    Cette étude a été conduite dans le but d’évaluer l’impact de la mutation du gène PCSK9 sur le métabolisme lipidique au cours de l’infection palustre chez les enfants âgés de 3 mois à 14 ans. Elle a porté sur des enfants âgées de 3 mois à 14 ans tirés d’une étude cas-témoin, qui s’est déroulée de juin 1999 à décembre 2003. Les polymorphismes E670G, I474V, A443T et C679X étaient présents dans la population d’enfants étudiés à Bandiagara à des fréquences variables. Les taux médians de cholestérol total, de HLD, et de LDL étaient bas au cours de l'accès palustre quel que soit sa gravitée. Contrairement aux autres lipides le taux médian de triglycéride était plus élevé chez les enfants présentant un paludisme grave. Comparé au gain de fonction, le polymorphisme perte de fonction était significativement associé à une protection contre la survenue du paludisme grave qui est associé à un faible de cholestérol total. La compréhension des mécanismes de cette association pourrait conduire au développement de nouveaux médicaments interagissant avec la PCSK9 et d’améliorer la prise en charge des cas graves de paludisme

    Predictability of surgical apgar score on short term outcomes of laparotomies at the university teaching hospital in Lusaka, Zambia

    ThesisThe Surgical Apgar Score (SAS) is a very easy and objective tool for triaging patients postsurgery. It can be used as an assessment tool for performance of hospitals, units or individual surgeons. However, not much research has been done to assess its utility in resource limited settings like Zambia. This study was undertaken to assess the predictability of SAS in laparotomy patients at UTH in Lusaka, Zambia.This was a prospective cohort study. A total of 50 patients were recruited, their intraoperative data collected and SAS calculated. The patients were followed up for 30 days. The primary end point was incidence of any major complication and/or death as defined by the Dindo-Clavien classification. Two out of the 50 patients enrolled were lost to follow up, leaving a total of 48 patients. Age ranged from 17 to 89 years with mean age of 38.8 years (SD 17 years). Out of the patients enrolled, 79 percent were male with 73 percent of operations being emergencies. Intestinal obstruction was the most common diagnosis accounting for 31.3 percent followed by viscus perforation. Of the patients enrolled, 58.3 percent had no complications. Overall, complications rate was 41.3 percent which was consistent with published literature. Deep surgical site infection was the most common complications at 12.5 percent. Patients who did not develop complications had a significantly higher SAS compared to patients who did (p<0.001). Mortality rate in the high risk group was higher than predicted by SAS (p=0.23). Period of operation and gender did not significantly affect the SAS of patients (p values =0.45 and 0.28 respectively). This study confirms that SAS is adequate at predicting outcome in laparotomy patients in resource limited settings like UTH. Key words; laparotomy, postoperative complications, Surgical Apgar scor

    Etude des facteurs associés à la survie des enfants nés de mères séropositives suivis à l’USAC du CNAM de 2007 à 2017

