that the levels of quality of life differed significantly in the married and the non-married respondents, t (112) = -2.64; p value =.01. But controlling the effect of the covariates in both groups, the result revealed that there was no significant difference in the levels of quality of life in non-married and married women, t (1J2) = -1.4, p value = .17. The apparent difference (t (112) = -2.64; p value =.01) in the levels of quality of life was found to be due to covariates variables
Les accidents vasculaires cérébraux au cours du diabète de type2, dans le service de Médecine et d'Endocrinologie de l'Hôpital du Mali.
Au cours de notre étude nous avons retenu conformément à nos critères d'inclusion 50 patients diabétiques de type 2, avec une fréquence de 5,257 p.100, tous diabétiques de type 2, et dont l'AVC avait été diagnostiqué. Cette étude rétrospectre de (Novembre 2015-Octobre 2016) et prospective de (Novembre 2016-Avril 2017) a concerné 50 patients diabétiques de type 2, avec une prévalence de 5,257 p.100 . Elle avait pour objectif général d'étudier les accidents vasculaires cérébraux (AVC) chez les diabétiques de type 2. Parmi ces patients 72 p.100 avaient une évolution de leur diabète allant de 0 à 10 ans ; on notait une prédominance féminine avec 70 p.100 . Nous avions remarqué une prédominance des complications dégénératives chez les patients, il n'existait pas de relation statiquement significative entre la survenue des complications macroangiopathiques chez les diabétiques de type 2 avec respectivement p égal 1.833 pour l'AVC, p égal 0.06 pour la cardiomyopathie et p égal 0.12 pour l'AOMI. Concernant les complications microangiopathiques, parmi les 50 patients diabétiques de type 2 ayant présenté un AVC, nous avons retrouvé une fréquence de 32 p.100 pour la rétinopathie, 10 p.100 pour la néphropathie, 58 p.100 pour la neuropathie. Il s'avère donc indispensable de convaincre les malades et les médecins de l'impérieuse nécessité et du bénéfice à attendre d'une normalisation de la glycémie des diabétiques ; de la prise en charge des facteurs de risque associés au diabète. La dyslipidémie, l'HTA, l'obésité, le tabagisme, l'hyper-uricémie, la néphropathie diabétique étaient les facteurs de risque associés au diabète
Les portes d’entrée tératogènes identifiées chez les tétaniques au centre hospitalier universitaire du point-g. Bamako. Mali
Objective: To determine the frequency,the main types of tetanogenic EP, to describe their topography and to evaluate the progressive factors associated with them
Patients and Method
We conducted descriptive and cross-sectional study with retrospective collection to CHU du Point G over a period of 17 years. Study population consisted tetanus patient whose PED was found at admission.The analyzed variations were collected from complete and analysable hospital records.The expected significance threshold has been set at p<0,05.
Results: we recorded 258 tetanus patients, 4,99% of hospitalizations. EP was found in 217 cases (84,10%) with a sex ratio (M/W)= 9,85 and an average age of 35,94=15,58 years. The majority occupations were manual workers (32,7%), peasants (25,8), pupils/students (10,1%) and housewives (5,1%) significant statistic in cases with EP not found (p=0,63).The main EPs were skin and mucosal injuries (65,4%), open fractures (10,6%), skin lesions (7,8%) and deep wounds (7,4%) . The most important anatomical sites were the lower limbs (62,2%), the upper limbs (26,3%), the head (5,5%), the uterus (2%). The generalized forms appeared more important with the anatomical seats located on the lower limbs and above with no statiscal difference as to the genesis of the clinical form (p=0,45). Hypertention (50%), malaria (28,6%) and one case of epilepsy were the major comorbidities noted in patients with PEBR and with no significant statistical difference with those with EP not found (p=0,97). The classically known EP risk factors were (42,1%) versus the other EP (42,5%) with p>0,05. Dakar’s poor prognosis scores were mostly noted in PEBR patients with 48,8% (score 2), 31,8% (score 3), 7,4% (scores 4), and 0,5 (scores 5 and 6) with a significant statistical difference (p=0,043). The overall lethality is 42,2%. Specifically 41,5% of deaths were noted in patients with PEBR versus EP not found (46,3%) with p=0,77.
