Case Report: Four cases of cardiac sarcoidosis in patients with inherited cardiomyopathy: A phenotypic overlap, co-existence of two rare cardiomyopathies or a second-hit disease
Cardiac sarcoidosis (CS), a rare condition characterized by non-caseating granulomas, can manifest with symptoms such as atrioventricular block and ventricular tachycardia (VT), as well as mimic inherited cardiomyopathies. A 48-year-old male presented with recurrent VT. The initial 18F-fluorodeoxyglucose positron emission tomography (18FDG-PET) scan showed uptake of the mediastinal lymph node. Cardiovascular magnetic resonance (CMR) demonstrated intramyocardial fibrosis. The follow-up 18FDG-PET scan revealed the presence of tracer uptake in the left ventricular (LV) septum, suggesting the likelihood of CS. Genetic testing identified a pathogenic LMNA variant. A 47-year-old female presented with complaints of palpitations and syncope. An Ajmaline provocation test confirmed Brugada syndrome (BrS). CMR revealed signs of cardiac inflammation. An endomyocardial biopsy (EMB) confirmed the diagnosis of cardiac sarcoidosis. Polymorphic VT was induced during an electrophysiological study, and an implantable cardioverter-defibrillator (ICD) was implanted. A 58-year-old woman presented with sustained VT with a prior diagnosis of hypertrophic cardiomyopathy (HCM). A genetic work-up identified the presence of a heterozygous MYBC3 variant of unknown significance (VUS). CMR revealed late gadolinium enhancement (LGE), while the 18FDG-PET scan demonstrated LV tracer uptake. The immunosuppressive therapy was adjusted, and no further VTs were observed. A 28-year-old male athlete with right ventricular dilatation and syncope experienced a cardiac arrest during training. Genetic testing identified a pathogenic mutation in PKP2. The autopsy has confirmed the presence of ACM and a distinctive extracardiac sarcoidosis. Cardiac sarcoidosis and inherited cardiomyopathies may interact in several different ways, altering the clinical presentation. Overlapping pathologies are frequently overlooked. Delayed or incomplete diagnosis risks inadequate treatment. Thus, genetic testing and endomyocardial biopsies should be recommended to obtain a clear diagnosis
Changes in Alcohol-Specific Mortality During the COVID-19 Pandemic in 14 European Countries
Aim: Exploring trends in 1) alcohol-specific mortality and 2) alcohol sales in European countries in the years before and during the
COVID-19 pandemic. Method: Complete data on alcohol-specific mortality and alcohol sales were obtained for 14 European countries (13 EU
countries and UK) for the years 2010 to 2020, with six countries having mortality data available up to 2021. Age-standardised mortality rates
were calculated and descriptive statistics used. Results: When compared to 2019, alcohol-specific mortality rates in 2020 increased by 7.7 %
and 8.2 % for women and men, respectively. Increases in alcohol-specific mortality were seen in the majority of countries and continued in
2021. In contrast, alcohol sales declined by an average of 5.0 %. Conclusion: Despite a drop in alcohol consumption, more people died due to

Proteinortho6: pseudo-reciprocal best alignment heuristic for graph-based detection of (co-) orthologs
Proteinortho is a widely used tool to predict (co)-orthologous groups of genes
for any set of species. It finds application in comparative and functional genomics,
phylogenomics, and evolutionary reconstructions. With a rapidly increasing
number of available genomes, the demand for large-scale predictions is also
growing. In this contribution, we evaluate and implement major algorithmic
improvements that significantly enhance the speed of the analysis without
reducing precision. Graph-based detection of (co-)orthologs is typically based
on a reciprocal best alignment heuristic that requires an all vs. all comparison of
proteins from all species under study. The initial identification of similar proteins is
accelerated by introducing an alternative search tool along with a revised search
strategy—the pseudo-reciprocal best alignment heuristic—that reduces the
number of required sequence comparisons by one-half. The clustering
algorithm was reworked to efficiently decompose very large clusters and
accelerate processing. Proteinortho6 reduces the overall processing time by
an order of magnitude compared to its predecessor while maintaining its small
memory footprint and good predictive quality
Psychometric properties of the modified Suicide Stroop Task (M-SST) in patients with suicide risk and healthy controls
The Cognitive Model of Suicide proposes a suicide attentional bias in individuals
with suicidal thoughts and behavior (STBs). The Suicide Stroop Task (SST) was
developed as a behavioral measure to assess this attentional bias. However,
prior studies demonstrated poor psychometric properties of the SST
Vaginale Lasertherapie der weiblichen Belastungsinkontinenz: Prospektive Untersuchung der Patientenergebnisse
Die hier vorgestellte Arbeit untersucht den therapeutischen Effekt der vaginalen Lasertherapie auf die Behandlung der Belastungsinkontinenz. Es wurde prospektiv eine Kohorte von 100 Patientinnen untersucht, welche zwischen Januar 2020 und Mai 2022 eine vaginale Laserbehandlung auf Grund der leichten bis mäßigen Belastungsinkontinenz in der Praxis vom Herr Dr. med. Tom Kempe in Leipzig erhielten.