    Objectif : Etudier les facteurs associés à la survie des enfants nés de mères séropositives suivis à l’USAC du CNAM Méthodologie : Il s’agissait d’une étude transversale de 5 mois, concernant les enfants nés de mère séropositive entre Juillet 2007 et Décembre 2017 et testés au VIH. Les données ont été recueillies à partir des dossiers médicaux et des registres de notification des grossesses, d’accouchement et de bilans sanguins de suivi. Elles ont été saisies et analysées avec le logiciel Epi info TM 7. Résultats : Au total, 268 enfants nés de 184 mères étaient enregistrés. L’âge moyen des enfants était de 44,24 mois. Toutes les femmes avaient bénéficié de la CPN au cours de laquelle la PTME leur a été expliquée. Parmi elles, 248 étaient sous ARV avant la grossesse et 64,20% avaient une charge virale indétectable. La voie basse représentait 89,92% du mode d’accouchement. Les enfants ont bénéficié de la prophylaxie ARV (94,00%) et du cotrimoxazole (71,60%) à la naissance. L’allaitement maternel exclusif était pratiqué à 76,50%, l’allaitement mixte à 1,87% et le sevrage après 12 mois de vie à 29,10%. La PCR1 était négative à 99.15%, la PCR2 à 100% et la sérologie à 99.15%. Neufs cas de décès ont été enregistrés. Conclusion : Les facteurs associés à la survie des enfants nés de mères séropositives sont la détectabilité de la charge virale, le suivi des CPN, la prophylaxie ARV et le cotrimoxazole, le suivi pédiatrique et le sevrage de l’enfant. Pour une meilleure PTME, les femmes en âge de procréer et les enfants exposés devront bénéficier d’un suivi de qualité. Ceci implique une formation du personnel médical, l’information et l’éducation des PVVIH, la disponibilité des bilans de suivi et des ARV surtout pédiatrique et un support pour les femmes désirant adopter l’allaitement artificie

    Nous avion réaliser une étude rétrospective descriptive allant du 1er décembre 2017 au 30 septembre 2018 dans le service de Gynécologie de l'hôpital du Mali. Elle visait à étudier les hystérectomies dans le service de Gynécologie de l'Hôpital du Mali durant cette période nous avions enregistré 70 cas d'hystérectomie sur 337 cas interventions chirurgicales Gynécologies et obstétricales furent réaliser soit une fréquence de 20, 77 p.100 . La tranche d'âge de 50 ans et plus a été la plus touchée soit une fréquence de 57,14 p.100 . L'âge moyen était de 49 ans avec des extrêmes de 20 et 54 ans. Le fibrome uterin était le diagnostic le plus fréquent avec 35,71 p.100 soit 25 cas La voie abdominale représente 71 p.100 des cas contre dans environ 1/3 des cas de la voie base. Il est souhaitable d'introduire dans le service l'hystérectomies par Coeliochirurgie

    Factors associated with short term outcomes of hypospadias repair at the University Teaching Hospital, Lusaka, Zambia

    ThesisHypospadias is a common congenital penile abnormality with an incidence of 1 in 250 per live male births. It occurs as a birth defect resulting in the urethral opening being located anywhere from the glans along the ventral aspect of the shaft of the penis up to the scrotum or perineum in extreme cases. In its moderate to severe forms surgery is always advocated because affected individuals fail to pass urine while standing, the penis is cosmetically different from others and importantly causes subfertility in adulthood. However surgery is not without complications. An audit done locally at UTH, Lusaka in the year 2017 showed high complication rates of 10 percent for distal hypospadias and 67% for proximal hypospadias. The objective of this study was to determine the short-term complications and factors that are associated with this outcome at UTH Lusaka. In this study we hypothesized that small glans size, narrow urethral plate, PDS suture for urethroplasty, age more than 18months at operation and Haemoglobin level are not associated with increased complication rate. This was a consecutive (total enumerative) observational cohort study done at D-block and Clinic 7 at UTH Lusaka over a period of nine months. A total of 41 patients were enrolled and their demographic data, hemoglobin, maximum glans width size and urethral plate size before creating glans wing or midline plate incision were noted and measured. Logistic regression assessed urethroplasty complications (urethrocutaneous fistula, glans dehiscence, infection, penile torsion, persistent chordee) on urethral plate size, glans size, hemoglobin, suture material and catheter problems. The average age of participants was 34months. Distal hypospadias was the most common type at 61%, while middle hypospadias was 27% and proximal hypospadias 12%.The complication rate was 34%. The most common complication seen was urethrocutaneous fistula 9(53%). The other complications seen included 2(11.8%) glanular dehiscence, 2(11.8%) infection 1(6%) persistent chordee and 1(6%) penile torsion. The risk factors for post-operative complications included, the location of the urethral opening. The more proximal the site of the hypospadias the higher the complication rate. Complications were strongly associated with hemoglobin levels of less than 11.5g/dl (p=0.001) and catheter blockage or dislodgment (p=0.008). There was no association of complications with Social economic status, Age at time of repair, glans width size or suture material. Contrary to the finding of many authors wider urethral plate in this study was associated with higher complication rate. The conclusion was that short term complication rate at the UTH Lusaka Zambia is at 34%. The most common complication is urethrocutaneous fistula while the main factors associated with increased complications are hemoglobin less than 11.5g/dl, blocked and dislodgment urethral catheters, proximal hypospadias type. The recommendation from this study is hypospadias repair to be done with a hemoglobin of 12g/dl or more and post operatively a close catheter care is imperative.     Dépistage de l'Artériopathies Oblitérante des membres inférieurs chez les diabétiques type 2 : Intérêt de l'Index de Pression Systolique

