Optimization of In-Beam Positron Emission Tomography for Monitoring Heavy Ion Tumor Therapy

    Get PDF
In-beam positron emission tomography (in-beam PET) is currently the only method for an in-situ monitoring of highly tumor-conformed charged hadron therapy. In such therapy, the clinical effect of deviations from treatment planning is highly minimized by implementing safety margins around the tumor and selecting proper beam portals. Nevertheless, in-beam PET is able to detect eventual, undesirable range deviations and anatomical modifications during fractionated irradiation, to verify the accuracy of the beam portal delivered and to provide the radiotherapist with an estimation of the difference in dosage if the treatment delivered differs from the planned one. In a first study within this work, a set of simulation and fully-3D reconstruction routines shows that minimizing the opening angle of a cylindrical camera is determinant for an optimum quality of the in-beam PET images. The study yields two favorite detector geometries: a closed ring or a dual-head tomograph with narrow gaps. The implementation of either detector geometry onto an isocentric, ion beam delivery (gantry) is feasible by mounting the PET scanner at the beam nozzle. The implementation of an in-beam PET scanner with the mentioned detector geometries at therapeutic sites with a fixed, horizontal beam line is also feasible. Nevertheless, knowing that previous in-beam PET research in Berkeley was abandoned due to detector activation (Bismuth Germanate, BGO), arising most probably from passive beam shaping contaminations, the proposed detector configurations had to be tested in-beam. For that, BGO was substituted with a state-of-the-art scintillator (lutetium oxyorthosilicate, LSO) and two position sensitive detectors were built. Each detector contains 32 pixels, consisting of LSO finger-like crystals coupled to avalanche photodiode arrays (APDA). In order to readout the two detectors operated in coincidence, either in standalone mode or at the GSI medical beam line, a multi-channel, zero-suppressing free, list mode data acquisition system was built. The APDA were chosen for scintillation detection instead of photomultiplier tubes (PMT) due to their higher compactness and magnetic field resistance. A magnetic field resistant detector is necessary if the in-beam PET scanner is operated close to the last beam bending magnet, due to its fringe magnetic field. This is the case at the isocentric, ion beam delivery planned for the dedicated, heavy ion hospital facility under construction in Heidelberg, Germany. In-beam imaging with the LSO/APDA detectors positioned at small target angles, both upbeam and downbeam from the target, was successful. This proves that the detectors provide a solution for the proposed next-generation, improved in-beam PET scanners. Further confirming this result are germanium-detector-based, spectroscopic gamma-ray measurements: no scintillator activation is observed in patient irradiation conditions. Although a closed ring or a dual-head tomograph with narrow gaps is expected to provide improved in-beam PET images, low count rates in in-beam PET represent a second problem to image quality. More importantly, new accelerator developments will further enhance this problem to the point of making impossible in-beam PET data taking if the present acquisition system is used. For these reasons, two random-suppression methods allowing to collect in-beam PET events even during particle extraction were tested. Image counts raised almost twofold. This proves that the methods and associated data acquisition technique provide a solution for next-generation, in-beam positron emission tomographs installed at synchrotron or cyclotron radiotherapy facilities

The Institute of Radiochemistry (IRC), one of the six institutes of the Forschungszentrum Rossendorf (FZR), performs application-oriented research in the fields of radiochemistry and radioecology. Motivation and background of our research are environmental processes relevant for the installation of nuclear waste repositories, for remediation of uranium mining and milling sites, and for radioactive contaminations caused by nuclear accidents and fallout. Due to their high radiotoxicity and long half-life the actinides are of special interest. Hence our research focuses on the chemical behavior of actinides at the molecular level in order to predict the relevant macroscopic processes in the environment. Within this framework, special emphasis is on the interface between geological and biological systems. In the last year our research topics were as follows: # Aquatic chemistry of actinides # Actinides in bio-systems # Interaction of actinides with solid phases # Reactive transport of actinides About 60 scientists, technicians and PhD students are employed in the Institute of Radiochemistry. We have achieved a wide range of new scientific results in the past year, which are presented in this Annual Report. Among them only a few can be highlighted here in this preface. For the first time it was possible to determine uranium speciation in situ in drinking and mineral waters e.g. by a dedicated fluorescence spectrometer at lowest µg/L concentrations. This methodical progress is an important prerequisite to study the uranium toxicity and its dependence on chemical speciation. We were very successful in the determination of formation pathways and structure of various actinide complexes, e.g., the surface complexes of uranium (VI) onto mica and iron hydroxides over a wide range of pH and carbonate concentration. These results contribute to a better understanding of actinide speciation in geo- and bio-systems, especially with respect to the chemical processes on the interfaces. Studies to the interaction of uranium with biofilms, green algae and bacteria coming from extreme habitats extended our research on the field of bio-systems. Major progress in the structural analysis of multiple uranium species has been achieved by applying Monte Carlo simulations and iterative transformation factor analysis to EXAFS spectroscopy. Furthermore, our new radiochemical experimental station at the Free Electron Laser of the Rossendorf accelerator ELBE is now in full operation. We have started first experiments on the uranium and neptunium complexation on selected mineral surfaces

Preface The Forschungszentrum Rossendorf (FZR) at Dresden is a multidisciplinary research center within the Wissenschafts-Gemeinschaft G. W. Leibniz (WGL), one of the German agencies for extra-university research. The center is active in investigations on the structure of matter as well as in the life sciences and in environmental research. The Institute of Nuclear and Hadron Physics (IKH) within the FZR avails for its research the coupling of radiation to matter in subatomic dimensions as well as to tissue, to cells, and to their components. Its research in the field of Subatomic Physics is part of the FZR-program Structure of Matter and its investigations concerning the interaction of Biostructures and Radiation contribute to the bf Life Science program of the FZR. In this field the IKH exploits possibilities for transfer and introduction of experimental and theoretical techniques from particle and nuclear physics to projects in radiobiology and biophysics. Much of this kind of interdisciplinary transfer is connected to the Radiation Source ELBE at the FZR. With its superconducting accelerator for relativistic electrons this large installation provides photons in the wide wavelength range from fm to mm - i.e. bremsstrahlung for the investigation of photonuclear processes, hard X-rays for radiobiological and other studies and infrared light for research on the structural dynamics of biomolecules. The investigation of radiation-induced processes not only dominates the projects in nuclear astrophysics as pursued at ELBE, it also is a central theme of the experimental and theoretical research performed by the IKH in close connection to the heavy ion synchrotron SIS and the upcoming FAIR facility at Darmstadt. ELBE also will deliver compact bunches of secondary neutrons and fission fragments; both offer new possibilities in laboratory studies related to the cosmic breeding of the chemical elements thus complementing the astrophysics-motivated studies with bremsstrahlung photons..