Conclusion: Tetanus remains a reality despite the availability of effective means of prevention. The implementation of good practices for the primary management of bacterial entry proteins was an important contribution to tetanus elimination strategies
The prevalence of incidental breast malignancy in women below 35 years old with firoadenoma at university teaching hospital, Lusaka, Zambia
The most common cause of a discrete breast mass is fibroadenoma. Malignancy within a fibroadenoma is very rare with a reported incidence of 0.1 to 0.3%. Despite an audit report from the University Teaching Hospital histology lab done from January to December in 2013 showing that 11% of clinically diagnosed fibroadenoma were found to be malignant on histology, the prevalence of incidental cancer within a fibroadenoma is not known.
This study focuses on the prevalence of incidental malignancy in patients with fibroadenoma. The findings will provide evidence based recommendations on the management of patients less than 35 years old with fibroadenoma at UTH.
The aim of this study was to determine the prevalence of malignancy in fibroadenoma in females less than 35 years old presenting at the University Teaching Hospital during the study period.
This was a cross-sectional study involving women less than 35 years of age old, presenting at University Teaching Hospital with a fibroadenoma. The target population was women with clinically and histologically confirmed fibroadenoma. The inclusion criteria was all women less than 35 years of age who presented with breast lumps that were either clinically or histologically confirmed as fibroadenoma. The breast lumps were excised under local anesthesia and the specimen analyzed for the presence of malignancy after confirming it being a fibroadenoma.
The sample size was 116 using the prevalence formula. The mean age was 22.8 years old and the mean size of fibroadenomas was 3.35 cm. None of the cases presented with breast cancer arising from within the fibroadenoma. The study has demonstrated for the first time in Zambia that the prevalence of the incidental breast malignancy in fibroadenoma was extremely rare in women less than 35 years. Therefore, management of histologically confirmed fibroadenoma in women less than 35 years can be safely managed conservatively.

Etude pilote sur les cephalées des plongeurs chez les collecteurs des sable dans le fleuve niger-station de Bamako.
Les berges du fleuve Niger-station de Bamako, ont servis de cadre à notre étude pilôte. Il s'agissait d'une étude transversale, allant d'Avril 2017 à Décembre 2018, soit une durée de 20 mois. Elle a inclu 101 plongeurs (collecteurs de sable) tous de sexe Masculin, plongeant en apnée sans matériels de plongée, recensés selon une technique d'échantillonnage par comodité sur un ensemble de quatre (04) sites de collecte de sable. Au terme de cette étude, nous avons obtenu les resultats suivants: Une fréquence globale des céphalées de 81,19 p.100 dont 27,72 p.100 étaient probablement des céphalées des plongeurs. La tranche d'age de 26-35 ans a été la plus représentée (soit 38, 61 p.100 ), la plupart des pongeurs étaient non-scolarisés (soit 76,24 p.100 ) et étaientt sans antécédents particuliers (soit 88,12 p.100 ). -Dans 95,05 p.100 des cas, les plongeurs faisaient une (01) activité/jour (partir à la collecte de sable une seule fois/jour). La durée d'immersion par activité était de 3-4 heures dans 50,49 p.100 des cas, la durée de submersion était inférieure à une (01) minute dans 46,53 p.100 des cas et la profondeur de submersion n'atteignait pas -10 mètres en général (soit 91,09 p.100 ). -Dans 34,15 p.100 des cas, les céphalées ont apparu pendant la plongée et étaient accompagnées des signes associés aux céphalées des plongeurs selon l'HIS (dyspnée, incoordination mortice, étourdissements). -Du point de vue thérapeutique: l'automédication a demeuré l'option première des plongeurs céphalalgiques. Les antalgiques (69,51 p.100 ) et/ou les thérapies traditionnelles (19,51 p.100 ) ont été leurs moyens les plus utilisés pour calmer les céphalées. Seul 10,98 p.100 ont opté pour un avis médical en cas de céphalées
Apport de la Protéine C-Réactive dans les pathologies infectieuses chez les enfants de 0 à 15 ans
La protéine c-réactive (CRP) est une glycoprotéine secrétée lors de la phase de réaction aiguë, dont le taux
augmente suite au processus inflammatoire, notamment en cas d’infections bactériennes (pneumocoque), de
maladies histolytiques et dans de nombreux autres états pathologiques. Dans cette thèse nous avons étudié
l’apport de la protéine C-réactive dans le diagnostic des pathologies infectieuses et inflammatoires chez les
enfants de 0 à 15 ans.