In die statistische Auswertung konnten schließlich 50 Frauen (n=50) eingeschlossen werden, da sie alle vier Fragebögen vollständig ausfüllten. Diese Kohortengröße ist mit den anderen Studien vergleichbar. Die Fragebögen wurden von den Patientinnen jeweils vor der Therapie, nach der Beendigung der Therapie, sowie in den Kontrollen in drei und sechs Monaten nach der Therapie ausgefüllt. Die Lasertherapie bestand aus insgesamt drei Lasersitzungen jeweils in 6- bis 8-wöchigen Abständen.
Die Laserbehandlung wurde mit dem Erbium:YAG-Laser der Firma Fotona durchgeführt, entsprechend dem IncontiLase® Protokoll.
Die Er:YAG Lasertherapie zeigte in unserem Kollektiv eine Verbesserung in allen Symptombereichen eines dysfunktionalen Beckenbodens (Blasenfunktion, Darmfunktion, Senkungsbeschwerden, Sexualbeschwerden). Ebenso konnte unsere Studie zeigen, dass der Effekt dieser Therapieoption weitgehend unabhängig vom Alter oder dem BMI der Patientin waren. Allein die Anzahl der vaginalen Geburten schien sich teilweise auf die Symptomverbesserung negativ auszuwirken.
Auch in den untersuchten Einzelfragen zeigte sich eine statistisch signifikante Verbesserung der Beschwerden. Insbesondere ist bezüglich der Blasenfunktion eine signifikante Verbesserung zu sehen. Interessanterweise zeigte sich auch eine Verbesserung der Beschwerden, welche auf eine Dranginkontinenz (Überaktive Blase) zurückzuführen waren.
Der positive Effekt der vaginalen Laserbehandlung auf die Sexualfunktion, in der Literatur als Vaginal-rejuvenation bekannt (Juhász et al. 2021), hat sich hier bestätigt. Unter Vaginal-rejuvenation versteht man ein breites Spektrum von ästhetischen und medizinischen Prozeduren, die vulvovaginale Atrophie, vaginale Laxizität und Harninkontinenz behandeln (Vanaman et al. 2016). Somit bestätigen unsere Ergebnisse den therapeutischen Effekt der Lasertherapie auf die Sexualfunktion.
Überraschenderweise zeigte sich zusätzlich eine Verbesserung der Wahrnehmung der Darmfunktion, die über die kontrollierte Beobachtungszeit erhalten geblieben ist. Unseres Wissens ist dies die erste Veröffentlichung des o.g. Indikationsspektrums.
Die Untersuchte Gruppe bestand aus Frauen, welche sich primär aufgrund der Belastungsinkontinenz vorstellten und keine manifeste Senkung hatten. Sodass die Senkungsbeschwerden bei diesen Frauen nicht im Vordergrund standen. Jedoch zeigte die Auswertung der Fragebögen, dass Senkungsbeschwerden bei einigen Patientinnen vor der Therapie vorhanden waren. Interessanterweise besserte sich nach Absolvierung der drei Lasertherapien auch die subjektive Wahrnehmung des Vaginalgefühls, die in der Beobachtungszeit ebenfalls statistisch signifikant geblieben ist. Letzteres ist dadurch zu erklären, dass die Lasertherapie zu einem Wärmeaufbau mit einer Straffung von Kollagen ohne dessen Denaturierung oder Destruktion führt.