    L'artériopathie oblitérante des membres inférieurs (AOMI) est un rétrécissement du calibre des artères à destination des membres inférieurs, qui entraîne une perte de charge hémodynamique, avec ou sans traduction clinique. Notre étude avait pour but le dépistage l'AOMI par la mesure de l'index des pressions systoliques (IPS) c'est une étude prospective qui s'est déroulée de Janvier 2018 à juillet 2018. Nous avons réalisé un IPS chez 170 sujets diabétiques type ; Les 56 qui avaient une anomalie de l'IPS ont fait l'objet de notre étude. L'évaluation de l'IPS était fait par le calcul de la sensibilité la spécificité les valeurs prédictives et les rapports de vraisemblances. Notre étude, était constituée en majorité de femmes (71,42 p.100 ) avec un sex ratio de 0,40. l'âge moyen était de 45,50 plus ou moins 11,87 ans. Nous avons retrouvé une moyenne de 4,5 plus ou moins 2,7 facteurs de risque cardiovasculaire par sujet. La sédentarité était le facteur de risque le plus retrouvé dans notre population 91,07 p.100 (n égal 51), suivi de la dyslipidémie 87,50 p.100 (n égal 49), l'HTA 76,78 p.100 (n égal 43), l'obésité/surpoids 60,70 p.100 ; l'obésité androide 53,57 p.100 (n égal 30) âge avance 48,21 p.100 (n égal 27) ; et le tabagisme 26,78 p.100 (n égal 15). Après estimation du risque cardiovasculaire des sujets, nous avons retrouvé que 69,64 p.100 (n égal 39) de notre population avaient un risque cardiovasculaire élevé, 19,64 p.100 (n égal 11) un risque moyen et 12,49 p.100 (n égal 7) un risque faible. Une AOMI (IPS inférieur à 0,90) a été retrouvée chez 67,85 p.100 (n égal 38) des sujets, tandis que 32,14 p.100 (n égal 18) avaient un IPS supérieur à 1,30. l'échographie doppler artérielle des membres inferieur a permis de diagnostiquer 5,35 p.100 (n égal 3) d'AOMI chez les 18 patients avec IPS supérieur à1,30. Nous avons recherché une association entre l'AOMI et, les facteurs de risque cardiovasculaire et nous avons trouvé que l'âge influençait de manière significative la survenue de l'AOMI (p égal 0,035). La prévalence de l'AOMI augmente avec le risque cardiovasculaire (p égal 0,002). Au terme de notre étude, nous avons fait les conclusions suivantes : la sédentarité est le facteur cardiovasculaire le plus retrouvé dans notre population ; nos sujets ont majoritairement un risque cardiovasculaire élevé ; l'AOMI est fréquente dans notre population avec une prévalence de 73,21 p.100, IPS est un bon outil permettant le dépistage de l'AOMI chez le diabétique avec une Se égal 0,92 e une Spé égal 0,8

    Complications périopératoires intra-hospitalières en chirurgie programmée : évaluation de la morbi-mortalité au CHU Gabriel-Touré

