    Full scale test on reinforced concrete elements of an existing bridge: on-site investigation and laboratory testing design

    The evaluation of mechanical characteristics and degradation level of existing bridges is a highly relevant topic today. This paper presents the initial phases of an integrated experimental-numerical research project focused on a pre-scheduled demolition of an existing reinforced concrete bridge. The controlled demolition of existing bridges represents an invaluable opportunity, allowing to deepen our comprehension on their behavior, especially when degradation analysis is concerned. Both on-site tests and subsequent laboratory experiments design are carried out on full-scale elements extracted from the bridge. These tests are complemented by Finite Element analyses conducted at various levels of detail. Specifically, after the description of the knowledge phases of the bridge performed on-site, the paper illustrates the preliminary numerical analyses carried out to support the customized design of the experimental setup, where mean values of all material parameters are assumed to acquire the essential information on the expected test results. Additionally, Monte Carlo analysis has been performed to evaluate the impact of statistical variability of material parameters on structural response. This aspect is particularly relevant for the design of the experimental test setup, due to the large forces involved and large displacements expected in order to reach the full collapse of the beams

    Surgical Management of Foot Syndactyly: The State of the Art and a Treatment Algorithm Based on a Literature Review

    Objectives: This systematic review provides an in-depth analysis of the surgical treatment of foot syndactyly, offering insights into the various options and their applications. The objective is to improve treatment standardization and optimize patient care in cases of foot syndactyly. Methods: By synthesizing the existing evidence, the authors propose a treatment algorithm to guide clinicians in decision-making, considering the type and severity of syndactyly. Results: Sixteen articles were included in the systematic review, comprising a total of 395 patients. The mean age at surgery was 6 years (range: 0.66–23 years). Clinical outcomes were assessed after a mean follow-up of 33.2 months (range: 4–82.8 months). Several surgical procedures were described in the included articles. Conclusions: Continued research efforts and collaborative initiatives are crucial to further refine our understanding of complications and enhance surgical practices in foot syndactyly procedures

    A Dedicated Algorithm for Endovascular Approach as a First-Line Treatment Option for Visceral Artery Aneurysms

    Objectives: Few data are reported in literature about visceral artery aneurysms (VAAs) management. The aim of the present study was to analyze VAAs management in a single institution, with a dedicated algorithm for endovascular approach as the first line treatment. Methods: A single-center retrospective cohort study was performed. Patients with a VAA submitted to either endovascular repair or open surgery from 2016 to 2023 were included. A dedicated algorithm was used to evaluate the endovascular approach feasibility assessing on the preoperative computed tomography angiography the following parameters: (a) the tortuosity of the involved artery (<150 degrees), (b) the healthy arterial diameter (>4 mm), (c) the VAA proximity to the hilum and/or the presence of a bifurcation of the aneurysmatic artery, and (d) the circumferential calcium presence. An endovascular approach was chosen if (a) and (b) criteria were satisfied without (c) and (d) ones. Otherwise, it was deemed a challenging anatomy, and an open surgical treatment was considered. In the absence of (a) and/or (b) criteria open surgery was the preferred option. Results: Thirty-one asymptomatic aneurysms (28 patients) were treated electively. The most frequent VAA location was the splenic artery (18 cases; 58%), followed by the renal arteries (6 cases; 19%), the common hepatic artery (5 cases; 16%), the gastroepiploic artery (1 case, 3.2%) and the pancreatoduodenal artery (1 case; 3%). Twenty-two aneurysms (71%) were initially treated by an endovascular approach (stent-graft deployment and/or transcatheter embolization) with 3 (13%) of them needing a surgical conversion. Nine aneurysms (29%) were submitted directly to a surgical treatment (aneurysm resection with or without interposition bypass) with no peri-operative and long-term complications. Technical success was 90.3%. Conclusions: Endovascular management as a first line approach is safe and effective in most cases. A preoperative dedicated algorithm is useful to identify suitable cases. Open surgery can be considered an alternative option in specific challenging anatomical situations or in case of endovascular failure

    Tonsillar Leishmaniasis: A Rare Clinical Entity Mimicking Malignancy in the Oropharynx - A Case Series from Northeastern Italy