Matériels et méthodes
Nous avons mené une étude prospective descriptive et analytique couvrant 110 patients sur une période de 7
mois chez les enfants de 0 à 15 ans reférés au laboratoire d’analyse médicale de l’hôpital du Mali, tous se
présentant pour une détermination de la CRP. Dans cette série, on a étudié l’âge des patients, le sexe, et la
corrélation entre la CRP et certains paramètres de l’hémogramme. Nos données ont été saisies sur le logiciel
Microsoft Office 2016 et l’analyse a été faite sur SPSS 22.0, Microsoft Excel 2016 et Epi Info 7.
Résultats
Pendant la période d’étude, 110 patients ont été inclus répondant aux critères d’inclusion. On a répertorié 72
enfants de sexe masculin et 38 de sexe féminin. L’âge moyen était de 4 ans (extrêmes de 0 à 15 ans). Lors de
cette étude, le lien a été fait entre la température et la CRP ; la corrélation entre une hyperleucocytose et
l’interférence avec un taux d’hémoglobine faible ont été décrites. La variation de la CRP par rapport à l’origine
de la pathologie a été également étudi

Evaluation de l’état nutritionnel des enfants de 02à59mois hospitalisés à l’unité de pédiatrie du CSRéf de Kalaban Coro.
Malgré des progrès constatés dans la région de Koulikoro en général, en matière de lutte contre la malnutrition certaines parties de la région restent encore durement touchées par ce fléau. C’est le cas notamment dans la commune de Kalaban Coro où une augmentation des admissions des cas de MAS a été constatée avec des pics observés après les campagnes de masse et les périodes de maladies infantiles telles que la diarrhée et le paludisme. C’est après ces constats que cette étude a été initiée avec comme but de déterminer la prévalence de la malnutrition chez les enfants de 02 à59 mois hospitalisés. Il s’agissait d’une étude transversale descriptive allant du 01er Janvier au 31Decembre 2018 au cour de laquelle 110 enfants ont fait l’objet de mensurations anthropométriques. Les enfants de 02 à 59 mois hospitalisés présentaient des états nutritionnels très préoccupants pour la malnutrition aigüe (49,1%), la malnutrition chronique (52,7%) et la malnutrition globale (42, 1%). Ce qui constituerait une situation nutritionnelle alarmante par rapport aux normes de l’OMS.
Il est donc important de noter que la malnutrition chronique ou retard de croissance, qui constitue un indicateur de pauvreté reste plutôt augmentée.
La gastroentérite, les infections respiratoires aigües et le paludisme étaient les maladies fréquemment associées à la malnutrition.
Cependant, le faible niveau de scolarisation (48,2 % de non scolarisées) et la grande multiparité des mères (37,3 % des mères) pourraient avoir une répercussion négative sur l’état nutritionnel des enfants.
La couverture vaccinale est d’un niveau peu satisfaisant avec 53,6 % et cela par rapport à la moyenne nationale qui est de 60 %
Analyse de la prescription des antibiotiques dans le service de Médecine Interne du Centre Hospitalier et Universitaire du Point G
Les antibiotiques font partie des médicaments les plus prescrits dans nos hôpitaux en raison de la prédominance des infections bactériennes L’objectif de la présente étude était d’analyser la prescription des antibiotiques dans le service de médecine interne du CHU Point G afin d’optimiser leur utilisation.
Il s’agissait d’une étude transversale avec une phase de recueil rétrospective des données sur une période de 6 mois (1er aout 2018 à 31 Janvier 2019) et une prospective sur 4 mois (1er Février 2019 à 31 Mai 2019). Elle a inclus les patients hospitalisés dans le service. Les prescriptions ont été recueillies à l’aide d’une fiche d’enquête qui nous renseignait sur l’indication, la posologie, la voie d’administration, les examens biologiques et les résultats de l’antibiogramme.