    L'objectif principal de la Commission mondiale de chirurgie est d'améliorer l'accès des patients aux traitements chirurgicaux. Si l'acte chirurgical est considéré comme une composante principale des soins de santé, en chirurgie non-cardiaque elle est corrélée à une mortalité précoce de 2 à 4 p.100 et une mortalité annuelle de 5 à 10 millions de personnes. Afin de déterminer l'incidence des complications et la mortalité intra-hospitalières péri-opératoires au CHU Gabriel Touré, cette étude prospective, analytique transversale a été réalisée, sur une période de six (6) mois allant de mai à novembre 2018. L'étude a inclus tous les patients opérés en chirurgie programmée dans trois services de chirurgie quelles que soient les techniques d'anesthésie, et hospitalisés pendant au moins 24 heures après l'intervention. Sur 262 patients opérés 245 ont été inclus dans notre étude. Complications Péri-opératoires intra hospitalières en chirurgie programmée : Évaluation de la morbi-mortalité au CHU Gabriel Touré. Nous avons enregistré 38,8 p.100 de complications péri-opératoires dont 33,5 p.100 de complications peropératoires et 13,5 p.100 de complications postopératoires. L'âge moyen des patients était de 43,3 plus ou moins 24,8 ans. Le sexe masculin est majoritaire avec un sex ratio de 3,3. La classe ASA 1 était la plus représentée soit 90 p.100 . Les complications retrouvées majoritairement sont : l'hypotension artérielle (80,5 p.100 ) en peropératoire ; et en postopératoire les infections du site opératoire (27,3 p.100 ), les saignements postopératoires (18,2 p.100 ) et les anémies (15,1 p.100 ) sont les plus représentés. Il ressort de cette étude que la survenue des complications péri-opératoires est influencée par : l'âge, aux comorbidités, la qualification de l'anesthésiste, du chirurgien, la classe d'Altémeier, la durée d'attente préopératoire, le type d'intervention, le type d'anesthésie, la durée de l'intervention et la durée d'hospitalisation postopératoire. La mortalité péri-opératoire reste encore élevée à 2 p.100 dont 1,2 p.100 le jour de l'intervention. Le pronostic dépend de la nature et de la gravité de la complication

    Risk of incarceration of inguinal hernias among paediatric patients awaiting elective surgery at the university teaching hospital, Lusaka, Zambia

    ThesisInguinal hernias in infants and children are very common. One of their most common complications before surgery is incarceration, a life threatening condition with potential to cause severe short term and long term morbidities like bowel ischaemia, testicular atrophy and its resultant infertility, in future. There appears to be, institutional variability in the rates and risks of inguinal hernia incarceration among infants and children. This study was conducted with a purpose to determine our own institutional risk of inguinal hernia incarceration among infants and children awaiting elective surgery. A retrospective review of infants and children under the age of 17 years, who had undergone inguinal hernia repair or had incarcerated, from January 2015 to January 2017 was performed. Interviewer completed questionnaires were used to abstract data from the clinical records. Three hundred and sixty-four patients were included for analysis, in this study. Descriptive statistics, Pearson Correlation Coefficient, Regression Analysis and Kaplan Meiers Survival Analysis were used. The infants and children were stratified into seven age groups. The incarceration rates and risks were evaluated for each group. The overall incarceration rate was 13.45%. The risk of incarceration was age dependent. It ranged from 0.52 daily risk of incarceration, among neonates, to zero daily risk of incarceration among older children. Neonates were as high as five times more likely to incarcerate as older infants and children. Likewise, younger children had higher risk of inguinal hernia incarceration than older ones. Longer wait times were associated with increased rate and risk of inguinal hernia incarceration. Overall, risk reduction, of inguinal hernia incarceration, of more than 50% could be achieved if inguinal hernia repair was done within 14 days of diagnosis. This study concluded that, the rate of incarceration was high (13.45%). The risk of incarceration was age dependent & higher in neonates (0.52), infants & younger children (0.01). Neonates had as much as 5 times higher risk as other children. Longer wait times, male sex, right sided hernias were associated with higher rates & risks. Older children had no incarceration. The best time to operate is before 3 weeks for neonates, before 2 months for infants & before 4 months for younger children to prevent incarceration.     Prevalence of acute deep vein thrombosis in HIV Seropositive Orthopaedic patients after major surgery at the University Teaching Hospital, Lusaka