Die Lasertherapie nimmt im Bereich der Urogynäkologie mittlerweile einen festen Stellenwert ein und ist in die Leitlinie der Behandlung der Belastungsinkontinenz eingegliedert. Diese Studie zeigt, dass die minimal invasive Er:YAG Lasertherapie die Symptome der leichten bis mäßigen Belastungsinkontinenz, sowie den anderen Symptomen des dysfunktionellen Beckenbodens wie Darmfunktion, Senkungsbeschwerden und Sexualität, verbessert.
Der Vorteil gegenüber der operativen TVT-Einlage ist die Möglichkeit der ambulanten Behandlung, einhergehend mit geringerem Aufwand für die Patientinnen. Zusätzlich ist diese Methode mit keinen oder nur geringen Komplikationen verbunden. Gemäß unseren Ergebnissen ist die Lasertherapie eine ideale Methode für Frauen die eine schnelle, risikoarme, minimalinvasive Behandlung ohne synthetische Fremdmaterialien suchen, sowie die Familienplanung noch nicht abgeschlossen haben. Die Lasertherapie kann auch den Frauen angeboten werden, bei denen die Beckenbodengymnastik die gehoffte Besserung nicht erbrachte sowie in Ergänzung dazu.
Die Lasertherapie stellt somit ein etabliertes Behandlungsinstrument im Rahmen der Behandlung der Beckenbodendysfunktion, vor allem der Belastungsinkontinenz und wird damit den Patientinnen in unserer Sprechstunde angeboten.:1. Titelblatt

Timing of complementary feeding for early childhood allergy prevention: an overview of systematic reviews
Objective: To summarise and critically appraise systematic review (SR) evidence on
the effects of timing of complementary feeding (CF) on the occurrence of allergic
sensitisation and disease.
Design: Overview of SRs. AMSTAR-2 and ROBIS were used to assess methodological
quality and risk of bias (RoB) of SRs. RoB 2 Tool was used to assess RoB of primary
randomised controlled trials (RCTs) (or extracted). The certainty of evidence (CoE)
was assessed using GRADE. Findings were synthesised narratively.
Data Sources: MEDLINE (via PubMed and Ovid), the Cochrane Library and Web of
Science Core Collection (2010 to 27 February 2023).
Eligibility Criteria: SRs investigating the effects of timing of CF in infants or young
children (0–3 years) on risk of developing food allergy (FA), allergic sensitisation,
asthma, allergic rhinitis, atopic eczema and adverse events based on RCT evidence.
Results: Eleven SRs were included. Only two SRs had low RoB; common issues were
failure to report on funding of primary studies and failure to provide a list of excluded
trials. Common limitations of included trials were lack of blinding of outcome assess-
ment or detailed trial preregistration, and inadequate handling of high loss to follow
up. Primary study overlap was very high for specific FA and slight to moderate for FA
in general and other primary outcomes. Introducing specific foods (peanut, cooked
egg) early probably reduces the risk of specific FA. Evidence for other allergic out-
comes was mostly very uncertain and based on few primary studies. Trials varied
regarding timing of CF, nature of complementary foods and population risk, which
limited comparability between SRs
Depressive and anxiety symptoms during the COVID-19 pandemic in the oldest-old population and the role of psychosocial factors: a multivariate and multi-wave analysis
Background: Since the oldest-old population was identified as a high-risk group
for a severe course of the coronavirus disease and higher mortality, it was assumed
that they might be particularly psychologically burdened. The aim of the study is
to analyze the development of anxiety and depressive symptoms over the course
of the pandemic from 2020 to 2022, as well as psychosocial factors associated
with these outcomes.
Method: We analyzed data of n = 135 participants aged 78 to 97 years old (2020:
M = 86.77, SD = 4.54) with three points of measurement from May to June
2020 (t1), March to May 2021 (t2) and November to January 2022 (t3). Besides
sociodemographic variables, worries about the Sars-Cov-2 virus, living situation,
perceived social support (ESSI), resilience (BRS), anxiety and depressive symptoms
(BSI-18) were assessed. We calculated multilevel mixed-effects generalized linear
models with a negative binominal distribution to model anxiety and depressive
symptoms over time.