Au cours de cette étude, 203 patients ont bénéficié d’une prescription d’antibiotiques avec un total de 348 antibiotiques prescrits. Au cours de l’antibiothérapie probabiliste, 295 antibiotiques ont été prescrit. Les antibiotiques probabilistes les plus prescrits étaient l’Amoxicilline + Acide clavulanique 38,6%, le Métronidazole (12,6%) et la Ceftriaxone (10,2%). Les Bêtalactamines et les Nitro-5-imidazolés sont les classes d’antibiotiques les plus prescrit en antibiothérapie probabiliste.
Les germes les plus isolés au cours des examens cytobactériologiques des liquides biologiques sont l’Escherichia coli (38,2%) et le Klebsiella pneumoniae (11,8%).
Les antibiotiques prescrit après antibiogramme étaient au nombre de 53 chez 41 patients ayant fait des examens biologiques suivit d’antibiogramme. Les classes d’antibiotiques prescrits après antibiogramme étaient les Fluoroquinolones et les bêtalactamines-carbapénèmes. Les antibiotiques les plus prescrits étaient la ciprofloxacine (24,5%) et l’imipenème (17,0%).
Les antibiotiques étaient prescrits majoritairement par voie orale (55%) pour toutes les antibiothérapies réalisées chez nos 203 patients.
Les bêtalactamines était les antibiotiques les plus fréquemment non conformes (15,5%) à l’indication. Les bêtalactamines semblaient être les antibiotiques les plus fréquemment non conforme à la posologie (0.6%), suivie des Nitro-5-imidazolés (0,3%).
L’Association Amoxicilline + Acide clavulanique + Ringer lactate représentait 70,3% des interactions médicamenteuses.
L’amélioration de la qualité de la prescription des antibiotiques et la lutte contre le taux élevé de résistance des bactéries aux antibiotiques demeurent indispensables pour une meilleure prise en charge des pathologies bactérienne
The prevalence of prostate cancer in men undergoing finger guided prostate biopsy at the university teaching hospital in Lusaka,Zambia
ThesisProstate cancer is the most common malignancy in men other than Skin cancer worldwide. In Zambia a review of histological reports showed that prostate cancer is the most common urological malignancy. Recent studies have shown that there is a high prevalence of this cancer in Africa where the population is predominantly black. From literature a lot of studies have reported a range of 30% to 60% prostate cancer detection rate in men undergoing finger guided prostate biopsies around the world. This study was undertaken to determine the prevalence and Characteristics of prostate cancer in men undergoing prostate biopsy at the University Teaching Hospital in Lusaka Zambia.
A prospective Cross Sectional Study was done at the UTH from April, 2014 to December, 2014 after getting ethical approval from UNZABREC. Patients undergoing prostate biopsy were recruited. A researcher administered Questionnaire was used to collect data on demographic details, Clinical presentation, Indication for biopsy and Prostate specific Antigen (PSA) test result. Then a finger guided, 6 core transrectal prostate biopsies were performed by experienced Urologists. The histological samples were analysed by pathologists at the UTH and other private laboratories. The obtained Data was analyzed using SPSS
A total of 146 men undergoing prostate biopsy were recruited in the study. The age range was 49 to 92 years and the Mean age was 71.57. The most common symptoms were Lower Urinary tract symptoms at 89% in all patients and only 4.3% of men came asymptomatic.56% of the patients had a suspicious DRE finding. The PSA values ranged from 2.69 to 5480ng/ml. Most patients had serum PSA more than 20ng/ml. Prostate cancer (Adenocarcinoma) was found in 61 % (89/146) and Benign prostatic hyperplasia was found in 31.4%. Chronic prostatitis was found in 5.8%. The low risk cancer with Gleason score of 2 to 6 was found in 11.9%. The intermediate risk cancer with Gleason score 7 was found in 44% while the high risk cancer was also 44%.
The prevalence of prostate cancer in men undergoing prostate biopsy at UTH was 61% and 44% of men had high risk cancers which are more aggressive with an unfavorable prognosis.