    Hypercoagulable states and immobilization are known risk factors in the development of DVT after major orthopaedic surgery. However, the added effect of HIV to major orthopaedic surgery in developing DVT is unknown. In addition the benefits of D-dimer screening of patients after major surgery is not emphasized. A total of 42 Patients, 23 (54 %) were HIV negative (control) and 19 (46 %) were HIV positive were recruited by convenient sampling. Prior to surgery demographic and HIV status data was recorded. After surgery a blood sample was tested for D-dimer levels. The patients were then monitored acute DVT clinically and confirmed by ultrasound. The HIV positive cohort recorded 19 (46%) and the control (HIV negative) cohort recorded 23 (54%) participants. The results showed no significant difference in the development of acute DVT; (5.3%) in the HIV positive group and (4.3%) in the HIV negative group. In addition, no significant difference between the two groups in the number of positive D-Dimers, (97.7%) in the HIV positive cohort and (95.7%) in the HIV negative cohort group. In both cohorts, hip and knee surgeries had high values for D-Dimers. There was a positive correlation between D-dimers value and the site of surgery HIV positive cohort R = +0.390, p = 0.049 and control R = + 0.398 p = 0.03. The study shows no added effect of HIV in the known risk of major orthopaedic surgery in the development of DVT. Major orthopaedic surgery is a risk in developing DVT as shown in this study by high levels of D-dimers in both groups. Therefore the study recommends a blood test for D-dimers be incorporated in clinical risk assessment tool.     Use of Complementary and Alternative Medicine among Cancer Patients in Meru County, Kenya.

    http://library.kemu.ac.ke/ijpp/index.php/ijpp/article/view/25Cancer is among the leading causes of death globally.Despite advancing technology in conventional methods of cancer diagnosis and treatment, access to services remains a big challenge especially in the developing countries.Out of desperation, patients may be tempted to use Complementary and Alternative Medicine (CAM) to improve their health outcomes.However, there is minimal data in Kenya and specifically Meru County on use of CAM among cancer patients.The study aimed to assess the level of use, sources and perceived effects of CAM among cancer patients in Meru County.A descriptive cross-sectional study design was adopted in this study.Data was obtained from 117 participants through a researcher administered questionnaire.Data was cleaned, coded and keyed manually into the statistical package of social sciences (SPSS) version 23.Data was then summarized using means,percentages, frequency tables and measures of dispersion.The results showed that almost half of the respondents (47.9%) were using CAM mainly to cure cancer (78.6%). here was no association between use of CAM and the socio-demographic factors (p>0.05).The common CAM methods used included; spiritual therapy (37.5%), vitamins and supplements (26.5%) and local/traditional herbs (19.6%).Friends (55.4%) and family members (53.6%) provided the major sources of information on CAM.Pastors and local chemists were the most common sources of CAMs used. Improved health (n=30, 53.6%) and ability to cope with the disease (n=16, 28.6%) were the most frequent perceived benefits reported by CAM-users. None of the CAM-users reported any adverse/side effects experienced after use of CAM.Conclusively, results depicted a significant percentage of cancer patients in Meru County use CAM; mainly spiritual therapy and vitamins and mineral supplements. Health care provider should pay close attention to use of CAM while taking history from cancer patients and advice accordingly.Meru county government should regulate all CAM products that are accessible to the public and ensure the providers are well trained and registered