    Purpose: Tegumentary leishmaniasis (TL) is a neglected disease in Europe, often underdiagnosed or misdiagnosed due to its variable clinical presentation. Mucosal leishmaniasis (ML) is a rare manifestation of TL, and isolated tonsillar leishmaniasis is an even rarer finding, with very few reported cases. This study aims to expand knowledge on this unusual clinical entity by describing five cases of isolated tonsillar leishmaniasis diagnosed in the Emilia-Romagna region (ERR), northeastern Italy, emphasizing diagnostic challenges and treatment outcomes. Methods: Between January 2014 and December 2024, all consecutive patients presenting with unilateral tonsillar swelling and pharyngodynia were evaluated at otolaryngology units in ERR hospitals. Histopathological analysis, special stains (H&E, Giemsa, PAS, Ziehl-Neelsen), and immunostaining (CD1a) were performed at the referring hospital. Microbiological confirmation was obtained through real-time PCR targeting Leishmania kDNA and rRNA. Treatment was administered according to current TL guidelines. Results: We included five patients who presented with unilateral tonsillar swelling mimicking malignancy and with a histological diagnosis of non-necrotizing granulomas. Histology revealed amastigotes in four cases. PCR confirmed Leishmania infection in all cases. Treatment with liposomal amphotericin B or pentamidine led to complete clinical remission. Conclusion: Isolated tonsillar leishmaniasis should be considered in the differential diagnosis of head and neck tumors, especially in endemic regions. Histological and molecular tools are essential for accurate diagnosis. Increased awareness among clinicians and pathologists is necessary to improve recognition and management of this rare presentation

    A three-dimensional agro-hydrological model for predictive analysis of shallow landslides: CRITERIA-3D

    model is an extension of the CRITERIA-3D free-source model for crop development and soil hydrology, developed by the Hydrometeorological service of the Regional Agency for Environmental prevention and Energy of Emilia-Romagna region (Arpae-simc). The soil-water balance is computed through the coupling of surface and subsurface flows in multi-layered soils over areas topographically characterized by Digital Elevation Model (DEM). The rainfall infiltration process is simulated through a three-dimensional version of Richards’ equation. Surface runoff, lateral drainage, capillarity rise, soil evaporation and plant transpiration contribute to the computation of the soil hydrology on an hourly basis. The model accepts meteorological hourly records as input data and outputs can be obtained for any time step at any selected depth of the soil profile. Among the outputs, volumetric water content, soil-water potential and the factor of safety of the slope can be selected. The validation of the proposed model has been carried out considering a test slope in Montu`e (northern Italy), where a shallow landslide occurred in 2014 a few meters away from a meteorological and soil moisture measurement station. The paper shows the accuracy of the model in predicting the landslide occurrence in response to rainfall both in time and space. Although there are some model limitations, at the slope scale the model results are highly accurate with respect to field data even when the spatial resolution of the Digital Elevation Model is reduced

    Effect of high-pressure carbon dioxide combined with modified atmosphere packaging on the quality of fresh-cut squash during storage

    The study evaluated the application of a novel high-pressure microbial inactivation method combining dense carbon dioxide with modified atmosphere packaging on organic fresh-cut squash (Cucurbita moschata). Approximately 4 g or 32 g of squash was packed in plastic pouches filled with CO2 to test two different gas-to-product ratios and treated with the high-pressure method at previously optimized process conditions (45 °C, 6.0 MPa and 40 min). The products were then stored for 21 days at 4 °C and assessed for enzymatic activity, product quality, sugar content, bioaccessibility (polyphenols, DPPH antioxidant activity, and carotenoids), and sensory acceptance, with products packed in air and CO2 serving as controls. The high-pressure treatment effectively inactivated inoculated E. coli to undetectable levels (inactivation >3.63 ± 0.53 Log CFU/g) and reduced the activity of the browning-responsible enzymes up to 50 %. During the shelf life, treated samples exhibited significantly higher scavenging activity for DPPH, ABTS, OH, O2−, and NO compared to non-treated samples, with minor exceptions at a high gas-to-product ratio. Additionally, treated samples showed increased levels of glucose and fructose and a comparable or higher bioaccessibility of antioxidants with respect to the products packed in air or in CO2. Sensory evaluation indicated that the treatment enhanced color and smell appreciation among panelists, demonstrating the potential of this method to improve both safety and quality of fresh-cut squash