Results: While there is an increase in depressive and anxiety symptoms in the
investigated oldest-old individuals in Germany from 2020 to 2021, there is no
further increase in symptomatology from 2021 to 2022. Participants of older age
reported higher levels of anxiety symptoms. Higher perceived social support was
associated with both less depressive and less anxiety symptoms, while resilience
was associated with less depressive symptoms only. More worries about the Sars-
Cov-2 virus were associated with higher anxiety levels.
Conclusion: Overall, the oldest-old population appeared to show rather stable
mental health after a slight increase in symptomatology within the first year of
the pandemic. Social support is an important factor to target in mental health
prevention programs for oldest-old individuals in times of future crises such as a
pandemic
Mathematical Simulation of Haemodynamic Flow Competition Affecting A Radial Arterial Coronary Bypass Graft
Zusammenfassung (Deutsch)
Hintergrund
Die koronare Herzkrankheit (KHK) stellt weiterhin die häufigste Todesursache in westlichen Industrienationen dar. Ein frühes Versagen arterieller Bypass-Transplantate tritt in etwa 4–7 % der Fälle auf und wird mit einem sogenannten „competitive flow“ aus der nativen Koronarzirkulation in Verbindung gebracht, wenn der Stenosegrad nicht ausreichend flusslimitierend ist. Die bisherige klinische Evidenz basiert überwiegend auf Beobachtungsstudien heterogener Patientenkollektive, sodass kausale Zusammenhänge zwischen Flusskonkurrenz und Transplantatversagen nur eingeschränkt beurteilt werden können. Die computergestützte Modellierung bietet daher die Möglichkeit, koronare Hämodynamik patientennah und kontrolliert zu untersuchen, um chirurgische Strategien bei der koronaren Bypasschirurgie (CABG) zu optimieren.
Methoden
Zur Erstellung eines dreidimensionalen Modells der Koronararterien auf Basis von CT-Daten wurde die Open-Source-Software SimVascular (Stanford University, CA, USA) verwendet. Modelliert wurde eine rechte Koronararterie mit variablem Stenosegrad sowie ein radialarterielles Bypass-Transplantat, das von der Aorta ascendens ausgeht und distal der Stenose anastomosiert ist. Der Stenosegrad wurde systematisch variiert, und mithilfe von Computational Fluid Dynamics (CFD) wurden der Blutfluss in der nativen Koronararterie, im Bypass sowie der distale Gesamtfluss simuliert. Die erforderliche Präzision der Berechnungen wurde durch iterative Optimierung der Netzerstellung und Visualisierung der Strömungsdynamik erreicht.
Ergebnisse
Das Modell konnte koronare Flussdynamiken über einen Stenosebereich von 30 % bis 80 % erfolgreich abbilden. Der distale Gesamtfluss blieb dabei weitgehend konstant, während sich die Anteile von nativem Koronarfluss und Bypassfluss abhängig vom Stenosegrad verschoben. Bei einem Stenosegrad von etwa 55 % zeigte sich ein Gleichgewicht zwischen nativem und graftabhängigem Fluss mit jeweils rund 20 mL/min. Unterhalb dieses Schwellenwerts dominierte der native Koronarfluss, oberhalb der Bypassfluss. Zwischen 30 % und 40 % Stenose war die native Flussvariation gering, während bei Stenosegraden ≥ 70 % nur noch ein geringer zusätzlicher Bypassfluss erreicht wurde.
Schlussfolgerung
Die CFD-basierte Modellierung identifiziert einen hämodynamisch relevanten Schwellenwert bei etwa 55 % Koronarstenose, an dem sich die Flussverhältnisse zwischen nativer Arterie und radialarteriellem Bypass deutlich zugunsten eines graftabhängigen Flusses verschieben. Diese Ergebnisse sprechen für eine stärker individualisierte, hämodynamikbasierte Entscheidungsfindung in der koronaren Bypasschirurgie. Eine enge interdisziplinäre Zusammenarbeit zwischen Herzchirurgie, computergestützter Modellierung und Softwareentwicklung ist entscheidend, um CFD-basierte Verfahren weiter in die klinische Planung und Ausbildung zu integrieren.:Table of Contents

Coronary artery disease (CAD) remains the leading cause of mortality in Western societies. Early arterial graft failure occurs in approximately 4–7% of cases and is thought to be primarily related to competitive flow from the native coronary circulation when the degree of stenosis is insufficient to be flow-limiting at rest. Existing clinical evidence is largely observational and derived from heterogeneous patient populations, limiting the ability to establish causal relationships between competitive flow and graft failure. Computational modelling has therefore emerged as a valuable tool to investigate coronary haemodynamics in a controlled, patient-specific setting, with the aim of optimizing coronary artery bypass grafting (CABG).
Methods
The open-source software SimVascular (Stanford University, CA, USA) was used to construct a three-dimensional model of the coronary arteries based on CT imaging data. A right coronary artery with a variable degree of stenosis and a radial artery bypass graft originating from the ascending aorta and anastomosed distal to the stenosis were modelled. Coronary stenosis severity was systematically varied, and blood flow in the native coronary artery, the bypass graft, and the total distal coronary circulation was simulated using computational fluid dynamics. Accurate mesh generation and visualization of flow dynamics were achieved through iterative refinement.
Results
The computational model successfully reproduced coronary haemodynamics across a stenosis range of 30% to 80%. Total distal coronary flow remained relatively constant despite marked changes in individual flow components. At approximately 55% stenosis, native coronary and graft flows were balanced at around 20 mL/min. Below this threshold, native coronary flow predominated, whereas above it graft-dependent flow became dominant. Minimal variation in native coronary flow was observed at stenosis levels between 30% and 40%, while graft flow was minimal at stenosis levels of 70% or greater.
Conclusion
Computational fluid dynamics modelling identified a haemodynamically relevant threshold at approximately 55% coronary stenosis, where competitive flow dynamics between the native artery and radial artery graft shift toward graft-dependent perfusion. These findings suggest that individualized, haemodynamics-based assessment may support a more tailored approach to CABG. Interdisciplinary collaboration between cardiac surgeons, computational scientists, and software developers will be essential to further integrate CFD-based tools into clinical decision-making and surgical planning.:Table of Contents

Sequenzanalyse von SARS CoV-2-Stämmen aus Leipzig von März bis September 2020
Die vorliegende Arbeit befasst sich mit der Sequenzanalyse von SARS-CoV-2-Stämmen aus Leipzig im Untersuchungszeitraum von März bis September 2020. Die Zielsetzung dieser wissenschaftlichen Arbeit besteht somit in der Analyse des Auftretens von SARS-CoV-2-Mutationen in Leipzig von März bis September 2020 sowie in der Erfassung des geografischen Ursprungs der im März 2020 untersuchten SARS-CoV-2-Infektionen. Die durchgeführte Analyse ergab, dass die Mutation D614G im Spike-Protein in Kombination mit der Mutation P323L bei 100 % der analysierten SARS-CoV-2-Stämme aus Leipzig von März bis September 2020 auftrat. Der geografische Ursprung zahlreicher SARS-CoV-2-Infektionen in Leipzig im März 2020 kann in Tirol vermutet werden, da 46 % der untersuchten COVID-19-Patienten im März 2020 eine Reiseanamnese mit Bezug zu Tirol aufwiesen. New Developments of Chiral Palladium-Aqua Complexes as Cooperative Brønsted Acid-Base Catalysts in Organic Synthesis
Based upon their exceptional reactivity as cooperative Brønsted
acid-base catalysts chiral diphosphane Pd-aqua complexes have
successfully been utilized in a broad range of asymmetric
transformations. Originally discovered by Sodeoka 25 years ago,
they readily form reactive chiral Pd enolates upon deprotona-
tion of β-dicarbonyl compounds and simultaneously, a Brønsted
acid liberated from the catalyst activates the electrophile. After
the initial report and some ensuing papers had been published
in the early 2000s, the field lay dormant for more than a
decade. In recent years, however, new developments towards
the enantioselective synthesis of various heterocycles and other
useful compounds have evolved which take advantage of the
unique properties of chiral Pd-aqua complexes
